A 12-year-old intact female Pomeranian dog was presented with vulvar discharge, lethargy, polyuria, and polydipsia. A complete blood count revealed leukocytosis and anemia. The serum biochemical analyses showed elevated serum alkaline phosphatase activity and blood urea nitrogen. Survey abdominal radiography revealed an enlarged left kidney. Abdominal ultrasonography identified multiple rounded hypoechoic subcapsular lesions in the left kidney. The bilateral uterine horn was enlarged, and contained echogenic fluid. Percutaneous ultrasonographic-guided aspiration of the renal subcapsular lesions was performed on the left kidney. Cytological examination revealed bacterial cocci, degenerated neutrophils, and suppurative inflammation. Bacterial culture produced growth of Escherchia coli. Left nephrectomy and ovariohysterectomy were performed. Dilated uterine fluid was also evaluated for bacterial culture, and showed growth of E. coli. Urine was examined by bacterial culture, and showed negative results. The treatment consisted of antibiotic therapy according to the antibiotic sensitivity test. Based on these findings, the diagnosis was renal subcapsular abscess associated with pyometra. This study suggests that pyometra should be considered as the cause of renal abscess.
Letters of Credit are the most common method of payment for goods in the export trade, and banking practice relating to letters of credit is standardised by the Uniform Customs and Practice for Documentary Credits, which are a set of rules issued by the International Chamber of Commerce. The current version is UCP 600, which took effect on July 1, 2007. To assist the practitioners of Documentary Credits, the KCCI(Koea Chamber of Commerce and Industry) authorised a new publication-Official Commentary on UCP 600. This new publication added the word "official" in the title. In order to being an official commentary, the terms and expression should be correct and unified. But there is some problems in official commentary and legal phraseology. The problem was appeared that ICC UCP 600's were translated into Korean UCP 600 version. For example, "Issuing Bank", "Applicant", "Port of discharge", "A date of pick-up", "Shipper's load and count", "Courier Receipt", "Charter Party", "Bill of Lading", "Cover Note", "Exclusion Clause", "Insurance Certificate", "Declaration, Underwriter". If can be used 'Official Commentary on UCP 600', the above statements should be a compliment though take a wide professional opinions or held a public hearings. The Purpose of this paper is to point out the problems and substitute the term used and unify the expression in official commentary.

The chlorophyll-a concentration in algae bloom warning system of Hangang basin did not predict the cyanobacteria dominance. In this study, suggest the predicting indicator of cyanobacteria dominance through analyzing the environmental factors affecting on the cell count of cyanobacteria. Firstly, the dominance of algae was analyzed with seasonal variation during Jan. 2012~Sep. 2013. The diatom dominated phytoplankton communities during the period of January~April. In the May~June, the green algae dominated. And, the dominance of algae was changed to cyanobacteria in the July~August. Also, the environmental factors affecting to cyanobacteria blooms ; nutrients (TN, TP), temperature, precipitation, dam-discharge were evaluated during the study period. Rather than temperature factor, relatively low dam discharge causes cyanobacteria to grow rapidly and create a blooms. The low dam-discharge may increase the water retention time. Finally, it is proved that a low ratio of TN to TP (<29:1) can favour the development of cyanobacteria blooms. Thus, the predicting indicator (TN:TP) have need to apply to the alarm bloom warning system of Hangang basin.

