Bioimpedence to Assess Breast Density as a Risk Factor for Breast Cancer in Adult Women and Adolescent Girls

  • Although high mammographic density is one of the strongest predictors of breast cancer risk, X-ray based mammography cannot be performed before the recommended screening age, especially not in adolescents and young women. Therefore, new techniques for breast density measurement are of interest. In this pilot study in Guam and Hawaii, we evaluated a radiation-free, bioimpedance device called Electrical Breast Densitometer$^{TM}$ (EBD; senoSENSE Medical Systems, Inc., Ontario, Canada) for measuring breast density in 95 women aged 31-82 years and 41 girls aged 8-18 years. Percent density (PD) was estimated in the women's most recent mammogram using a computer-assisted method. Correlation coefficients and linear regression were applied for statistical analysis. In adult women, mean EBD and PD values of the left and right breasts were $230{\pm}52$ and $226{\pm}50{\Omega}$ and $23.7{\pm}15.1$ and $24.2{\pm}15.2%$, respectively. The EBD measurements were inversely correlated with PD ($r_{Spearman}=-0.52$, p<0.0001); the correlation was stronger in Caucasians ($r_{Spearman}=-0.70$, p<0.0001) than Asians ($r_{Spearman}=-0.54$, p<0.01) and Native Hawaiian/Chamorro/Pacific Islanders ($r_{Spearman}=-0.34$, p=0.06). Using 4 categories of PD (<10, 10-25, 26-50, 51-75%), the respective mean EBD values were $256{\pm}32$, $249{\pm}41$, $202{\pm}46$, and $178{\pm}43{\Omega}$ (p<0.0001). In girls, the mean EBD values in the left and right breast were $148{\pm}40$ and $155{\pm}54{\Omega}$; EBD values decreased from Tanner stages 1 to 4 ($204{\pm}14$, $154{\pm}79$, $136{\pm}43$, and $119{\pm}16{\Omega}$ for stages 1-4, respectively) but were higher at Tanner stage 5 ($165{\pm}30{\Omega}$). With further development, this bioimpedance method may allow for investigations of breast development among adolescent, as well as assessment of breast cancer risk early in life and in populations without access to mammography.

Assessing the Impact of Climate Change on Water Resources: Waimea Plains, New Zealand Case Example

  • Climate change is impacting and will increasingly impact both the quantity and quality of the world's water resources in a variety of ways. In some areas warming climate results in increased rainfall, surface runoff, and groundwater recharge while in others there may be declines in all of these. Water quality is described by a number of variables. Some are directly impacted by climate change. Temperature is an obvious example. Notably, increased atmospheric concentrations of $CO_2$ triggering climate change increase the $CO_2$ dissolving into water. This has manifold consequences including decreased pH and increased alkalinity, with resultant increases in dissolved concentrations of the minerals in geologic materials contacted by such water. Climate change is also expected to increase the number and intensity of extreme climate events, with related hydrologic changes. A simple framework has been developed in New Zealand for assessing and predicting climate change impacts on water resources. Assessment is largely based on trend analysis of historic data using the non-parametric Mann-Kendall method. Trend analysis requires long-term, regular monitoring data for both climate and hydrologic variables. Data quality is of primary importance and data gaps must be avoided. Quantitative prediction of climate change impacts on the quantity of water resources can be accomplished by computer modelling. This requires the serial coupling of various models. For example, regional downscaling of results from a world-wide general circulation model (GCM) can be used to forecast temperatures and precipitation for various emissions scenarios in specific catchments. Mechanistic or artificial intelligence modelling can then be used with these inputs to simulate climate change impacts over time, such as changes in streamflow, groundwater-surface water interactions, and changes in groundwater levels. The Waimea Plains catchment in New Zealand was selected for a test application of these assessment and prediction methods. This catchment is predicted to undergo relatively minor impacts due to climate change. All available climate and hydrologic databases were obtained and analyzed. These included climate (temperature, precipitation, solar radiation and sunshine hours, evapotranspiration, humidity, and cloud cover) and hydrologic (streamflow and quality and groundwater levels and quality) records. Results varied but there were indications of atmospheric temperature increasing, rainfall decreasing, streamflow decreasing, and groundwater level decreasing trends. Artificial intelligence modelling was applied to predict water usage, rainfall recharge of groundwater, and upstream flow for two regionally downscaled climate change scenarios (A1B and A2). The AI methods used were multi-layer perceptron (MLP) with extended Kalman filtering (EKF), genetic programming (GP), and a dynamic neuro-fuzzy local modelling system (DNFLMS), respectively. These were then used as inputs to a mechanistic groundwater flow-surface water interaction model (MODFLOW). A DNFLMS was also used to simulate downstream flow and groundwater levels for comparison with MODFLOW outputs. MODFLOW and DNFLMS outputs were consistent. They indicated declines in streamflow on the order of 21 to 23% for MODFLOW and DNFLMS (A1B scenario), respectively, and 27% in both cases for the A2 scenario under severe drought conditions by 2058-2059, with little if any change in groundwater levels.

