Computed Tomography (CT) Simulated Fluoroscopy-Guided Transdiscal Approach in Transcrural Celiac Plexus Block

  • Conventional transcrural CPB via the "walking off" the vertebra technique may injure vital organs while attempting to proximally spread injectate around the celiac plexus. Therefore, we attempted the CT-simulated fluoroscopy-guided transdiscal approach to carry out transcrural CPB in a safer manner, spreading the injectate more completely and closely within the celiac plexus area. A 54-year-old male patient with pancreatic cancer suffered from severe epigastric pain. The conventional transcrural approach was simulated, but the needle pathway was impeded by the kidney on the right side and by the aorta on the left side. After simulating the transdiscal pathway through the T11-12 intervertebral disc, we predetermined the optimal insertion point (3.6 cm from the midline), insertion angle (18 degrees), and advancement plane, as well as the proper depth. With the transdiscal approach, we successfully performed transcrural CPB within a narrow angle, and the bilateral approach was not necessary as we were able to achieve the bilateral spread of the injectate with the single approach.

Expansive Laminoplasty for Cerical Compression Myelopathy (경수 압박으로 인한 척수증 환자에서의 경추강 확장술)

  • In an attempt to eliminate some negative aspects of conventional extensive laminectomy, 4 cases of multiple level of cervical compression myelopathy, 1 OPLL(ossification of posterior longitudinal ligament) and 3 cervical stenosis, were treated with a technique of expansive laminoplasty. Operative results in all patients were satisfactory without surgical complications and all patients had a neurological improvement. We suggest that our technique is more effective one for cervical canal enlargement with preservation of stability for treating multiple level of cervical canal stenosis, OPLL, and spondylosis than conventional extensive laminectomy.

Body fluid identification in forensics

  • Determination of the type and origin of the body fluids found at a crime scene can give important insights into crime scene reconstruction by supporting a link between sample donors and actual criminal acts. For more than a century, numerous types of body fluid identification methods have been developed, such as chemical tests, immunological tests, protein catalytic activity tests, spectroscopic methods and microscopy. However, these conventional body fluid identification methods are mostly presumptive, and are carried out for only one body fluid at a time. Therefore, the use of a molecular genetics-based approach using RNA profiling or DNA methylation detection has been recently proposed to supplant conventional body fluid identification methods. Several RNA markers and tDMRs (tissue-specific differentially methylated regions) which are specific to forensically relevant body fluids have been identified, and their specificities and sensitivities have been tested using various samples. In this review, we provide an overview of the present knowledge and the most recent developments in forensic body fluid identification and discuss its possible practical application to forensic casework.

  • Ovarian cancer is the tenth most common cancer in Korean female, and the third most common cancer of female reproductive organ after breast cancer and uterine cervical cancer. In spite of develop of conventional treatment, high modality of ovarian cancer comes from difficulty of an early diagnosis. Recent studies revealed that combining conventional and integrative medical treatment can reduce the adverse effect of surgical operation, chemotherapy and radiotherapy. Also it can improve survival rate, quality of life. However there isn't still a systemic clinical protocol for ovarian cancer in Korean medicine. This study will be helpful to establish clinical practice guidelines of Korean Medicine for ovarian cancer. And further studies on integrative ovarian cancer treatment are needed to build the clinical practice guidelines of ovarian cancer.

The Clinical Study on Hua-Tuo-Jia-Ji-Xue Acupuncture Treatment to Patient with Herniation of Nucleus Pulposus (요각통(腰脚痛) 환자의 화타협척혈(華陀夾脊穴) 침치료에 대한 임상적 연구)

  • Objectives : This study was designed to evaluate clinical effects of Hua-Tuo-Jia-Ji-Xue acupuncture to the patients with herniation of nucleus pulposus(HNP) of lumbar. Methods : Total of 40 patients with HNP of lumbar were participated in this study. We divided them into two group; A group using conventional acupuncture and B group using conventional acupuncture and Hua-Tuo-Jia-Ji-Xue acupuncture at the same time. All the subjects were asked to complete the VAS(visual analogue scale) and ODI(Oswestry disability index) before and after treatment. Results : Both A and B group represented effective improvement in VAS and ODI. And, B group is more effective than A group statistically. Conclusions : A comparision of VAS and ODI results of the two groups show that Hua-Tuo-Jia-Ji-Xue acupuncture trement can give be more effective method on lumbago patients, if it is added conventional acupuncture. Further clinical studies are needed to verify these findings.

