The Upper Ascending Reticular Activating System between Intralaminar Thalamic Nuclei and Cerebral Cortex in the Human Brain

  • Purpose: The ascending reticular activating system (ARAS) is responsible for regulation of consciousness. In this study, using diffusion tensor imaging (DTI), we attempted to reconstruct the thalamocortical projections between the intralaminar thalamic nuclei and the frontoparietal cortex in normal subjects. Methods: DTI data were acquired in 24 healthy subjects and eight kinds of thalamocortical projections were reconstructed: the seed region of interest (ROI) - the intralaminar thalamic nuclei and the eight target ROIs - the medial prefrontal cortex, dorsolateral prefrontal cortex, ventrolateral prefrontal cortex, orbitofrontal cortex, premotor cortex, primary motor cortex, primary somatosensory cortex, and posterior parietal cortex. Results: The eight thalamocortical projections were reconstructed in each hemisphere and the pathways were visualized: projections to the prefrontal cortex ascended through the anterior limb and genu of the internal capsule and anterior corona radiata. Projections to the premotor cortex passed through the genu and posterior limb of the internal capsule and middle corona radiata; in contrast, projections to the primary motor cortex, primary somatosensory cortex, and posterior parietal cortex ascended through the posterior limb of the internal capsule. No significant difference in fractional anisotropy, mean diffusivity, and fiber volume of all reconstructed thalamocortical projections was observed between the right and left hemispheres (p>0.05). Conclusion: We reconstructed the thalamocortical projections between the intralaminar thalamic nuclei and the frontoparietal cortex in normal subjects. We believe that our findings would be useful to clinicians involved in the care of patients with impaired consciousness and for researchers in studies of the ARAS.

Consideration on Human in World of Post-Heaven: Focusing on perfect man of Right I Ching (후천세계가 지향하는 인간상 탐구- 정역의 지인(至人) 중심으로 -)

  • The theory of post-heaven provided Korea's many emerging national religions with spiritual ground. The I Ching originated in China emphasizes the principles of change or static aspects of world, while Right I Ching, which Il-Bu Kim suggested, underlines dynamic aspects of world. Il-Bu Kim is perceived to open a new stage in the history of philosophy of I Ching in this regard. His most remarkable contribution is the view of Great Opening Era of Post-Heaven, according to which the clock of cosmos shows the era of great change from Pre-Heaven to Post-Heaven and in the world of Post-Heaven everything will take its proper place. As to human society, Il-Bu Kim foresaw the change from disharmony to harmony, imbalance to balance and era of xiaoren to that of junzi. The advent of such a new world, however, asks human's moral revolution as a prerequisite. In the tradition of East Asian thought, human is the center of the Samjae, Heaven-Earth-Human and the only being which could participate in the growth of Heaven and Earth. Without the change of human mind, however, human can neither participate in the growth of Heaven and Earth nor expect the right change of Heaven and Earth. Even though the world of nature changes according to the principle of cosmic harmony, as the Right I Ching predicts, the real change of Heaven and Earth can be achieved only with moral change of human. The human in the Right I Ching is a free man who sticks to nothing and communicates with cosmos with empty mind. This is the very image of perfect man(至人). The perfect man in the Right I Ching is the main agent of everything, that's to say, the main agent of the great opening to Post-Heaven Era. This is the reason why most leaders of Koran emerging religions concentrate on the change of human. In this regard, Jeung-San Kang encouraged people to become the perfect man through the purification of mind and body and the great opening of inner man, depending on the metaphysical principle of the Right I Ching. Cheok and Resolution of grievances for the mutual beneficence of all life are what he suggested as a method of the great opening of inner man. Such a view is based on the thought that the accumulated sinful behaviors are obstacles to cosmic change and the time of mutual beneficience opens with the removal of the obstacles. If such religious practices are so effective, as Jeung-San Kang says, that future world could consist of perfect men who have achived the great opening of inner man, moral change, and change of consciousness, we could expect that the activities of xiaoren will be shrinked and junzi will be welcomed, as the Right I Ching predicts.

