함치성 낭종에 대한 방사선학적 연구 (A RADIOLOGIC STUDY OF DENTIGEROUS CYSTS)

  • A radiologic study of 139 dentigerous cysts (128 patients) indicated the followings: 1. There was higher incidence in males (62.99%) than in females (37.01%). The most common age group was 11-20 years and the average age was 23.6 years. 2. The most common clinical symptoms was swelling of the jaws (49 cases, 33.09%), and no symptoms was presented in 26 cases (18.71%). 3. Dentigerous cysts were found to be slightly more common in the maxilla (53.24%) than in the mandible (46.76%) and there was almost no difference in occurance between right and left side. The maxillary supernumerary tooth was the most frequent site of the dentigerous cyst and no dentigerous cyst was found to be related to a deciduous tooth. 4. The most common radiographic findings was root resorption of the adjacent teeth (33.09%), and the central dentigerous cysts were 78 cases (56.12%), the lateral dentigerous cysts were 61 cases (43.88%). 5. The increased radiolucency at the crown portion of the tooth in a cystic cavity was seen in 44 cases (31.65%), and a case of dentigerous cyst was found in a edentulous patient, and 16 cases of multiple dentigerous cysts were found in 5 patients. 6. The ameloblastic changes in 8 cases, the keratinization of the cyst wall in 2 cases, and a case of epidermoid carcinoma were confirmed microscopically.

A Novel Homozygous CLCNKB Mutation of Classic Bartter Syndrome Presenting with Renal Cysts in 6-year-Old Identical Twin Boys : A Case Report

  • Bartter syndrome is an autosomal recessive hypokalemic salt-losing tubulopathy, and classic Bartter syndrome is associated with mutations in the CLCNKB gene. While chronic hypokalemia is known to induce renal cyst formation in different renal diseases, renal cyst formation in Bartter syndrome is rarely reported. Russian six-year-old identical male twins were referred to our hospital for the evaluation of renal cysts, which were incidentally detected on abdominal sonography due to diarrhea. Both twins had shown symptoms of polydipsia, polyuria, and nocturia since they were one year olds. Vital signs including blood pressure were normal in both twins. Renal sonography revealed nephromegaly, increased echogenicity of renal cortex, and various sized multiple cysts in both kidneys for both twins. Laboratory findings included hyponatremia, hypokalemia, hypochloremia, and metabolic alkalosis. Bartter syndrome with renal cysts were suspected. Genetic analysis for both twins confirmed a homozygous c.1614delC deletion on exon 15 of the CLCNKB gene, which was confirmed as a previously unreported variant to the best of our knowledge. They were managed with potassium chloride, nonsteroidal anti-inflammatory drugs, and angiotensin-converting-enzyme inhibitors. Metabolic alkalosis, hypokalemia, hypochloremia, and polyuria partially improved during the short course of treatment. This is the first report of a homozygous mutation in the CLCNKB gene in an identical twin, presenting with renal cysts.

Histological classification of canine ovarian cyst types with reference to medical history

  • Ovaries of 21 bitches presented with gynecopathies were surgically removed and histologically examined. Standard histological, as well as immunohistochemical, classification of 193 cystic structures resulted in the classification of 72 cysts of subsurface epithelial structures (SES), 61 follicular cysts (FCs), 38 cystic rete ovarii (CRO), 13 lutein cysts (LCs), and 9 non-classifiable cysts (NCCs). In addition to the histological classification, results were interpreted according to subject medical history, clinical examination outcome, and macroscopic observations during ovariohysterectomy. Dogs with ovarian cysts (OCs) and associated reproductive perturbations were mostly nulliparous, of large breed, and had an average of $9.5{\pm}3$ years. Prolonged or shortened inter-estrus intervals of past heats, however, seemed to be relatively low-risk factors for the development of OCs in dogs. Furthermore, we provide histological observations of a rarely seen canine LC including a degenerated oocyte in the central cavity.

