과민성대장증후군의 형상의학적 고찰 -동의보감(東醫寶鑑)을 중심으로- (Hypersensitive Large Intestine Syndrome in Hyungsang Medicine)

  • The writer reports the conclusions gained from study about the cause of the hypersensitive large intestine syndrome with Dongeuibogam as the central figure through researching the disharmony among Body Essence, Vital Energy, Mentality, and Blood, mutual action of five viscera and six bowels, and external shapes. The hypersensitive large intestine syndrome is generally chronic and recurred in many cases, so it is more efficacious than symptomatic to treat according to find the contradictions of individual shapes. The shapes and cases suffering frequently the hypersensitive large intestine syndrome are Gi-kwa and Sin-kwa, having a long nose, having a bruised spot on Triple warmer, man with inclined mouth, Taeeum type, man with congested fluids, man with colic symptoms. The hypersensitive large intestine syndrome in Oriental medicine is recognized of diarrhea, constipation, abdominal pain, abdominal distention and fullness caused by seven emotions. In Dongeuibogam it can be found out the similarity in depressive symptoms due to disorder of Gi, stagnation of Gi, dysphasia due to disorder of Gi, diarrhea due to disorder of Gi, fullness of due to Gi, diarrhea due to phlegm-retention, retention of undigested food, immoderate drinking, hypo-function of the spleen, or deficiency, abdominal pain from colic symptom, and difficulty in defecation and urination, internal injury, diarrhea due to weakness and fatigue. If the Jung, Gi, Sin, and Hyul composed the human body is broken harmony, the function of large intestinal transmission would be fallen, so similar symptoms like the hypersensitive large intestine syndrome are gotten. Especially Gi-kwa suffers diarrhea, constipation abdominal pain, and abdominal distention and fullness due to depressive symptoms from disorder of Seven emotions or Seven Gi. And Sin-kwa suffers from the hypersensitive large intestine syndrome due to emotional restlessness having an influence on rhythmic movement of abdomen. Examining between five viscera and six bowels and the hypersensitive large intestine syndrome, Liver cannot disperse well having influence on mutual relation of Liver-Large intestine, Heart reduces the function of defecation and urination not to control the seven emotions, Lung having exterior and interior relation with intestine has an influence on primordial energy and let the main symptoms occur, Spleen circulating the body fluid let the main symptoms occur due to malfunction of circulation, Kidney locating in lower part of the body has deep connection with intestine, so let the disorder. Urinary bladder is connected with intestine in moisture metabolism, Stomach is connected in receive and transmission, Small intestine is connected in absorption and excretion, from small intestine pain disturbing the abdominal movement, Samcho managing the catharsis of lower heater if declined its function causes the hypersensitive large intestine syndrome. The colic symptoms of Front private parts which disorder in lower abdomen give rise to abdominal pains, difficulty in defecation and urination due to Cold are similar to the hypersensitive large intestine syndrome. The treatments of applying the shapes of colic syndrome advocated by Master Park can be efficacious cure in clinic. Researching after the clinical cases of Master Park advocating Hyungsang medicine, we came to know that plenty of prescriptions of internal injury are applied and take good effects.

동연동성 좌측결장을 이용한 식도재건술 (Esophageal Reconstruction with Isoperistaltic Interposition of Left Colon)

  • The surgical experience on 18 patients with benign or malignant stricture of the esophagus who underwent isoperistaltic interposition of left colon from April 1989 to July 1991 was reviewed. During same period 22 esophageal reconstructions with colon were performed, but 3 patients who had intraabdominal adhesion in the left upper quadrant and one patient who had uncertainty of blood supply of left colic artery could not undergo iso-peristaltic interposition of left colon. There were 12 male and 6 female patients ranging from 16 to 65 years of age. 12 patients had corrosive esophageal stricture, two had cancer of esophagus, and another two had hypopharyngeal cancer. The postoperative complications developed in 7 patients [38.8%] and most frequently encountered complication was cervical anastomotic leakage, which was successfully managed with simple drainage in all cases but one malignant patient. There was no operative mortality. The esophageal reconstruction with isoperistaltic left colon resulted in good function in 14 patients[77.8%], fair in 3 patients[16.7], and poor in 1 patient[5.6%]. In this experience esophageal reconstruction using isoperistaltic left colon is a satisfactory method that can be accomplished with acceptable morbidity and mortality.

