Effects of Aqua-Acupuncture of Yukmijihwangtang and Palmijihwangtang Water Extracts on the Renal Function (육미지황탕(六味地黃湯)및 팔미지황탕(八味地黃湯)의 약침(藥鍼)이 신장기능(腎臟機能)에 미치는 영향(影響))

  • The aim of this experiment was to investigate the effects of Aqua-Acupuncture of Yukmijiwangtang(六味地黃湯) and Palmijihwang-tang(八味地黃湯) water extracts applied at the meridian points BL 23(賢兪) and GV 4(命門) to test the renal function in normal rats. The results obtained were as follows; 1. Among the effects of Aqua-Acupuncture of Yukmijihwangtang water extract at the merdian point BL 23 group, there were significant changes in water balance and urine volume over a 1 week period; Both decreasing and decreasing trends were exihibited. Urinary excretion of sodium and free water clearance changed significantly over a 2 week period; Both decreasing and increasing trends were exihibited, Urinary excretion of potassium, chloride and creatinine, plasma renin activity, plasma levels of aldosterone and atrial natriuretic peptide showed no significant differences compared to the contral group. 2. Among the effects of Aqua-Acupuncture of Palmljihwangtang water extract at the merdian point BL 23 group, there were significant changes in water balance and free water clearance over a 2 week period; a decreasing trend. Urinary excretion of creatinine changed significantly over a 2 week period; an increasing trend. Urinary excretion of chloride changed significantly over 1 week period; an increasing trend. The plasma levels of atrial natriuretic peptide changed significantly over 1 and 2 week period; an increasing trend. Urine volume, urinary excretion of sodium and pottasium, plasma renin activity and plasma level of aldosterone showed no significant differences compared to the control group. 3. Among the effects of Aqua-Acupuncture of Yukmijihwangtang water extract at the meridian point GV 4 group, there was significant decrease in water balance over a 2 week period; there was significant decrease in urine volwne and urinary excretion of sodium and creatinine over a 1 week period, followed by an increasing trend after 2 weeks. Urinary excretion of free water clearance demonstrated significant changes over both 1 and 2 week period; both increasing and decreasing trends were exihibited. Urinary excretion of chloride and plasma levels of aldosterone increased significantly over 1 and 2 week period. Plasma levels of atrial natriuretic peptide also decreased significantly. Plasma renin activity showed no significant differences compared to the control group. 4. Among the effects of Aqua-Acupuncture of Palmijihwangtang water extract at the meridian point GV 4 group, water balance and urinary excretion of chloride, plasma levels of aldosterone decreased significantly over both 1 and 2 week period. Urine volume and urinary excretion of pottasium decreased significantly. Urinary excretion of creatinine and urinary excretion of sodium changed significantly over both 1 and 2 week period. Urinary excretion of free water clearance, plasma renin activity and plasma level of atrial natriuretic peptide showed no significant differences compared to the control group. Seeing these results, I come to know that the effects Aqua-Acupuncture of Yukmijihwangtang and Palmijihwangtang water extracts at the meridian point BL 23 and GV 4 have affected the renal function differently. Seeing the results that BL 23 is a meridian point for Aqua-Acupuncture directly related to the kidney, I think, we can use Aqua-Acupuncture of Yuk-mijihwangtang and Palmijihwangtang water extracts to prevent and to treat the diseases related to kidney.

Effects of Astragali Radix (AR) on Chronic Renal Injury in Rats (황기가 흰쥐의 만성 신손상에 미치는 영향)

  • Objective : This study was designed to investigate the possibility of AR for chronic renal injury. Methods : The author first investigated the expression levels of DNA by inducing of chronic renal injury. Then, the author investigated the effects of AR on chronic renal injury induced by combination treatment with Adriamycin and cisplatin in terms of changes in body weights and renal tissues, urine volume, BUN and creatinine levels, creatinine clearance and histopathological changes in renal tissues. Total expression levels of 546 genes were elevated or lowered by induction of chronic renal injury. Genes of which whose expression levels were elevated by induction of chronic renal injury were related to the PPAR signaling pathway and fatty acid mechanism, etc. Genes of which expression levels were lowered by induction of chronic renal injury were related to the neuroactive ligand-receptor signaling pathway. Results : Oral administration of AR restored renal mass which was reduced by induction of chronic renal injury. AR also restored creatinine clearance and lowered serum BUN level. In histopathological observation, the AR group has a tendency to prevent tissue damages as shown in the chronic renal injury group. Conclusions : AR can be used to treat patients with chronic renal injury although further study will be needed to elucidate the exact mechanisms in the efficacy of AR on chronic renal injury.

Pharmacokinetics of Furosemide in Rabbits with Renal Failure (신장장해 가토에서 Furosemide의 약물동태)

  • The Pharmacokinetics of furosemide (5 mg/kg iv) was investigated in rabbits with folate (75 mg/kg, 150 mg/kg 300 mg/ug, iv) induced renal failure. The plasma concentration was increased and urinary excretion was decreased significantly compared with those of normal rabbits. ${\alpha},\;{\beta}\;and\;K_{12},\;K_{21},\;K_{10}$ were decreased, $t_{1/2}$ and AUC were increased significantly. Correlation of serum creatinine concentration and AUC, renal clearance have linear relationship respectively. In short, dosage regimen of furosemide is considered to be adjusted in the dose size and the dosing interval by degree of serum creatinine concentration.