Sinus lifts in the presence of pseudoantral and mucous retention cysts

  • Objectives: Mucous retention cysts and pseudoantral cysts are mainly located within the floor of the maxillary sinus. Most of these maxillary cysts are asymptomatic and often only require observation. However, the presence of these benign maxillary cysts may create problems when maxillary sinus all types of implants are needed. Various treatment methods have been introduced. The selected treatment option depends on the type, size, and location of the cyst and its symptoms. Patients and Methods: The case reports of four patients with maxillary cysts were reviewed retrospectively. These patients received a sinus lift between January 2016 and October 2021 at the Wonkwang University Dental Hospital. Results: To reduce unnecessary operations and the duration of treatment, a conservative treatment method is required. A sinus lift in the presence of maxillary cyst will not typically cause sinus problems if the lifted sinus membrane does not interfere with ventilation of the maxillary sinus. Conclusion: When proper treatment is provided, sinus perforation during a sinus lift performed in the presence of maxillary cyst and contamination of bone graft materials by cystic fluid does not necessarily result in adverse outcomes.

성대낭종의 임상적 특성 (Clinical Characeristics of Intracordal Cysts)

  • 본 교실에서 시행했던 후두미세수술 1900례중에서 술후 성대낭종으로 확진된 83례를 대상으로 다음과 같은 결과를 얻었다. 1) 전체 후두미세수술 1900례 중 성대낭종이 차지하는 비율은 83례로 4.4%였다. 2) 성대낭종 83례중 유표피낭종은 27례로 32.5%. 저류낭종은 56례로 67.5%를 차지하였다. 3) 성별분포는 83례의 성대낭종중 여성이 55례(66.3%). 남성이 28례 (33.7%)로 여성에서 호발하였고 저류 낭종에서는 남녀비가 17 : 39. 유표피낭종에서는 11 : 16으로 나타났다. 4) 이학적검사 소견으로서 유표피낭종은 보다 진단이 용이하여 술전 이학적 검사상 63%에서 낭종을 의심할 수 있었으나 저류낭종은 성대 폴립이나 결절 등으로 오인되는 경우가 많았고 술전 진단과 술후 진단이 일치하는 경우는 34%에 지나지 않았다. 5) 병리 조직학적 검사상 유표피낭종은 중층 평편상피로 구성된 막을 가지고 있고 내부는 대개 PAS 음성인 케라틴과 콜레스테롤양(cholesterol-like)물질을 포함하고 있고 저류낭종은 원주 또는 입방세포로 구성되어있는 막을 가지고 있으며 주로 PAS양성인 점액 분비물을 내부에 포함하고 있다. 6) 애성 발생과 관련된 유발 요인으로는 성대과용후 33례, 상기도감염후 16례로 성대과용과 상기도감염이 주요한 유발 요인으로 나타났고 성대낭종의 발생에 수다스러움(talkativeness)이 관련성 있게 나타났고 저류낭종에서 관련성이 더욱 높게 나타났다. 7) 발병부위는 대부분에서 막성대유리연의 중간부 및 전방부 1/3에 발생하였고 수술결과는 저류낭종 50례 중 12례에서만 낭종의 파열없이 완전제거가 가능하였으며 유표피낭종은 24례 중 16례에서 낭종의 파열없이 완전제거가 이루어져 저류낭종이 유표피낭종에 비해 파열없이 완전제거가 매우 어려움을 알 수 있었다. 8) 수술전후의 음성만족도는 전체적으로 59%를 나타내어 성대폴립에서의 만족도와의 유사한 결과를 보였다.m)은 각각 0.60, 0.44, 0.40%로 시험초기와 비슷한 수준이었다.의성있게 최대발성지속시간은 감소하고, 평균호기류율은 증가하였으며, 발성기류량과 성문하압은 여자환자에서만 의미 있게 각각 감소하거나 증가하였다. 이상의 결과는 정상인과 성대용종 환자에 있어서 객관적인 공기역학적 검사결과를 보여주며 이는 성대용종 환자의 음성이상에 대한 상태판정이나 치료 후 효과 판정에 기본자료로 이용할 수 있을 것으로 생각된다.야 할 것으로 생각된다. 발생과 피해정도를 예측하여 벼멸구 방제에 중요한 자료로 활용될 수 있을 것으로 생각한다.5) 실험 3에서는 zygote의 VS1 에 노출시간에 따른 생존율을 조사한 결과 융해후 2-cell (91.6, 88.5 및 88.9%) 및 배반포기 (83.3, 74.3 및 69.4%) 까지 배발달율은 1,2 및 3분간의 노출시간에 따른 유의적인 차이를 보이지 않았다. 또한 융해후 노출시간에 따른 할구수에서도 유의적인 차이를 보이지 않았다 (36.4$\pm$4.76, 32.4$\pm$4.67 및 27.6$\pm$4.52). 이상의 결과에서 OPP vitrification 방법은 EFS 또는 EDS 동결보존액에 따른 유의적인 차이 없이 이용될 수 있는 것으로 판단된다. 배발달단계에 따른 생존율은 zygote 의 초기배는 2, 4, 8, 상실배 및 배반포기보다 유의적으로 저조한 생존율을 보였다. Zygote의 VS1에 노출시간에 따른 생존율도 1 분간의 노풀시간에서 높은 배발달율을 보였다. OPP vitrification 동결보존방법으로 생쥐수정란의 동결보존에 유용하게 이용가능한 것으로 판단된다.l the training process, the subject, and area of research, can be pointed out one of the flimsy in our researches of science education.

