농촌(農村) 주민(住民)들의 의료필요도(醫療必要度)에 관(關)한 연구(硏究) (A Study Concerning Health Needs in Rural Korea)

  • Today most developed countries provide modern medical care for most of the population. The rural area is the more neglected area in the medical and health field. In public health, the philosophy is that medical care for in maintenance of health is a basic right of man; it should not be discriminated against racial, environmental or financial situations. The deficiency of the medical care system, cultural bias, economic development, and ignorance of the residents about health care brought about the shortage of medical personnel and facilities on the rural areas. Moreover, medical students and physicians have been taught less about rural health care than about urban health care. Medical care, therefore, is insufficient in terms of health care personnel/and facilities in rural areas. Under such a situation, there is growing concern about the health problems among the rural population. The findings presented in this report are useful measures of the major health problems and even more important, as a guide to planning for improved medical care systems. It is hoped that findings from this study will be useful to those responsible for improving the delivery of health service for the rural population. Objectives: -to determine the health status of the residents in the rural areas. -to assess the rural population's needs in terms of health and medical care. -to make recommendations concerning improvement in the delivery of health and medical care for the rural population. Procedures: For the sampling design, the ideal would be to sample according to the proportion of the composition age-groups. As the health problems would be different by group, the sample was divided into 10 different age-groups. If the sample were allocated by proportion of composition of each age group, some age groups would be too small to estimate the health problem. The sample size of each age-group population was 100 people/age-groups. Personal interviews were conducted by specially trained medical students. The interviews dealt at length with current health status, medical care problems, utilization of medical services, medical cost paid for medical care and attitudes toward health. In addition, more information was gained from the public health field, including environmental sanitation, maternal and child health, family planning, tuberculosis control, and dental health. The sample Sample size was one fourth of total population: 1,438 The aged 10-14 years showed the largest number of 254 and the aged under one year was the smallest number of 81. Participation in examination Examination sessions usually were held in the morning every Tuesday, Wenesday, and Thursday for 3 hours at each session at the Namchun Health station. In general, the rate of participation in medical examination was low especially in ages between 10-19 years old. The highest rate of participation among are groups was the under one year age-group by 100 percent. The lowest use rate as low as 3% of those in the age-groups 10-19 years who are attending junior and senior high school in Taegu city so the time was not convenient for them to recieve examinations. Among the over 20 years old group, the rate of participation of female was higher than that of males. The results are as follows: A. Publie health problems Population: The number of pre-school age group who required child health was 724, among them infants numbered 96. Number of eligible women aged 15-44 years was 1,279, and women with husband who need maternal health numbered 700. The age-group of 65 years or older was 201 needed more health care and 65 of them had disabilities. (Table 2). Environmental sanitation: Seventy-nine percent of the residents relied upon well water as a primary source of dringking water. Ninety-three percent of the drinking water supply was rated as unfited quality for drinking. More than 90% of latrines were unhygienic, in structure design and sanitation (Table 15). Maternal and child health: Maternal health Average number of pregnancies of eligible women was 4 times. There was almost no pre- and post-natal care. Pregnancy wastage Still births was 33 per 1,000 live births. Spontaneous abortion was 156 per 1,000 live births. Induced abortion was 137 per 1,000 live births. Delivery condition More than 90 percent of deliveries were conducted at home. Attendants at last delivery were laymen by 76% and delivery without attendants was 14%. The rate of non-sterilized scissors as an instrument used to cut the umbilical cord was as high as 54% and of sickles was 14%. The rate of difficult delivery counted for 3%. Maternal death rate estimates about 35 per 10,000 live births. Child health Consultation rate for child health was almost non existant. In general, vaccination rate of children was low; vaccination rates for children aged 0-5 years with BCG and small pox were 34 and 28 percent respectively. The rate of vaccination with DPT and Polio were 23 and 25% respectively but the rate of the complete three injections were as low as 5 and 3% respectively. The number of dead children was 280 per 1,000 living children. Infants death rate was 45 per 1,000 live births (Table 16), Family planning: Approval rate of married women for family planning was as high as 86%. The rate of experiences of contraception in the past was 51%. The current rate of contraception was 37%. Willingness to use contraception in the future was as high as 86% (Table 17). Tuberculosis control: Number of registration patients at the health center currently was 25. The number indicates one eighth of estimate number of tuberculosis in the area. Number of discharged cases in the past accounted for 79 which showed 50% of active cases when discharged time. Rate of complete treatment among reasons of discharge in the past as low as 28%. There needs to be a follow up observation of the discharged cases (Table 18). Dental problems: More than 50% of the total population have at least one or more dental problems. (Table 19) B. Medical care problems Incidence rate: 1. In one month Incidence rate of medical care problems during one month was 19.6 percent. Among these health problems which required rest at home were 11.8 percent. The estimated number of patients in the total population is 1,206. The health problems reported most frequently in interviews during one month are: GI trouble, respiratory disease, neuralgia, skin disease, and communicable disease-in that order, The rate of health problems by age groups was highest in the 1-4 age group and in the 60 years or over age group, the lowest rate was the 10-14 year age group. In general, 0-29 year age group except the 1-4 year age group was low incidence rate. After 30 years old the rate of health problems increases gradually with aging. Eighty-three percent of health problems that occured during one month were solved by primary medical care procedures. Seventeen percent of health problems needed secondary care. Days rested at home because of illness during one month were 0.7 days per interviewee and 8days per patient and it accounts for 2,161 days for the total productive population in the area. (Table 20) 2. In a year The incidence rate of medical care problems during a year was 74.8%, among