Development and implementation of national competency standard: based on preventive dentistry and practice (일개대학 National Competency Standard (NCS) 교과목 운영에 관한 연구: 예방치학 및 실습 교과목 중심으로)

  • Objective: The purpose of the study is to investigate the curriculum development and operation based on national competency standard (NCS). Methods: The duty of the dental hygienist was analyzed based on DACUM by ten experts in January, 2011. The duty model of the dental hygienist was inspected after duty analysis. The subjects of choice were preventive dentistry and practice. The satisfaction with the subjects were carried out from March to June, 2015. Results: The duty analysis of dental hygienist by DACUM produced preventive dental treatment(11 tasks), oral health education(3 tasks), comprehensive dental hygiene treatment(6 tasks) and 12 categories(156 tasks). Preventive dental treatment was divided into preventive dentistry and practice, oral health education was changed into oral health education and practice, and comprehensive dental hygiene treatment was replace by comprehensive dental hygiene and practice. The contents of preventive dentistry and practice included outline, learning objective, related knowledge and self evaluation. Professional evaluation required mutual experience and evaluation of the students. The mutual evaluation of the students was $4.61{\pm}0.506$(dental plaque control) and $1.80{\pm}0.316$(tooth brushing). The professional evaluation was $1.73{\pm}0.274$(dental plaque control) and $1.60{\pm}0.322$(tooth brushing)(p<0.01). The satisfaction with preventive dentistry and practice was $4.61{\pm}0.506$(improvement in practical work ability), $4.58{\pm}0.511$(knowledge improvement) and $4.55{\pm}0.572$(NCS educational environment) in order. Conclusions: The operation of NCS curriculum is considered to improve practical work ability and to solve skill mismatch between dental industries and educational training institutions.

Characteristics Analysis on Budget Distribution of Master Plan for Comprehensive Development Projects of Rural Villages (농촌마을종합개발사업의 기본계획 사업비 특성분석)

  • This study analyzed the budget investment plans for the unit-project items(UPI) of 176 project districts for the rural village comprehensive development projects (RVCDP). This study classified the master plan reports of 176 project districts into 88 unit project items in aspect of project management, in order to analyze characteristics of distribution of budget in each project item. Most of all unit project items have similar types of uniform distribution with plus skewness in frequency pattern analysis except the total budget of the project district. This study analyzed the characteristics of budget distribution per province, year, and geographical types of region. Furthermore this paper also analyzed ratio of budget in unit project items to find out distribution pattern of each budget between project items over time. The hierarchical system for UPI of RVCDP consisted of three steps, which are 4 items of the first step on Strength of Rural-urban Exchange & Regional Capability (RURC), Green-income Infrastructure & Facility (GIF), Culture- health-welfare Facility, and Eco-environment & Landscape facility (ELF), 13 items for the second one, and 52 items for the third project items. From the results of the budget investment analysis for 5 years from 2004 to 2008, the budget investment ratios of RURC and ELF have steady state for every year, while GIF in decreasing and ELF in increasing over time. The ratios of UPI on infrastructure were decreased, whereas those on culture, health, and welfare were increased. Portion of tow project items among 52 items, which are community centers for village residents and rural experimental study facility, has 30% of total budget investment. Futhermore, the budget ratios of seven project items showed 50% of total budget. Average value of project budgets for five years was optimized as a type of exponential function in the case of decent array for ranking order.

The Change of Characteristics in Clinical Decision Making in Novice Critical Care Nurses (중환자실 신규간호사의 임상의사결정 특성의 변화)

  • The main purpose of this research is to describe comprehensively the processes of clinical decision making in novice critical care nurses through clinical experience. This research was an exploratory, longitudinal study using a fieldwork approach incorporating "think-aloud" method and in-depth interviews with the study participants. The study participants consisted of 5 novice nurses assigned to critical care units at a tertiary medical center located in Seoul, among a group of 27 novice nurses who started at the same period at this hospital. The data were collected from March 1999 to April 2000. The major findings of the study is that the novice nurses followed the analytic linear model of clinical decision making in the beginning, but were changed increasingly to follow the comprehensive, integrated model of clinical decision making. Through repeated experience that resulted in increasing repertoire of clinical schema and familiarity of task environments of clinical practice the novice nurses expanded their ability to arrive at comprehensive integration of information and to arrive at accurate and time-efficient decisions. Both the analytic, linear model mostly used at the beginning period and the comprehensive, integrated model that seems to be the mode significantly dependent upon experience seem to have strengths and weaknesses as decision making processes in clinical situations. Hence, it is imperative to develop an effective orientation and training program for novice nurses through the use of clinical preceptors. In addition, students should be exposed to the process of clinical decision making early in their nursing education through an appropriate clinical experiences and clinical assignments.

