측두하악관절의 panoramic double TMJ 방사선사진상에서 하악과두와 인접구조의 관계 (Relationship between the condyle and adjacent structures in double temporomandibular joint view using panorama)

  • Purpose: To investigate the ability of double TMJ view by multifunctional panorama to view the bony components and the space of the temporomandibular joint. Materials and Methods: Ten dry skulls fitted with resin shims over the articular surface of the condyle were used to reproduce the temporomandibular joint space. Fine metal wires were attached to the three portions of contours of the condylar head and the articular eminence. With 10 dry skulls and 20 cases having TMJ dysfunction, double TMJ views by multifunctional panorama (Planmeca 2002 Proline CC) and transcranial views were taken, analyzed from the anatomical view point, and compared statistically in view of the widths of the posterior joint space and the condylar head. Results: In double TMJ view, the supero-anterior part of the condyle represented the lateral 1/3, the most superior part represented center portion, and the posterior part medial l/3 of the condyle. In maximum mouth opening, no other structures were superimposed with the condyle in double TMJ view. In double TMJ view, petrous bone was moderately superimposed with the superior part of the condyle and the posterior increment of angle exposure made wider the images of the articular eminence and the condyle. The tendency of reduction in the posterior joint space appeared in the side of TMJ dysfunction compared with the normal side. The posterior joint spaces in double TMJ view were statistically wider (p<0.05) than those in transcranial view. The correlation coefficient was 0.5179 between the widths of the posterior joint spaces in two radiographic views. Conclusions: Double TMJ view can be substituted for transcranial view in evaluating the TMJ dysfunction.

  • This study was designed to observe the adaptive changes of mandibular condyles to displacement of mandibular condyle in adult animals. In this study, 16 rabbits weighting about 3.5 kg was selected. Four rabbits were preserved for control group and 8 animals were divided into 3 groups, 2 weeks, 4 weeks and 8 weeks. The experimental animals were performed oblique osteotomy on both mandibular ramus and internal wiring at mandibular border. The experimental animals were sacrificed consecutively on the 2 weeks, 4 weeks and 8 weeks after oblique osteotomy and mandibular condyles were dissected out carefully to produced tissue specimen. The specimens were fixed with 10% N formaline solution for 24 hours and rinsed with phosphate buffer solution. It was decalcified with 5% nitric acid for 15 days. Thereafter the specimens were dehydrated in alcohol series and embedded paraffin as usual manner. The mebedded specimen were sectioned in $4-6{\mu}m$ microtome, stained with hematoxylin-eosin and azan stain and observed through light microscope. The following results were observed from this experiment. When there was postional change of condyle head after mandibular ramus oblique osteotomy in adult rabbit, 1. The density of chondrocyte was generally increased at condylar cartilage and the thickness of condylar cartilage was increased posterosuperior aspect of the mandibular condyle slightly. 2. The density of chondrocyte was increased at proliferative zone so fibrous articular zone, porliferative zone and hypertrophic zone was clearly distinguished. 3. Active endochondral bone formation was occurred at mandibular condyle.

  • Osteochondroma is the one of the most benign tumors of the axial skeleton, but is rarely found in the facial bones. Typical facial features of condylar osteochondroma include striking facial asymmetry, malocclusion with openbite on the affected side, and prognathic deviation of the chin and crossbite to the contralateral side. In this case, twenty four year-old female showed facial asymmetry, chin deviation, openbite on the affected side but have no symptoms of pain or dysfunction. Concomitantly she had maxillary occlusal cant and hemimandibular hypertrophy. Panoramic radiograph showed radiopaque mass on right mandibular condyle extended along the lateral pterygoid muscle. Computed tomogram demonstrated enlarged condylar head and bony spur on posteromedial side of condyle and 99Tc bone scintigraphy showed a focal hot image. These findings were correspond with osteochondroma. The lesion was treated with condylectomy and residual facial asymmetry was corrected with 2-jaw orthognathic surgery. Herein, we report a case of osteochondroma of the mandibular condyle and accompanying facial asymmetry.