them health problems which required rest at home was 37 percent. Estimated number of patients in the total population during a year was 4,600. The health problems that occured most frequently among the interviewees during a year were: Cold (30%), GI trouble (18), respiratory disease (11), anemia (10), diarrhea (10), neuralgia (10), parasite disease (9), ENT (7), skin (7), headache (7), trauma (4), communicable disease (3), and circulatory disease (3) -in that order. The rate of health problems by age groups was highest in the infants group, thereafter the rate decreased gradually until the age 15-19 year age group which showed the lowest, and then the rate increased gradually with aging. Eighty-seven percent of health problems during a year were solved by primary medical care. Thirteen percent of them needed secondary medical care procedures. Days rested at home because of illness during a year were 16 days per interviewee and 44 days per patient and it accounted for 57,335 days lost among productive age group in the area (Table 21). Among those given medical examination, the conditions observed most frequently were respiratory disease, GI trouble, parasite disease, neuralgia, skin disease, trauma, tuberculosis, anemia, chronic obstructive lung disease, eye disorders-in that order (Table 22). The main health problems required secondary medical care are as fellows: (previous page). Utilization of medical care (treatment) The rate of treatment by various medical facilities for all health problems during one month was 73 percent. The rate of receiving of medical care of those who have health problems which required rest at home was 52% while the rate of those who have health problems which did not required rest was 61 percent (Table 23). The rate of receiving of medical care for all health problems during a year was 67 percent. The rate of receiving of medical care of those who have health problems which required rest at home was 82 percent while the rate of those who have health problems which did not required rest was as low as 53 percent (Table 24). Types of medical facilitied used were as follows: Hospital and clinics: 32-35% Herb clinics: 9-10% Drugstore: 53-58% Hospitalization Rate of hospitalization was 1.7% and the estimate number of hospitalizations among the total population during a year will be 107 persons (Table 25). Medical cost: Average medical cost per person during one month and a year were 171 and 2,800 won respectively. Average medical cost per patient during one month and a year were 1,109 and 3,740 won respectively. Average cost per household during a year was 15,800 won (Table 26, 27). Solution measures for health and medical care problems in rural area: A. Health problems which could be solved by paramedical workers such as nurses, midwives and aid nurses etc. are as follows: 1. Improvement of environmental sanitation 2. MCH except medical care problems 3. Family planning except surgical intervention 4. Tuberculosis control except diagnosis and prescription 5. Dental care except operational intervention 6. Health education for residents for improvement of utilization of medical facilities and early diagnosis etc. B. Medical care problems 1. Eighty-five percent of health problems could be solved by primary care procedures by general practitioners. 2. Fifteen percent of health problems need secondary medical procedures by a specialist. C. Medical cost Concidering the economic situation in rural area the amount of 2,062 won per residents during a year will be burdensome, so financial assistance is needed gorvernment to solve health and medical care problems for rural people.

가정학교육 영역에서의 인구교육문제에 관한 조사연구 -선임가정학자들을 대상으로- (A Study of the Attitude of/and Problems Encountered by Senjor Home Economist Toward the Integration of Family Planning Education in the Korean Formal School System)

  • Under the full consideration of the growing need and importance of population education in the field of home economics in Korea, the study was carried out to verify and assess the following facts on the current issues of population education of home economists who are presently engaging in teaching professions as the teachers of middle and high school and professors of college and universities by setting its primary objectives of the study as followings; 1) to assess the degree of general knowledge and attitudes of home economists toward population education in the field of home economics, 2) to verify the problems encountered in implementing population education by home economists in its field, 3) to find an existing status of previous trainings received and other activities of population education of home economists aimed at utilizing these findings as a part of reference materials when the population education is conducted in the field of home economics. In order to attain these objectives described above, the questionnaire was carefully designed to house a total of 40 questions with good combination of multiple-choice and the simple answer questions. The mail questionnaire survey was conducted by establishing teachers of home economics at middle/high schools and college/universities as Senior Home Economists(SHE) who are from public, private liberal arts and vocational schools. The rate of response observed during the survey was 45.6 percent and the findings of the survey research are as follows: 1) Examining the status of the respondents by residence and religion, it was found that 45 percent of middle & high school teachers ar.d 59. 1 percent of college professors are residing in Seoul city area and that the largest percent of them are christian in their religion. Analyzing respondents by their ages, 56 percent of middle/high school teachers are in their 30s, 45 percent of college professors are in their 40s, and 37 percent of college teachers are in their 30s. In addition, 13 percent of the total respondents are found to be unmarried. The study also revealed that 71 percent of the college professors finished Master Degree course and 82 percent of middle/high school teachers are graduated from college level lasting 4 years. Looking over the status cf major fields of respondents, 68.4 percent of middle/high school teachers are specialized in home economic education and the college professors, on the other hand, show relatively even prortion by specializing in the order of food & nutrition science, clothes & textile science and home managerial science. As far as the length of teaching experience is concerned, a relatively longer period of teaching experience is observed in the college professors in comparison with that of middle/high school teachers. In other words, 33.3 percent of middle/high school teachers are experienced in teaching from 6 to 10 years on average while 43.9 percent of college professors show more than 16 years of experience. 