The Determinants of Community Service Utilization Among Family Caregivers of the Impaired Older Persons (만성질환 및 기능손상노인 가족수발자의 재가복지서비스 이용 결정요인에 관한 연구)

  • This Study explored three issues in regard to the determinants of community service utilization among family caregivers of the dependent older persons. First, the differences between users and nonusers of community services were compared regarding to predisposing, enabling, need characteristics of the primary caregiver as well as the elder care recipient. Second, the variables which determined the contact of community services were examined. Third, the influence of the variables on duration of the community services among users was also examined. The data collected from 164 family caregivers were used for analyses. Findings suggest that community service users had higher education and higher emotional support, lower family income than nonusers. The level of cognitive impairment of the elder was also higher for users than nonusers. The entry into community services is more likely for elders cared for by caregivers who have higher educational attainment, lower family income and lower level of instrumental support. Once interaction terms for relationship between need factors and social support are entered, caregivers with poorer level of his/her perceived physical health and lower level of instrumental support are more likely to report use of community services. Among those reporting contact with services, more extensive use occurs for caregivers with lower emotional support and lower depression. The entry of interaction terms for relationship between need and support reveals that the combination of lower support(instrumental and emotional) and elder's greater physical impairment are associated with longer period of community service use. However, the effect of caregivers' depression followed the different pattern. For caregivers with greater depression, more supports are related with more service utilization. According to the results, implications for research and practice are discussed.

  • This study aims to enhance the continuous utilization of community mapping by structurally understanding the intentions behind its ongoing use and the impacts related to it, grounded in prior research. By analyzing existing community mapping cases through literature studies on community mapping, this research identifies the characteristics of community mapping that motivate its sustained use. Moreover, based on the Uses and Gratifications Theory and the Participatory Communication Model (PCM), this study proposes a research model to investigate the factors influencing the intention to continue using community mapping. Unlike previous studies, which primarily focused on developing and applying community mapping processes for case studies, this research not only develops and applies a community mapping process but also aims to apply the findings related to the intention of continued use in future community mapping activity processes for sustained engagement.

Outcome Assessment of a Demonstration Project on Integration of Home Visiting Health Care and Social Welfare Services (보건소 중심의 방문보건.재가복지 통합시범사업 성과)

  • Purpose: The objectives of this study was to measure the outcomes of interventions on the health and social welfare of the elderly in a rural community in Korea. The project involved integrating services of one public health center with that of one social welfare agency, which were under different administrative structures. Method: A single group pretest-posttest design was used for this research. Seventy-five elderly residents living alone in a rural community participated in the study. All of them had coverage of free basic medical care and social welfare services by the government. Major activities for the intervention included: developing partnerships among community leaders/institutes; forming committees of community residents; educating care providers and volunteers; developing 8 integrated service programs and instruments; and organizing the networks. The 20-month intervention was care-managed by a public health nurse whom collaborated with social worker, and was assisted by volunteers. The t-test was utilized to analyze the outcome variables including the elder's health, social welfare and quality of life. A major limitation of this study was the lack of a control group. Results: The outcome of the intervention was shown by improved elder's health, social welfare needs, and quality of life. Integrating the services of public health centers with those of social welfare agencies is an effective way to improve the health of the elderly in the community. Conclusion: Developing community capacity with such integrated services will pay an important role in improving the health of the elderly who live alone.

Providing End-of-Life Care in the Community: What Are the Challenges in Malaysia?

  • A 72-year-old woman with metastatic lung cancer to bone and brain and with left external iliac vein thrombosis was under the care of a community palliative care provider. She experienced an acute pain crisis due to acute limb ischemia of the left lower limb. Goals-of-care discussions were held with the patient and her family; she prioritized symptom control and end-of-life care at home. The family and patient were aware of her short prognosis. Her complex pain was managed by the community palliative team, and her family was empowered to give subcutaneous injections. We illustrate a case showing the importance of community health services with palliative care support in providing symptom management and support to patient and family caregivers throughout the course of a life-limiting illness. It also highlights family caregivers' potential psychological distress in delivering subcutaneous injections in terminal care for a patient at home.

Pharmacists' Perceptions of Barriers to Providing Appropriate Pharmaceutical Services in Community Pharmacies (지역약국 약료서비스 제공의 장애요인: 약사 대상 설문조사)

  • Background: In order to achieve the goals of community pharmacy practice, its legal, labour-related, and economic barriers need to be identified. This study examined pharmacists' perceptions of constraints on providing optimal pharmacy services in order to identify underlying factors and analyse the associations between barriers and pharmaceutical services in community pharmacies. Methods: A survey targeting pharmacy owners was conducted from May to June 2012 using a structured questionnaire including nine pharmaceutical service items. According to the service provision level, we classified pharmacists as inactive (fewer than 5 items among the listed 9 service items) and active providers (5 or more items). Principal component analysis was used to group significant factors for barriers into four thematic components. Associations between the participants' demographics and pharmacy characteristics and the services provided were explored by logistic regression analyses. Results: Participants were 402 pharmacists. Over 60% provided disease management services for hypertension, diabetes, and hyperlipidaemia. Variables that affected pharmaceutical services included the lack of separate areas for patient counselling (OR: 2.12, 95% CI: 1.18-3.80), and clinical knowledge and information-related barriers (OR: 0.59, 95% CI: 0.36-0.97). Conclusion: Strategies for improving clinical knowledge and providing expeditious information are necessary in order to improve community pharmacy services.