백서 외측익돌근 제거가 하악골 성장에 미치는 영향에 관한 실험적 연구 (AN EXPERIMENTAL STUDY ON THE MANDIBULAR GROWTH FOLLOWING RESECTION OF THE LATERAL PTERYGOID MUSCLE IN RAT)

  • The purpose of the study is to note the effects of the resection of the lateral pterygoid muscle on the mandibular growth in the growing rats. Twenty four female Wistar rats were used in the experiment. They were divided into three groups: group 1 ; bilateral sham operation group 2 ; bilateral resenction of the lateral pterygoid muscle group 3 ; unilateral resection of the lateral pterygoid muscle (The right lateral pterygoid muscle was resected and the left one was sham-operated.) Groups 1&2 were sacrificed eight weeks lateral and group 3, four weeks later. All specimens were measured with calipers, and stained with hematoxylin and eosin. Groups 1 & 2 were compared with each other. In group 3, the right side was compared with the left. The results were as follows: 1. In the growing rats, the resection of the lateral pterygoid muscle was followed by a decrease of the mandibular growth. After eight weeks, the condyle-mental foramen distance, the angular process-mental foramen distance, the size of the condylar head, the supradentale-first molar distance, and the diameter of the symphysis were significantly smaller than the control. 2. Resection of the lateral pterygoid muscle resulted in decrease of the thickness of the cartilage layer and the prechondroblastic-chondroblastic layer after four weeks. 3. After eight weeks, group 1 and group 2 were not different significantly in the histologic sections. 4. The condylar cartilage was stabilized eight weeks after the experiment.

악관절의 자기공명영상과 시상단층 방사선촬영상에 관한 비교연구 (A COMPARATIVE STUDY ONMAGNETIC RESONANCE IMAGE AND SECTOGRAPH OF HUMAN TNJ)

  • For understanding of anatomy, physiology, and diseases of human TMJ, it is required to evaluate quantitatively the movement of the disc and condyle head of mandible. The histologic section of cadaver TMJ were examined, and the magnification of the MR image and its details of anatomy were evaluated. And then a quantitative analytic method, by comparing the Sectograph and the MR image of vital human TMJ, was proposed. For this study, 15 subjects(Male, 24~35years) were selected from a prosthodontic examination randomly, and each subject’s five interocclusal rubber registration records were made on the ICP, and 5, 10, 15, and 20mmjaw opening positions. All subjects were radiographed with a Denar Quint Sectograph Image System(Denar Corp., USA), and imaged with a MRP-20EX MR Image System(0.2T, Permanent Magnet Type, Hitachi Medical Corp., Japan) using an 100mm diameter bilateral type surface coil. These images were traced on the acetate tracing paper, and analyzed In this study, the findings led to the following conclusions. 1. In comparison of the histologic section of autopsy specimen with the MR image at the same section, the size(dimension) of MR image was 70% of the real one. It was possible to recognize the shape of articular disc, anterior and posterior attachments, and adjacent soft tissues, because of the excellent reproducibility of anatomical structure. 2. When we compared the amount of joint space on MR image with that of joint space on sectograph, the amount of joint space on sectograph was significantly greater than that of joint space on MR image, except at the top of condylar head. 3. The position of minimum joint space on sectograph at intercuspal position didn't coincide with the middle position of articular disc on MR image, and was approximately in the anterior third of posterior band of articular disc. 4. The amount of condylar movement on MR image at opening movement was greater than that of articular disc movement. From Intercuspal position to 5mm jaw-opening movement, the condylar movement showed hinge one, and over the range 5mm jaw-opening it suggested hinge & translatory one. 5. In terms of area variation of articular disc measured on MR image in sagittal plane, the area of posterior band increased with increasing the amount of Jaw opening, but the area of anterior band decreased conversely.