2) Examining the status of existing number of children cf the respondents, one boy and one daughter pattern is predominant, showing 28.5 percent in middle/high school teachers and 21.1 percent in college professors. As for the desired number of children of unmarried respondents, it is observed that 43.8 percent of middle/high school teachers desire to have one boy and one girl, and 31.3 percent of college professors want to have one child regardless of the sex. By assessing the degree of awareness of the population education through their students, it is observed that 53 percent of middle/high school teachers and 50 percent of college professors are aware of population education in some extent and that a majority of respondents took the positive attitudes toward an inclusion of family planning components into the formal school education. Another noteworthy to observe is that a total of 84.8 percent out of middle/high school teachers pointed that the population education currently conducted at schools as a part of home economics are less sufficient than it should be. 3) Analyzing the tendency as to whether the respondents were experienced in receiving population education during the time when they were students, 75 percent of college professors and 59 percent of middle/high school teachers responded negative answers in the survey. In the mean time, a total of 50 percent of the respondents replied that they began to acknowledge the importance of population education mainly through the participation of some sort of population-education orientend seminars, experienced by 40 percent of college professors and 80 percent of middle/high school teachers. 4) What it calls attention in this study was to find that 96.5 percent of middle/high school teachers and 72 percent of college professors conduct population education to some extent during their lecture hours and that more than 80 percent of them are never experienced in teaching population and family planning contents in their regular classes. It is, on the other hand, found that no more than once was the response of those who believe themselves that they are experienced in teaching these relevant components to their students. Analyzing the contents of the subjects being taught in the class, a large percent of them are found to be consisted of population and family planning contents. According to this study, the current population education through the formal school is quite inactive. Analyzing the facts, 44.9 percent of the college professors responded that the population and family planning components are quite apart from their specialization which eventually generates lack of interest in the field. 5) It is also noticed through the study that the degree of frequency of commenting on population and family planning contents during the classes was depending significantly on their specializations which means that the degree of frequency varies from a major to another. Those who majored in home managerial science was the first one, as compared to others who majored in different specializations. Glancing over the status of correlations between ages of the respondents and numbers of seminar paticipation, it is quite clear that the aged group participated more than the younger group did, and that the most highest number of participations made by college professors were those who are in 50s. In addition, it is also found that those who are aged 20s and 60s of the respondents were the group who comments least on the contents of population and family planning at their classes. The suggestions and recommendation made through this survey research are as follows. 1) No one denies that the rapid increase of population, as compared to the limited size of land and resources, will certainly affect adversly to an enhancement of individual life quality which will, eventually, bring forth the poverty of the nation. This is the reasson why we are insisting that the world population be controlled up to an optimum level with a matter of global concerns. It is our understading that the primary aim for reducing number of population is believed to be attained only by conducting the systematic and comprehensive population education through the formal schools. Therefore, the role of home economists in the field of population/family planning education is considered very importment due to the fact that an ultimate goal of population education is placed in elevating the quality of family life by having optimum number of children through family planning program. 2) It is quite clear that home economists as teachers of formal school in all level are invited to pay their attention on redefining the ultimate goal of education and that of population education. We also understant that the primary objective of population education is to change the norm and value of the clients by replenishing the students with pertinent knowledge and attitudes on population and its related problems through a sort of education in order to attain the ultimate goal for enhancing the quality of life. There is no exception in the theory of home economics. An altimate goal of home economics is to elevate the general quality of life through an establishment of value existed in daily life. Considering the relations between population education and home economics, it is quite indespensable to bandle population components as an integral part in the field of home economics. We believe, therefore, that the senior home economists positive participation in the effort population control is more needed than it has been. 3) It is also strongly urged that population education should be a part of instructor training course for home economics. In other words, the teacher of home economics should be well aware of population and its problems by teaching interrelationship between population education and home economics, needs, contents and methods of population education during the instructor training courese for home economics. In addition, the senior home economists should be encouraged through positive participation on the short term training by types of domestic and international seminar, workshop, etc. 4) We certainly believe that the population education can not sustain itself without any backing-up of information and findings' of various and comprehensive researches of natural and social sciences. Accordingly, every senior home economist is invited to exert their maximum effort to conduct systematic study with an aim to utilize these findings and information at best in population education in the field of home economics. Therefore, we consider that the development of training material is imminent in order to provide effective and efficient population education through the for training of home economies. It should be noted that these training materials must be carefully designed, tailored and developed to meet the different classes of trainees under the considerations as to whether it is easily adaptable and infusable into the curricula of every field of home economics, and it is acceptable in the degree of difficulty and quality in its contents. 5) It is true that there are many domestic and international research rapers, reports and findings in the field of population education and family planning. However, there is a tendency that the most of research papers are heavily relying on the authors intension and preferences in its expression and publication. Under these circumstances, it is urged that the home economists should aware of the growing need of the technical training in order to keep these available information and research findings reprocessed and redesigned to insure the practical application into the population education in the field of home economics in Korea.

