One Case of Stroke in Childhood (소아중풍(小兒中風) 환자(患者)에 대한 치험(治驗) 1례(例))

  • Objectives : The purpose of this study is case report of stroke in childhood. The childhood cerebrovascular diseases are interesting than those seen in adulthood, because the etiology and prognosis of the disease in children are quite different from those of adults. Cerebrovascular diseases in children is more common than once recognized. However, relatively little attention has been paid to the epidemiology of childhood stroke. Methods : We experienced one case of ischemic stroke in childhood whose symptoms are similar to that of adults. There is no evidence of cerebrovascular malformation at MRI, MRA film. The cause of stroke is undetermined and just we can see the little infarction lesion on Lt. pons. We treated the patient with herb medicine, acupuncture-Tx, negative-Tx, moxa-Tx and physical treatment. Results : After the treatment of oriental medicine, the patient recovered from the stroke symptoms. Conclusion : we thought that the more follow-up is needed. So we repoert one case of cerebral infarction in children that recovered from stroke symptoms after the treatment of oriental medicine.

  • Tempormandibular Joint pain and dysfunction is a common and important clinical problem. With the recent advances in imaging technology, radiologists have made major contribution to the understanding of TMJ disease. The MRI has several advantages over the conventional imaging methods. It is possible to obtain surprisingly precise images of internal hard and soft tissues with MRI. It is not invasive without ionizing radiation. The abnormal disk position has been thought as the cause of TMJ internal derangement. But there are few methods to relate disk position to TMJ internal derangement quantatively. The object of our study is to determine the amount of anterior displacement fo articular disk in symptomatic patients related each symptoms. Using the method of Dr. Drace, we studied the 38 joints of 22 persons with susceptable TMJ internal derangement. 1. In determining the anterior displacement of TMJ articular disk, the junction between the posterior band and bilaminar zone was useful. 2. The mean anterior displacement of disk in reduction group and without reduction group were $51.0^{\circ}C$ and $81.1^{\circ}C$ respectively. The difference was significant. 3. In the mean anterior displacement of disk, the pain without clicking group showed $84.8^{\circ}C$ and the clicking and pain group $70.8^{\circ}C$, the clicking group respectively.

Antituberculosis Medication in Children (소아의 항결핵제 사용 실태)

  • Tuberculosis in children is an important disease because of higher incidence and mortality, especially in developing and underdeveloped countries. The objectives of this study were to evaluate the cause of antituberculosis medication in children and to find out the basic data for proper drug regimen. We reviewed the medical records of 198 patients who had been treated with antituberculosis drugs from Jan. 1991 to Dec. 1993 in Anam Hospital of Korea University Medical Center. The results are as following; 1) Of 198 patients, 69 cases(34.8%) had treated due to BCG complications. They were all medicated with INH. The durations of medication were 3 months in 46 patients(66.7%), 4~6 months in 17 patients(5.8%), 7~9 months in 4 patients(5.8%), 10-12 months in 2 patients(2.9%). 2) Of 198 patients, 68 cases(34.3%) had treated due to chemoprophylaxis, 59 patients (29.8% of all cases) had histories of house hold contact. Of 68 cases, 51 patients (86.4%) were medicated with INH only, 8 patients (13.6%) were medicated with INH and RFP. 3) Other causes of antituberculosis medication were tuberculous lymphadenitis(14.1%), pulmonary tuberculosis(10.6%), meningitis, miliary tuberculosis(2.0%), and pleurisy(2.0%). Most common causes of antituberculosis medications in children were complication of BCG vaccination and chemoprophylaxis after household contact. So early detection of adult tuberculosis and development of convenient diagnostic methods and safe vaccine for childhood tuberculosis is necessary.

Intussusception in Neonates: Clinical Characteristics of Eight Cases in a Single Center

  • Purpose: Intussusception is the most common cause of bowel obstruction in children; however, it is rarely diagnosed in newborn infants. This study aimed to describe the clinical features of intussusception in newborn infants. Methods: Medical records of eight patients diagnosed with intussusception during the newborn period at Ulsan University Hospital between March 2007 and March 2020 were retrospectively reviewed. Results: Among the eight cases, two occurred in the intrauterine period and six occurred in the postnatal period. Intrauterine intussusception presented with symptoms of bowel obstruction within 1 to 2 days after birth, and ileal atresia was diagnosed simultaneously through exploratory laparotomy. All the postnatal patients were extremely low birth weight infants (median gestational age and birth weight: 25+6 weeks and 745 g, respectively). Four cases were diagnosed preoperatively using abdominal ultrasonography. One patient was diagnosed by exploratory laparotomy because the clinical symptoms were nonspecific and difficult to differentiate from those of necrotizing enterocolitis, a more prevalent complication in preterm infants. The site of intussusception in all six patients was the small bowel. Meckel's diverticulum (one case) and meconium obstruction (two cases) were found to be the lead point. Conclusion: Neonatal intussusception tends to show different clinical features according to its period of occurrence. Intussusception, especially in preterm infants, has nonspecific clinical features; therefore, clinicians should always be cautious of this disease for its early diagnosis.

