Suicide is a major problem in Korean health care and a serious social problem. In Korea, 12,463 people (24.3 per 100,000) lost their lives due to suicide in 2017. Although the government has established three National Comprehensive Plan of Suicide Prevention (2004, 2009, 2016), and National Action Plan of Suicide Prevention (2018), the suicide rate is still high. The suicide rate of the elderly is especially high. This is due to the economic vulnerability of the elderly in Korea. Therefore, in order to prevent suicide in Korea, mental health care approach and social welfare approach should be integrated. The intervention of preventing suicide of suicide attempters should include social welfare services as well as mental health program and should be based on community. There are many health problems, including prevention of suicide, which can not be solved only by the efforts of health care. Many health problems are social problems and the integrated approach is needed to solve them. In order to solve many health care problems and improve health, integrated approach of health, social science, and humanities is needed.
본 연구는 장애노인의 실태를 통해 제 특성을 파악하고 전반적인 삶의 만족도에 어떠한 요인이 영향을 미치는지 검증하여 장애노인의 삶의 질 향상을 위한 제언을 하는데 목적이 있다. 이를 위하여 한국보건사회연구원이 수행한 2014년 장애인실태조사 원 자료를 활용하였으며 분석대상은 65세 이상 장애노인 3,181명이다. 자료 분석을 위해 기술통계를 제시하고 삶의 만족도 영향요인 분석을 위해 로지스틱회귀분석을 수행하였다. 분석결과, 성별, 연령, 배우자유무(인구학적 요인), 가구소득, 주택소유여부, 수급자여부, 공적연금 가입여부, 사회적 차별정도(사회 경제적 요인), 장애정도, 주관적 건강상태, 일상생활 도움필요정도, 일상생활 도와주는 사람유무, 도움충분여부, 혼자외출가능여부, 교통수단이용 어려움 여부, 건강검진여부(장애 및 건강요인)가 삶의 만족도에 유의미한 영향을 미치는 요인이었다. 이러한 분석결과에 근거하여 본 연구에서는 장애노인 중 취약계층인 독거장애노인에 대한 돌봄서비스 등 집중적인 관심과 지원, 공 사적연금 등 다양한 소득보장을 위한 제도적 보완, 장애차별에 대한 사회적 인식 및 환경개선, 보건의료서비스 지원 및 건강관리체계 마련, 장애인복지와 노인복지의 통합적 서비스 제공, 보건과 복지의 통합적 서비스 제공 등을 제언하였다.
On September 12, 2018, President Jae-In Moon announced the Comprehensive Plan for Lifelong Care for People with Developmental Disabilities, with representatives from the associated government branches (Ministry of Health and Welfare, Ministry of Education, and Ministry of Employment and Labor) in attendance. The goals of this plan are to provide health, medical, rehabilitative, special education, and social welfare services according to the life-stages of the affected individuals; to reduce parental pressure; to promote social interventions; and to enhance community-level participation in order to create a 'welfare society in harmony.' However, in order for the plan to succeed, additional efforts must be made in the following areas. First, an epidemiological survey is needed to understand the scale, prevalence, and incidence of developmental disabilities and to establish an evidence base to support policy development. Second, accurate definitions of developmental disabilities must be established in order to avoid policy discrimination based on impairment type and age. Third, personal evaluations to assess disabled individuals' unmet needs and customized service designs to deliver those needs are required. Fourth, the plan must fulfill the goals of accessibility and fairness that the government intends to provide. Fifth, the government should consider an integrated financial support system and to propose a detailed plan for monetary distributions. Finally, an integrated system that links health, medical, employment, educational, and welfare services must be constructed.