한국전쟁의 교훈과 대비 -병력수(兵力數) 및 부대수(部隊數)를 중심으로- (The lesson From Korean War)

  • Just before the Korean War, the total number of the North Korean troops was 198,380, while that of the ROK(Republic of Korea) army troops 105,752. That is, the total number of the ROK army troops at that time was 53.3% of the total number of the North Korean army. As of December 2008, the total number of the North Korean troops is estimated to be 1,190,000, while that of the ROK troops is 655,000, so the ROK army maintains 55.04% of the total number of the North Korean troops. If the ROK army continues to reduce its troops according to [Military Reform Plan 2020], the total number of its troops will be 517,000 m 2020. If North Korea maintains the current status(l,190,000 troops), the number of the ROK troops will be 43.4% of the North Korean army. In terms of units, just before the Korean War, the number of the ROK army divisions and regiments was 80% and 44.8% of North Korean army. As of December 2008, North Korea maintains 86 divisions and 69 regiments. Compared to the North Korean army, the ROK army maintains 46 Divisions (53.4% of North Korean army) and 15 regiments (21.3% of North Korean army). If the ROK army continue to reduce the military units according to [Military Reform Plan 2020], the number of ROK army divisions will be 28(13 Active Division, 4 Mobilization Divisions and 11 Local Reserve Divisions), while that of the North Korean army will be 86 in 2020. In that case, the number of divisions of the ROK army will be 32.5% of North Korean army. During the Korean war, North Korea suddenly invaded the Republic of Korea and occupied its capital 3 days after the war began. At that time, the ROK army maintained 80% of army divisions, compared to the North Korean army. The lesson to be learned from this is that, if the ROK army is forced to disperse its divisions because of the simultaneous invasion of North Korea and attack of guerrillas in home front areas, the Republic of Korea can be in a serious military danger, even though it maintains 80% of military divisions of North Korea. If the ROK army promotes the plans in [Military Reform Plan 2020], the number of military units of the ROK army will be 32.5% of that of the North Korean army. This ratio is 2.4 times lower than that of the time when the Korean war began, and in this case, 90% of total military power should be placed in the DMZ area. If 90% of military power is placed in the DMZ area, few troops will be left for the defense of home front. In addition, if the ROK army continues to reduce the troops, it can allow North Korea to have asymmetrical superiority in military force and it will eventually exert negative influence on the stability and peace of the Korean peninsular. On the other hand, it should be reminded that, during the Korean War, the Republic of Korea was attacked by North Korea, though it kept 53.3% of troops, compared to North Korea. It should also be reminded that, as of 2008, the ROK army is defending its territory with the troops 55.04% of North Korea. Moreover, the national defense is assisted by 25,120 troops of the US Forces in Korea. In case the total number of the ROK troops falls below 43.4% of the North Korean army, it may cause social unrest about the national security and may lead North Korea's misjudgement. Besides, according to Lanchester strategy, the party with weaker military power (60% compared to the party with stronger military power) has the 4.1% of winning possibility. Therefore, if we consider the fact that the total number of the ROK army troops is 55.04% of that of the North Korean army, the winning possibility of the ROK army is not higher than 4.1%. If the total number of ROK troops is reduced to 43.4% of that of North Korea, the winning possibility will be lower and the military operations will be in critically difficult situation. [Military Reform Plan 2020] rums at the reduction of troops and units of the ground forces under the policy of 'select few'. However, the problem is that the financial support to achieve this goal is not secured. Therefore, the promotion of [Military Reform Plan 2020] may cause the weakening of military defence power in 2020. Some advanced countries such as Japan, UK, Germany, and France have promoted the policy of 'select few'. However, what is to be noted is that the national security situation of those countries is much different from that of Korea. With the collapse of the Soviet Unions and European communist countries, the military threat of those European advanced countries has almost disappeared. In addition, the threats those advanced countries are facing are not wars in national level, but terrorism in international level. To cope with the threats like terrorism, large scaled army trops would not be necessary. So those advanced European countries can promote the policy of 'select few'. In line with this, those European countries put their focuses on the development of military sections that deal with non-military operations and protection from unspecified enemies. That is, those countries are promoting the policy of 'select few', because they found that the policy is suitable for their national security environment. Moreover, since they are pursuing common interest under the European Union(EU) and they can form an allied force under NATO, it is natural that they are pursing the 'select few' policy. At present, NATO maintains the larger number of troops(2,446,000) than Russia(l,027,000) to prepare for the potential threat of Russia. The situation of japan is also much different from that of Korea. As a country composed of islands, its prime military focus is put on the maritime defense. Accordingly, the development of ground force is given secondary focus. The japanese government promotes the policy to develop technology-concentrated small size navy and air-forces, instead of maintaining large-scaled ground force. In addition, because of the 'Peace Constitution' that was enacted just after the end of World War II, japan cannot maintain troops more than 240,000. With the limited number of troops (240,000), japan has no choice but to promote the policy of 'select few'. However, the situation of Korea is much different from the situations of those countries. The Republic of Korea is facing the threat of the North Korean Army that aims at keeping a large-scale military force. In addition, the countries surrounding Korea are also super powers containing strong military forces. Therefore, to cope with the actual threat of present and unspecified threat of future, the importance of maintaining a carefully calculated large-scale military force cannot be denied. Furthermore, when considering the fact that Korea is in a peninsular, the Republic of Korea must take it into consideration the tradition of continental countries' to maintain large-scale military powers. Since the Korean War, the ROK army has developed the technology-force combined military system, maintaining proper number of troops