상악 전치부에 발생한 함치성 낭종의 임상적 특징과 치료에 대한 후향적 연구 (Retrospective Study of Characteristics and Treatment of Dentigerous Cysts Involving the Maxillary Anterior Tooth)

  • 본 연구는 상악 전치부에 이환된 함치성 낭종의 다수의 증례 분석을 통해 임상적 특징 및 치료 결과에 대해 알아보고자 하였다. 상악 전치부에서 함치성 낭종으로 진단 받은 55명의 환아 중에서 절치부에서 33증례, 견치부에서 22증례를 분석하였다. 그 결과 절치부에서는 남자가 7.2배 더 높았고 종창을 주소로 내원한 경우가 많았으며, 견치부는 여자가 1.7배 더 높았고 정기검진에서 발견된 경우가 더 많았다. 이환된 낭의 영향으로 92.7%에서 치아 변위가 관찰되었고 치료는 41증례에서 조대술이, 14증례에서 적출술이 시행되었다. 낭종 처치 후 자발적 맹출은 견치부보다 절치부에서 더 높았다. 하지만 자발적 맹출 이후 절치부에서는 부정교합으로 인해 교정치료가 시행된 경우가 많았다. 본 연구를 통해서 상악 전치부에 발생한 함치성 낭종의 분포 및 특성을 알 수 있었고, 치료 결과를 제공함으로써 향후 치료계획을 세우는데 도움이 될 수 있으리라 생각된다.

Clinical and Neuroimaging Outcomes of Surgically Treated Intracranial Cysts in 110 Children

  • Objective : The indications and optimal surgical treatments for intracranial cysts are controversial. In the present study, we describe long-term clinical and neuroimaging results of surgically treated intracranial cysts in children. The goal of this study is to contribute to the discussion of the debate. Methods : This study included 110 pediatric patients that underwent surgeries to treat intracranial cysts. Endoscopic cyst fenestrations were performed in 71 cases, while craniotomies and cyst excisions (with or without fenestrations) were performed in 30 patients. Cystoperitoneal shunts were necessary for nine patients. Long-term results were retrospectively assessed with medical and neuroimaging records. Results : Clinical and radiological improvement was reported in 87.3% and 92.8% of cases, respectively, after endoscopic neurosurgery, and in 93.3% and 100% using open microsurgery whereas 88.9% and 85.7% after shunt operation. There were no statistical differences in clinical outcomes (p=0.710) or volume reductions (p=0.177) among the different surgeries. There were no mortalities or permanent morbidities, but complications such as shunt malfunctions, infections, and subdural hematomas were observed in 56% of the patients that had shunt operations. A total of 13 patients (11.8%) underwent additional surgeries due to recurrences or treatment failures. The type of surgery performed did not influence the recurrence rate (p=0.662) or the failure rate (p=0.247). Conclusion : Endoscopic neurosurgeries are less invasive than microsurgeries and are at least as effective as open surgeries. Thus, given the advantages and complications of these surgical techniques, we suggest that endoscopic fenestration should be the first treatment attempted in children with intracranial cysts.