Due to abnormal climate phenomena and climate change in Korea, overgrowth of algae in rivers and reservoirs occurs frequently. Algae in rivers are classified into green algae, blue-green algae, diatom, and other types, and some species of blue-green algae cause problems due to odor and the discharge of toxic substances. In Korea, an algae alert system is in place, and it is issued based on the number of harmful blue-green algae cells. Thus, measuring harmful blue-green algal blooms is very important, and currently, the analysis method of algae involves taking field samples and determining the cell count of green algae, blue-green algae, and diatoms through algal microscopy, which takes a lot of time. Recently, the analysis of algae concentration through Phycocyanin, an alternative indicator for the number of harmful algae cells, has been conducted through remote sensing. However, research on the analysis of the number of blue-green algae cells is currently insufficient. In this study, we water samples for algal analyses were collected from river and counted the number of blue-green algae cells using algae microscopy. We also obtained the Phycocyanin concentration using an optical sensor and acquired algae spectra through a hyperspectral sensor. Based on this, we calculated the equation for estimating blue-green algae cell counts and estimated the number of blue-green algae cells.
Introduction: Febrile neutropenia is a relatively frequent event in cancer patients treated with chemotherapy and improvement in absolute neutrophil count (ANC) has been linked directly to improved outcome. Evaluation of granulocyte colony stimulating factors (GCSFs) for treatment has shown reduced incidences of episodes of prolonged neutropenia and protracted hospitalization. To determine absolute neutrophil counts with GCSF in febrile neutropenic cancer patients admitted to a tertiary care centre and to co-relate the improvement in ANC with mortality and hospital discharge. Methods: A prospective cross sectional study was carried at an oncology ward at Aga Khan University hospital from January 2010 to June 2011. All adult patients who were admitted and treated with GCSF for chemotherapy induced febrile neutropenia were included. Multivariable regression was conducted to identify the factors related with poor outcomes. Results: A total of 131 patients with febrile neutropenia were identified with mean age of 43.2 (18-85) years, 79 (60%) being ${\leq}50$. Seventy-five (57%) had solid tumors and 56 (43%) hematological malignancies, including lymphoma. Fifty seven (43.5%) had an ANC less 100 cells/$mm^3$, 34 (26%) one between 100-300 cells/$mm^3$ and 40 (31%) an ANC greater than 300 cells/$mm^3$. Thirty (23%) patients showed ANC recovery in 1-3 days, and 74(56%) within 4-7 days. Thirteen (10%) patients showed no recovery. The overall mortality was 18 (13.7%) patients. The mean time for ANC recovery seen in hematological malignancies was 6.34 days whereas for solid tumors it was 4.88 days. Patients with ANC <100 cells/$mm^3$ were more likely to die than patients with ANC >300 cells/$mm^3$ by a factor of 4.3. Similarly patients >50 years of age were 2.7 times more likely to die than younger patients. Conclusion: Our study demonstrated that use of GCSF, in addition to intravenous antibiotics, in treatment of patients with chemotherapy induced febrile neutropenia accelerates neutrophil recovery, and shortens antibiotic therapy and hospitalization. We propose to risk classify the patients at the time of admission to evaluate the cost effectiveness of this approach in a resource constrained setup.

Research on condition monitoring and diagnosis of power facilities has been conducted to improve the safety and reliability of electric power supply. Although insulation diagnostic techniques for unit equipment such as gas-insulated switchgears and transformers have been developed rapidly, studies on monitoring of cables have only included aspects such as whether defects exist and partial discharge (PD) detection; other characteristics and features have not been discussed. Therefore, this paper dealt with PD characteristics against void sizes and positions, and with defect localization in XLPE cable. Four types of defects with different sizes and positions were simulated and PD pulses were detected using a high frequency current transformer (HFCT) with a frequency range of 150kHz~30MHz. The results showed that the apparent charge increased when the defect was adjacent to the conductor; the pulse count in the negative half of the applied voltage was about 20% higher than that in the positive half. In addition, the defect location was calculated by time-domain reflectometry (TDR) method, it was revealed that the defect could be localized with an error of less than1m in a 50m cable.

Objectives: With the aging of the population, the incidence of a variety of diseases is increasing. Cerebrovascular disease is a major cause of mortality worldwide and therefore requires timely treatment and management. We aimed to evaluate the clinical effectiveness of herbal medicines in treating cerebrovascular disease by screening the most frequently prescribed herbal medicines and analyzing the clinical data of patients with cerebrovascular disease taking these medicines. Methods: In this retrospective study, we analyzed the electronic medical records of patients with cerebrovascular disease who received herbal medication at the Pusan National University Korean Medical Hospital from 10 March 2010 to 31 March 2017. The inclusion criteria were as follows: diagnosis of cerebrovascular disease, use of frequently prescribed herbal medicines, available records of laboratory tests and functional evaluation that was conducted within two weeks before and after admission and discharge, and herbal medicine usage exceeding half the period of hospitalization. Results: Of the total patients with cerebrovascular disease, the number of patients analyzed in this study was 49. There were some changes in the various variables examined after frequently used herbal medicines had been taken, such as complete blood count, liver function test, lipid test, and functional evaluation. No serious adverse events related to herbal medicine were found in this study. Conclusions: We concluded that herbal medicines frequently used for cerebrovascular disease may have some clinical effectiveness. However, because of the retrospective nature of this study and only one hospital involved, the generalizability of our findings is limited.