and units and pursuing 'select few' policy at the same time. This has been promoted with the consideration of military situation in the Koran peninsular and the cooperation of ROK-US combined forces. This kind of unique military system that cannot be found in other countries can be said to be an insightful one for the preparation for the actual threat of North Korea and the conflicts between continental countries and maritime countries. In addition, this kind of technology-force combined military system has enabled us to keep peace in Korea. Therefore, it would be desirable to maintain this technology-force combined military system until the reunification of the Korean peninsular. Furthermore, it is to be pointed out that blindly following the 'select few' policy of advanced countries is not a good option, because it is ignoring the military strategic situation of the Korean peninsular. If the Republic of Korea pursues the reduction of troops and units radically without consideration of the threat of North Korea and surrounding countries, it could be a significant strategic mistake. In addition, the ROK army should keep an eye on the fact the European advanced countries and Japan that are not facing direct military threats are spending more defense expenditures than Korea. If the ROK army reduces military power without proper alternatives, it would exert a negative effect on the stable economic development of Korea and peaceful reunification of the Korean peninsular. Therefore, the desirable option would be to focus on the development of quality of forces, maintaining proper size and number of troops and units under the technology-force combined military system. The tableau above shows that the advanced countries like the UK, Germany, Italy, and Austria spend more defense expenditure per person than the Republic of Korea, although they do not face actual military threats, and that they keep achieving better economic progress than the countries that spend less defense expenditure. Therefore, it would be necessary to adopt the merits of the defense systems of those advanced countries. As we have examined, it would be desirable to maintain the current size and number of troops and units, to promote 'select few' policy with increased defense expenditure, and to strengthen the technology-force combined military system. On the basis of firm national security, the Republic of Korea can develop efficient policies for reunification and prosperity, and jump into the status of advanced countries. Therefore, the plans to reduce troops and units in [Military Reform Plan 2020] should be reexamined. If it is difficult for the ROK army to maintain its size of 655,000 troops because of low birth rate, the plans to establish the prompt mobilization force or to adopt drafting system should be considered for the maintenance of proper number of troops and units. From now on, the Republic of Korean government should develop plans to keep peace as well as to prepare unexpected changes in the Korean peninsular. For the achievement of these missions, some options can be considered. The first one is to maintain the same size of military troops and units as North Korea. The second one is to maintain the same level of military power as North Korea in terms of military force index. The third one is to maintain the same level of military power as North Korea, with the combination of the prompt mobilization force and the troops in active service under the system of technology-force combined military system. At present, it would be not possible for the ROK army to maintain such a large-size military force as North Korea (1,190,000 troops and 86 units). So it would be rational to maintain almost the same level of military force as North Korea with the combination of the troops on the active list and the prompt mobilization forces. In other words, with the combination of the troops in active service (60%) and the prompt mobilization force (40%), the ROK army should develop the strategies to harmonize technology and forces. The Korean government should also be prepared for the strategic flexibility of USFK, the possibility of American policy change about the location of foreign army, radical unexpected changes in North Korea, the emergence of potential threat, surrounding countries' demand for Korean force for the maintenance of regional stability, and demand for international cooperation against terrorism. For this, it is necessary to develop new approaches toward the proper number and size of troops and units. For instance, to prepare for radical unexpected political or military changes in North Korea, the Republic of Korea should have plans to protect a large number of refugees, to control arms and people, to maintain social security, and to keep orders in North Korea. From the experiences of other countries, it is estimated that 115,000 to 230,000 troops, plus ten thousands of police are required to stabilize the North Korean society, in the case radical unexpected military or political change happens in North Korea. In addition, if the Republic of Korea should perform the release of hostages, control of mass destruction weapons, and suppress the internal wars in North Korea, it should send 460,000 troops to North Korea.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury. 소음 특수건강진단 자료를 이용한 순음청력검사 평가 (Evaluation of Puretone Threshold Using Periodic Health Examination Data on Noise-exposed Workers in Korea)

  • 이 연구는 특수건강진단기관의 소음성 난청 진단결과의 유소견자$(D_1)$와 요관찰자(C)를 하나의 평가 지표로 설정하여, 첫째 소음 특수건강진단 결과 소음성 난청의 실태 파악, 둘째 소음성 난청 요관찰 자의 청력장애 평가, 셋째 정력장애 정도 에 따른 각 주파수 영역별 기도순음청력 검사 결과를 통해 청력손실의 정도를 파악하고 이의 판정기준에 따른 진단의 적정성을 검토하고자 하였다. 1. 1994년 l월부터 12월까지의 73개 특수건강진단기관의 특수건강진단 실시 사업장은 27,347개이며 이중 소음 특수 건강진단 설시 사업장은 16,388개(59.9%)이었으며, 전체 특수건강진단 수진 근로자는 731,029명이며 이중 소음 특수 건강진단 수진 근로자는 343,457명 (47.0%)이었다. 소음성 난청 요관찰자는 38,058명, 소음성 난청 유소견자는 1,358 명으로 소음성 난청 요관찰률은 11.1%, 유소견율은 0.44%이었다. 지역에 따라 소음성 난청 요관찰률의 차이를 보여주며 판정기준의 적용에 따른 기도순음 청력평균손실치가 일부 적정하게 판단되지 못하였음을 보여 주었다. 2. ISO 기준의 3분법에 의한 청력 평가시 97%가 경도난청 이하였으며, 회화음 역에서의 4분법에 비해 거의 비슷하였으나 약간 정상역이 많았고, 고음역을 포함하여 평가하는 4분법과 6분법의 적용시 정상자의 경도난청으로의 가능성이 높다고 볼 수 있어 청력평가시 평가방법의 적용에 따라 내재적인 판별능의 차이를 보여준다고 볼 수 있다. 3. 우측귀의 청력역치를 ISO 기준에 의해 평가한 후 양귀의 청력역치의 분포 및 차이를 보면, 우측귀의 평균역치(표준편차)가 20.54(9.56) dB, 좌측귀의 평균역치가 20.54(9.57) dB로 좌측귀의 평균역치가 우측보다 높았다. 양귀의 청력이 75.4%에서 정상역이었으며, 21,562명 (90.6%)의 양귀 청력역치 차이의 범위가 10dB이내였다. 4. 소음성 난청 요관찰자의 회화음역에 속하는 500, 1,000 및 2,000 Hz에서의 기도청력역치를 산술평균으로 하여 구하는 3분법의 청력손실도(표준편차)를 주파수 별로 보면, 우측귀에서 500 Hz 21.08(10.23), 1,000 Hz 18.44(10.01), 2,000 Hz 22.09(13.46), 4,000 Hz 52.36(16.38) dB이었다. 평균청력손실도를 10 dB 간격으로 구분한 후 각각의 주파수별 청력역치를 살펴보면, 정상역인 20 dB미만에서 고음역인 4,000 Hz에서 회화음역인 500, 1,000 및 2,000 Hz에서 보다 평균 30-40 dB 이상의 역치를 보이는 $C_5-dip$ 현상을 특징적으로 보였다. 평균정력손질이 증가함에 따라 4,000 Hz에서의 역치 증가 현상이 점차적으로 감소하다 평균청력손실이 50 dB 이상에서는 10dB 내외의 차이만을 나타내었다. 이상과 같이 소음성 난청 요관찰자에 대한 분석에서 소음성 난청의 평가방법 에 따른 실태와 의미, 소음에 의한 조기청력손실의 특정과 소음성 난청의 판정기준에 따른 진단의 적정성을 확인할 수 있었으며, 소음성 난청 요관찰자에 대한 관리의 필요성을 제언할 수 있겠다.

