The Development of a Intrahepatic Biliary Cyst after Kasai Operation for Biliary Atresia (카사이 수술 후 발생한 간내 담관 낭종)

  • A 6 and a half year-old girl developed recurrent cholangitis following hepatic portoenterostomy for biliary atresia. Computed tomogram showed an ovoid cyst ($4.5{\times}4.0$ cm in size) in the left hepatic lobe and another tubular dilatation ($2.0{\times}0.8$ cm in size) in the right hepatic lobe. Percutaneous transhepatic cholangiodrainage(PTCD) and cystogram showed an ovoid cyst in the left hepatic lobe (Tsuchida type A), measuring $6.6{\times}5.0$ cm in size. Following drainage and administration of parenteral antibiotics she became afebrile and anicteric. However she continued to drain 45-150 cc of bile per day via the tube over the next 2 weeks. The patient successfully underwent intrahepatic cystojejunostomy with intraoperative ultrasonographic guidance. This case illustrates relapsing cholangitis caused by Tsuchida type A intrahepatic cyst, successfully managed with PTCD followed by an internal drainage procedure.

  • The so-called eruption cyst, which occurs mostly on the third molar, is dentigerous cyst, arising at a late stage of tooth development. The authors have treated a dentigerous cyst occurred in angle region of the left mandible in 27-year-old woman who has complained of the facial asymmetry, by means of enucleation of the cyst in tis entirety and tooth removal, and resulted in a fairly good prognosis.

  • Plantar corn is a circumscribed and sharply demarcated hard mass of traumatic hyperkeratosis, which has a central core impacting the dermis deeply and causes pain while walking. The cause of the corn is the mechanical stress to the skin induced by several causative factors; extrinsic(tight shoes) or intrinsic(bony prominence), or combined(the claw toe). We found 7 cases of patients with a painful recurrent plantar corn which had an epidermal cyst under its lesion. These corns and cysts were excised totally and there was no recurrence in all cases during two to four months follow-up period. We think that an epidermal cyst may be another intrinsic factor for the development of a plantar corn. So, an epidermal cyst should be considered once in a recurrent plantar corn to be recalcitrant against popular treatments.

  • The soybean cyst nematode, Heterodera glycines, is a major plant-parasitic nematode that has caused important economic losses to Korea's soybean production. Four species of cyst nematodes, H. schachtii, H. glycines, H. trifolii, and H. sojae, all belong to schachtii group are coexist in field soil in Korea. The rapid identification of the nematode is crucial for preventing crop damage and in decision making for controlling this nematode. This study aimed to develop a species-specific primer set for quantitative PCR (qPCR) assay of H. glycines. The specific primer set (HGF1 and HGR1) for H. glycines was designed based on the cytochrome c oxidase subunit I (COI) sequence of mitochondrial DNA. After optimization, it is possible to identify the H. glycines using a qPCR assay with DNA extracted from a single cyst and single second-stage juvenile (J2). The specificity was confirmed by the absence of SYBR fluorescent signals of three other Heterodera species. A serial dilution of DNA extracted from a single cyst was obtained for the sensitivity test. The result showed that the standard curve of the test had a highly significant linearity between DNA concentration and Ct value (R2 = 0.996, slope = -3.49) and that the detection limit concentration of DNA of the primer set was 10 pg of DNA per reaction. Our findings suggested that H. glycines could be distinguished from H. sojae and other Heterodera species when a qPCR assay is used with a specific primer set.

  • In order to understand fine structural and histochemical changes in the cyst cell and he interstitial cell in the testis of the spottybelly greenling Hexagrammos agrammus associated with the reproductive cycle from January to December, 1992, there cells were studied by electron microscopy and light microscopy. The cyst cells in the mature testis show a weak affinity to haematoxylin. while they become larger in size. At this time, these cells appear to be more functional than those on the growing stage because several mitochondria, endoplasmic reticulum, glycogen particles and a few lipid droplets appear in the cytoplasm of the cyst cell. It appears, therefore, that the cyst cell of this species has vital functions for nutrition, secretion and steroidogenesis. Well-developed interstitial cells contain large rod-shape or spherical mitochondria with tubular cristae and the large quantities of smooth endoplasmic recticulum and electron-dense materials in the vesicle at the mature and spawning stage. The interstitial cells of this species show characteristics of steroid interstitial cells having a vesicular nucleus, mitochondria with tubular cristae, and smooth endoplasmic reticulum. However, these interstitial cells of teleost give negative histochemical reactions for Sudan black B.

