건강한 남녀의 관상동맥질환 위험 예측요인 (Predictors of Coronary Heart Disease Risk in Healthy Men and Women)

  • Objective: The purpose of this study was to identify predictors of coronary heart disease risk factors in healthy men and women. Method: The subjects of this study were 346 people (173 men and women aged 20 years and over) who received health screenings. Data was collected from December 1, 2005 to February 28, 2006. The FANTASTIC Lifestyle Assessment Inventory except smoking and the Framingham risk score of subjects were investigated. Data was analyzed by descriptive analysis, t-test, ANOVA, pearson correlation coefficients and stepwise multiple regression using the SPSS 10.0 program. Results: The mean score of the lifestyle of the women (64.24) was higher than that of the men (59.12). The mean score of the risk of coronary heart disease of the men (5.28%) was higher than that of the women (0.28%). The framingham risk for men was significantly related to lifestyle such as dietary habit, use of caffeine and drugs, anxiety and depression, job satisfaction, and closeness with family. The main predictors of framingham risk for men and women were 'use of caffeine and drugs', and 'menopause' which explained 16.5%, and 30.7% respectively. Conclusion: Since lifestyles can be changed with effort, coronary heart disease can be prevented while people are healthy.

  • Purpose: This study was to investigate factors influencing the Framingham risk score-Coronary heart disease (FRS-CHD) according to gender and body mass index (BMI) of adults who participated in the 5th Korea National Health and Nutrition Examination Survey (KNHANES V-3). Methods: This study used a cross-sectional design with secondary analysis with KNHANES V-3. The FRS-CHD scores were measured with ages, sex, blood pressure, cholesterol, high density lipoprotein, smoking, and diabetes mellitus. With demographic characteristics, family history of ischemic heart disease, types (intensity) and days of physical activities, perceived stress, drinking, menopause (in female), and BMI scores were measured. The data were analyzed with descriptive statistics, Pearson's correlation coefficients, and multiple regressions. Results: FRS-CHD was significantly associated with types (intensity) and days of physical activities, educational level, occupation, and marital status, explaining 19.1~76.8% of the variance in men. FRS-CHD was significantly associated with types (intensity) and days of physical activities, menopause, and education level, explaining 55.0~59.5% of the variance in women. Conclusion: Factors influencing FRS-CHD were significantly different according to gender and BMI. To reduce the risk of coronary artery disease, it is necessary to develop gender-specific physical activity programs according to BMI.

Framingham Coronary Risk Score를 이용한 화병과 심혈관계 질환과의 관련성 연구 (Corelationship Study between Hwa-Byung and Coronary Heart Disease, by using Framingham Coronary Risk Score)

  • Objectives : This study was to research the relationship between Hwa-Byung and Framingham coronary risk score(FRS), cardiovascular disease. Methods : 649 people participated in the community based cohort study in Wonju City of South Korea from July 2nd to August 30th in 2006. Educated investigators checked up systolic & diastolic blood pressure and surveyed Hwa-Byung Diagnostic Interview Schedule(HBDIS), cohort questionnaire about gender, age, smoking, diabetes. Blood sample was collected from participants to analyze total cholesterol, HDL-cholesterol. FRS was calculated from collected data. 10-year prediction of coronary heart disease was determined from FRS by using score sheet that is estimated by Wilson et al. Collected data were analyzed by the chi-square test. Results : 1. Low risk number of people was 18(52.9%) in Hwa-Byung group, 263(42.8%) in non Hwa-Byung group. p-value was 0.472. Difference of the two group was invalid. 2. The number of people below or equal to average 10-year prediction of coronary heart disease as gnder & age, Hwa-Byung group was 19(55.9%), non Hwa-Byung group was 412(67.0%). p-value was 0.251. Difference of the two group was invalid. Conclusions : There was no correlationship Between Hwa-Byung and 10-year prediction of coronary heart disease.

일 건강검진센터에 내원한 여성을 대상으로 심혈관질환 위험요인을 활용한 고위험군 간호의 우선순위 설정 (Determining Nursing Care Priorities among Women in the High Risk of Coronary Heart Disease)

  • Purpose: This study was designed as a epidemiologic study for determining priorities of nursing care in women with high risk groups of coronary heart disease(CHD) using risk factors. Method: Subjects were 1015 women who received health screenings at a hospital in Kyunggi, Korea, over one year period from January to December 2006. Results: The 37.9% of women had multiple risk factors for developing coronary heart disease. The most significant risk factors on CHD were the age older than 70 years (OR=11.45), the age between 60-69 (OR=4.65), the age between 50-59 (OR=3.20), having $HbA_1C$ over 7% (OR=2.38), high risk groups of L/H ratio (OR=1.98), and the waist to stature ratio (WSR) over 0.5 (OR=1.33). Conclusion: The findings suggest that women older than 50 years should be considered as an overt target population for CHD prevention, even in the absence of other risk factors. Also, $HbA_1C$ and WSR can be efficient indicators for CHD screening.

