Proven Cytomegalovirus Colitis Associated with Dasatinib Administration in Two Pediatric Allogeneic Hematopoietic Stem Cell Transplantation Recipients

  • Gastrointestinal (GI) bleeding is a rare adverse event of dasatinib, which is known to be caused by dasatinib-induced colitis, severe thrombocytopenia, and platelet dysfunction. We present two cases of pediatric patients who developed hematochezia during treatment with dasatinib after hematopoietic stem cell transplantation (HSCT). A colonic tissue biopsy was performed to differentiate the cause of GI bleeding. Both patients were diagnosed with proven cytomegalovirus (CMV) colitis, but only one was treated with ganciclovir. The patient who did not receive antiviral therapy experienced recurrent GI bleeding during dasatinib administration, leading to multiple treatment interruptions. During dasatinib therapy after HSCT, patients with GI bleeding and confirmed CMV colitis may benefit from antiviral therapy to reduce interruptions in dasatinib therapy.

Human cytomegalovirus 증식에 미치는 cAMP의 영향 (Effect of cAMP on the Replication of Human Cytomegalovirus)

  • Human cytomegalovirus (HCMV) 의 immediate early(IE) 유전자의 promoter/enhancer 부위에 cAMP response element가 있다는 것으부터 caMP 가 HCMV 의 증식에 관여할 것이라고 생각할 수 있다. 이러한 가능성을 알아보기 위해 8-bromoadenosine 3', 5'-cyclic monophosphate (BrA) 와 papaverine 같은 세포내 cAMP 농도를 변화시키는 약제를 사용하여 HCMV 의 증식, DNA 합성 및 IE 유전자 발현에 대한 영향을 알아보았다. HCMV 증식과 DNA 합성은 papaverine 에 의해 억제된 반면, BrA 는 HCMV 의 증식에는 큰 영향을 주지 않았고 DNA 합성은 오히려 촉진시키는 것을 알 수 있었다. HCMV IE promoter 에 의해 작동되는 CAT 유전자를 함유한 plasmid pCMVIE/CAT 을 세포내로 transfection 시켰을 때, papaverine 을 처리한 세포에서는 CAT 효소 활성도가 감소한 반면 BrA 를 처리한 세포에서는 증가하였다. HCMV 에 감수성이 없는 HeLa 세포에서는 CAT 활성도가 감소성 세포인 HEL 세포에서보다 높게 나타난 반면, Vero 세포에서는 낮게 나타났다. 이들 비감수성 세포인 HEL 세포에서 보다 높게 나타난 반면 Vero 세포에서는 낮게 나타났다. 이 들 비감수성 세포에서의 CAT 활성도는 BrA 를 처리하여 주었을 때 모두 증가하였다. 이상과 같은 결과로부터 cAMP 는 HCMV 증식과 IE 유전자 발현에 어느 정도 관여한다는 것을 알 수 있다.

  • Herpes virus family is highly infectious to patients, their families and dentists. The diagnosis of herpes infection is based on the characteristic clinical appearance and the location of the lesions. Herpes Simplex Virus(HSV) usually acquired through direct contact with infected lesions or body fluids, and the prevalence of HSV infection increases progressively from childhood. Primary infections provoke herpetic gingivostomatis typically affects the tongue, lips, gingival, buccal mucosa and palate. Recurrent infections give rise to vesiculo-ulcerative lesions at vermilion border of lip(herpes labialis). In the form of chickenpox, Varicella Zoster Virus(VZV) usually is infected in childhood. VZV spreads in the affected primary afferent nerve to the skin and produces a vesicular rash and pain. Epstein-Barr Virus(EBV) infects B cells and cause infectious mononucleosis. Latent EBV infection has also been implicated in Burkitt lymphoma, nasopharyngeal carcinoma. Cytomegalovirus(CMV) is associated with immune-compromised patient such as organ transplantation and AIDS patients.

