농업용수 수요량 분석을 위한 잠재증발산량 공간 분포 추정

  • Weather station based PET(Potential Evapotrarspiration) analysis has often been inadequate to meet the needs of regional-scale irrigation planning. A map of continuous PET surface would be better a solution for the spatial interpolation considering spatial variations. Using a normal PET data collected at the 54 meteorological stations in Korea, 10-days spatial distribution PET map was created using universal Kriging(UK). These estimation methods were evaluated by both visual assessments of the output maps and the quantitative comparison of error measures that were obtained from the cross validation. The universal Kriging method showed appropriate results in spatial interpolation from weather station based PET to spatial PET with low statistical errors.

Ultimate outcomes of three modalities for non-surgical gallbladder drainage in acute cholecystitis with or without concomitant common bile duct stones

  • Backgrounds/Aims: In moderate and high-surgical risk patients with acute cholecystitis, studies comparing percutaneous cholecystostomy (PC) vs. endoscopic transpapillary gallbladder stenting (ETGS) vs. endoscopic ultrasound-guided transmural gallbladder stenting (EUGS) are limited. Thus, the aim of this study was to compare efficacy and recurrence of cholecystitis after PC, ETGS, or EUGS during follow-up. Methods: We reviewed 143 moderate and high-surgical risk patients with acute cholecystitis with or without concomitant common bile duct stones who underwent PC, ETGS, or EUGS at our hospital. Technical success rate (TSR), clinical success rate (CSR), and recurrence were compared. Results: TSR in PC or EUGS group was higher than that in the ETGS group for those with concomitant common bile duct stones (100% vs. 100% vs. 73.2%; p = 0.07) and for those without concomitant common bile duct stones (100% vs. 100% vs. 77.3%; p < 0.001). CSR in ETGS or EUGS group was higher than that in the PC group for those with concomitant common bile duct stones (96.2% vs. 100% vs. 87.5%; p = 0.41) and for those without concomitant common bile duct stones (94.1% vs. 100% vs. 63.0%; p = 0.006). Using Kaplan-Meier analysis, the overall recurrent risk was the highest in the PC group (p = 0.004). Conclusions: In moderate and high-surgical risk patients with acute cholecystitis, EUGS provides significantly higher CSR with comparable TSR to PC. Thus, ETGS should be the first choice in those with concomitant common bile duct stones. Among the three patient groups, those who received PC had the highest rate of recurrence.

Minimally Invasive Cardiac Surgery versus Conventional Median Sternotomy for Atrial Septal Defect Closure

  • Background: Median sternotomy is the standard approach for atrial septal defect (ASD) closure. However, minimally invasive cardiac surgery (MICS) has been introduced at many centers in adult/grown-up congenital heart patients. We retrospectively reviewed the results of right anterolateral thoracotomy compared with conventional median sternotomy (CMS) for ASD closure at Seoul National University Hospital. Methods: We retrospectively analyzed 60 adult patients who underwent isolated ASD closure from January 2004 to December 2013 (42 in the CMS group, 18 in the MICS group). Preoperative, operative, and postoperative data were collected and compared between the 2 groups. Results: The MICS group was younger (44.6 years vs. 32.4 years, p=0.002) and included more females (66.7% vs. 94.4%, p=0.025) than the CMS group. Operation time (188.4 minutes vs. 286.7 minutes, p<0.001), cardiopulmonary bypass time (72.7 minutes vs. 125.8 minutes, p<0.001), and aortic cross-clamp time (25.5 minutes vs. 45.6 minutes, p<0.001) were significantly longer in the MICS group. However, there were no significant differences in morbidity and mortality between groups. Only chest tube drainage in the first 24 hours (627.1 mL vs. 306.1 mL, p<0.001) exhibited a significant difference. Conclusion: MICS via right anterolateral thoracotomy is an alternative choice for ASD closure. The results demonstrated similar morbidity and mortality between groups, and favored MICS in chest tube drainage in the first 24 hours.

The Flow Characteristic Variation by Installing a Movable Weir having Water Drainage Equipment on the Bottom (저층수 배출식 가동보 설치에 따른 흐름특성)

  • Generally, water is taken through channels and rivers, in which there are many weirs and structures, which cross rivers and temporally hold up water. But this way has its own shortcomings. It is main reason that the water flows through structures, and backwater come into being. So it causes many water quality problems and some flood side-effects and so on. In this study, among the various movable weirs, we installed bottom-discharged and air pressure movable weir in the experimental channel. And we analyzed flowing influence, which is followed by the angle variation of movable weir. We also make further study the flow characteristic variation followed by installing entrance at the bottom to discharge the bottom water. The analysis result was that installed weir angle was increased, and the discharge also gradually increased. The installed weir angle depended on the water quantity, which can be excluded in the bottom. In case of velocity, there was increased as maximum 21.9 times, according to there is entrance or not at the bottom. And in case of water level, it showed the water level of locally above the average decrease in the upper river of weir.