Purpose: The purpose of this study was to follow-up the frailty of the old who received home health care by Registered Nurse in Public Health Center over 8 years. Methods: We used the second wave data which was a comprehensive longitudinal data set, Public Health Information System of a public health center located in Seoul from 2010 to 2018. For statistical analysis, a mixed model of repeated measures by R program was used. Results: Frailty (range: 0~31) was getting worse significantly from 5.38 on registration to 6.54 on 4th year, 7.40 on 7th year, 7.69 on 8th year with adjustment for age, sex, economic status, the number of family, and the number diseases. The coefficient of parameters with frailty change was serviced year (β=0.29, p<.001), age (70~79 to 60~69; β=0.98, p=.018) and sex (female to male; β=2.55, p<.001). Conclusion: This study showed that the home visiting health service needs to take attention to aged 70s and over, female. The home health care of public health center need to be extended more practical and effective services in terms of 'community care'and 'ageing in place'.
Considering the characteristics of the Korean family which maintain a close connection with their patients from the moment of their falling sick to hospitalization to discharge, the family is the most important environmental factor of the social supporting system, and is the important object of the client as well as activity system. The medical social work intends to meet the practical needs of aged chronic patients, providing them and their family with a professional human service. The end of this study is to find out the hardships of both the aged chronic patients and their family as well as their needs for the social welfare service, and to search out the way of comprehensive social work service. The summary of the analysis of the survey is as follows: 1. The needs of aged chronic patients are divided into those of the solution of the problems of falling ill, social welfare program and discharge. Those needs arc affected by the various factors of the types of hospitals, the patients' age, the kinds of insurance, and the supporting systems, etc. Accordingly, the assessment of the needs of the patients are asked to be done comprehensively in accordance with the kinds of diseases and social environments. 2. The importance of the family to the aged chronic patients is evident. The family plays a decisive role in the patients' hospitalization and discharge, the family being an important supporting system and making it necessary to take an approach to client system. The family has difficulty in getting connection of community resources, in adapting to social life after the patient's discharge, and in paying the treatment. The family suffers the secondary hardships more than the burden of the treatment expenses. 3. For this reason various interventions are needed to reduce the stress caused by supporting and nursing patients. Thus the social welfare service for the aged chronic patients and their family needs the following prepositions: 1. It is the characteristics of the aged chronic patients that they need continuous care and that the strengths of the patients and their family cannot be too much emphasized, and that comprehensive assessment based on the connection 'with the community and the mutual interchange 'with the environment, is much emphasized. 2. The family of the aged chronic patient is a resources system as well as a client one. 3. Another characteristic of the aged chronic patients is that with the resources connection in mind, it needs an active intervention of social workers in the community. With these prepositions considered, the development of practical social work service for the aged chronic patients is thought urgently needed.
Purpose: A study of the family planning and Maternal-Child Health Services that disciplined itself in primary health care post form 1980 to 2009. Method: Investigation studies family planning in primary health care post and a change process of a Maternal-Child Health Services into case by case until 2009 from 1980. This study of elderly households living conditions to determine the overall status in the housing environment is to understand. To this end, the stage of elderly as a framework to target living consciousness, living space composition, housing Environment composition of elderly households with a comprehensive understanding and analyzes were employed. For this research is a survey conducted on senior citizens(over 60) in Busan, Korea. The major findings are as followings; In the living consciousness, the elderly households showed high perception of dependency but the age of the elderly increases, depends on the parent to the child. Also depending on the aging of the elderly considered health more important than economic conditions. The requirements for living space composition, most respondents prefer to stay in the same place where they currently reside. Move into the new housing was also identified. It was recognized that elderly households not only required living in co-residential space with their families, but also the necessity for individual space. The home environments composition, health care and social welfare support services appears overall the elderly of stage. The age of the elderly increases, a strong demand for care and support service were identified. Health activities in leisure and social activities and social activities, as age increased mainly done by the convenience of public transportation has been identified that are important.