도시지역 주민의 관상동맥질환 위험인자에 관련한 비만지표의 유용성 검토 (The Usefulness of Obesity Indices for the Coronary Risk Factors in an Urban Inhabitants)

  • Objectives : To determine obesity for the screening of individuals at high risk of coronary heart disease in urban areas. Methods : Data were obtained from 4,137 adults between 19 and 85 years of age (2,372 males, 1,765 females), not recognized as taking medicines for cardiovascular diseases, who underwent a health check-up at the health promotion center of university hospitals in cities between Jan. 2003 and Dec. 2004. The variables studied were divided into two broad categories, and their relationships examined. obesity indices and risk factors for coronary heart disease. To reveal the relation between each of the obesity indices and the proportion of individuals at risk of coronary heart disease, the obesity indices were stratified and odds ratios obtained after age adjustment. Results : From a gender comparison of anthropometric measures, men were found to have significantly greater heights, weights, and waist and hip circumferences than women. From a gender comparison by the obesity indices, women were found to have significantly higher BMI, %Fat, waist to hip and waist to stature ratios than men. As obesity indices, the waist to stature ratio and the waist circumference were strongly correlated with coronary risk factors, both in men and women. The age-adjusted odds ratio of coronary risk factors increased significantly with increasing waist circumference, BMI, %fat, waist to hip and waist to stature ratios, and were highest specifically for the waist to stature ratio and the waist circumference. Conclusions : The study results showed that the waist to stature ratio and the waist circumference, as obesity indices, were most closely correlated with coronary risk factors. It is suggested that the waist to stature ratio and, specifically, the waist circumference can be effectively used in the field of health management for screening those with high levels of coronary risk factors.

여성의 무증상 갑상샘 기능이상과 관상동맥질환 발생과의 관련성 연구 (A Study of Subclinical Thyroid Function Disorder and the Risk of Coronary Heart Disease in Women)

  • Purpose: This paper has attempted to investigate the association between the subclinical thyroid function disorder caused by natural change in thyroid hormones or thyroid remedies and the incidence of CHD. Methods: Data was analyzed of 7,675 women who had undergone health examinations in a hospital in Gyeonggi-do between January 2007 and December 2008. The covariation of the coronary risk factors such as age, BMI, HbA1C, systolic blood pressure, LDL:HDL-cholesterol ratio, CRP, smoking and exercise were analyzed by using logistic regression analysis. Results: A significant increase in serum TSH was observed with higher age (F=26.91, p=.00). In terms of age, the risk of CHD started to gradually increase since the 40s and sharply since the 60s (${\chi}^2$=113.29, p=.00). The serum TSH was the most significant parameter influencing CHD (B=.12, p=.00). The risk of coronary heart disease was 3.12 times higher in the subclinical hypothyroidism group (OR=3.12) while no significant difference was observed in the subclinical hyperthyroidism group. Conclusion: Subclinical hypothyroidism may be an independent risk factors for CHD. A nurse nursing patients with thyroid disorder should be well informed of their state of subclinical thyroid function disorder and make efforts to extend their health expectancy.

관상동맥질환자의 교육 요구 -일개 종합병원 내원환자를 중심으로- (A Study on the Educational Needs of the Coronary Heart Disease Patients in a Municipal General Hospital)