Use of magnetic resonance imaging for evaluation of therapeutic response in breast cancer-related lymphedema: A systematic review

  • Breast cancer treatment-related lymphedema (BCRL) is a common comorbidity in breast cancer survivors. Although magnetic resonance imaging (MRI) is widely used to evaluate therapeutic response of patients with various medical conditions, it is not routinely used to evaluate lymphedema patients. We conducted a systematic review of the literature to identify studies on the use of MRI to evaluate therapy for BCRL. We hypothesized that MRI could provide information otherwise not possible through other examinations. On October 21, 2019, we conducted a systematic review on the PubMed/MEDLINE and Scopus databases, without time frame or language limitations, to identify studies on the use of MRI to evaluate therapy for BCRL. We excluded studies that investigated other applications of MRI, such as lymphedema diagnosis and surgical planning. Of 63 potential articles identified with the search, three case series fulfilled the eligibility criteria. In total, 53 patients with BCRL were included and quantitatively evaluated with MRI before and after manual lymphatic drainage. Authors used MRI or MR lymphagiography to investigate factors such as lymphatic vessel cross-sectional area, tissue water relaxation time (T2), and chemical exchange saturation transfer. The only study that compared MRI measurement with standard examinations reported that MRI added information to the therapy evaluation. MRI seems to be a promising tool for quantitative measurement of therapeutic response in patients with BCRL. However, the identified studies focused on only manual lymphatic drainage and were limited by the small numbers of patients. More studies are necessary to shed light on the topic.

Discussions on the Distribution and Genesis of Mountain Ranges in the Korean Peninsular (I) : The Identification Mountain Ranges using a DEM and Reconsideration of Current Issues on Mountain Range Maps (한국 산맥론(I) : DEM을 이용한 산맥의 확인과 현행 산맥도의 문제점 및 대안의 모색)

  • In recent years, there are some confusions related to the definition and existence of mountain ranges, which have been described in current geography text books. We contend that these confusions came from the lack of understanding on the geomorphological processes that form the mountain system in Korea. This research attempts to clarify the definition of mountain ranges and offer geological and geomorphological explanations about the formation of them. Based on the analyses of the social and cultural causes underlying the recent debates on the existence of mountain ranges, we tried to identify the relationships among the definition of mountain ranges, geological structure of Korea, and the forming processes of mountain ranges. The current and past mountain range maps were compared with geological structures, geological maps, surface curvature, and hill shade maps. The latter two maps were derived from a Digital Elevation Model of the Korean Peninsular. The results show that we are able to prove the existence of most mountain ranges, which provides a useful framework to understand the geological evolution of Korean peninsular and formation of mountainous landscape of Korea. In terms of their morphological continuity and genesis, however, we identified five different categories of mountain ranges: 1) Uplift mountain ranges(Hamkyeong Sanmaek, Nangrim Sanmaek, Taebaek Sanmaek), which were formed by the uplift processes of the Korean Peninsular during the Tertiary; 2) Falut mountain ranges(Macheonryeong Sanmaek, Sobaek Sanmaek, Buksubaek Sanmaek), whirh were directly related to the uplift processes of the Korean Peninsular during the Tertiary; 3) Trust mountain ranges(Jekyouryeong Sanmaek, Kwangju Sanmaek, Charyeong Sanmaek, Noryeong Sanmaek), which were formed by the intrusion of granite and consequent orogenic processes during the Mesozoic era; 4) Drainage divide type mountain ranges, which were formed by the erosion processes after the uplift of Korean Peninsular; 5) Cross-drainage basin type mountain ranges (Kangnam Sanmaek, Eunjin Sanmaek, Myelak sanmaek), which were also formed by the erosion processes, but the mountain ranges cross several drainage basins as connecting mountains laterally We believe that the current social confusions related to the existence of mountain ranges has partly been caused by the vague definition of mountain ranges and the diversity of the forming processes. In order to overcome theses confusions, it is necessary to characterize the types of them according the genesis, the purpose of usages and also the scale of maps which will explains the mountain systems. It is also necessary to provide appropriate educational materials to increase the general public's awareness and understanding of geomorphological processes.