Since the establishment of health centers in the 1960s, the centers have been played an important role in providing basic health care for the people. Although the health centers made a great effect to prevent diseases and promote the health status of the people for the last three decades, the function of health centers should be strengthened to meet the health care need of individual, family and community. Over the last ten years, there have been great changes and developments in health related environments, such as population size and age, rapid urbanization, up-grading of the educational level, increase of income, health care demand for promotive health care measures and practical measures for chronic diseases and also practicing healthy life. According to the great changes in health related environments, the health centers should be reformed. The following policy options are recommended as a summary; First, the function of health centers should be converted from providing basic health services into promotive and preventive health care services, to meet changing needs of people. Second, the health center personnel should be reinforced for their competency to provide a qualitative services to people and also the operation of health center should be reactivated. Third, a close linkage of health centers with the private sector is an essential requirement for the operation of the health care delivery system within a health district in order to improve the health status of people. Fourth, type of manpower mix, scope of organization and health care program should be varied, based on the health care needs of people, geographical characteristics and size of population etc. Fifth, a comprehensive health care delivery system should be developed, for maintaining healthy life style of people and also the health and welfare services should be integrated in order n ensure an effective service.
Objectives: The purpose of this study was to verify a change for family nursing phenomena and satisfaction of clients of vulnerable families in an urban community. Methods: The study subjects were 711 families, randomly selected, who had chronic diseases(arthritis diabetes, stroke, hypertension, mental disease, cancer, dementia) with basic social welfare services from the nation and had disabled persons in an urban community, South Korea. The instruments used were the family nursing phenomena in Korea by ICNP and client satisfaction. Client satisfaction consisted of client satisfaction on home visiting nurses(4 Likert scales) and home visiting services(3 Likert scales). Results: The average visiting number is 3.82. The service number of education and counseling is 3.16, patient and symptom management 3.08, assessment and diagnosis 3.08, test 2.02, medication service 1.71회, dressing 1.01, referral to social welfare institute 1.00회, referral to medical service institute 0.21. In both, pre home visiting and post home visiting, the highest rated phenomenon was the 'lack of social support system' and following that 'deficit of financial management skill and support'. 'lack of family interaction in community', and 'social isolation' 'unhealthy life style' and 'inadequate care management of sick member', in that order. The percentages of phenomena besides 'deficit of financial management skill and support' decreased. The satisfaction level of clients towards the nurses was 3.27 points on a scale of 4, and the nursing services was 2.70 points on a scale of 3. Conclusion: Home visiting nursing services should continue to provide comprehensive healthcare services and support for vulnerable families, in urban communities.
본 연구는 2018년 보건복지부에서 발표한 「독거노인 종합지원대책」을 중심으로 독거노인에 대한 제도적 지원의 문제점 및 해결방안에 관하여 연구하였다. 연구결과는 다음과 같다. 첫째, 맞춤형 독거노인 지원제도를 도입해야 한다. 독거노인의 생활만족도 향상을 위해 가장 기본적인 일상생활 관련 욕구에 맞는 프로그램 개발이 필요하며, 서비스를 연계할 구체적인 방안과 지원체계가 마련되어야 한다. 둘째, 독거노인에 대한 사회적 관심도 증대가 필요하다. 독거노인에 대한 문제해결은 독거노인에 대한 사회적 관심도가 선행되어야 한다. 이를 위해서는 사회적 관계망의 강화가 필요하다. 셋째, 독거노인을 위한 법률 제·개정을 제안한다. 기존 법률의 일부 개정만으로는 한계가 있고, 다양한 노인일자리 유형에 대한 기준과 정의, 노인일자리 사업 전담기관을 포함한 전달체계에 대한 재정비, 근로자성 관련 규정, 노인생산품 우선구매제도 등 개별 법률 제정을 통하여 해결하는 것이 바람직하다. 결론적으로 독거노인에 대한 복지지원은 종합적이고 포괄적이어야 한다. 독거노인의 복지는 우선적으로 맞춤형 돌봄서비스가 시행되어야 하며, 사회적 관심도의 증대를 토대로 해서 독거노인에 대한 사회적 지지를 향시키고, 독거노인을 위해 법제·개정이 능동적이고 선제적으로 이루어져야 한다.