세계 무인항공기 운용 관련 규제 분석과 시사점 - ICAO, 미국, 독일, 호주를 중심으로 - (Analysis and Implication on the International Regulations related to Unmanned Aircraft -with emphasis on ICAO, U.S.A., Germany, Australia-)

  • 무인항공기 규제 법률은 ICAO의 경우 1944년 '시카고협약'을 기준으로 'RPAS manual(2015)'에 상세하게 규정하고 있으며, 미국의 경우 '연방항공규칙 (14CFR), Public Law (112-95)', 독일의 경우 EASA의 Regulation (EC) No.216/2008을 기본으로 150kg 미만의 무인항공기의 경우 항공운송법, 항공운송명령, 항공운송허가명령 (무인항공기 운영규칙에 관한 법률에 의한 개정), 호주의 경우 '민간항공법 (CAA 1998), 민간항공규칙 101장 (CASR Part 101)'로 정하고 있다. 공통적으로 이러한 법률들이 규제하는 대상에 여가선용 목적의 모형항공기는 제외하고 있으며, 반드시 무인항공기를 통제할 수 있는 조종자를 두어야 하는데, 이때 조종자란 항공 기내가 아닌 지상에서의 조종과 통제를 하는 사람을 의미한다. 또한 무인항공시스템이라는 구조 하에서 조종자는 물론이고 무인항공기를 운용에 필요한 모든 관리 즉, 법률의 규정이 정하는 범위 안에서 안전하고 효율적으로 시스템을 운용하기 위한 모든 관리를 포함하는 것을 의미한다. 구체적 운용방식에 관하여는 각 나라는 25kg 이하의 항공기로 분류하여 규정하고, 호주와 독일은 그 이하의 중량에서 다시 세분화하여 규정하고 있다. ICAO는 시카고협약 제6부속서에 따라 상업적운용을 포함하여 일체의 일반항공 운용을 규정하고 있으며 RPAS 운용의 경우에도 적용된다. 다만, RPA를 이용한 여객운송은 제외하고 있다. RPA의 운용범위가 타국의 영공을 포함하는 경우 비행일 7일 이전에 해당 국가의 특별허가를 요건으로 하며, 이때 비행계획서를 함께 제출하여야 한다. 미국은 연방항공규칙 107장에 따라, 비레저용 소형무인기는 책임조종자 또는 관찰자의 시야 범위 내에서 (주간에만) 지표 또는 수면으로부터 122m(400피트)까지, 시속 161km (87노트) 이내로 운용 가능하다. 소형무인기는 다른 항공기에 경로를 양보해야 하고, 위험물질을 수송하거나 1인이 동시에 2대 이상의 무인기를 운용하는 것은 금지된다. 독일의 경우 무인항공기 운영규칙에 관한 법률에 따라 무인항공시스템과 무인모형항공기에 관한 규정(여가선용 용도 제외)은 공중충돌 방지의무와 더불어 지상의 안전 및 개인의 사생활 보호도 함께 고려되어 2017년 3월 제정되었다. 5kg 이하의 상업용 무인항공기는 종전의 규제규정을 완화하여 더 이상 허가를 요건으로 하지 않지만, 중량에 상관없이 모든 무인항공기는 지속적인 감시자와 조종자의 통제 범위 내에서 100m이하의 높이에서만 자유롭게 운용되어질 수 있다. 호주는 2001년 무인항공기를 규제한 첫 국가로 ICAO 및 FAA, EASA 등의 무인항공기 관련법제에 영향을 주었다. 2016년 개정을 통하여 저위험도로 고려되는 무인항공기의 운용에 대하여 활용성을 증대시키고자 '배제 무인항공기'라는 항목을 추가하여 규제조건을 완화시켰으며, 이에 해당하는 경우 상업적 목적이라 할지라도 특별한 허가 없이 운용할 수 있도록 하였다. 나아가 현재 규제의 유연성을 위하여 새로운 표준 매뉴얼에 대하여 논의 중이다.