  • Changes in the soil physical property and the topographic condition derived from agricultural activities like as farming activities, land clearance and cutting down resulted in environmental and economic problems including the outflow of nutrient from farms and the water pollution. Several theories on the soil conservation have been developed and reviewed to protect soil erosion in the regions having a high risk of erosion. This study was done using the USLE model developed by Wischmeier and Smith (1978), and model for the slope length and steepness made by Desmet and Govers (1996), and Nearing (1997) to evaluate the potential of the soil erodibility. Therefore, several results were obtained as follows. First, factors affecting the soil erosion based on the USLE could be extracted to examine the erosion potential in farms. Soil erodibility (K), slope length (L), and slope steepness (S) were used as main factors in the USLE in consideration of the soil, not by the land use or land cover. Second, the soil erodibility increased in paddy soils where it is low in soil content, and the very fine sandy loam exists. Analysis of the slope length showed that the value of a flat ground was 1, and the maximum value was 9.17 appearing on the steep mountain. Soil erodibility showed positive relationship to a slope. Third, the potential soil erodibility index (PSEI) showed that it is high in the PSEI of the areas of steep upland and orchard on the slope of mountainous region around Dokjigol mountain, Dunji mountain, and Deummit mountain. And the PSEI in the same land cover was different depending on the slope rather than on the physical properties in soil. Forth, the analysis of land suitability in soil erosion explained that study area had 3,672.35ha showing the suitable land, 390.88ha for the proper land, and 216.54ha for the unsuitable land. For unsuitable land, 8.71ha and 6.29ha were shown in fallow uplands and single cropping uplands, respectively.

시추공 텔리뷰어 및 BIPS의 영상자료 해석을 통한 파쇄매질의 투수율텐서 계산 프로그램 개발 및 현장 적용성 평가 (Program Development to Evaluate Permeability Tensor of Fractured Media Using Borehole Televiewer and BIPS Images and an Assessment of Feasibility of the Program on Field Sites)

  • 시추공 텔리뷰어 및 BIPS의 공내 검층자료를 이용하여 파쇄매질이 나타내는 투수율텐서를 수치적으로 계산하는 프로그램을 개발하였다. 본 프로그램은 검층자료의 분석을 통해 제공되는 불연속면의 방향과 두께등의 기하학적인 정보를 입력자료로 사용하며, EPM 모델에 근거한 관계식을 적용하여 투수율텐서를 구성하는 9개 성분의 상대적인 값을 계산한다. 거제시 일운면에 위치하는 연구지역 시추공을 대상으로 수리간섭 시험을 실시하여 암반의 이방성을 추정하였으며 BIPS 검층자료로부터 프로그램에 의하여 산정된 이론값과 비교, 분석하여 프로그램의 현장 적용성을 평가하였다. 수리간섭시험을 통하여 추정한 연구지역 암반의 수평적인 이방성비와 주축방향은 2.8 및 $N77^{\circ}CE$인 반면, 프로그램을 적용하여 산정한 값은 3.0 및 $N63^{\circ}CE$로 나타냈다. 두 결과가 불일치한 이유로는 수리간섭시험 해석결과의 신뢰성, EPM모델 및 3승법칙의 현장 적용성, 프로그램에서 가정한 단열의 크기-두께 관계식의 단순화 등의 문제점을 들 수 있다. 개발된 프로그램은 투수율텐서를 계산하는 초기모델로서 단열면의 거칠기, 충전물질의 충전정도 및 수리특성을 반영하는 수리인자의 적용을 통한 개선이 요구되며, 암반의 투수성이 양호하며 시추공 간의 거리가 가까운 연구지역에서 시추공간 수압시험을 실시할 때 3차원의 현장적용성 평가가 가능할 것으로 판단된다.