  • In almost all biologic systems, mechanically induced electric charge separation is a fundamental phenomenon. Since the hypothesis was established that the generation of electric potentials in bone by mechanical stress including muscular force might control the activity in bone by mechanical stress including muscular force might control the activity of osseous cells and their biopolymeric byproduct, the concept of electrically mediate growth mechanism, which involves biological growth and bone remodeling by any means, in living systems has been applied clinically and experimentally to orthopedic fracture repair, the regulation of orthodontic tooth movement, epiphyseal cartilage regeneration, etc. On the other hand, recent numerous research data available show apparently that the mandibular condyle has the characteristics of growth center as well as growth site. In addition, there exists a considerable difference of opinion as to the role of external pterygoid muscle in condylar growth. In view of these evidences, this. experiment was performed to investigate the effect of the galavic current on the growth of the mandible and condyle for elucidating the nature of condylar growth. The bimetallic device was composed of silver and platinum electrode connected with resistor (3.9 Mohm), which was expected to produce galvanic current of 23.6 nA according to the galvanic principle. The 25 Sprague-Dawley rats were divided into two group, 2 week group comprising 8 animals exposed to satanic current for 2 weeks and 3 control animals not exposed for 2 weeks, 4 week group comprising 10 animals in experimental group and 4 animals in control group applied for 4 weeks respectively. The experimental rats were subjected to application of the galvanic current invasively to codylar head surface and the control groups with sham electrode. On the basis of anatomic and histologic data from the mandibular condyle of experimental and control group, the following results were obtained. 1. After 2 weeks, there was no increase of mandibular size in experimental group over that of the control group. 2. After 4 weeks, the size of the condylar head was larger in experimental group than that of the control. 3. In 2 week group, the thickness of the mitotic compartment and hypertrophic chondroblastic layer was increased in experimental group. 4. In 4 week group, the number and the size of the hypertrophic chondroblasts were increased significantly on experimental group over that of the control group. 5. The application of the satanic current caused an increase in chondrocytic hypertrophy and intercellular matrix in both groups.

악관절 기능장애 환자의 하악과두 위치에 관한 단층 및 두부방사선 계측학적 연구 (TOMOGRAPHIC AND CEPHALOMETRIC STUDY OF CONDYLAR POSITION IN TEMPOROMANDIBULAR DISORDER PATIENTS)

  • The author analysed tomographic and cephalometric radiographs of 82 temporomandibular joints from 41 symptomatic patients and 40 temporomandibular joints from 20 asymptomatic young adults. The results were as follows; 1. The mean condylar angulation in control group and patient group was 21.72±6.48° 20.13±9.14° respectively and there was no significant difference between two groups. 2. The mean depth of cut was 6.63±0.38㎝ in control group and 6.57±0.46㎝ in patient group. 3. Mean height and width of condylar head were 6.66±1.83㎜, 12.42±0.49㎜ in control group and 6.22±1.36㎜, 11.93±l.92㎜ in patient group. 4. The mean height of articular fossa was 10.20±2.04㎜ in control group and 9.89±1.98㎜ in patient group. The mean width of articular fossa was 21.08±2.08㎜ in control group and 21.24±3.03㎜ in patient group. 5. In centric occlusion the superior joint space was largest (4.15±0.93㎜), followed by the posterior joint space (2.99±0.97㎜) and the anterior joint space (2.70±0.73㎜) in control group. The superior joint space (3.47±1.31㎜) and posterior joint space (3.47±7.07㎜) were same in patient group. There was significant difference in left superior joint spaces between two groups (p<0.05). 6. The condylar position in articular fossa was displaced anteroinferiorly (0.99±3.65㎜ anteriorly, 1.75±1.01㎜ inferiorly) in control group and posteroinferiorly (3.20±4.69㎜ posteriorly, 1.25±1.87㎜ inferiorly) in patient group with 1 inch opening. In maximum opening, it was displaced anteroinferiorly (6.09±3.55㎜ anteriorly, 1.38±2.47㎜ inferiorly) in control group and anteroinferiorly (1.70±5.96㎜ anteriorly, 1.37±1.85㎜ inferiorly) in patient group. There was significant difference in anteroposterior position of both condyles with 1 inch opening and maximum opening between two groups (p<0.01). 7. The mean inclination of upper central incisor and the posterior inclination of articular eminence in control group was 65.60±6.04° 58.88±9.18° in control group, and 67.14±8.41°, 59.70±9.08° in patient group respectively. There was no significant correlation between two groups.

