Bone Replacement and Grafting with a Biologically Active Ceramic Composite

  • A composite of $Ca_3$(PO$_4$)$_2$ and MgAl$_2$O$_4$ spinel is biologically active and has enduring strength. Its strength depends on the spinel phase. The flaws in the spinel depend on the grain size of the calcium phosphate phase and are not altered by dissolution. The calcium phosphate, ${\alpha}$ tri-calcium phosphate, controls the tissue response. Bone bonds to the implant. A design for a bone graft as a replacement for a section of the diaphysis of a canine femur provides for tensile, compressive, torsional and bending load; and for the physiological processes of bonding and remodeling. A bone plate, used to stabilize the implant at time of surgery was removed after about one year. Over seven years of service have been achieved without internal or external fixation.

Bentall's operation of ascending aorta aneurysm with aortic regurgitation in Marfan's syndrome (Bentall씨 수술치험 2예)

  • We have experienced two cases of ascending aorta aneurysm with aortic regurgitation in Marfan`s syndrome. There were abnormal findings in cardiovascular system associated with abnormalities of skeletal systems. They had total replacement of the ascending aorta and aortic valve with Bjork-Shiley`s aortic valve composite graft and reimplantation of coronary ostia on the graft. Their postoperative courses were uneventful and discharged with good clinical results for follow up.

  • Two young male patients were operated on for the Marfan syndrome complicating ascending aortic aneurysm and a moderate degree of aortic regurgitation. We replaced the ascending aorta and aortic valve with Bjork-Shiley aortic valve composite graft and implanted the coronary ostia in the sides of the graft directly. Postoperatively, the atrial fibrillation occurred in one case and the other had uneventful course. They showed improvement in activity at follow-up.

  • A 64 year-old male patient with annulo-annulo-aortic ectasia[AAE] due to cystic medial necrosis was successfully treated with Cabrol operation with Cabrol trick. The technique consist of implantation of a composite valve graft within the aneurysmal sac with reattachment of the coronary ostia using a separate, small tube graft and creation of a communication between the closed perigraft space and right atrium for bleeding control. The patient had a postoperative gastrointestinal bleeding but successful recovery was achieved eventually.

  • A 31 year-old male patient underwent surgical treatment of the pseudoaneurysm of the ascending aorta complicating after the Bentall operation, He had undergone the replacement of the ascending aorta using the composite valved graft with direct coronary reimplantation under the diagnosis of the annuloaortic ectasia of ascending aorta associated with Marfan syndrome. Eleven months after the operation, he started to feel dyspnea and anterior chest pain, and was diagnosed as pseudoaneurysm around the ascending aortic graft. The second operation consisted of the dacron patch closure of the defect of the aortic graft which was the hole for previous coronary reimplantation, and the anastomosis between the coronary orifice and the aortic graft with the intermediate graft of a 10mm woven dacron tube, and suture closure of the fistula opening from the aneurysm. His postoperative course was uneventful and discharged without complication. He is doing well 10 months postoperatively.

Soft Tissue Augmentation with Silk Composite Graft

  • Purpose: The objective of this study was to evaluate the interaction between 4-hexylresorcinol (4HR) and antibody as that affects the performance of a silk-4HR combination graft for soft tissue augmentation in an animal model. Methods: The silk graft materials consisted of four types: silk+10% tricalcium phosphate (TCP) (ST0), silk+10% TCP+1% 4HR (ST1), silk+10% TCP+3% 4HR (ST3), and silk+10% TCP+6% 4-HR (ST6). The antibody binding assay tested the 4HR effect and scanning electron microscopic (SEM) exam was done for silk grafts. The animal experiment used a subcutaneous pocket mouse model. The graft - SH0 or SH1 or SH3 or SH6 - was placed in a subcutaneous pocket. The animals were killed at one, two, and four weeks, postoperatively. The specimens were subjected to histological analysis and lysozyme assay. Results: Groups with 4HR applied showed lower antibody binding affinity to antigen compared to groups without 4HR. In the SEM examination, there was no significant difference among groups. Histological examinations revealed many foreign body giant cells in ST0 and ST1 group at four weeks postoperatively. Both ST3 and ST6 groups developed significantly lower levels of giant cell values compared to ST0 and ST1 groups (P < 0.001) at four weeks postoperatively. In the lysozyme assay, the ST1 and ST3 groups showed denser signals than the other groups. Conclusion: 4HR combined silk implants resulted in high levels of vascular and connective tissue regeneration.