Attitudes of Hospice Volunteers towards Death with Dignity (호스피스 자원봉사자의 존엄사에 대한 태도요인)

  • The purpose of this study was analyzed the factors of influencing toward attitude to death with dignity to hospice volunteers. The data was collected for 21 days from 14 March to 3 April 2010. Among a total of 220 cases of the questionaries, only 195 cases were used. To data were analyzed by factor analysis, independent t-test, one-way anova and logistic regression using PASW statistics 18.0. The results were as follows; The attitudes towards death with dignity according to general characteristics was high in those with will to agreed to the passive euthanasia than those opposite to the attitude factors, namely, acceptive, the right to decide, negative, and dereliction of duty attitude factors. Significant variables for effects of death with dignity were gender, acceptive attitude factor and dereliction of duty attitude factors. Given that main provider of human organs is the brain-dead and we don't have enough organ donation, death with dignity should be linked with activating policy of organ donation, while solving donation shortage problem. This way, constructing social implementation and sharing consciousness on organ donation, would be diluting the bio-ethic controversies.

Successful Management of a Comatose Patient with Traumatic Brain Exposure with a Fronto-Parieto-Occipital Flap

  • Composite skull defects in patients with severe head injuries are very challenging to manage. The dilemma when deciding whether to perform a definitive reconstruction is how long to wait for physiological recovery before an intervention complicates the situation. The inability of such patients to tolerate prolonged anesthetic exposure is a driving factor for performing the minimal intervention necessary to facilitate recovery. Herein, we present a case involving the successful immediate reconstructive treatment of a severely head-injured adolescent with a composite scalp defect secondary to trauma. A 14-year-old boy sustained a severe head injury from a motor vehicle accident with a composite scalp defect in the right fronto-parietal region. The frontal lobe was exposed, and the right eye was crushed and devitalized. The patient was deeply unconscious for 3 days, without any significant improvements before reconstructive surgery was proposed due to fear of possible meningitis resulting from the exposure of brain structures. We successfully managed the patient with a fronto-parieto-occipital flap, after which the patient promptly recovered consciousness.

Studies on Electroacupuncture Anesthesia of Pigs (돼지의 전침 마취에 관한 연구)

  • To establish the electroacupuncture anesthesia for pigs, 6 piglets (Landrace mixed, 1 month old, 3.7~5.3 kg, female: 3 heads and male: 3 heads) were examined. The acupoints used for porcine electroacupuncture anesthesia were An-shen, and Tian-ping and Bai-hui. After horizontal insertion of needle to left and right An-shen, and perpendicular insertion to Tian-ping and Bai-hui, respectively, positive electrode was connected at right An-shen and Tian-ping, and negative electrode was connected at left An-shen and Bai-hui using veterinary electroacupuncture anesthesia apparatus, respectively. Electric condition was 3 V and 30 Hz. To examine the effect of electroacupuncture anesthesia, laparotomy (4 heads) and castration (2 heads)were applied. The induction time of electroacupuncture anesthesia was very rapid and about 1 minute, and pain of the body surface including the extremities was not detected. As for the reactions with electroacupuncture anesthesia, the head was directed to backward, nasal part was curved to one side, and blepharum was closed in part of changed nasal part. In addition, consciousness was vivid, and muscle tension and lift of the tail were observed in early stage but was relaxated afterwards. The pain was not found during incision and suture, bleeding was comparatively small volume and the class of anesthesia effect was excellent.