Estimation or Threshold Runoff on Han River Watershed (한강유역 한강유출량 산정)

  • In this study, threshold runoff which is a hydrologic component of flash flood guidance(FFG) is estimated by using Manning's bankfull flow and Geomorphoclimatic Instantaneous Unit Hydrograph(GcIUH) methods on Han River watershed. Geographic Information System(GIS) and 3' Digital Elevation Model database have been used to prepare the basin parameters of a very fine drainage area($1.02\~56.41km^2$), stream length and stream slope for threshold runoff computation. Also, cross-sectional data of basin and stream channel are collected for a statistical analysis of regional regression relationships and then those are used to estimate the stream parameters. The estimated threshold runoff values are ranged from 2 mm/h to 14 mm/6hr on Han River headwater basin with the 1-hour duration values are$97\%$ up to 8mm and the 6-hour values are $98\%$ up to 14mm. The sensitivity analysis shows that threshold runoff is more variative to the stream channel cross-sectional factors such as a stream slope, top width and friction slope than the drainage area. In comparisons between the computed threshold runoffs on this study area and the three other regions in the United States, the computed results on Han River watershed are reasonable.

Characteristics of the Dalseong Acid Mine Drainage and the Role of Schwertmannite (달성폐광산 산성광산배수의 발달특징과 슈베르트마나이트의 역할)

  • The Dalseong acid mine drainage were studied focused on the characters of schwertmannite that controls geochemistry of the stream. Besides chemical analysis of stream water, particle size analysis, XRD SEM and TEM were performed on precipitates of streams and on wasted metalliferous ores. The AMD discharged from the abandoned mine reveals a decrease of pH and EC downward stream. Euhedral sulfur occurs as equigranular aggregates on the altered pyrite while fine acicula goethite coalesces to form cross, star, or starfish-like shapes. Water chemistry plotted on the Eh-pH diagram shows that schwertmannite and ferrihydrite are stable phases. Reddish brown precipitates consist of mostly schwertmannite with less goethite, whereas yellowish brown precipitates are composed of geothite with less schwertmannite. The particle size of precipitates ranges $d(0.1)\;0.861{\mu}m{\sim}3.769{\mu}m,\;d(0.5)\;3.984{\mu}m{\sim}15.255{\mu}m,\;and\;d(0.9)\;9.875{\mu}m{\sim}56.726{\mu}m$. Schwertmannite is characterized by equigranular spheric form. Pincushion or spicule with 100nm width and $200{\sim}300nm$length form on schwertmannite sphere with radial growth patterns. It is highly probable that reddish or yellowish brown precipitates formed in many AMDs may contain schwerhnannite. Because it can serve as sink for removing heavy elements by adsorption in AMD system, there is a need to correctly identify schwertmannite in precipitates and to characterize its phase stability.

A Study on Carrying Capacity for Floodplain (홍수(洪水)터의 통수능(通水能)에 관한 연구(研究))

  • The object of this study is the analysis of the factors which have significant effects in the floodplain and distribution pattern of carrying capacity in stream cross-section, which play important roles in the floodplain management. The conclusion are as follows, It has been found that the return period of bankfull discharge is about 1 to 2.5 years in Geum river system and the factors which have most important effects in the floodplain are the $Q_{100}$, the drainage area and the stream length. In Gong ju gauging station which has a recreation area, the carrying capacity is smaller than those of the other self-recording gauging station in Geum river basin. This shows that the complex cross-section of stream plays an important role in the distribution pattern of carrying capacity in floodplain.

Titanium Mesh Cage for Anterior Stabilization in Tuberculous Spondylitis : Is It Safe?

  • Objective : The safety of titanium metal cages in tuberculous spondylitis has not been investigated. We evaluated the outcome and complications of titanium mesh cages for reconstruction after thoracolumbar vertebrectomy in the tuberculous spondylitis. Methods : There were 17 patients with 18 operations on the tuberculous spondylitis in this study. Sixteen patients were operated with anterior corpectomy and reconstruction with titanium mesh cage followed by posterior transpedicular screw fixations on same day, two pateints were operated by either anterior or posterior approach only. After the affected vertebral body resection and pus drainage from the psoas muscle, titanium mesh cage, filled with morselized autogenous bone, was inserted. All the patients had antituberculosis medication for 18 months. The degree of kyphosis correction and the subsidence of cage were measured in the 15 patients available at a minimum of 2 years. Outcome was assessed with various cross-sectional outcome measures. Recurrent infection was identified by serial ESR[Erythrocyte Sedimentation Rate] and CRP[Cross Reactive Protein]. Results : There was no complication from the use of a titanium mesh cage. Recurrent infection was not detected in any case. Average preoperative of $9.2^{\circ}$ was reduced to $-2^{\circ}$ at immediate postoperative period, and on final follow up period kyphotic angle was measured to be $4.5^{\circ}$. Postoperatively, subsidence was detected in most patients especially at ambulation period, however further subsidence was prevented by the titanium mesh cage. Osseous union was identified in all cases at the final follow-up. Conclusion : The cylindrical mesh cage is a successful instrument in restoring and maintaining sagittal plane alignment without infection recurrence after vertebrectomy for tuberculous spondylitis.