The effects of autogenous composite grafts on bone regeneration after sinus elevation (상악동거상술시 자가골 복합이식이 골재생에 미치는 영향)

  • The purpose of this present study was to investigate the effect of autogenous bone with histological evaluation of regenerated bone after sinus elevation. The study involved genaral healthy 6 patients participated in this study and were treated with 2-stage sinus elevation procedures using a combination of demineralized freezed-dried bone allograft (DFDBA) and coralline calcium carbonate with or without autogenous bone. At 6months after sinus elevation, bone specimens were obtained and stained with Hematoxylin-Eosin for light microscopic evaluation. The results of this study were as follows : 1. Autogenous bone grafts present trabecular patterns at 6 months in test groups, consist of woven bone and lamellar bone, but more compact than control groups. 2. Resorption of bone graft particles, osteoblast-like cells, newly formed osteoid tissue were observed at 6 months in test groups, but seems to be more frequently than control groups. 3. New osteoid tissue was formed from the surface of graft materials and gradually expanded around them. 4. The appearance of connective tissue around graft materials was densely formed, but more prominent in test groups than control groups. 5. Bone graft particles were resorbed incompletely and slight inflammatory infiltrate, newly formed capillaries, and adipocytes were observed. From the above results, autogenous bone is effective in bone regeneration after sinus elevation, could provide favorable conditions in implant placement.

Surgical Treatment of Annuloaortic Ectasia - A Case Report - (Annuloaortic Ectasia 에 대한 Bentall 씨 수술치험)

  • A case of Annuloaortic Ectasia associated with Marfan syndrome and mitral regurgitation is treated surgically by Bentall`s method and mitral annuloplasty. The Annuloaortic Ectasia is frequently accompanied with Marfan syndrome, its definition is simply explained as the following; the marked dilatation of the sinuses of Valsalva and the aortic annulus as well as the huge aneurysm of the ascending aorta. As the operative finding, the intimal tearing was shown as circular and the both coronary ostia were changed the position into high up. The patient was taken a corrective operation replacing the ascending aorta and aortic valve with a composite graft[St. Jude medical valve 29mm, woven Dacron tubular graft 31mm]. The both coronary ostia were reimplanted on the graft with 4-0 prolene by continuous suture. Mitral annuloplasty was performed. After the operation, the patient developed both spontaneous pneumothorax, he improved state by the closed thoracostomy. He has been doing well, postoperatively.

Surgical Management of the Aneurysm of the Ascending Aorta with Aortic Regurgitation [A Report of 17 consecutive Patients] (대동맥 판막부전증이 동반된 상행 대동맥류의 외과적 치료: 17례 보고)

  • Seventeen patients underwent operations for aneurysm of ascending aorta with aortic regurgitation from August 1979 to October 1985. 10 patients underwent complete replacement of the ascending aorta and the aortic valve with a composite graft and implantation of coronary ostia on the graft. Seven patients underwent supracoronary noncomposite graft replacement and aortic valve replacement. The patients ranged in age from 25 to 55 years [mean 37.6 years]. There were 11 male and 6 female patients. All patients had aortic incompetence and aneurysmal dilatation of the ascending aorta. Seven of the patients has concomitant aortic dissection in ascending aorta and one had dissection in abdominal aorta. Eight patients had signs of Marfan syndrome and the other 3 patients had cystic degeneration in the medial layer of the aorta. There was one hospital death[5.8%]. He died of sepsis on the 23rd postoperative day. All survivors showed improvement in NYHA functional classification in the 34.9 patient-year follow-up period.

  • 1. 서론 : 본 연구는 시간이 경과함에 따라 free volume감소로 나타나는 PTMSP[Poly(1-trimethylsilyl-1-propyne)] memebrane의 pysical aging을 늦추거나 방지할 목적으로 PTMSP polymer를 합성하여 여기에 hydroxy-terminated PDMS를 graft시켜 PTMSP/PDMS graft copolymer를 제조하였다. 용매증발법에 의해 PTMSP memebrane 및 PTMSP/PDMS graft copolymer memebrane을 제막한 후 PTMSP막의 물리적 노화를 관찰하기 위한 시점에서 조업시간에 따른 이들 막의 transport property을 살펴 보았다. 또한 이들 polymer을 사용하여 0.5 wt%의 희박 dope solution을 제조한 후 여기에 상전환법에 의해 제조된 비대칭 PEI(polyetherimide)지지막을 dip-doping시켜 PTMSP-PEI, PTMSP/PDMS-PEI 복합막을 제조하여 상기의 두 막과 투과증발 특성을 상호 비교하여 보았다. 그리고 객관적 비교 자료를 얻을 목적으로 PDMS막과 PDMS-PEI 복합막을 각각 제막하여 동일조건에서 실험을 수행하였다. 따라서 본 연구는 수중에 미량 용해된 chloroform, trichloroethylene, perchlororthylene, 1,1,1-trichloroethane 등의 유기염소계화합물 제거 실험을 통해 PTMSP, PTMSP/PDMS 등의 dense membrane과 asymmetric composite membrane 사이의 상관관계 및 이들 막들의 투과특성을 서로 비교, 분석하는데 목적을 두었다.
