Disseminated Mycobacterium intracellulare Infection in an Immunocompetent Host

  • Disseminated Mycobacterium avium complex (MAC) infection can occur in immunocompromised patients, and rarely in immunocompetent subjects. Due to the extensive distribution of the disease, clinical presentation of disseminated MAC may mimic malignancies, and thorough examinations are required in order to make accurate diagnosis. We report a case of disseminated Mycobacterium intracellulare disease in an immunocompetent patient, which involved the lung, lymph nodes, spleen, and multiple bones. F-18 fluorodeoxyglucose positron-emission tomography imaging showed multiple hypermetabolic lesions, which are suggestive of typical hematogenous metastasis. However, there was no evidence of malignancy in serial biopsies, and M. intracellulare was repeatedly cultured from respiratory specimens and bones. Herein, we should know that disseminated infection can occur in the immunocompetent subjects, and it can mimic malignancies.

A Refolding Strategy for Recombinant Metalloprotease

  • The partial cDNA of the MT-c clone encoding snake venom metalloprotease was subcloned and expressed in E. coli. The expressed metalloprotease was purified by affinity chromatography in the presence of urea, and then successfully refolded into its functional form, retaining metalloprotease activity that hydrolyzes fibrinogen. The simple and convenient refolding strategy established in this work was highly efficient in recovering the recombinant enzyme activity. Experimental evidence suggests that the C-terminal amino acid stretch of 16 residues is a critical sequence for proper folding of the metalloprotease domain.

  • This paper presents a experimental evidence of the generation of the earth potential and communication line noise from the electric railway. There is a critical measurement err in case of measuring the electrical power induced noise voltage and degree of cable balance in the field of earth potential generated. As a results, it has been found that the conventional cable has more noise immunity than shielded cable near the railway where the earth current flows through the sheath layer.

  • Ko(1998) proposed a procedure to enhance the efficiency of double sampling plans by allowing second-stage sample size and critical region to depend on first-stage evidence using constraint optimization approaches. In this study further developments of such plans by incorporating several practically possible researcher's aims into the optimization are considered. Comparisons are made with the optimal ordinary double sampling plan and also among them It is observed that it is to some extent possible to match the details of the optimization to certain qualitative methodological aims.

  • This preliminary study held in two Indonesian state-owned enterprises (SOEs) showed empirical evidence that top management commitment was perceived to be the most important critical factor contributing to enterprise resource planning (ERP) implementation success, compared to top management support and top management involvement. Therefore, top management and middle management must continually show commitment during the ERP implementation process. This finding could serve as a reference for further study in a larger number of Indonesian SOEs.

  • Tetralogy of Fallot (TOF) is an index lesion for all paediatric and congenital heart surgeons. In designing an appropriate operation for children with TOF, the predicted postoperative physiology must be taken into account, both for the short and long term. A favourable balance between pulmonary stenosis (PS) and pulmonary insufficiency (PI) may be critical for preservation of biventricular function. A unified repair strategy to limit both residual PS and PI is presented, along with supportive experimental evidence. A strategy for dealing with coronary anomalies and some comments regarding best timing of operation are also included.

  • One of the areas of second language acquisition that enjoyed popularity in recent years is interlanguage pragmatics. The main reason for this popularity lies in the critical role of pragmatic competence in appropriate use of a target language. The aim of this paper was to examine L2 learners' pragmatic behavior in their speech act performance and determine main sources causing pragmatic difficulty. Four major sources of pragmatic errors were identified: linguistic proficiency, L1 transfer, waffling and teaching activities. Each source was discussed with empirical evidence in some detail, and teaching suggestions were provided for developing learners' pragmatic competence in EFL classrooms.

Effects of Burner Distance on Flame Characteristics at Low Strain Rate Counterflow Edge Flames (저 신장율 대향류 확산화염에서 화염 특성에 관한 버너 간격 효과)

  • Experimental study is conducted to identify the existence of a shrinking flame disk and to clarify its flame characteristics through the inspection of critical mole fraction at flame extinction and edge flame oscillation at low strain rate flames. Experiments are made as varying global strain rate, velocity ratio, and burner distance. The transition from a shrinking flame disk to a flame hole is verified through gradient measurements of maximum flame temperature. The evidence of edge flame oscillation in flame disk is also provided through numerical simulation in microgravity. It is found at low strain rate flame disks in normal gravity that buoyancy effects are importantly contributing to lateral heat loss to burner rim, and is proven through critical mole fraction at flame extinction, edge flame oscillation, and measurements of flame temperature gradient along flame disk surface.

Development of the Forensic Nursing Competency Scale for Nurses (간호사의 법의간호 역량 측정도구 개발)

  • Purpose : This study aims to develop the Forensic Nursing Competency Scale suitable to South Korea. Methods : The initial items were generated through literature review and interviews. Items were evaluated by experts for content and face validity. By conducting a pilot test, 74 measurement items were developed. In total, 333 copies of the questionnaire were analyzed regarding the validity and reliability of the study. Data were analyzed using SPSS 23.0 program and AMOS 18.0 program. Results : The study selected 53 questions after testing the content, face, and construct validity of the scale. The final 53 questions were composed of 15 sub-components in eight domains. These eight domains encompassed nursing practice, communication, critical thinking, human caring and relationships, evidence management, active information collection, professional development, and knowledge integration. Conclusions : In order to verify the effectiveness of the scale, future studies need to compare the forensic nursing competency in two groups to assess differences in these competency. Moreover, it is necessary to develop a forensic nursing educational program and validate its effectiveness by using this scale.

Development of a Standardized Education Protocol for Peritoneal Dialysis Patients (복막투석 환자를 위한 표준화된 간호교육 프로토콜의 개발)

  • Purpose: The purpose of this study was to develop a standardized education protocol for peritoneal dialysis patients. Methods: First, the demand for education was investigated from the 27 patients on peritoneal dialysis and 16 guardians, and later, the standardized nursing education protocol has been developed through the comprehensive literature review and the critical analysis of the related studies. Contents validity and applicability have been tested through the expert group survey. Results: The standardized peritoneal dialysis nursing education protocol with 9 areas and 20 items has been developed. The content validity for timing and duration, contents, method, and place of education proposed by the protocol were all above than 0.8. In terms of the applicability of the protocol, the experts scored between 4.20 to 4.93 for all items except one. The item on home visit education acquired the lowest applicability score, because there might exist potential restrictions in visiting patient's home. Conclusion: The standardized education protocol developed in this study turned out to be adequate by representing high content validity for all items and the applicability also was high except the item on home visit education. Further researches to test the effects of this education protocol should be done.