  • This paper aims at studying how to express and be used the characteristics of New age consciousness in the popular culture. New age consciousness is used interchangeably with new age movement. The philosophy of new age is based on humanism, which the infinite potential power of human can change the human himself and the earth forward to be shown the new age of peace, hope and love in an instant. This consciousness is based on the Western secret art and Eastern consciousness against the Christianity and has begun to develop in the alternative religion influenced by Postmodernism and Counterculture movement on 1970's which is against the corruption of American society on 1960's. However, new age consciousness has tried to change to a social movement for the culture, the peace, the happiness of mankind and the healing society rather than a religion idea because of the limitation of the theory itself. Nevertheless, the new age culture contains the characteristics of the religion, and the characteristics has been easily borrowed to the mess media and infiltrated to us. In a broad sense, the religion includes both own religion beliefs and behavior and the social movements and tendencies about the established religion which pursues the holy and the highest value. The religion is valued as a complex culture phenomenon, which the religion presents the right direction and the meaning of life as well as contributes to the maintenance of social order and the social unification. For this reason, the religion and the culture have a deep relationship between mess media and needs to be captured and listen to religious longing lurking deep within the human consciousness in order to give the audience impressed and fun. In the study, the characteristics of New age such as reincarnation, illusion, meditation, monism and pantheism is analyzed for what affects students studying mess media and the popular culture. And then the study is analyzed the characteristics of new age consciousness in the animation through 'Avatar'. This study shed new light on the influence of religion which is ideological identity of popular culture, and I expect to be a little help to have the insight to examine the mess media with a various angles.

Choroid Plexus Carcinoma in an Adult

  • Choroid plexus carcinomas are extremely rare in adults. They can behave aggressively and their optimal management is uncertain. A 35-year-old woman was admitted with an episode of loss of consciousness. Magnetic resonance imaging showed a homogeneously enhancing mass in the trigone of the right lateral ventricle. Detailed examinations found no evidence of an extraneural primary focus. She underwent total removal of the tumor. Pathological diagnosis was confirmed as a choroid plexus carcinoma. She is doing well eight months after surgery.

Cerebellar Anaplastic Oligodendroglioma Presenting with Hemorrhage

  • The authors describe an anaplastic oligodendroglioma of the cerebellum which is distinctly uncommon. The patient presented with sudden onset of decreased consciousness associated with hemorrhage in the cerebellum, which appeared to origin from a vascular malformation or a tumor on a initial computed tomography (CT) scan. Subsequent magnetic resonance (MR) imaging suggested a high grade glioma with abundant vascularity in the right cerebellum. The histological examination revealed the findings compatible with those of an anaplastic oligodendroglioma. A complicated clinical course had led him to a poor outcome.

  • Since the human race set foot on the earth, an unremitting effort to the free oneself from the fear of death. It was used ways and means like religion, power and a war to this end that the achievements of one's lifetime for a great man have power and wealth proved. But, there are remain proposition, all man will be die in spite of a lot of effort. The purpose of this study is recognize theoretical background of funeral culture based on present studies and a recent statistical data, and provide data for err from the right path of a funeral policy and improve consciousness of person. And, There wish inflected a basic data when Architectural Planning of funeral facilities to institutional background and facilities situation.

Severe Respiratory Depression and Intracranial Air after Epidural Morphine -Subdural or Epidural Injection?- (경막외 Morphine 투여에 의한 극심한 호흡억제 및 두개강내 공기음영)

  • Massive extradural spread, distinguished from subarachnoid injection that sometimes follows the introduction of small amounts of local anesthetics or narcotics during attempted epidural anesthesia or analgesia, has been attributed to subdural injection. A 64-year-old woman was admitted for partial radical hysterectomy under general anesthesia after insertion of lumbar epidural cathter by loss of resistance technique with 5 ml of air. In this case, we experienced severe respiratory depression and loss of consciousness after administration of 4 mg of morphine for postoperative pain control. We confirmed air shadows at right silvian and suprasella cisterna region by CT scanning. Patients was recovered without sequele after 2 days, As this case resembles a "massive epidural", it is suggested that subdural injection rather than epidural injection may explain the phenomenon.