전정감각, 고유감각 중심의 감각통합중재가 발달장애 아동의 공간 지각력과 시-운동 협응력에 미치는 영향 (The Effect of Vestibular-Proprioceptive Based Sensory Integration Intervention on Spatial Relations and Visual-Motor Coordination in Children with Developmental Disabilities)

  • 목적 : 본 연구를 통하여 발달장애 아동에게 적용한 전정감각, 고유감각 중심의 감각통합 중재가 공간 지각력, 시-운동 협응력에 어떠한 영향을 미치는지 알아보고자 한다. 연구방법 : 본 연구는 발달 장애 아동 만 5~6세 아동 3명을 대상으로 개별실험연구방법(single subject research design) 중 ABA' 설계를 사용하였다. 매주 3회기씩 6주간 30분씩 총 18회기를 진행하였다. 기초선 3회기와 재기초선 3회기는 중재 없이 공간 지각력과 시-운동 협응을 평가하였고 중재기 12회기는 전정감각, 고유감각 위주 감각통합 중재를 실시하였다. 중재의 효과를 알아보기 위한 매회기 측정으로는 전산화 인지 프로그램(CoTras-C)을 이용하였으며, 공간지각력 평가를 위한 퍼즐하기, 시-운동 평가를 위한 미로 찾기를 실시하였다. 연구의 사전, 및 사후 평가는 카우프만 아동용 지능 검사(Kaufman Assessment Battery for Children; K-ABC II)와 한국판 시지각 발달 검사(Korean-Developmental Test of Visual Perception-2: K-DTVP-2)의 하위평가를 이용하였다. 결과 : 발달장애 아동에게 전정감각, 고유감각 중심의 감각통합중재를 적용하기 전보다 후에 공간지각력과 시-운동 협응력 점수에 유의한 차이가 있었고, 세 아동 모두 통계적으로 유의한 차이가 있었다. 결론 : 전정, 고유감각 중심의 감각통합 치료가 발달장애 아동의 공간지각력과 시-운동 협응력에 유의미한 영향을 미쳤다.

Residual Multi-Dilated Recurrent Convolutional U-Net을 이용한 전자동 심장 분할 모델 분석 (Fully Automatic Heart Segmentation Model Analysis Using Residual Multi-Dilated Recurrent Convolutional U-Net)

  • 본 논문에서는 딥 러닝 기반의 전-자동 심장 분할 알고리즘을 제안한다. 본 논문에서 제안하는 딥 러닝 모델은 기존 U-Net에 residual recurrent convolutional block과 residual multi-dilated convolutional block을 삽입하여 성능을 개선한 모델이다. 모델의 성능은 테스트 데이터 세트를 전-자동 분할한 결과와 영상의학 전문가의 수동 분할 결과를 비교하여 분석하였다. CT 영상에서 평균 96.88%의 DSC, 95.60%의 precision과 97.00%의 recall 결과를 얻었다. 분할된 영상은 3차원 볼륨 렌더링 기법을 적용하여 시각화한 후 관찰하여 분석할 수 있었다. 실험 결과를 통해 제안된 알고리즘이 다양한 심장 하부 구조를 분할하기에 효과적인 것을 알 수 있었다. 본 논문에서 제안하는 알고리즘이 전문의 또는 방사선사의 임상적 보조역할을 수행할 수 있을 것으로 기대한다.