Severe chronic neutropenia is classified as severe congenital, cyclic, autoimmune, or idiopathic. However, there is a lot of uncertainty regarding the diagnosis of severe congenital neutropenia (SCN) and chronic idiopathic neutropenia, and this uncertainty affects further evaluations and treatments. A 20-year-old man presented with fever and knee abrasions after a bicycle accident. On admission, his initial absolute neutrophil count (ANC) was 30/µL. He had no medical history of persistent severe neutropenia with periodic oscillation of ANC. Although his fever resolved after appropriate antibiotic therapy, ANC remained at 80/µL. Bone marrow (BM) aspiration and biopsy were performed, and a BM smear showed myeloid maturation arrest. Moreover, genetic mutation test results showed a heterozygous missense variant in exon 4 of the neutrophil elastase ELANE: c597+1G>C (pV190-F199del). The patient was diagnosed with SCN. After discharge, we routinely checked his ANC level and monitored any signs of infection with minimum use of granulocyte colony-stimulating factor (G-CSF), considering its potential risk of leukemic transformation. Considering that SCN can be fatal, timely diagnosis and appropriate management with G-CSF are essential. We report the case of a patient with SCN caused by ELANE mutation who had atypical clinical manifestations. For a more accurate diagnosis and treatment of severe chronic neutropenia, further studies are needed to elucidate the various clinical features of ELANE.
Canine pyometra generally occurs in intact female dogs of more than four years of age. However, in rarely cases, pyometra can occur in young bitches. In the present report, two cases of pyometra in thirteen and eighteen months of age were presented. In both cases, absent of vaginal discharge represented that both patients had closed-cervix pyometra and diagnosis was somewhat complicated. In complete blood count, elevated level of leukocyte with left shift was found in both cases. Serum chemistry analysis showed elevation of alkaline phosphatase in both cases. Additionally, mild elevation of aspartate transferase and total protein were also found in $2^{nd}$ case. Radiographic and ultrasonographic findings show the enlargement of uterus and accumulation of fluid contents inside the uterine lumen. Ovariohysterectomy followed by antibiotics and anti-inflammatory drugs for post-surgery medication was performed for treatment. After a week, both patients fully recovered. To reduce mortality of pyometra in young intact female dogs, radiography or ultrasonography should be indicated immediately in the bitches showing severe infectious inflammation.
Purpose: Electrical burn of scalp is uncommon. Much more, chronically exposed dura in unstable burn scar is quite exceptional. Hence, we report a case of chronically exposed dura following electrical burn. Methods: A 63-year-old man presented with an about 40 years history of an ulcerative lesion arising from electrical burn scar with 'squeeze like sensation' around wound. Wound was about $6{\times}8$ cm. Area in the center was $3{\times}3$ cm nonviable dura without sequestrum. Tangential excision with an intraoperative neurosurgical consultation and transposition flap under general anesthesia was done. Intraoperative biopsy was done. The wound was diagnosed as chronic osteomyelitis, not Marjolin ulcer. Flap was taken successfully. But after 5 days, infectious discharge had been appeared during 2 weeks, despite irrigation and drainage. As flap was re-evaluated, we could see remnant necrotic dura. After that, latissimus dorsi muscle free flap with meshed split thickness skin graft was transferred without excision of necrotic dura. Results: Flap was taken successfully. Follow-up at 10 weeks has been uneventful, with good and stable coverage of the wound. Conclusion: It is true that complete excision of devitalized tissue with sagittal sinus obliteration is prerequisite to flap taken. But necrotic dura was tangentially excised instead of total dura excision, because, posterior two-thirds of the sagittal sinus was involved underneath. Muscle is rich in blood vessels and decrease the recipient-site bacterial count effectively. In this case, muscle flap with skin graft without total dura excision is an alternative treatment.
