A Clinical Case Study on Guillain-Barre Syndrome Complaining Both Lower Extremity Weakness with Oriental Medical Treatment (양하지 소력을 주소로 하는 길랑-바레 증후군 환자에 대한 한방치료 증례보고 1례)

  • This study was performed to report the effectiveness of oriental medical treatment on a patient with Guillain-Barre syndrome complaining both lower extremity weakness. The patient was diagnosed as Guillain-Barre syndrome and treated with acupuncture, electroacupuncture, physical therapy and herbal medicine. We have evaluated the efficacy of oriental medical treatment by measuring changes of motor grade and ambulatory condition according to period of hospitalization. After treatment, motor grade and ambulatory condition were improved. These results suggest that oriental medical treatment was effective on the patient with Guillain-Barre syndrome.

Two cases of craniofacial dysostosis (두개안면이골증의 두 증례)

  • Craniofacial dysostosis is considered to be one of rarely observed syndromes characterized by premature closing of all cranial sutures. The first patient was a 4-year-old male infant who had been complaining of empyema. Clinical findings showed exophthalmos, hypertelorism and facial asymmetry. Conventional radiographs demonstrated abscence of cranial sutures and underdeveloped maxilla. CT scan demonstrated the digital impressions of the inner surface of the cranial vault, enlarged and depressed sella turcica. The second patient was a 2-year-old female infant who had been complaining of facial deformity. Clinical findings showed hypertelorism and underdeveloped maxilla. Radiographs showed premature synostosis of all cranial sutures, depressed and enlarged sella turcica, and hypoplastic maxilla. 3 years after operation, her look improved. However, resurgery may be considered to decreasing intracranial pressure and for correction of facial deformity. Two interesting cases showing ‘cloverleaf’ skulls were presented.

  • This study was performed to report the effectiveness of oriental medical treatment on a patient with avascular necrosis of femur head complaining lower extremity pain. The patient was diagnosed as avascular necrosis of both femur head and treated with acupuncture, cupping therapy, physical therapy and herbal medicine. We have evaluated the efficacy of oriental medical treatment by measuring changes of visual analogue scale and range of motion according to period of hospitalization. After treatment, pain and range of motion were improved. These results suggest that oriental medical treatment was effective on the patient with avascular necrosis of femur head.

THE DEVELOPMENT OF INTERPRETATION FOR TEMPOROMANDIBULAR JOINT ROENTGENOGRAMS (악관절증 환자의 X선사진 판독법 개발에 관한 연구)

  • The authors analyzed the morphological change of bone structure from 3,140 radiographs (1570 joints) of 785 patients with temporomandibular joint arthrosis, which were obtained by the oblique lateral transcranial projection and orthopantomographs. The interrelation of bone change and clinical symptoms, duration of the diseases were examined. Also, the bone changes of articular eminence, condyle, articular fossa were examined according to positional change of the condyle in the mouth open and close state. The results were as follows. 1. In the 785 patients with TMJ arthrosis, 782 patients (99.62%) show the positional change of the condyle. Among them 691 patients (88.03%) show the bone change. 2. In TMJ arthrosis patients with bone changes 451 patients (65.27%) showed both the condylar positional changes and bone changes bilaterally. 198 patients (28.65%) show the condylar positional changes bilaterally and bone changes unilaterally. 3. The bone changes in the TMJ arthrosis were in order of frequency eburnation (647 cases, 32.8%), erosion (548 cases, 27.79%), flattening (418 cases, 21.20%), deformity (138 cases, 6.99%). sclerosis (115 cases, 5.83%), marginal proliferation (106 cases, 5.38%). The region of bone change in TMJ arthrosis with condylar positional changes were in order of frequency the articular eminence (43.97%) condylar head (38.64%), articular fossa (17.39%). In the patients with bone changes, their clinical symptoms were pain (44.34%), clicking sound (33.5%), limitation of mouth opening (22.52%). In the patients complaining pain the most frequent bone change was erosion (28.60%), in the patients complaining clicking sound, eburnation (28.97%) in the patients complaining the limitation, eburnation (29.40%). Also in the patients with the duration below 1 year most common bone change was eburnation. 5. The most common condylar positional change was downward position (39.94%) in closed state, restricted movement of condyle (30.07%) in open state. The condylar positional changes and bone changes according to the region were as follows: a) In the condylar head the most frequent bone change was erosion (30.45%) and the most frequent condylar positional change was downward position (37.40%) in closed state, restricted movement of condyle (33.2%) in open state. b) In the articular eminence the most frequent bone change was eburnation (39.91%) and the most frequent condylar positional change was downward position (39.79%) in closed state, restricted movement of condyle (27.22%) in open state. c) In the articular fossa the most frequent bone change was eburnation (53.94%) and the most frequent condylar positional change was downward position (42.57%) in closed state, restricted movement of condyle (30.32%) in open state.

