관절각도에 따른 근 피로도와 등척성 훈련 전이효과 (Muscle Fatigue according to Joint Angle and the Transfer Effect with Isometric Training)

  • This study examined whether there is a transfer effect to other joint angles and the angular specificity of muscle fatigue after 6 weeks of isometric training of the vastus medialis. Twenty subjects were randomly assigned to 30° and 90° knee flexion groups and were trained at 80% maximal voluntary isometric contraction(MVIC) three times a week for 6 weeks. The pre-and post-training values of the 80% holding time(endurance time) of MVIC, the Fatigue Index(FI), and the MVIC at 30°, 60°, and 90° were compared. After isometric training for 6 weeks, in the 30° knee flexion group, FI decreased significantly(p<0.05) only at 30°, which was the training angle; there was no change at other angles. By contrast, in the 90° knee flexion group, FI decreased significantly(p<0.05) at both the trained angle and at the other angles, indicating a transfer effect of training. MVIC did not increase significantly(p<0.05) at any trained angle in either the 30° or 90° knee flexion groups after 6 weeks of isometric training, neither did the 80% holding time of MVIC differ significantly compared with pre-training in either group. These findings suggest that training at 90° of knee flexion is more effective than training at 30° of knee flexion for obtaining a training transfer effect on muscle fatigue in the vastus medialis.

  • 인류는 과학기술의 발전과 함께 의학기술도 눈부신 발전을 거듭해왔다. 그러나 과거에도 난치성 질환은 여전히 존재하였듯이 지금도 난치성 질환이 존재하는데, 그 중 턱관절장애가 있다. 현재 의료선진국이라 불리는 대한민국 의료진들의 진단은 환자의 발언, 의사의 청음진단과 자를 이용한 진단, X-ray 촬영 진단 방식을 고수하며 시대에 뒤떨어지고 있다. 그렇기 때문에 환자의 정확한 증상 여부, 의사 본인의 진단 실력과 경력이 중요하고, 증상의 경중에 따른 진단 횟수의 증가와 이로 인해 발생하는 의료비용은 막대하다. 본 연구에서는 이를 해결하기 위해 저작운동(최대폐구)시 교근에서 발생되는 근전도 신호를 %MVC를 통해 정량화하였다. 정량화된 근전도는 Cortex로 비교, 평가하여 턱관절 상태 평가 기준 지표를 확립할 것이다.

만성 뇌졸중 환자에서 발목관절 각도에 따른 근 수축이 내측 비복근의 근 구조에 미치는 영향 (The Effect of Muscle Activity on Muscle Architectural of Medial Gastrocnemius in Chronic Stroke Patient Based on Ankle Joint Degree)

  • 본 연구의 목적은 만성 뇌졸중 환자에서 발목관절 각도에 따른 근 수축이 내측 비복근의 근 구조에 미치는 영향에 대해 알아보고자 하였다. 연구대상은 MAS(modified Ashworth scale) 2등급인 만성 뇌졸중 환자 10명을 대상으로 하였다. 근육 두께, 우모각, 근섬유속 길이, 최대 수의적 등척성 수축력(MVIC) 측정은 초음파 영상 촬영장치와 동력계로 이완 시(resting)와 최대 수의적 등척성 수축 시를 측정하였다. 측정결과, 근육 두께는 마비측이 정상측에 비해 유의하게 얇은 것으로 나타났고(p<.001), 발목이 저측굴곡 될수록 얇아짐을 알 수 있었다. MVIC에서는 이완 시보다 근육 두께가 유의하게 얇아지는 것을 알 수 있었다(p<.001). 우모각은 마비측이 정상측에 비해 유의하게 작은 것으로 나타났고(p<.001), 발목이 저측굴곡 될수록 커짐을 알 수 있었다. MVIC에는 이완 시보다 우모각이 유의하게 커짐을 알 수 있었다(p<.001). 근섬유속 길이는 마비측이 정상측보다 유의하게 짧은 것으로 나타났고(p<.001), 발목이 저측굴곡 될수록 짧아짐을 알 수 있었다. MVIC에는 이완 시보다 근섬유속 길이가 유의하게 짧아지는 것을 알 수 있었다(p<.001). 본 연구를 통해 만성 뇌졸중 환자에서 발목 각도에 따른 근 수축이 내측 비복근의 근 구조에 영향을 미치는 것을 알 수 있었다. 따라서, 만성 뇌졸중 환자의 영상구조적 특성을 평가하는 것은 임상적 진단과 치료에 매우 유용한 자료라 생각된다.