Dermoid cysts: Epidemiology and diagnostic approach based on clinical experiences

  • Background Dermoid cysts are congenital tumors that are benign. Dermoid cysts with intracranial extension can cause serious neurological complications. It is important, therefore, to determine whether a patient has a dermoid cyst when their chief concern at a doctor's visit is a mass in the head or neck area. In this study, we present a literature review of dermoid cysts and an analysis of the authors' experiences, with the goal of providing guidance useful for the diagnosis and treatment of dermoid cysts. Methods This study retrospectively analyzed the medical records of 62 patients who visited the two medical clinics with which the authors are affiliated. The patients were enrolled between October 2003 and January 2017. Results Of the 62 patients analyzed in this study, 32 were 0 to 5 years of age (52%) and 23 were 17 years of age or older (37%). Forty-seven patients underwent 1 or more imaging study during the process of diagnosis. Thirty-two patients were suspected to have a dermoid cyst. Forty-nine patients were analyzed to determine the depth of the cyst. Bone was seen in 43 patients through imaging tests or during actual surgery, and nine of the 43 had bony problems (21%). Conclusions This study found that dermoid cysts were present in many adults, and that a high rate of deep lesions was observed, as well as many cases in which even the bone was affected. These results suggest, therefore, that dermoid cysts should be considered, and medical professionals should actively conduct imaging studies.

The effectiveness of decompression for patients with dentigerous cysts, keratocystic odontogenic tumors, and unicystic ameloblastoma

  • Objectives: The aim of this study was to verify the clinical effectiveness of decompression in decreasing the size of a cyst. In addition to the different types of cysts, we tried to reveal what effect host factors, such as the initial size of the lesion and the age of the patient, have on the velocity of cyst shrinkage. Materials and Methods: With the aid of a panoramic view, we measured the size of the cysts before and after decompression in 13 dentigerous cysts (DCs), 14 keratocystic odontogenic tumors (KTOCs), and 5 unicystic ameloblastoma (UA) cases. The velocity of shrinkage in the three cystic groups was calculated. Relationships between the age of the patient, the initial size of the cyst, and the shrinkage velocity were investigated. Results: The three types of cysts showed no inter-type differences in their velocity of shrinkage. However, there was a statistically meaningful relationship between the initial size of the lesion and the absolute velocity of shrinkage in the DC group (P=0.02, R=0.65) and the KTOC group (P=0.02, R=0.56). There was also a significant relationship between the age of the patient and the absolute velocity of shrinkage in the KTOC group (P=0.04, R=0.45) and the UA group (P=0.04, R=0.46). Conclusion: There was no difference in the decrease in size due to decompression among the different types of cysts. However, the age of the patient and the initial size of the lesion showed a significant relationship with the velocity of shrinkage.

피하 흉터 조직에서 발견된 후천성 유피낭종 (Acquired Dermoid Cysts within Subcutaneous Scar Tissue)

  • Purpose: Dermoid cysts are almost always caused by congenital events. The most widely accepted pathogenesis is that the cysts are dysembryogenetic lesions that arise from ectodermic elements entrapped during the midline fusion. We report a rare case of dermoid cyst, which occurred not congenitally but newly in the subcutaneous scar tissue secondary to trauma. Methods: A 26-year-old man had a deep submental laceration caused by a car accident and got a primary wound closure 16 months ago. There were 18 cm-long submental hypertrophic scar and newly developed palpable masses inside the subcutaneous layer at the center of the scar. Initial impression was an epidermal cyst or a thyroglossal duct cyst. Ultrasonographic finding showed two cystic masses inside the scars at the submental area, but impressed dermoid cysts. The cystic masses were completely removed with W-plasty and histological examination were followed. Results: The histological diagnosis was dermoid cysts which were mainly composed of keratotic squamous epithelium in their inner surface linings and numerous skin appendages such as sebaceous glands, sweat glands, and hair follicles in their cystic lumens histopathologically. During the follow-up period of 25 months, there was no recurrence of any subcutaneous mass in the site of scar. Conclusion: We report a very unusual case of dermoid cysts developed by an acquired cause, considering that the accidental inclusion of deep skin elements caused by a trauma can be a critical origin of dermoid cysts.