The Analysis of the Effects of Comprehensive Health Promotion Program for RA on Changes in Health Promotion Strategies, Knowledge, and Health Status (류마치스 관절염을 위한 포괄적 건강증진 프로그램이 (Comprehensive Health Promotion Program for Rheumatoid Arthritis) 건강증진 전략, 질환에 대한 지식과 건강상태에 미치는 효과 분석)

  • This study was conducted to test whether a comprehensive health promotion program for promotion strategies and knowledge about the disease, thus leading to the improvement of health status by using repeated measure of quasi- experiment design. Eighteen RA patients who visited the RA clinic of an university hospital located in Inchon were invited to participate in the CHPPRA. According to the study results, it was shown that the CHPPRA had significant effects on the patients' health status such as pain, depression, and functional disability. Also, that the improvement of health status was achieved by a positive change in the four health promotion strategies, which consisted of goal setting, positive thinking, exercise, and knowledge about the disease. Goal setting, positive thinking, and knowledge about the disease could also affect the patients' depression. Thus it can be interpreted that the improvement of these strategies may result in a remarkable decrease of depression. In addition, alleviation of functional disability may be due to increase of exercise. However although the strategies which were directly associated with pain management were not significantly improved, pain was significantly reduced. On the other hand, the study result showed that the other health promotion strategies included in CHPPRA such as pain management, positive thinking, stress management, asking for assistance and communication were not significantly increased. although the health status such as pain, depression, and functional disability, which are final goals of the program, were significantly improved through the exposition of patients to those health promotion strategies.

Development of a Comprehensive Flood Index through Standardizing Distributions of Runoff Characteristics (유출특성 분포함수의 표준화를 통한 종합홍수지수의 개발)

  • This study developed a flood index which evaluates runoff characteristics. Runoff characteristics expressed in a hydrograph were reflected in the flood index in the form of characteristic factors such as a rising curve gradient, a peak discharge, a flood response time, and a flood discharge volume prior to peak. This study applied the standardization method to estimate the relative severity of the characteristic factors by transforming the distribution of characteristic factors into the standard normal distribution. The flood index developed in this study is a comprehensive flood index (CFI) which makes up for the weak points of a flash flood index (FFI) in determining relative severities. The CFI was applied to Han River basin and Selma River basin, and was compared with the FFI based on the correlation analysis and the regression analysis. The CFI could comprehensively evaluate flood runoff characteristics because the CFI is not dominated by a specific characteristic factor, and the CFI could explain more efficiently the relationship between rainfall and runoff than the FFI.

Total bilirubin level as a biomarker for dampness-heat differentiation in traditional Korean treatment for jaundice

  • Objectives: Classifying the pattern of jaundice during diagnosis will significantly improve the outcome of common KM interventions. This study aimed at determining an objective index for accurately diagnosing heat and dampness KM patterns in patients with jaundice. Methods: We systematically reviewed laboratory findings from case reports published in the scientific literature of Korean medicine. Cases were classified as following either the heat or dampness pattern. Biochemical indices were compared using a Bayesian factor (BF) analysis and standard t-tests. Results: The laboratory findings of 32 patients were evaluated. The heat pattern was observed in 17 patients and the dampness pattern in 15. No significant differences were observed between the 2 groups in terms of white blood cell count (BF=1.659); hemoglobin concentration (BF=2.627); platelet count (BF=1.019); or levels of direct bilirubin (BF=1.453), aspartate aminotransferase (BF=1.226), alanine aminotransferase (BF=1.340), alkaline phosphatase (BF=2.344), or gamma-glutamyl transpeptidase (BF=2.782). However, total bilirubin levels were significantly higher in the dampness pattern group (BF=0.854, P-value=0.070). Conclusions: Patients with high total bilirubin levels may predominantly follow the dampness pattern, while those with low levels may predominantly follow the heat pattern. These results are expected to be useful for the development of timely and efficient KM treatments as well as new integrative therapeutic approaches for jaundice. However, further studies are essential to fully validate the utility of total bilirubin as a biomarker for differentiating between heat and dampness patterns.