  • Salivary duct carcinoma is a high-grade adenocarcinoma arising from the ductal epithelium and has very low prevalence. We report a case of salivary duct carcinoma in high risk group with satisfactory result. A 65-year-old male was referred to our clinic complaining of mass on Rt. cheek. Preoperative CT and MRI shows $2.0{\times}1.9cm$ sized multilobulated, cystic mass on the superficial lobe of Rt. parotid gland and multiple lymph node enlargement thorough the Rt. internal jugular chain. Total parotidectomy and modified radical neck dissection with adjuvant radiation therapy was performed. Pathologic result was salivary duct carcinoma and resection margin was free. Postoperative radiation therapy with 6400 cGy($200cGy{\times}12fx$) was performed. During the 24-months of follow up periods, recurrence or complications associated with operation and radiation therapy was not observed. Salivary duct carcinoma is rare disease with very poor prognosis. Lymph node metastasis is commonly accompanied at the time of diagnosis. Distant metastasis is the most common cause of death. Total parotidectomy, radical neck disssection and adjuvant radiation therapy can be the appropriate modality for the control of the salivary duct carcinoma especially in high risk group.

Clinical Characteristics and Ultrasonographic Findings of Acute Bacterial Enterocolitis in Children

  • Purpose: This study clarified the bacterial pathogens currently causing acute infectious enterocolitis (AIE) in children and evaluated the clinical characteristics and ultrasonographic findings according to the different pathogens. Methods: Medical records regarding age, sex, clinical symptoms, laboratory data, identified enteropathogens, ultrasonographic findings, treatment, and outcome of 34 patients who were diagnosed with AIE via stool examination using multiplex polymerase chain reaction (PCR) or culture, were retrospectively reviewed. Results: Twenty-four patients (70.6%) were male. The mean age of the patients was $8.5{\pm}6.2$ (range, 1.1-17.1) years. Six bacterial pathogens were isolated: Salmonella species (spp.) (32.4%), Campylobacter spp. (20.6%), verotoxin-producing Escherichia coli (14.7%), Staphylococcus aureus (11.8%), Clostridium difficile (8.8%), and Shigella spp. (2.9%). Abdominal pain occurred in all patients regardless of pathogen. The patients infected with Salmonella were older than those infected with verotoxin-producing E. coli (p<0.05). C-reactive protein levels were higher in patients with Salmonella and Campylobacter infections than in those with verotoxin-producing E. coli infection (p< 0.05), the other clinical and laboratory data were indistinguishable between pathogens. Ultrasonography demonstrated diverse involvement of bowel segments according to pathogen. Wall thickening of both the ileum and the entire colon was the most common lesion site regardless of pathogen. Conclusion: Various bacterial agents cause AIE and the symptoms are diverse symptoms, however, all most children recovered spontaneously. Use of multiplex PCR on stool samples warrants improvement of its sensitivity for diagnosis of enteropathogenic bacteria. Ultrasonographic examination is useful for diagnosis of AIE; it can also detect the disease extent and severity.

The Effects of Salvia Miltiorrhiza on Renal Function and Histopathological Changes in Streptozotocin-induced Diabetic Nephropathy Rat Model (단삼이 당뇨병성 신병증 Rat의 신기능 및 조직학적 변화에 미치는 영향)