  • The radioisotopic measurement of glomerular filtration rate (GFR) has required analysis of serial blood or urine samples over several hours, and does not allow measurement of separate renal function unless separate catherterization of individual ureters is done. Gates described isotopic method for the measurement of global and unilateral GFR based on the determination by scintillation camera of the fraction of the injected dose of $^{99m}Tc-diethylenetriaminepentaacetic$ acid (DTPA) present in the kidneys 2-3 minutes after its administration. We calculated GFR according to Gates' method in 58 adult patients with various levels of global renal function using $^{99m}Tc$ DTPA and validated this technique by correlation with 24 hour creatinine clearance. A good correlation was observed between 24 hour creatinine clearance and GFR calculated by Gates' formula, with an r value of 0.91 (p<0.01). We concluded that determination of GFR according to the Gates' formula allows good and reproducible prediction of GFR with great rapidity and simplicity rendering this technique suitable for clinical practice.

  • The study was performed to investigate the effects of administration of aluminum compound in Kidney metabolism and plasma hormone of rats. Seventy frve male Sprague-Dawley strains rats were divided into five groups consisting of the control, 250ppm AlCl$_3$group, 500ppm AlCl$_3$ group, 250ppm $Al_2$(SO$_4$)$_3$group, 500ppm $Al_2$(SO$_4$)$_3$group and kept on the diet for 2 weeks. The body weight gain was increased by the administration of AlCl$_3$ but decreased by the administration of $Al_2$(SO$_4$)$_3$as compared to the control. The urinary excretion of sodium and creatinine were increased and free water clearance and urine volume were decreased significantly after AlCl$_3$adminstration group as compared to the control. The water balance, free water clearance, excretion of sodium and creatinine were increased and the excretion of chlorine was decreased after $Al_2$(SO$_4$)$_3$ administration as compared to the control. Plasma renin activity was increased and plasma aldosterone content was compared to the control. Plasma renin activity was increased and plasma aldosterone content was significantly decreased after adminstration of aluminum compounds as compared to the control.

Effect of Renal Failure on Pharmacokinetics of Atenolol in Rabbits (아테놀올의 체내동태에 대한 신장해의 영향)

  • The pharmacokinetics of atenolol (25 mg/kg, i.v.) in the folate-induced renal failure rabbits was studied. Renal failure was induced by the i.v. injection of folate (50, 100, and 200 mg/kg). At folate dose of 100 and 200 mg/kg, the serum creatinine concentration (Scr) and blood urea nitrogen (BUN) increased significantly compared with control rabbits. Plasma concentrations and AUC of atenolol increased significantly at folate dose of 100 and 200 mg/kg. The elimination rate constant $(K_{el})$ and total body clearance $(CL_t)$ of atenolol decreased significantly, and half-life ($t_{1/2}$) and mean residence time (MRT) of atenolol increased significantly at folate dose of 100 and 200 mg/kg. The serum creatinine concentration $(S_{cr})$ correlated well (p<0.05) with half-life $(t_{1/2})$ and elimination rate constant $(K_{el})$ of atenolol, as well as BUN with AUC and total body clearance $(CL_t)$ of atenolol.

Assessment of 8-isoprostane (8-isoPGF2α) in Urine of Non-Small Cell Lung Cancer (NSCLC) Patients Undergoing Chemotherapy

  • 8-isoprostane (8-$isoPGF_{2{\alpha}}$) is a reliable marker and considered a gold standard for lipid peroxidation. There are very few reports of 8-isoprostane levels in cancer patients, and in patients undergoing chemotherapy. Oxidative stress is however expected and has been observed in patients with cancer. This study measured 8-isoprostane levels in urine by ELISA of 25 patients undergoing chemotherapy for advanced non-small cell lung cancer, at cycles 1, 2, and 3 of treatment. It considers the creatinine clearance of the patients, and correction of 8-isoprostane levels by creatinine clearance, and overnight urine volume methods. The average 8-isoprostane levels in urine increased more than 6 to 12 fold on chemotherapy treatment, from $532{\pm}587$ pg/mL at cycle $1,6181{\pm}4334$ at cycle 2, and $5511{\pm}2055$ at cycle 3. Similar results were obtained if 8-isoprostane levels were corrected for overnight urine volume, giving averages of $285{\pm}244{\mu}g$ at cycle $1,4122{\pm}3349$ at cycle 2, and $3266{\pm}1200$ at cycle 3. No significant difference was seen in average total overnight urine volume or number of urinations between chemotherapy cycles except for a large variation in urine volume between cycle 2 and 3. Creatinine levels were significantly different only between cycles 1 and 2 (p=0.016). In conclusion, cisplatin therapy has been shown to induce high levels of lipid peroxidation in lung cancer patients and can be assessed from the 8-isoprostane marker in overnight urine, with or without urine volume correction.