Pharmacotherapy for Patients Complaining With Somatic Symptoms (신체증상을 호소하는 환자의 약물치료)

  • Patients complaining with somatic symptoms are very common in clinical practice, and are often consulted to medical department. But it is difficult to treat well. The treatment of somatic symptom disorder is multi-modal as none of the methods on their own provide a satisfactory outcome. The treatment of somatic symptoms disorders is complicated by lack of boundary, conceptual clarity, and overemphasis on psychosocial causation and effectiveness of psychological treatments. In clinical practice all classes of psychotropics are used to treat somatic symptoms disorder. Drugs such as tricyclic antidepressants, serotonin reuptake inibitors(SSRI), serotonin and noradrenalin reuptake inhibitors (SNRI), atypical antipsychotics are studied. The evidence indicates that these drugs are effective in somatic symptom disorders. All classes of antidepressants seem to be effective against somatic symptom disorders. SSRIs are more effective against hypochondriasis and body dysmorphic disorder, and SNRIs appear to be more effective than other antidepressants when pain is predominant. The author suggest that psychiatrists should know how to treat patients complaining with somatic symptoms by using not only psychotherapeutic approach but also pharmacological treatment. It will be helpful to reduce suffering and increase quality of life of these patients.

Result of modified Heller operation with gastric fundoplication in esophageal achalasia (Achalasia 의 외과적 치료)

  • For recently 2 years, 3 cases of esophageal achalasia were surgically treated by modified Heller operation with gastric fundoplication. Esophageal achalasia is functional disorder of lower esophagus of which symptoms are dysphasia, regurgitation, and weight loss. Preoperative diagnosis was made by clinical manifestations and radiologic examination, esophagoscopy and confirmed at operative table. Operative procedures are variable, but modified Heller operation is common method to handicap reflux esophagitis and postoperative esophageal stricture. Now, our patients who were surgically treated were well alive without complaining of specific symptoms for followed period.

  • The Plummer - Vinson syndrome Is characteristic difficulty In swallowing, iron defeciency anemia, and upper esophageal or hypopharyngeal web. It is rarely disease in Korea. We have experienced a case of the Plummer-Vinson syndrome, complaining of dysphagia with iron deficient anemia, hypopharyngeal web who was thirty - two years old woman has treated with bullous pemphigoid. The patient has treated sucessfully in the fasion with microsuspentional CO2 Laser for removal of hypopharyngeal web. We report one case with review of the literature.

  • Objective : A Patient complaining of dizziness was diagnosed and treated with the Shanghanlun disease pattern identification diagnostic system and was analyzed to report cases Methods : Based on the Shanghanlun disease pattern identification diagnostic system, we analyzed a case treated with the Baekho-tang and counseling. Results : Baekho-tang showed a rapid improvement in the patient. During the period of 20 days, dizziness were greatly improved. Conclusions : The Baekho-tang, which is not well known yet, can show rapid effect and can be diagnosed frequently through the Shanghanlun disease pattern identification diagnostic system.

The Use and Needs on Commun Rehabilitation Service of Industr Accident Victims at Home (재가 산재장애자들의 지역사회 재활서비스 이용 실태 및 요구도)

  • Objectives: This study described the features of home-bound industrial accident victims and their needs for rehabilitation services. This study was also aimed to find a future direction of development of community rehabilitation programs that are suitable for their needs demands. Methods: This study is a descriptive study, were collected through two phases using structured questionnaire. In the first stage, su were performed via telephone interviews. In the se stage, surveys were performed via home visit Subjects in the first stage included 2203 indu injured victims staying at home, of whom. individuals complaining of post-traumatic complic became the subjects of the second stage. Results: This study showed that the home-bound industrial accident patients were complaining of complications from the injury even after receiving treatment by IACI. However, they were neglecting their health problems without any intervention. Even if they use health care services. the treatment is mainly focused on acute medical care, which may not effective for them. Furthermore, they had unstable employment status and suffered from financial burden for health care costs. The Labor Welfare Organization has established a plan to remove barriers of industrial accident victims in reinstatement, and has been preparing various programs in order to establish an all-embracing service system for industrial accident victims from accident occurrence to reinstatement. However, these rehabilitation services can be truly helpful only when the injured are able to obtain enough information about them. The current restrictive system is also not appropriate for solving health problems of the industrial accident victims. Therefore, it is necessary to develop a plan that can provide industrial accident victims high-quality rehabilitation services so that they can use those services in the community without being dependent on hospitals. This study proposes visit nursing services as a way to provide various health services within community for the industrial accident victims.