  • It is now more than two decades since the AIDS epidemic began with a cluster of Pneumocystis carinii pneumonia (PCP) in a community of homosexual men. Since then, many other infections have been characterized as opportunistic infections secondary to HIV infection. These include, but are not limited to, infections with Toxoplasma gondii, Cytomegalovirus (CMV), Mycobacterium avium complex (MAC), and Cryptococcus neoformans. Over the last two decades, there have been dramatic improvements in diagnosis, prevention and treatment of all these infections. As a result, in North America and Western Furope the rates of opportunistic infections secondary to AIDS have decreased substantially. We will review these common opportunistic infections below.

  • 본 연구실에서는 새로운 베큘로바이러스 벡터 시스템을 구축하였다. 즉 본 벡터 시스템은 polyhedron promoter, vesicular stomatitis virus G (VSVG), polyA, cytomegalovirus (CMV) promoter, enhanced green fluorescent protein (EGFP), protein transduction domain (PTD)을 코딩하는 유전자들로 구성 되어있다. 이렇게 새로이 제작된 베큘로바이러스 벡터 시스템과 대조군의 벡터 시스템과 효율과 발현율을 비교하였다. 그 결과 본 연구실에서 제작된 베큘로바이러스 벡터 시스템이 다른 대조군의 벡터 시스템에 비해 효과적임을 확인할 수 있었다.

  • 거대세포바이러스(CMV: Cytomegalovirus)에 의한 폐렴은 기관지계의 염증 및 폐쇄로 인해 폐실질의 낭종성 변화를 유발할 수 있다. 본 교실에서는 심한 호흡부전을 동반한 생후 2주된 유아에서 선천성 폐낭종 질환 진단하에 폐구역 절제술을 시행하였다. 수술 후 병리소견 및 혈청학적 검사를 통해 선천성 감염에 의한 거대세포바이러스(CMV) 세기관지폐렴을 확인하였기에 보고하는 바이다.

  • polyhedron promoter, vesicular stomatitis virus G (VSVG), polyA, cytomegalovirus (CMV) promoter, enhanced green fluorescent protein (EGFP), protein transduction domain (PTD) 유전자가 포함된 재조합 베큘로바이러스를 구축하였다. 본 재조합 베큘로바이러스 시스템은 인간 섬유아세포와 여러 가지 조직에 감염하여 시험하였고 재조합된 유전자의 전달과 유전자 발현을 대조 벡터시스템과 비교하였다. 본 연구의 결과로 제작된 재조합 베큘로바이러스 시스템은 유전자의 전달과 발현에 있어서 대조 벡터시스템 보다 더욱 효과적이고 안전적이었다.

Cytomegalovirus Infection under a Hybrid Strategy in Pediatric Liver Transplantation: A Single-Center Experience

  • Purpose: To evaluate the outcomes of a hybrid prophylactic strategy to prevent cytomegalovirus (CMV) disease in pediatric liver transplantation (LT) patients. Methods: CMV DNAemia was regularly monitored by quantitative nucleic acid amplification test (QNAT) and was quantified in all children. CMV infection and disease were defined according to the International Consensus Guidelines. The hybrid strategy against CMV infection consisted of universal 3-week prophylaxis and preemptive treatment of intravenous ganciclovir regardless of the recipient's serostatus. Results: A total of 143 children who underwent living donor LT were managed using the hybrid strategy. The overall incidence of CMV infection by QNAT was 48.3% (n=69/143). The highest CMV DNAemia positivity was observed in 49.2% (n=60/122) of children in the D+/R+ group, followed by 46.7% (n=7/15) in the D+/R- group. CMV disease was noted in 26.1% (n=18/69) patients. Forty-three (62.3%) children had undergone preemptive therapy consisting of intravenous ganciclovir. No symptomatic patients developed tissue-invasive disease, resulting in no CMV-associated mortality. Conclusion: The incidence of CMV infection was high in pediatric LT patients despite the hybrid strategy. However, tissue-invasive disease in pediatric LT did not occur.