  • Objective : Diabetic nephropathy is the most common cause of end stage renal disease. AGE, $TGF-{\beta}1$ type IV collagen, and macrophage/monocyte infiltration are the main factors of diabetic nephropathy. We investigated the effects of Salvia miltiorrhiza on renal function and histopathological changes in streptozotocin(STZ)-induced diabetic nephropathy rat model. Methods : Diabetes was induced in male Sprague-Dawley rats(290${\pm}$10g) by injecting STZ(45mg/kg) into the tail vein. Rats were divided into 3 groups(n = 6): normal, control, and salvia. After 8 weeks of administration of Salvia miltiorrhiza extract on the Salvia group, we checked 24 hrs urine, blood biochemistry and renal tissue to evaluate renal function and histopathological changes by examining parameters including albuminuria, BUN, creatinine, cholesterol, LDL, TG, macrophage/monocyte antigen(ED-1), $TGF-{\beta}1$, AGE, and type IV collagen. Results : Salvia miltiorrhiza decreased the amount of 24hrs proteinuria, and inhibited histopathological changes of diabetic nephropathy including the expression and accumulation of various factors which could promote development of diabetic nephropathy. Conclusion : These findings suggest that Salvia miltiorrhiza might protect the renal function and inhibit the development of renal injury by regulating factors including AGE, $TGF-{\beta}1$ Type IV collagen, macrophage and monocyte infiltration. So Salvia miltiorrhiza can be used for diabetic patients to prevent the progression of diabetic nephropathy.

  • The common pathogen Salmonella enteirtidis (S. enteritidis) is the major cause of foodborne disease. Protein identification by peptide mass fingerprinting using the matrix-assisted laser desorption ionization time of fight (MALDI-TOF) mass spectrometry (MS) can analysis unambiguously identity the spots from 2-dimensional electrophoresis (2-DE) gel. In this report, we examined protein components from patterns of S. enteritidis proteins. In addition, antigens that are recognized by sera can be identified by immunoblotting. This study that 2-DE analysis of S. enteritidis yields useful information concerning S. enteritidis proteome, the results that have been obtained led to a more detailed understanding of Salmonella pathology and open further interesting fields for future work.

Subunit Principle of Vulvar Reconstruction: Algorithm and Outcomes

  • Background Vulvar defects result chiefly from oncologic resection of vulvar tumors. Reconstruction of vulvar defects restores form and function for the purpose of coitus, micturition, and defecation. Many surgical options exist for vulvar reconstruction. The purpose of this article is to present our experience with vulvar reconstruction. Methods From 2007 to 2013, 43 women presented to us with vulvar defects for reconstruction. Their mean age at the time of reconstruction was 61.1 years. The most common cause of vulvar defect was from resection of vulvar carcinoma and extramammary Paget's disease of the vulva. Method s of reconstruction ranged from primary closure to skin grafting to the use of pedicled flaps. Results The main complications were that of long term hypertrophic and/or unaesthetic scarring of the donor site in 4 patients. Twenty-two patients (51%) were able to resume sexual intercourse. There were no complications of flap loss, wound dehiscence, and urethral stenosis. Conclusions We present a subunit algorithmic approach to vulvar reconstruction based on defect location within the vulva, dimension of the defect, and patient age and comorbidity. The gracilis and gluteal fold flaps are particularly versatile and aesthetically suited for reconstruction of a variety of vulvar defects. From an aesthetic viewpoint the gluteal fold flap was superior because of the well-concealed donor scar. We advocate the routine use of these 2 flaps for vulvar reconstruction.

Presence of Sarcopenia and Its Rate of Change Are Independently Associated with Long-term Mortality in Patients with Liver Cirrhosis

  • Background: Sarcopenia is associated with a poor prognosis in patients with liver cirrhosis. However, it is not known whether the rate of skeletal muscle depletion is also associated with a poor prognosis. We investigated the prognostic impact of the rate of skeletal muscle depletion in patients with liver cirrhosis. Methods: We included retrospectively all patients with liver cirrhosis who underwent both multiple computed tomography scans and hepatic venous pressure gradient (HVPG) measurements. Results: A total of 131 patients with liver cirrhosis were enrolled. The mean age of the patients was 53.7 years and alcoholic liver disease was the most common cause (61.8%). Sixty-four patients (48.9%) were diagnosed with sarcopenia. The median changes in skeletal muscle area per year (${\Delta}SMA/y$) were -0.89%. During a median follow-up period of 46.2 months (range, 3.4-87.6), 45 patients (34.4%) died. In multivariate analyses, age, Child-Pugh score, HVPG, presence of sarcopenia and ${\Delta}SMA/y$ were independently associated with mortality. Cumulative mortality was significantly higher in patients with ${\Delta}SMA/y$ < -2.4% than those with ${\Delta}SMA/y{\geq}-2.4%$ (log-rank test, P < 0.001). Conclusion: Both the presence and rate of change of sarcopenia are independently associated with long-term mortality in patients with liver cirrhosis.