Choledochal Cyst in Children (소아의 담관낭종)

  • To study the role of anomalous pancreatico-biliary ductal union (APBDU) in the development of choledochal cyst, we reviewed 23 cases. APBDU is defined as a long common channel(>0.4 mm). The patients ages ranged from 1 week to 112 months and the mean age was 22.5 months. Right upper quadrant pain was the most prevalent symptom. The diagnosis was made by ultrasonography and operative cholangiography in most patient. The preoperative diagnosis was made in 100 % of the cases. Gallstones were found in 5 cases. Todani type I and type IV were prevalent. A long common channel was found in all cases. The operative treatment consisted of cyst excision and Reux-en-Y hepaticojejunostomy or choledochojejunosotmy. One patient had postoperative pancreatitis. There was no mortality. We conclude that detection of choledochal cyst is occurring at a younger age and APBDU seems to play an important role in the pathogenesis of type I and IV cysts. Cyst excision is the treatment of choice to eliminate repeated cholangitis and malignant transformation.

  • The testis of Neoditrema ransonneti is testicular tubule type, each testicular tubule consists of numerous testicular cysts which contain numerous germ cells showing the same developmental stage. During spermatogenesis, well developed rough endoplasmic reticula and the Golgi complex are observed in the cyst cell. Secretory activity of cyst cell was the highest in the late spermiogenesis. Sperm binding materials of spermatozeugmata are secreted by testicular cyst cell. One spermatozeugmata is produced by a testicular cyst during spermatogenesis. The capsular structure was not found in the spermatozeugmata discharged from male. According to observations under transmission electron microscopy approximately 1,500 to 1,700 of sperm tails were observed in the cross sectioned spermatozeugmata.

  • Choledochal cyst is a rare developmental malformation of the biliary tree and has serious problem of transformation to malignancy. The development of cholangiocarcinoma related to choledochal cyst increases as the age of patient increases and is more common in Orientals with female predominance of 2.5 times. Prevalence rate of cholangiocarcinoma is various from 2.5 to 15.6% in adult with choledochal cyst, but very low in children. We experienced a case of cholangiocarcinoma with multiple liver metastases arising from type I choledochal cyst in a 13-year-old girl who complained of sudden onset of right upper quadrant abdominal pain.

Natural Course and Treatment of Fetal Ovarian Cysts (산전 진단된 난소낭의 자연 경과 및 치료)

  • With the development of fetal ultrasonography, detection of fetal ovarian cysts has been increased. Although ovarian cyst formation during the perinatal period is a self limiting process, there is still considerable controversy regarding the best treatment of the fetal ovarian cyst. The purpose of this study is to evaluate the natural history of fetal ovarian cysts and to analyze the result of treatment. From 1995 to 2004, 31 consecutive fetuses with ovarian cysts were followed by ultrasonography during the perinatal period. The fetal ovarian cyst was diagnosed by prenatal ultrasonography between 25weeks and 38 weeks and the mean size of the cysts was 5cm (ranged from 2 to 8cm). At birth, 3 cysts disappeared. In 2 cases, the diagnoses were changed to multicystic kidney disease and intestinal duplication. During following up of 26 cysts, 15 cysts have resolved completely. Seven cysts required oophorectomy because of cyst torsion (n=3), differentiation of tumorous condition (n=2), increased size of cyst (n=1), and large size (8cm) of cyst at birth (n=l). Fetal ovarian cyst should primarily be observed, and only in the limited cases, surgical treatment would be required for the risk of complications such as torsion and differentiation from benign to malignant pathology.

  • A ventriculo-peritoneal shunt is a standard surgical management for hydrocephalus, but complications may impede the management of this disease. Obstruction of the catheter is one of the most common complications and manifests clinically in various ways. Intraparenchymal cyst development after shunt malfunction has been reported by several authors, but the underlying mechanism and optimal treatment methods are debatable. The authors report a case of intraparenchymal cyst formation around a proximal catheter in a premature infant after a ventriculo-peritoneal shunt and discuss its pathogenesis and management.