  • 본 연구의 목적은 소아 수면무홉증 환자와 정상 소아의 수면 구조를 비교하고 잘 드러나지 않는 차이를 뇌파의 비선형적 분석을 통해 비교 분석하는데 있다. 코골이 증상을 호소하는 소아 중 수면다원검사를 통해 폐쇄성 수면무호흡 지수가 시간당 1회 이상으로 소아기 폐쇄성 수면무호흡증으로 진단받은 12명(남아 10명, 평균 6.4${\pm}$3.4세)과 정상 대조군으로 연구에 참여한 15명(남아 8명, 평균 6.0${\pm}$2.2세)의 수면다원검사 자료를 비교하였다. 즉, 수면 구조 관련 변인과 뇌파 채널(C3/A2, C4/A1, O1/A2, O2/A1)에서 탈경향변동분석 과정을 거쳐 계산된 축적 지수를 비교하였다. 수면 구조에서는 1단계 수면의 비율이 폐쇄성 수면무호흡증 환아에서 9.3${\pm}4.3$%로 정상 소아의 5.6${\pm}4.3$2.8%보다 유의하게 높았고, 렘수면 입면잠복시간에서 폐쇄성 수면무호흡증 환아가 181.5${\pm}4.3$59.9 분으로 정상 소아의 133.5${\pm}4.3$42.0분보다 유의하게 길었다. 2, 3, 4단계 수면과 렘수면의 비율은 폐쇄성 수면무호흡증 환아와 정상 소아에서 유의한 차이가 없었다. 소아의 수면 뇌파에서 계산된 축적 지수(${\alpha}$)는 장기 시간적 연관성(long-range temporal correlation)의 특징을 갖는 것으로 1단계에서 4단계 수면으로 갈수록 ${\alpha}$ 값이 증가 하였다. 각각의 뇌파 채널에서 폐쇄성 수면무호흡증 환아의 축적 지수는 정상 소아의 축적 지수보다 유의하게 낮았다(C3/A2: 1.36${\pm}4.3$0.05 vs. 1.41${\pm}4.3$0.04, C4/A1: 1.37${\pm}4.3$0.04 vs. 1.41${\pm}4.3$0.04, O1/A2: 1.37${\pm}4.3$0.05 vs. 1.41${\pm}4.3$0.05, O2/A1: 1.36${\pm}4.3$0.07 vs. 1.41${\pm}4.3$0.05, p<0.05). 폐쇄성 수면무호흡증 환아에서 낮은 ${\alpha}$ 값을 보인 것은 자기조직적 고비성(self-organized criticality)이 줄어든 것으로 뇌 체계에서 자체 기전에 의한 에너지 축적이 감소한 것으로 해석해 볼 수 있다. 8시간에 이르는 수면 뇌파의 정량화가 수면무호흡증과 같은 질병 상태를 반영한다면 향후 연구에서 그 유효성과 범용성을 여러 질병에서 확인해 보아야 할 것이다.

오메가 3계 지방산 강화 식품류의 연구개발 동향 (Research and Development Trends on Omega-3 Fatty Acid Fortified Foodstuffs)

  • 오메가 3계 지방산의 섭취가 동맥경화, 고혈압 및 뇌졸중 등의 심혈관질환을 비롯하여 당뇨병, 류마티즘 등의 성인병 예방과 치료에 효과가 있슴이 여러 가지 역학조사 결과 밝혀지면서 의 학계를 위시한 여러 생명과학 분야에서 관심을 모으는 연구주제가 되고 있다. $\omega$3계 지방산은 인체내에서 생성이 되지 않고 외부로부터 섭취해야 하는 필수 지방산으로서 이러한 필수성은 식이로부터 섭취 부족시 생겨나는 결핍증의 치료효과에 기인 한다기 보다는 합성과정에 필요한 적당한 desaturase가 존재하지 않거나 효소활성이 제한되어 있기 때문에 그 필수성이 강조되는 물질로서 이 분야에 대한 관심이 더 많이 요구된다. 또한 $\omega$3계 지방산이 필수지방산 이므로 심혈관질환의 치료 및 예방과 관련, 어느 정도 섭취 해야 하며 부족시 인체에 나쁜 영향을 막기에 충분한 양은 얼마인지에 대한 구체적 자료가 제시되어 있지는 않은 실정이라 이에 대한 연구도 활성화되어야겠다. $\omega$3계 지방산이 심혈관질환 등에 유익한 효과를 보여주는 기전은 혈액지방 분획에 바람직한 변화를 유도하기 때문으로 혈액 중성지 방, 초저밀도지단백-콜레스테롤, 초저밀도지단백-중성지방 등의 감소나 고밀도지단백-콜레스테롤의 상승을 초래하므로써 얻어 지는 효과로 보이며 이에 대한 결과가 항상 일관되게 만 보고되고 있지는 않다. 또한 $\omega$3계 지방산은 AA로부터 대사되는 혈소판 응집반응 촉진물질의 생성을 억제하며 항혈전생성물질의 합성을 도모하는 기능을 하게 되므로 $\omega$6/$\omega$3계 지방산의 적정한 섭취비율이 고려되어야만 한다. $\omega$3계 지방산의 식이급원으로는 LNA가 풍부한 식물성유나 장쇄 불포화지방산인 EPA, DHA 등의 급원으로 생선 및 어유가 있다. LNA는 EPA와 DHA로의 전환이 일어나나 사람에서는 전환율이 여러 가지 조건에 따라 다르며 제한적이다. 생선의 경우 비록 EPA, DHA가 타식품에 비해 풍부한 편이나 생선종류나 동일한 생선내에서도 부위별로 그 함량이 다르므로 $\omega$3계 지방산 급원으로 섭취 할 때에는 이점을 잘 고려해야 하겠다. $\omega$3계 지방산 강화식품의 연구 개발 동향은 $\omega$3계 지방산의 인체에 대한 유익한 효과에 대한 연구결과에 힘입어 이루어진 것으로 여러 가지 식품에 정제 혹은 농축된 $\omega$3계 지방산을 첨가하는 경우와 사료에 $\omega$3계 지방산 급원을 첨가, 동물에게 이행시켜 $\omega$3계 지방산 강화식품을 제조하는 두 가지 방법이 있을 수 있는데 전자는 통조림, 과자, 건강보조식품류에 다용되며 후자는 다양한 기능성 축산식품에 활용되고 있다. $\omega$3계 지방산 강화 축산식품에는 돼지고기, 우유, 치이즈, 계란, 분유, 햄 등 다양한 제품에 활용, 개발되고 있으며 최근에는 이러한 식품 개발에 더욱 박차가 가해지고 있다 그러나 이와 같은 기법을 이용해 생산, 시판된 제품의 현황조사에 따르면 식품별로 함량표시가 없는 제품이 있어 제품에 대한 소비자의 신뢰도 제고에 장애가 될 수 있으며 제품의 품질관리면에서도 이러한 미비점은 개선되어야 할 것으로 보인다. 한편 소비자 입장에서도 $\omega$3계 지방산이 강화된 식품의 개발로 다양하면서도 차별화된 식품을 선택, 소비할 수 있는 잇점이 있으나 강화식품이 아닌 $\omega$3계 지방산이 풍부한 천연식품을 일상의 식탁에서 섭취하는 대신 가공된 제품을 무차별적으로 소비하거나 첨가된 $\omega$3계 지방산 함량이 일반식품에 비해 그다지 높지 않은 경우 이들 식품의 소비가 더 이상 장점이 되지 못하므로 이들 식품 소비에만 의존하는 식생활은 지양되어야 하겠다.