Effects of Skin Anti-Aging Wellness Program on Factors Related to Wellness Index and Skin Health (피부건강 웰니스 프로그램이 웰니스지수 및 피부건강지표에 미치는 영향)

  • Purpose : Wellness tourism is defined as travel for the purpose of promoting health and well-being through physical, psychological, or spiritual activities. The development and verification of a comprehensive wellness program for health care for workers is needed. The purpose of this study is to investigate the wellness and skin health effects of skin health programs in order to develop high-value health care services. Methods : The subjects were 15 middle-aged women who understood the research and volunteered and participated in the two-day skin health wellness program. Participants were measured to determine their wellness index and skin health twice: before and after participating in the program. Wellness index measures include comprehensive wellness, physical wellness, mental wellness, emotional wellness, social/cultural wellness, financial wellness, and environmental wellness. Skin health measurement items consisted of skin oil, skin water, skin pore, skin elasticity, skin wrinkle, and skin pigment. The skin wellness program included aerobic exercise, endurance exercise, functional food, cosmetics, herbal tea, massages, spa treatments, meditation, and marine leisure sports. The t-test analysis was used to compare the difference between wellness index and skin health measurement items before and after the program. Results : Among the wellness index items, the comprehensive wellness index (p<.05), mental wellness index (p<.05) and environmental wellness index (p<.05) showed statistically significant differences. Among skin health items, skin oil (p<.05), skin elasticity (p<.01) and skin wrinkle (p<.01) all showed statistically significant differences. There was no statistically significant difference in physical wellness, emotional wellness, social/cultural wellness, financial wellness, skin water, skin pore and skin pigment. Conclusion : This study confirmed that the two-day complex wellness program is an effective program for some items of wellness index and skin health.

The Surgical Impact of E-Cigarettes: A Case Report and Review of the Current Literature

  • We report a case of a 51 years old female with a 25 pack year smoking history who underwent bilateral mastectomy and immediate tissue expander reconstruction for newly diagnosed right breast cancer. The patient reported herself as a non-smoker despite significant e-cigarette use, with resulting significant mastectomy skin flap necrosis and breast reconstruction failure. Little is known about the physiologic effect of e-cigarettes on wound healing and tissue perfusion. To this end, we provide an updated review of the impact of e-cigarettes on surgical outcomes. PubMed, Ovid MEDLINE, and PRS GO were searched for the terms "e-cigarette", "electronic cigarette", "e-cig", "electronic nicotine delivery system", "vaping", "surgery", "surgical", "peri-operative", "operate", "operative", and "wound healing". Abstract review of all articles was performed. 123 articles returned that contained both variants of e-cigarettes and surgery as keywords. Of those, manual assessment returned three articles which were found to be relevant to e-cigarette use in the surgical patient. No articles were found that compared perioperative complications in e-cigarette versus traditional cigarette users in humans. In conclusion, our case report depicts the potential dangers associated with e-cigarette use in the surgical patient. There is a public misconception that e-cigarettes are healthier than traditional cigarettes and as such their use may go unreported by patients. Early evidence suggests e-cigarettes may induce some of the same physiologic changes as traditional cigarettes, and may have a significant deleterious effect on wound healing.

  • This study aims to offer base line data for efficient conflicts control measures by first, analyzing the current situation of the regional comprehensive development project and second, identifying level, type and cause of the conflicts appeared during the development business promotion. For this purpose, theoretical side of this study establishes a basic frame by reviewing the existing researches in order to analyze the characteristics of the conflicts between the participation subjects in the development business, and the empirical side conducts survey and analysis on the awareness on these conflicts. As a result of the analysis, the survey revealed the followings. First, the respondents were aware of the overall promotions of the business quite positively, second, the conflicts were considered as a major obstacle against the development project promotion just as the other negative factors, third, majority of the responses on the seriousness of the conflicts were neutral which meant that the conflicts could be deepened by situations, fourth, most respondents had positive recognition on the needs of conflict control training and its effectiveness, and last but not least, most respondents of the survey revealed their intentions to take parts in trouble shooting from the conflicts. Based on the results above, we were able to extract several elements to consider preparing the countermeasures for the conflicts. First, specific plan should be arranged and institutionalized in order to control the conflicts between the residents at the initial stage of the development business or even before. Second, the specific plan must lower its threshold for easy participation of the residents in that region and make the residents main body of the conflict control. Third, enhanced conflict control education must be provided to the residents of the region where comprehensive development plan is in progress or being prepared.

A Comprehensive Framework for Estimating Pedestrian OD Matrix Using Spatial Information and Integrated Smart Card Data (공간정보와 통합 스마트카드 자료를 활용한 도시철도 역사 보행 기종점 분석 기법 개발)

  • TOD (Transit-Oriented Development) is one of the urban structure concentrated on the multifunctional space/district with public transportation system, which is introduced for maintaining sustainable future cities. With such trends, the project of building complex transferring centers located at a urban railway station has widely been spreaded and a comprehensive and systematic analytical framework is required to clarify and readily understand the complicated procedure of estimation with the large scale of the project. By doing so, this study is to develop a comprehensive analytical framework for estimating a pedestrian OD matrix using a spatial information and an integrated smart card data, which is so called a data depository and it has been applied to the Samseong station for the model validation. The proposed analytical framework contributes on providing a chance to possibly extend with digitalized and automated data collection technologies and a BigData mining methods.