The Use and Needs on Commun Rehabilitation Service of Industr Accident Victims at Home (재가 산재장애자들의 지역사회 재활서비스 이용 실태 및 요구도)

  • Objectives: This study described the features of home-bound industrial accident victims and their needs for rehabilitation services. This study was also aimed to find a future direction of development of community rehabilitation programs that are suitable for their needs demands. Methods: This study is a descriptive study, were collected through two phases using structured questionnaire. In the first stage, su were performed via telephone interviews. In the se stage, surveys were performed via home visit Subjects in the first stage included 2203 indu injured victims staying at home, of whom. individuals complaining of post-traumatic complic became the subjects of the second stage. Results: This study showed that the home-bound industrial accident patients were complaining of complications from the injury even after receiving treatment by IACI. However, they were neglecting their health problems without any intervention. Even if they use health care services. the treatment is mainly focused on acute medical care, which may not effective for them. Furthermore, they had unstable employment status and suffered from financial burden for health care costs. The Labor Welfare Organization has established a plan to remove barriers of industrial accident victims in reinstatement, and has been preparing various programs in order to establish an all-embracing service system for industrial accident victims from accident occurrence to reinstatement. However, these rehabilitation services can be truly helpful only when the injured are able to obtain enough information about them. The current restrictive system is also not appropriate for solving health problems of the industrial accident victims. Therefore, it is necessary to develop a plan that can provide industrial accident victims high-quality rehabilitation services so that they can use those services in the community without being dependent on hospitals. This study proposes visit nursing services as a way to provide various health services within community for the industrial accident victims.

Future Residents' Opinions about the Services and Programs in the University-Based Retirement Community - Focusing on the Opinion Differences Depending on Residents' Intention to Live in the UBRC - (대학 연계형 은퇴주거단지의 서비스 및 프로그램에 대한 잠재 수요자의 의견 분석 - 거주의사 유무에 따른 차이를 중심으로 -)

  • The purpose of this study was to identify services and programs that will be needed for helping senior residents to be aging healthfully and comfortably in the university-based retirement community (UBRC). This study conducted a questionnaire survey targeting about 900 faculty and staff members in one of national universities. A total of 214 valid responses were analyzed for identifying necessary services and programs. The study also analyzed the differences in resident's opinions depending on their intention to live in the UBRC. Over 65% of the respondents were willing to live in the UBRC. The most desirable services were healthcare services including emergency care and nursing care and cleaning and laundry services. Regarding the educational and recreational programs, future residents chose physical exercises such as swimming and tennis and music activities such as playing instruments and singing as the most interesting programs they want to experience in UBRC. Most of the respondents were interested in mentoring college students or international students who come to the university for their degrees. The findings from the study should provide a guideline when offering services, programs, and activities in the future UBRC in Korea.

Services for Abused Children : Implications from a Comparative Case Study of Programs in the USA and Sweden (피학대아동을 위한 서비스의 국가간 비교를 통한 한국적 함의 - 미국과 스웨덴을 중심으로 -)

  • Even though social intervention in child abuse has begun in Korean society, there are many problems in terms of basic principles of child welfare services. Focusing on the principle of family preservation in child welfare services, this comparative study reviewed services for abused children and their families in the USA and Sweden. Results indicated directions for effective services for abused children are comprehensive and should be provided by community resources. Comprehensive services and follow-up of reported cases should be managed by public agencies and community networks. Education curricula for professionals and inter-disciplinary collaboration should be organized. A most important task in the future is to set up firm laws and policies for effective services in Korean society.

  • This study is designed to evaluation of the Community based Opthalmologic Health Services(CBOHS) for elderly carried out for last three years from 1994. The subjects were total 236 aged over 57 in Chuncheon city. Kangwon province. and the period of data collection was from Feb. 24th. 1996 to Feb. 27th. 1996. For evaluation of CBOHS. the questionnaire was consisted of participation rate. and perception about the service. the level of satisfaction and things to improve. Result were obtained as follows: 1. The number of participants was $95(40.3\%)$ among total subjects. The places provided services were the health service center of Seoul National University in Chuncheon city and community health center. The participants were known from announcements of community staff$(66.3\%)$. 2. Non participants were $59.7\%$ among total subjects. mam causes of missing wasn't known'. 3. The services that participants screening test$(41.2\%)$. provided provided operation$(45.5\%)$. 4. The question about things to improve were continuous follow up and more many glasses provision These fingings may indicate that the CBOHS need more announcement. continuous follow up and provide more many glasses. As proved the satisfaction of CBOHS for elderly. the next study can be performed about the relevant factor of the services.