Diagnosis of Rotator Cuff Tears with Non-Arthrographic MR Imaging: 3D Fat-Suppressed Isotropic Intermediate-Weighted Turbo Spin-Echo Sequence versus Conventional 2D Sequences at 3T

  • Purpose: To assess the diagnostic performance in detecting rotator cuff tears at 3T of non-arthrographic shoulder magnetic resonance imaging (MRI) using 3D isotropic turbo spin-echo (TSE-SPACE) sequence as compared with 2D sequences. Materials and Methods: Seventy-four patients who were arthroscopically confirmed to have underwent non-arthrographic shoulder MRI with 2D sequences and TSE-SPACE were included. Three independent readers retrospectively scored supraspinatus and infraspinatus tendon (SST-IST) and subscapularis tendon (SCT) tears on 2D sequences and TSE-SPACE. Results: The mean sensitivity, specificity, and accuracy of the three readers were 95%, 100%, and 95% on TSE-SPACE and 99%, 93%, and 98% on 2D sequences for detecting SST-IST tears, respectively, whereas those were 87%, 49%, and 68% on TSESPACE and 88%, 66%, and 77% on 2D sequences for detecting SCT tears, respectively. There was no statistical difference between the two sequences, except for in the specificity of one reader for detecting SCT tears. The mean AUCs of the three readers on TSE-SPACE and 2D sequences were 0.96 and 0.98 for detecting SST-IST tears, respectively, which were not significantly different, while those were 0.71 and 0.82 for detecting SCT tears, respectively, which were significantly different (P < 0.05). Conclusion: TSE-SPACE may have accuracy and reliability comparable to conventional 2D sequences for SST-IST tears at non-arthrographic 3T shoulder MRI, whereas TSE-SPACE was less reliable than conventional 2D sequences for detecting SCT tears.

Attitude of Medical Students and Doctors towards Complementary, Alternative and Integrative Medicine: A Single-Center, Questionnaire-Based Study

  • Objectives: Our study aimed to determine the attitudes of second- and final-year medical students and doctors (teaching faculty) of modern medicine towards complementary and alternative medicine (CAM) using the Complementary, Alternative, and Integrative Medicine Attitude Questionnaire (CAIMAQ). Methods: We invited 248 second-year medical students, 245 final-year medical students, and 48 faculty members to participate in the study. The CAIMAQ consists of 30 items, divided into five categories assessing various aspects of CAM, and scored using a 7-point Likert scale. The median scores obtained were compared between groups; a p-value < 0.05 was considered statistically significant. Results: A total of 138 medical students and faculty responded and participated in the study, of which, 24 (17.4%) were faculty, 40 (29%) were final-year medical students and 74 (53.6%) were second-year medical students. The overall attitude towards the various CAM concepts and therapies was positive. In general, the faculty were significantly less likely to consider referring patients for CAIM treatments, integrating them with conventional medicine, referring patients to alternative healthcare providers, considering the use of subtle energy fields as an ethical form of treatment, or considering CAIM treatments to be less invasive and harmful compared with conventional medicine. There was no significant difference in the attitudes of second- and final-year students. Conclusion: The attitude of medical students and doctors towards CAM is positive, and although the medical faculty have reservations in recommending specific types of CAM therapies or integrating them with conventional care, building evidence for supporting CAM therapies in specific diseases is likely to increase its uptake among health care professionals.

A Review on the of External Ointment Treatment for Diabetic Foot Ulcer (당뇨병성 족부 궤양의 외용 연고 처치에 대한 고찰)

  • Objectives : Diabetic Foot Ulcer(DFU) is one of the common complications of diabetes. DFU is difficult to treat compared to other chronic wounds and clinically effective treatments are limited. The purpose of this study is to review the of external ointment treatment for DFU. Methods : 8 databases such as PubMed, EMBASE, CENTRAL, CNKI, NDSL, RISS, KISS, OASIS including English, Korean and Chinese were searched by structured search strategies that consist of terms as 'diabetic foot', 'diabetic foot ulcer(DFU)' and 'randomized' from January 2001 to January 2021. All randomized controlled trials(RCTs) involving treatment group as external ointment or in combination with conventional treatment were included. Results: A total of 20 RCTs was identified and analyzed. In treatment group, ointment was applied based on conventional treatment. A total of 16 different ointments were used, and the frequency of use was highest in the order of Heat-clearing drug, Blood-activating and stasis-dispelling drug, Tonifying and Replenishing drug and Orifice-opening drug. The most used herbs were in the order of Coptis chinensis Franch., Phellodendron chinense Schneid., Borneolum syntheticum., Angelica sinensis(Oliv.) Diels. In treatment group, a clinically effective effect was obtained compared to the contrast group. Conclusions : The result of this study suggest that external ointment treatment based on conventional treatment can be applied to DFU.

  • A technique to improve the analysis of micronuclei(MN) in lymphocytes as a cytogenetic indicator is reported. For the purpose of diminishing the variation of the result from individual reader and making it easier to distinguish accurately a cytokinesis blicked(CB) lymphocyte and micronuclei, we tried a modified one-step silver staining technique as a method for detection of the argyrophilic nucleolar organizer region(AgNOR) with or without conventional Giemsa stain in the slide from CB method. Compared with the conventional Giemsa stain, the preparation processed with this method are especially useful for the accurate analysis of MN of cultured lymphocyte with cytochalasin B. This method will be a useful technique for automated calculation of MN.

  • Hemifacial spasm is a distressing condition characterized clinically by paroxysmal and an involuntary movement in muscles innervated by the facial nerve on one side of the face. Blockade of the facial nerve can be performed percutaneously, without any serious complications. There are certain clinical problems associated with the conventional procedure, such as severe pain and technical difficulties to find facial nerve. This report describes a fluoroscope guided facial nerve block. This new technique reduced the difficulties in identifying the facial nerve and decreased the suffering associated with the conventional way of facial nerve block.