혈소판유래성장인자-BB가 성견 치근이개부병변의 조직재생에 미치는 효과 (THE EFFECTS OF THE PLATELET-DERIVED GROWTH FACTOR-BB ON THE PERIODONTAL TISSUE REGENERATION OF THE FURCATION INVOLVEMENT OF DOGS)

  • 치주질환 진행의 최종적인 결과는 하반 지지조직의 소실로 치아상실을 초래하는 것이다. 이러한 치주질환의 이상적인 치유형태는 부착상실 예방을 비롯 상실된 조직의 재상 즉 신생 치조골과 백악질이 형성되고 두 조직사이에 치주인대가 재형성되는 것이다. 최근까지의 치주조직재생을 위한 처치법으로 이환된 병소부의 단순제거, 치관변위판막술, 약제의 치근면처리법, 조직유도재생술, 골전도물질 삽입, 골유도 혹 골형성물질 사용등 다양한 방법이 제시되었으나 아직 이상적인 치료법은 밝혀지지 않고 있다. 치아지지의 가장 중요한 역할을 하는 치조골의 재생에 대해 많은 연구들과 아울러 조직화학적 연구에서 Polypeptide Growth Factor(PGF) 가 다양한 종류의 세포증식과 이주 및 기질합성에 촉진효과가 있다고 하여 조직재생에 사용 가능성이 보고된 바 있었다. 이 PGF중 혈소판유래성장인자가 섬유아세포와 골아세포의 유사분열 및 단백질합성에 촉진작용이 있으며 조직재생에 영향을 미친다는 보고도 있었다. 본 연구의 목적은 총 8 마리의 실험동물을 이용하여 치근이개부병변을 형성한 후 혈소판유래성장인자를 처리하고 기존 조직결손부에 사용하였던 Tricalcium Phosphate(TCP) 와 콜라젠을 성장인자 함유매개체로 하여 이개부병변에 병용삽입한 경우 치유과정에 미치는 효과를 규명하여 임상적용의 가능성을 규명하고 실제 임상적용방법을 개발할 목적으로 실시하였다. 실험동물 Pentobarbital Sodium으로 전신마취를 시킨후, 초음파치석제거기등을 이용하여 구강위생을 청결하게 한 다음, 치은열구절개를 이용하여 전층판막을 형성하였다. 피질골과 치조골을 삭제하고 형성된 이개부 결손부에 치근면활택술을 시행하였으며, 구연산으로 치면처리 후, 계획된 재료를 삽입하고, 치관변위판막술과 유사한 형태의 봉합을 시행하였다. 실험동물을 2, 4, 8, 12 주에 관류고정과 아울러 회생시켜 악골을 채취하고 통법에 의해 후고정, 탈회, 탈수과정을 거쳐 파라핀으로 포매한 후 $7{\mu}m$두께로 절편하고 H & E 염색후 광학현미경으로 관찰하여 다음과 같은 결과를 얻었다. PDGF-BB 만 처리한 군에서는 2주 소견에서부터 결손부 전체에 걸쳐 활성도가 높은 조골세포들이 균일하게 분포하면서 이들로부터 생성된 골양조직이 기초적인 골소주의 형태를 이루고 있음이 관찰되었으며 그후 매우 빠른 골형성이 계속되어 8주 소견에서는 결손부 정상에 이르기까지 성숙된 치밀골이 채워져 있었다. 신생골형성의 전반적 형태는 치근면의 외형에 따라 치근이개부상단부로 형성되는 양상이었다. PDGF-BB 군에서 신생백악질의 형성은 2주소견 에서는 미약하였으나 4주이후 치근면에 수직으로 배열된 교원섬유들과 함께 균일한 두께로 형성되기 시작하여, 8주에 이르러서는 비 손상부위에서보다 더욱 두터운 신생백악질이 형성되었고 전형적인 샤피스씨 섬유가 완성되어 있음이 관찰되었다. PDGF-BB와 TCP를 병용한 경우 및 PDGF-BB와 콜라젠을 병용한 경우에서는 PDGF-BB군에 비해 신생골 및 신생백악질의 형성, 치주인대강의 발달이 상대적으로 미약하였으며 이러한 현상은 수술후 초기단계에서 더욱 두드러져 함유매개체로서의 기능을 기대하였던 이들 이식재들이 도리어 급속하게 분화 증식되는 세포들의 이동에 장애물로서 작용하는 것으로 나타났다. 결론적으로 PDGF-BB의 치근면 처리가 치근이 개부병변 치료에 있어 전반적으로 조직재생의 속도가 빠르고 그 치유양상도 시간경과에 따라 치주조직 고유형태로 진행됨이 관찰되어 동일 병소치료에 응용가능성을 확인하였다. 또한 차후 결손부에 주입방법과 성장인자의 관리법 및 적용량 그리고 적응중의 규명을 위해 연이은 연구가 있어야 할것으로 사료된다.