  • The purpose of this study is to find out the educational needs for the coronary heart disease patients of a general hospital in a municipal general hospital. The study subjects were 90 outpatients who had been diagnosed as myocardial infection or angina at the hospital from January 3, 1998 to January 31, 1998. The questionaire was consisted of risk factors and educational needs for coronary heart disease patients. The qusetions on the education needs of the coronary heart disease were consist of 34 questions in 6 fields. In analyzing the data, the number and the percentage were calculated in patients general characteristics, the average and the standard-deviation were calculated to understand the educational needs, t-test and ANOVA were used to find out inter group difference on educational needs. In ANOVA, the advanced analysis was done by Scheffe-test. The results of this study were as follows: 1. 52(57.8%) of the patients (90%) live in Kangnung-city and 95% of the patients live in the middle-east part of Korea. At the age distribution 39 to 80, 41.1% of the patients were in their sixties and average age was 62.4 years old. 40% of the patients left school in mid-course or graduated from middle school or high school 2. As a result of the study on the risk factors of the coronary heart disease, the patients who have hypertension and diabetes were 41.1% and 20% respectively. In smoking and drinking, smokers were 30% and drinkers were 40% and average of smoking and drinking duration over 30 years old. 47.8% were over 140mgHg systolic - BP and 45.8% were over 90mmHg diastolic - BP 3. The patients' educational needs average 94.88 (maximum 134), and item average 2.79(maximum 4.0), the highest needs field was the characteristic of disease 33.22(maximum 44). Item average 3.02 (maximum 4.0) and the lowest education needs field was the physical activity and exercise 10.21(maximum 16). Item average 2.55(maximum 4.0) 4. General characteristics which were different from the general characteristic educational needs were statistically different by group of educational level, occupation, age and sex. 5. The risk factors which were different from the coronary heart disease patients' risk factors educational needs were statistically different by groups of the heart disease patients in family and the patients who take regular exercise. with a result the nurses need to improve the educational programs while considering the high educational needs field and the characteristics which showed significant concerns statistically.

비타민 A 과량 투여가 흰쥐의 체내 지방 대사상 및 장기의 비타민 A 함량에 미치는 영향 (Effects of Excess Vitamin A Feeding on Lipid Profiles and Vitamin A Contents of Rats)

  • This study was performed to examine the effect of excess vitamin A feeding on lipid profile and vitamin A content of rats. 12 male Sprague-Dawley rats. weighing 114.1$\pm$0.9g, were divided into control and excess vitamin A group, and fed experimental diet for 6 weeks. In the excess vitamin A group, the level of vitamin A was 400, 000 1.U./kg diet(100 times of standard level). It was observed that vitamin A excess group had higher levels of plasma total lipid, total cholesterol, and trighlyceride and lower HDL ratio, indicating a high risk to coronary heart disease. In the liver, total CHOL and TG content were also high, and fatty liver was produced by excess Vitamin A. Vitamin A contents of organs were higher when excess vitamin A was given. These results suggested that excess vitamin A could be a dietary factor increasing the lipid contents of plasma and liver. Therefore, it could be said that vitamin A megadose can be one of the risk factors to coronary heart disease.

  • New biochemical and genetic markers will be required to be more successful in the prevention of coronary heart disease. Postprandial lipid metabolism has received considerable attention since it was shown that postprandial triglyceride-rich lipoproteins are independently involved in the development of atherosclerosis. Multiple genes and environmental factors work in concert to alter these lipid. In this paper, postprandial lipemia, genetic variation and cardiovascular risk will be reviewed.

한국 30세 이상 성인에서 식사 섭취와 Framingham risk score에 의한 10년 관상심장질환 위험과의 연관성: 2019-2020년 국민건강영양조사 자료 활용 (Association of Dietary Intake with 10-Year Risk for Coronary Heart Disease Predicted from Framingham Risk Score in the Korean Adults: Korea National Health and Nutrition Examination Survey 2019-2020)

  • This study investigated the association between dietary intake and 10-year risk for CHD predicted from Framingham risk score in Korean adults using the Korea National Health and Nutrition Examination Survey 2019-2020. Eight thousand subjects (3,382 men and 4,618 women) aged 30 years or older were classified into three groups according to the 10-year CHD risk (%). The sociodemographics, anthropometrics indices, blood profiles, and dietary intake were collected. After adjusting for confounding variables, the mean adequacy ratio (MAR) decreased for both men and women as the disease risk increased. The high-risk group was below the MAR criteria, indicating that the overall quality of the meal was not good. The highest consumers of legumes (OR 0.56, 95% CI 0.37-0.84), fruits (OR 0.67, 95% CI 0.48-0.93), and fish (OR 0.60, 95% CI 0.42-0.87) had a 44%, 33%, and 40% lower 10-year CHD risk than lowest consumers in men, respectively. In women, there is no significant relationship between food groups and disease risk. Therefore, improving lifestyle habits such as weight control, increased activity, and adequate food intake, especially legumes, fruits, and fish rich in antioxidant nutrients and bioactive substances, appears to have a potential association with preventing coronary heart disease in Korean adults.
