Effective strategy in the treatment of aortobronchial fistula with recurrent hemoptysis

  • Aortobronchial fistula (ABF) involves the formation of an abnormal connection between the thoracic aorta and the central airways or the pulmonary parenchyma and is associated with an increased risk of mortality. An ABF typically manifests clinically with symptoms of hemoptysis, and currently, there is a lack of defined guidelines for its treatment. Here, we report the cases of two patients who suffered from recurrent hemoptysis due to ABF with pseudoaneurysm. We propose that removal of the aorta with concomitant lung resection and coverage of the aorta using the pericardial membrane is a definite treatment to lower recurrence of ABF and persistent infection.

pH-Dependent Electrochemical Behavior of N-Monosubstituted-4,$4^{\prime}$-Bipyridinium Ions

  • The pH-dependent reduction behavior of N-monosubstituted-4,4'-bipyridinium ions ($RBPY^+: R=methyl(C_1)$; benzyl; n-octyl; n-dodecyl) has been investigated by electrochemical and spectroelectrochemical techniques. At acidic condition, $RBPY^+$ is protonated and the protonated species are reduced by two consecutive one-electron processes. The $2e^-$ reduced species undergoes a chemical reaction with $H^+$. The second-order rate constant $(k_H)$ of the homogeneous chemical process is $(3.7{\pm}0.3){\times}10^3M^{-1}s^{-1}$ for the two electron reduction product of $C_1BPY^+$. At high pH, the electrode reduction of $RBPY^+$ is one-step $2e^-$ transfer process with concomitant addition of $H^+$, which is confirmed by cyclic voltammetric study using a microdisk electrode.

Hydrolysis of p-Nitrophenyl Acetate and p-Nitrophenyldiphenyl Phosphate in Micellar Solution by N-Chloro Compounds : Involvement of Counter Ions in Micellar Catalysis

  • Hydrolysis of p-nitrophenyl acetate (PNPA) and p-nitrophenyldiphenyl phosphate (PNPDPP) by N-chloro compounds in micellar solution were studied. N,N'-dichloroisocyanuric acid sodium salt (DCI) in cetyltrimethylammonium chloride (CTACl) micellar solution gave pseudo first-order kinetics. But, DCI in cetyltrimethylammonium bromide (CTABr) micellar solution showed typical series first-order kinetics - fast hydrolysis of the esters and concomitant slow decay of the hydrolyzed product, p-nitrophenolate. The hydrolysis rate was decreased as the hydrophobicity of N-chloro compounds was increased, which is the opposite trend to the usual bimolecular micellar reaction. This curious behavior of the N-chloro compounds in the catalytic hydrolysis of PNPA and PNPDPP in a cationic micellar system can be best explained by participation of counter ions of the surfactants during hydrolysis.

  • Pharmacokinetic interaction of theophylline with pefloxacin following i.v. administrations was investigated in rabbits. Pefloxacin was coadministrated at doses of 10 and 20 mg/kg or previously administered for 6 days 10 and 20 mg/kg. Plasma concentration and AUC of theophylline were increased significantly (p<0.05) and the renal clearance $(CL_r)$, total body clearance $(CL_r)$ and the volume of distribution $(Vd_{ss})$ were decreased significantly (p<0.005) by the pretreatment. It demonstrates that adjustment of dosage regimen of theophylline should be considered when concomitant administration of pefloxacin is prescribed.

Food allergies and food-induced anaphylaxis: role of cofactors

  • Food allergies and food-induced anaphylaxis are important health problems. Several cofactors modulating the onset of anaphylaxis have been identified. In the presence of cofactors, allergic reactions may be induced at lower doses of food allergens and/or become severe. Exercise and concomitant infections are well-documented cofactors of anaphylaxis in children. Other factors such as consumption of nonsteroidal anti-inflammatory drugs, alcohol ingestion, and stress have been reported. Cofactors reportedly play a role in approximately 30% of anaphylactic reactions in adults and 14%-18.3% in children. Food-dependent exercise-induced anaphylaxis (FDEIA) is the best-studied model of cofactor-induced anaphylaxis. Wheat-dependent exercise-induced anaphylaxis, the most common FDEIA condition, has been studied the most. The mechanisms of action of cofactors have not yet been fully identified. This review aims to educate clinicians on recent developments in the role of cofactors and highlight the importance of recognizing cofactors in food allergies and food-induced anaphylaxis.