프리스트레스트 콘크리트 깊은 보의 전단거동에 관한 해석적 연구 (Analytical Study on the Shear Behavior of Prestressed Concrete Deep Beams)

  • 이 연구는 프리스트레스트 콘크리트 깊은 보의 전단거동을 파악하고 합리적이면서 경제적인 설계기준의 개발을 위한 자료를 제공하는데 그 목적이 있다. 정확하고 올바른 평가를 위하여 신뢰성 있는 비선형 유한요소해석 프로그램을 이용하였다. 사용된 프로그램은 철근콘크리트 구조물의 해석을 위한 RCAHEST이다. 재료적 비선형성에 대해서는 균열 콘크리트에 대한 인장, 압축, 전단모델과 콘크리트 속에 있는 철근모델을 조합하여 고려하였다. 사용된 부착 또는 비부착 텐던요소는 유한요소법에 근거하며 프리스트레스트 콘크리트 부재의 콘크리트와 텐던의 상호작용을 구현할 수 있다. 이 연구에서는 프리스트레스트 콘크리트 깊은 보의 전단거동을 파악하기 위해 제안한 해석기법을 신뢰성 있는 다른 연구자의 실험 결과와 비교하여 그 타당성을 검증하였다.

  • 우리는 수 과학 영재 고등학생들의 정보과학 창의성 향상을 위한 효과적인 정보과학 교수-학습 방법을 제안하였다. 학생들은 현실적 응용문제를 해결하는 정보과학 팀 프로젝트의 수행을 통하여 창의적 문제해결 능력과 팀워크 능력을 향상할 수 있다. 이러한 교육목표를 효과적으로 달성하기 위하여 기본과정과 응용과정으로 구성되는 2단계 정보과학 수업 방법과 체계적 프로젝트 수행 단계를 설계하고, 소규모 협력 학습 전략과 입체적 평가 방안을 적용하였다. 논문에서 제안한 프로젝트 기반 교수-학습 방법의 학습 효과를 분석한 결과, 정보과학 창의성의 지적 정의적 및 사회적 영역에서의 효과적인 교육 목표 달성을 확인할 수 있었다.

3D 스캐너와 3D 프린터를 활용한 손목보호대 개발 (The Development of a Wrist Brace using 3D Scanner and 3D Printer)

  • The purpose of this study was to develop a customized wrist brace using 3D scanner and 3D printer. This study included in-depth interviews with people who had wrist pain and ever used wrist braces. The wrist brace has been designed and modeled by the CAD program after 3D scanning the wrist of subjects. Based on the results of the in-depth interviews on wrist brace users and design investigation in the market, a prototype of the wrist brace has been created. The wrist brace does not compress the hand and is easy to put on. In addition, it is adjustable to the wrist of users, allowing them to move his or her wrist without any restrictions. A computer-modeling program produced solid files for the design of the wrist brace after 3D scanning ten subjects' hands. It features a lattice-patterned surface, a velcro adjustment and trimming to smoothen the surface. PLA filaments were used to 3D print the wrist brace. As for the assessment, the wrist brace has been evaluated to be easy to put on and adjust to user's wrist. However, when it comes to appearance and comfortability, it was rated 3.0 or slightly above. This is because 3D printing materials are firm and not flexible compared to other materials such as neoprene or stretchable bands.

갑상샘 유두암종의 세포진단에서 형태학적 계측의 분석 (Morphometric Analysis for Cytological Diagnosis of Thyroid Papillary Carcinoma)

  • The diagnosis of papillary thyroid cancer is generally based on the findings of intranuclear cytoplasmic inclusions and nuclear grooves. Although anisokaryosis and poikilokaryosis, in papillary thyroid cancer, are not distinct when compared to other cancers, cytological examination can provide useful preoperative information. Our study evaluated the diagnostic role of computer-assisted image analysis for the pre-surgical assessment of papillary thyroid carcinoma. Thyroid aspirates from twenty female patients who were histologically confirmed to have both papillary carcinoma and benign nodules were studied. Different populations of 50 benign cells and 50 malignant cells were analyzed. Five morphometric parameters were selected for analysis: nuclear area, perimeter, maximum length, maximum width and intensity standard variation. The values obtained for papillary carcinomas were higher than the surrounding benign nodules as follows: nuclear area 63.5 vs. 36.1 (p=0.000), nuclear perimeter were 29.4 vs. 22.0 (p=0.000), maximum length 9.6 vs. 7.1 (p=0.000), maximum width 8.2 vs. 6.3 (p=0.000), the ratio between maximal length and maximal width 1.16 vs. 1.13 (p=0.000), the standard variation of intensity 14.9 vs. 15.9 (p=0.101) respectively. Therefore, morphometric information can be helpful for the differential cytological diagnosis of papillary thyroid carcinoma.