계단 오르기와 내리기 동안 다축범위(multi-radius) 무릎인공관절 수술자의 운동역학적 비교분석 (A Biomechanical Comparative Analysis of the Multi-Radius Total Knee Arthroplastry System for Go up Stair and Go down Stair)

  • The primary purpose of a TKA is to restore normal knee function Therefore, ideally, a TKA should: (a) maintain the natural leverage of the knee joint muscles to ensure generating adequate knee muscle moments to accomplish daily tasks such as rising from climbing stairs; (b) provide adequate knee joint stability. A 16-channel MyoResearch XP EMG system was used to collect the differential input surface electromyography signals VM, VL, RF, BF, ST during climbing/descending stair tests. A Peak Motion Measurement System was used to collect the kinematic and kinetic data. AKIN-COM Ill isokinetic dynamometer was used for EMG of VM, VL, RF, BF and ST during maximal voluntary contraction. I Quadriceps EMG results for the VM of the passed 1year group limb demonstrated significant less RMS EMG than that of the passed 3year group limb $60^{\circ}-15^{\circ}$ of knee flexion(p<0.05). The VL of the passed 1year group limb also demonstrated significants less RMS EMG than that of the passed 3year group limb from $60^{\circ}-45^{\circ}$ of knee flexion(p<0.05). Similar to the VM and VL, the RF of the passed 1year group limb showed less RMS EMG than that of the passed 3year group limb from $60^{\circ}-30^{\circ}$ do knee flexion(p<0.05). Hamstring EMG results for the BF of the passed 1year group limb demonstrated less RMS EMG than that of the passed 3year group limb from $75^{\circ}-15^{\circ}$ of knee flexion(p<0.05). The passed 1year group limb tended to have less ADD displacement(p<0.071) than that of the passed 3year group limb. There was no significant difference of the ABD displacement between the passed 1year group and the passed 3year group limbs(p<0.73). The passed 3year group used compensatory adaptation movement strategies to compensate for the strength deficit of passed 3year group limbs. The passed 3year group limb also increased the quadriceps muscle activation level to produce more knee extension moment to compensate for the short quadriceps moment arm. The passe 3year group limb might have an unstable knee joint in the medio-Iateral direction during the climbing/descending by showing a tendency of more ADD displacement and greater hamming co-activation EMG than the passed 1year group limbs. The TKA design was not able to help the knee joint to produce adequate knee extension moment with less quadriceps muscle effort. I think that old man needs continuous exercise for muscle strength.

태권도 주춤서 지르기에서 Stretch-Shortening Cycle 이 관절파워에 미치는 효과 (The Effect of Stretch-Shortening Cycle on the Joint Power of the Jireugi in the Taekwondo Juchumseogi Stance)

  • The purpose of this study was to investigate the muscle mechanical properties of the pelvic axial pre-rotational movement for the Jireugi in the Taekwondo Juchumseogi stance. Eleven elite Taekwondo Poomsae athletes participated. Each participant performed 5 right hand Jireugi in Juchumseogi stance as fast and strong as possible while their motion was recorded by a 3D motion analysis system and the ground reaction forces by two force plates. The power and work of the muscular group surrounding the waist were analyzed to verify the effect of the stretch-shortening cycle (SSC) theory. The cause of the greater power seems to be the application of the SSC by the muscles surrounding the waist during the preparation phase of the pre-rotation group. For the none pre-rotation group, they only used the concentric contraction of the muscles surrounding the waist. Because the pre-rotation group used the SSC theory, they had the effect of shortening of the range of movement, creating a fast and more powerful rotation, thus anticipating the increase the magnitude of impact.

윗팔두갈래근의 근력과 근육직경 간의 상관성에 대한 초음파 영상 연구 (A Correlation Study Using the Sonogram Between the Muscle Strength and the Diameter of Biceps Brachii Muscle)

  • Background: The purpose of our experimental study, we want to know a correlation study using the sonogram between the muscle strength and the diameter of Biceps brachii muscle by gender. So we set to hypothesis that Biceps brachii contracted maximally would be a diameter of the muscles increased according to the muscle strength will be increased. Methods: In this study, we measured the muscle strength of biceps brachii on contracting its maximum using the Biodex and the diameter of the muscles using Myo-Lab (25Gold). We experimented 30 subjects of twenty-aged men and women. The subjects is comprised of a persons who has not external injuries and pain of elbow joint, no mobility limitation of ROM, no experiences of surgery in past history. Result: We found that the men have a highly correlation between maximum strength and the diameter of the muscles more than women (p<.05). Conclusion: The results of our experimental study between the muscle diameter and the muscle strength to biceps brachii on relaxation and contraction is a statistically significant relation (r=.394), especially in rather men then women (p<.05). So this results would be based data for further clinical muscle efficiency application in physical therapy field.