Population Pharmacokinetics for Gentamicin in American and Korean-American Appendicitis Patients Using Nonparametric Expected Maximum(NPEM) Algorithm (비모수적 기대최대치(NPEM)연산방법에 의한 미국인과 재미동포 충수돌기염 환자에게 겐타마이신의 모집단 약물동태학)

  • Population pharmacokinetics for gentamicin were compared with 24 American patients (16 male and 8 female) and 16 Korean-American appendicitis patients(12 male and 4 female). Two to six blood specimens were collected from all patients at the following times: just before a regularly scheduled infusion and at 1/2 hour after the end of a 1/2 hour infusion. Nonparametric expected maximum(NPEM) algorithm for population modeling was used. The estimated parameters were the elimination rate constant(K), the slope of the relationship between K versus creatinine clearance(KS), the apparent volume of distribution(V), the slope of the relationship between V versus weight(VS), gentamicin clearance(CL) and the slope of the relationship between CL versus creatinine clearance and the VS(CS). The output includes a 3-dimensional plot of the joint probability density function(PDF), two marginal PDF, means, medians, modes, variance, skewness, kurtosis, and CV%. The mean K(KS) were 0.424$\pm$0.139(0.00429$\pm$0.00164) and 0.411$\pm$0.135 hr$^{-1}$ (0.00475$\pm$0.00180[hr.mL/min/1.73m$^{2}]^{-1}$) for American and Korean-American populations, respectively. The mean V(VS) were not different at 15.6$\pm$4.77(0.233$\pm$0.0526) and 15.1$\pm$3.84L(0.239$\pm$0.0492 L/kg) for American and Korean-American populations, respectively (P>0.2). The mean CL (CS) were 6.28$\pm$1.85(0.0634$\pm$0.0191) and 5.70$\pm$1.77 L/hr(0.0701$\pm$0.0215 L/kg[hr.mL/min/1.73m$^{2}$)] for American and Korean-American populations, respectively. There are no differences in gentamicin pharmacokinetics between American and Korean-American Appendicitis patients.

Effect of low-dose Aprotinin on Postoperative Bleeding and Renal Function after Cardiopulmonary Bypass (체외순환후 출혈감소와 신기능에 미치는 저용량 aprotinin효과)

  • High-dose aprotinin(Hammersmith regimen) has been widely used for years to control postoperative bleeding and reduce blood consumption in cardiac surgery but had known to cause some side-effects and had disadvantage in cost-effectiveness. The prospective controlled study of 33 patients undergoing cardiopulmonary bypass was performed to evaluate the efficacy for reducing postoperative bleeding and unfavorable effects of low-dose aprotinin. The level of hemoglobin and platelet in the blood and the amount of postoperative bleeding were assessed preoperatively, and postoperatively for the study of hemostatic function. The level of BUN and serum creatinine in the blood, levels of urine creatinine, total protein, albumin, alpha-1-microglobulin and creatinine clearance were assessed before and after the operation for the study of renal function. The aprotinin group had a significant reduction in chest tube drainage; 243$\pm$ 123 ml versus 406$\pm$303 ml(P=0.037) during 6 hours immediate-postoperatively, 494$\pm$358 ml versus 869$\pm$570 ml(P=0.045) during 24 hours postoperatively. The ratio of alpha-1-microglobulin/creatinine and microalbumin/creatinine in the urine were slightly increased in the aprotinin group postoperatively in comparison with the control group but there were no statistically significant difference(55$\pm$23 versus 24$\pm$10 in the alpha-1-microglobulin/creatinine, 56$\pm$19 versus 38$\pm$25 in the microalbumin/creatinine at post- operative 3rd day). There were no significant difference between two groups in other parameters of renal function, too. This study showed that low-dose aprotinin is an effective means of reducing postoperative bleeding without inducing significant renal dysfunction.

Effect of Boron Supplementation on Ca and Bone Metabolism in Rats during Growth (성장기 흰쥐에서 붕소의 보충이 체내 칼슘 및 골격 대사에 미치는 영향)

  • It has been reported that boron may be beneficial for optimal calcium metabolism and, thus, optimal bone metabolism. Therefore, we designed a study to determine the effect of boron supplementation on Ca and bone metabolism in rats. The rats of 80-l40g body weight were given a control(0ug), 5$\mu\textrm{g}$, 10$\mu\textrm{g}$, 20$\mu\textrm{g}$, 40$\mu\textrm{g}$, or 80$\mu\textrm{g}$ boron supplement per Is diet for 4-weeks. The results are summarized as follows. There were no differences in total food intake and weight gain among the experimental groups. fecal Ca excretion, urinary Ca excretion, apparent Ca absorption, Ca retention, serum alkaline phosphatase activity, and urinary hydroxyproline were not affected by boron supplementation. There was no difference in serum creatinine. Whereas, urinary creatinine excretion was increased with increasing boron supplementation, and conse-quently creatinine clearance was increased with boron supplementation. No differences were found in length, weight, density, Ca content of femur and scapular. The findings suggest that boron supplementation was not effective in Ca and bone metabolism in growing rats fed normal Ca diet. (Korean J Nutrition 31(6) : 1039-1048, 1998)