Clinical analysis of neck node metastasis in oral cavity cancer

  • Objectives: The purpose of this study was to evaluate the neck node metastasis pattern and related clinical factors in oral cavity cancer patients. Materials and Methods: In total, 76 patients (47 males, 29 females) with oral squamous cell carcinoma (OSCC) who had no previous malignancies and were not undergoing neoadjuvant concomitant chemoradiotherapy or radiotherapy were selected for analysis. Results: Occult metastases were found in 8 of 52 patients with clinically negative nodes (cN0, 15.4%). Neck node metastases were found in 17 patients (22.4%). There was a statistically significant relationship between neck node metastasis and T stage (P=0.014) and between neck node metastasis and distant metastasis (Fisher's exact test, P=0.019). Conclusion: Neck node metastasis was significantly related to tumor size and distant metastasis during follow-up.

  • Acute lateral ankle sprain, which is the most common musculoskeletal injury, can be treated effectively with appropriate evidence-based initial care using PRICE (protection, rest, ice, compression, and elevation) and functional rehabilitation. Many systemic reviews reporting a high-level of evidence supporting the clinical usefulness and necessity of primary surgical repair for acute lateral ankle sprain have been insufficient. Regardless of the severity of ligament complex injuries, the surgical treatment for acute lateral ankle sprain without concomitant pathologies is not recommended and should be considered only in young professional athletes with complete ligament rupture (grade III) and severe instability.

  • Synovial chondromatosis is an uncommon disorder characterized by cartilaginous proliferation within the synovial membrane of the articular joint. Smaller joints are rarely affected and it may be progressed to osteochondromatosis after ossification or calcification of metaplastic cartilage. It is commonly presented in the third to fourth decade of life, but rarely presented in adolescence. We report a unique case of synovial osteochondromatosis of the subtalar joint in 14-year-old baseball player. Arthroscopic removal of loose body and complete excision of the osteochondral mass with concomitant synovectomy resulted in satisfactory outcome without recurrence at final follow-up.

  • Mechanism of electrochemical polymerization of pyrrole (Py) on a Pt electrode in acidic aqueous solutions was studied by means of potentiostatic measurements, cyclic voltammetry and chronopotentiometry. Pyrrole molecule appeared to be initially oxidized via two-electron transfer step to produce oxidized pyrrole ion ($Py^+$), which was coupled with a non-oxidized pyrrole to yield a dimerized species, Py-Py. The Py-Py thus formed was further oxidized again via two-electron transfer step, which was followed by coupling with non-oxidized monomer and by concomitant expulsion of a $H^+$. Then the latter chain extension process was repeated. The chain extension and polypyrrole oxidation reactions occurred competitively.

Atomic Absorption Spectrophotometric Determination of Trace Cadimuim after Preconcentration by Extracting Its 8-Hydroxyquinoline Complex into Molten Benzophenone

  • A sensitive method for the determination of trace cadmium after the preconcentration by extracting its 8-hydroxyquinoline complex into a molten benzophenone was developed. Several experimental conditions such as the pH of solution, the amounts of 8-hydroxyquinoline and benzophenone, stirring time, and standing time were optimized. Trace cadmium in 100 mL water sample was chelated with 2.5 mL of 0.001 M 8-hydroxyquinoline at pH 8.0. After 0.07 g benzophenone was added, the solution was heated to about 70 ℃ and stirred vigorously for 1 minute to dissolve the complex quantitatively in a molten benzophenone, and stood for 30 minutes to reproduce the microcrystalline benzophenone. The benzophenone containing Cd-8-hydroxyquinoline complex was filtered and dissolved in acetone. Cadmium was determined by a flame atomic absorption spectrophotometry. The interfering effects of diverse concomitant ions were investigated and eliminated. This method could be applied to natural water samples and the recovery of more than 90% was obtained in the real samples.