  • The purpose of this study was to investigate the neurophysiological mechanisms, assessment of muscle spasm and treatments that could apply to clinical field. Spasm is a painful pattern of contraction of muscle caused by chronic or acute trauma, excessive tension, or organic disorders. Aside from pain, muscle spasm creates shortenning of muscles and limits motion. Untreated spasm and protective immobility due to pain lead to decreased local blood flow in the muscles and result in a vicious cycle of muscle spasm and paul. The assessment of muscle spasm involve muscle tone assessment, tissue compliance, and joint ROM. Each of these aessessments utilize as a part of the patient's condition Clinical managements involve drug management and physical therapy. Expecially, physical therapy is one of the most important techniques to reduce muscle spasm. Physical therapy includes applying heat and cold, electrical stimulation, massage, and traction. This investigation should entourage phisical therapists to experiment further with various techniques to reduce muscle spasm.

퇴행성 슬관절염 환자에 대한 수정된 대퇴사두근과 슬괵근 운동의 동시수축 효과 (Effect of a modified maneuver for quadriceps muscle setting with co-contraction of the hamstrings on patients with knee joint osteoarthritis)

  • Purpose : The purpose of this study was to investigate a modified maneuver for quadriceps setting exercise for patients with knee osteoarthritis. Methods : The patients were randomly divided into a modified maneuver for quadriceps setting exercise group(MQG) and conventional quadriceps setting exercise group(CQG). Total of 28 patients received a modality with training for 1 hour, three times a week for 12 weeks. Results : Each group showed significant reductions in the WOMAC(Western Ontario McMaster Universities Osteoarthritis) Index, mobility, and muscle strength after 12 weeks. In pain, physical function, and hamstring muscle strength, there was a statistically significant difference between groups. Conclusion : According to the results, MQG experienced less pain, physical function, and increased more hamstring muscle strength than NQG.

급확대-축소 연결부 주변의 원관 난류유동에 관한 실험적 연구 (Experimental Study on the Turbulent Flow Field in a Sudden Expansion-Contraction Pipe Joint)

  • 본 연구에서는 H를 고정하고 L을 변화시켜가며 내부의 유동구조가 어떻게 변하는가를 살펴보고, 특히 재부착이 일어나는 경우에는 급확대 부분만 존재하는 기존 실험결과와 비교분석하여 하류의 급축소부분이 전체 유동구조에 어떤 영향을 미치는가를 살펴보고자 한다. 실험에서 사용된 작동유체는 공기이며, 입구관 직경은 110mm, 급확대점과 급축소점사이의 연결부 직경은 220mm, 연결부의 길이는 L=300, 600 그리고 900mm의 3가지를 선택하였으며 기준속도는 입구관의 중심속도로 9.71 m/s이다. 입구직경(110mm)을 기준으로 한 Reynolds 수는 $R_{e}$=73,000 이고 입구관반경과 연결부반경의 차이인 계단높이(H=55mm)를 기준으로 하면 $R_{e=36}$ ,500이다. 연결부 의 급확대부분에서 입구관반경을 기준으로 한 반경확대비는 2이고 급축소부분의 반경 축소비는 1/2이다. 측정항목은 유동방향의 벽면압력분포, 유동방향의 평균속도분포 및 난류강도 등이며, L=900mm인 경우는 반경방향과 원주방향의 난류강도, Reynolds 전단응력도 측정되었다.

  • The role of botulinum toxin as a therapeutic agent is expanding rapidly in otolaryngology. Botulinum toxin is a protease that blocks the release of acetylcholine from nerve terminals. Its effects are transient and nondestructive, and largely limited to the area in which it is administered These effects are also graded according to the dose, allowing for individualized treatment of patients and disorders. Botulinum toxin has been used primarily to treat disorders of excessive or inappropriate muscle contraction. In the field of otolaryngology, these include spasmodic dysphonia, oromandibular dystonia, and blepharospasm, vocal tics and stuttering, cricopharyngeal achalasia, various tremors and tics, hemifacial spasm, temporomandibular joint disorders and a number of cosmetic applications. Botulinum toxin treatment has recently begun to show some benefit in the control of pain from migraine and tension headache. It may also prove useful in the control of autonomic dysfunction, as in Frey syndrome, sialorrhea, and rhinorrhea. In over 20 yews of use in humans, botulinum toxin has accumulated a considerable safety record, and in many cases represents relief for thousands of patients unaided by other therapy.