Bony window approach for a traumatic bone cyst on the mandibular condyle: a case report with long-term follow-up

  • Traumatic bone cyst (TBC) occurs preferentially on the mandibular symphysis and body, but rarely on the mandibular condyle. When TBC occurs in the condylar area, it can usually be related with or misdiagnosed as a temporomandibular joint disorder. A 15-year-old female patient visited the Temporomandibular Joint Clinic with a 5-year history of pain and noise localized in the left temporomandibular joint. On imaging, a well demarked oval-shaped radiolucent lesion was observed on the left condyle head. The patient underwent cyst enucleation and repositioning of the bony window on the lateral cortex of the affected condyle head under the impression of subchondral cyst or TBC; however, no cystic membrane was found. The bone defect resolved and showed no recurrence on the serial radiographic postoperative follow-up for 43 months after surgery.

악관절원판의 전방변위에 따른 관절간극의 변화 (CHANCE OF JOINT SPACE ACCORDING TO ANTERIOR DISC DISPLACEMENT OF TMJ)

  • Objective: Disc displacement may affect the joint space narrowing between condylar head and glenoid fossa. This study was designed to evaluate the correlation between the joint space change and the severity of anterior disc displacement. Materials and Methods: Two hundreds temporomandibular joints MR images of TMD patients(170 joints) and asymptomatic volunteers(30 joints) were evaluated for this purpose. Anterior disc displacement was divided into 3 stages(normal, little to mild, and moderate to severe displacement) based on sagittal images, then joint spaces were measured at medial, central and lateral parts of condyle head on coronal MR images, respectively The joint spaces of 3 groups divided according to the severity of anterior disc displacement were compared. Results: The reduction of joint space was related to the severity of the anterior disc displacement at lateral, medial side and especially at center Conclusion: The temporomandibular joint space was affected by the severity of the anterior disc displacement.

CEPHALOMETRIC CHARACTERISTICS OF OPEN-BITE CASES WITH DEGENERATIVE JOINT DISEASE(DJD) OF TMJ

  • The purpose of this study is to investigate the cephalometric characteristics of the open-bite patients with DJD of TMJ. The DJD open-bite cases were compared with normal samples and Class II open-bite cases with normal TMJ respectively. Twenty three open-bite patients with bilateral DJD of TMJ($13.9\~35.3$ yens old, Group I) were selected from the Department of Orthodontics, SNUDH. Group ll consisted of thirteen Class II open-bite cases($13.2\~27.4$ years old) with no TMD signs/symtoms and good condylar shapes. Group III samples were the forty eight healthy dental students who have Class I molar relationships with no history of orthodontic treatment, good facial balance and no TMD symptoms($20.0\~26.8$ years old). First, sixty measurements in the lateral cephalometric radiographs and analysis of variance(P<0.05, Scheffe) were used to compare these three groups. The seven measurements showed significant difference(p<0.05) between Group I and Group II. After analysis of variance, six of them were used for the discriminant analysis(Wilks' stepwise analysis) and the discrminant function for Group I/Group II was obtained. The results and conclusions were as follows : In most of the measurments, Group I and Group II showed the same skeletal and dental characteristics. But seven of the sixty measurements(FH-PP angle, SNB, FH-ArGo angle, articulare angle, genial angle, upper gonial angle and Ar-Go length) were significantly different(p<0.05) between Group I and Group II. These differences may be explained by the fact that in DJD cases the mandible rotated backward due to the shortening of the ramus following the degenerative destruction of condylar head and its surrounding structures. The resulting discriminant function was : $D={-0.120X}_1+{0.066X}_2+{0.144X}_3-{0.058X}_4+2000,\;where\;X_1=ArGo\;length(mm),\;X_2=SArGo\;angle(degree),\;X_3=FH-PP\;angle(degree),\;X_4=Gonial\;angle(degree)$. Mean of the group centroids was -0.555 and percent of the 'grouped' cases correctly classified was $88.89\%$.