A Case of Biliary Ascariasis in Korea

  • Biliary ascariasis is still the leading cause of surgical complication of ascariasis, though its incidence has been dramatically reduced. Herein, we report a case of biliary ascariasis for the purpose of enhancing awareness of parasitic infections as a possible cause. A 72-year-old male visited the emergency room of Dankook University Hospital on 12 July 2015, complaining of right-upper-quadrant pain. By endoscopic retrograde cholangiopancreatography (ERCP), a tubular filling defect in the right hepatic duct was detected. The defect was endoscopically removed and diagnosed as an adult female of Ascaris lumbricoides worm, of 30 cm length. Upon removal of the worm, the pain subsided, and the patient was discharged without any complication. When treating cases of biliary colic, physicians should not neglect biliary ascariasis as the possible cause.

  • Careya arborea Roxb. (Lecythidaceae) is a significant medicinal plant known as kumbhi in Ayurveda. Though, most of the plant parts are used in traditional systems of medicine, bark and leaves constitute the important medicinal parts. The present review gives an account of the updated information on its phytochemical and pharmacological properties. The review reveals that numerous phytochemical constituents have been isolated from the plant possessing hepatoprotective, antioxidant, antimicrobial, anticoagulant, analgesic, antidiarrhoeal and various other important activities. Leaves are used in filaria, colic, loose motions and ulcers. Bark is used as an antipyretic, abortifacient, antipruritic and in smallpox, urinary discharges and rheumatic pain. Since last few decades, extensive exploration has been done to establish the biological activities and pharmacology of the extracts and plentiful chemical constituents including flavonoids, tannins, alkaloids, terpenoids and many other have been isolated.

  • The sleeplessness in childhood is quite different from that in adulthood in terms of causes, developmental process, and treatment. Sleep behavior in childhood is strongly influenced by parental personality and familial and cultural background. In understanding and management of sleeplessness of children, it is especially important to understand the separation anxiety and the ways of its management in bedtime because bedtime routine with children one of separation process from parents. Co-sleeping, parental intervention, transional object and bedtime routines can be appeared in order to reduce the anxiety from bedtime separation. Causes of sleeplessness in infant and toddler are bad sleep-onset association, nocturnal drinking, colic, and food allergy. In preschool and school aged children, limit-setting sleep problem and fears and nightmare can be causes of sleeplessness. When good sleep environment and habits are established sound sleep and more mature personality can be developed.

  • Much recent research on protein and amino acid (AA) digestive physiology of pigs has been concerned with measurement of the ileal apparent and true digestion and absorption. For measurement of the ileal apparent digestibility of AA, the steered ileo-caecal valve cannulation (SICV) and the ileo-rectal anastomosis (IRA) techniques appear to be the more reliable and simple methods, when compared with any methods requiring use of a marker for calculation of digestibility, or with the complex techniques of ileo-caecal re-entrant cannula (ICRC) and the postvalve ileo-colic re-entrant cannula (IPVC). On the other hand, the peptide alimentation ultrafiltration methods might be a better choice for measurement of the ileal endogenous nitrogen (N) and AA flow in a routine feedstuff analysis, although the classical method of $^{15}N-isotope$ dilution method is still a standard method for N and AA nutrition research in pigs.

  • Congenital duplication of the gallbladder, including true duplication and septated gallbladder, is an uncommon but potentially complicated malformation. It presents biliary colic associated with acute or chronic cholecystitis. It can be diagnosed preoperatively by various imaging tools such as real-time ultrasonography, biliary scintigraphy, oral cholecystography, or endoscopic retrograde cholangiography. It can be safely managed by either laparoscopic or open procedures. This report describes a 6-year-old girl with true duplication of gallbladder diagnosed preoperatively by real-time ultrasonography and treated by open cholecystectomy.

  • The bark of Careya arborea Roxb. (Barringtoniacae) is used against various disorders in Indian systems of medicine, namely astringent, anthelmintic, in tumors, dyspepsia, colic, bronchitis, urinary discharges, piles, lecucoderma, skin diseases, epileptic fits, diarrhoea. The juice of the fresh bark with honey is given as a demulcent in coughs and colds, infusion of the fresh bark is used to treat snake bite by applying to the bitten part and also taken internally. The present communication deals with the detailed pharmacognostical evaluation of the bark sample using light and confocal microscopy, WHO recommended physico-chemical determinations and authentic phytochemical procedures. The physico-chemical, morphological and histological parameters presented in this paper may be proposed as parameters to establish the authenticity of C. arborea bark and may possibly help to differentiate the drug from its other allied species.

Birth Weight and the Development of Functional Gastrointestinal Disorders in Infants

  • Purpose: To assess the association between birth weight and the development of functional gastrointestinal disorders (FGIDs) in the first year of life. Methods: This is a secondary analysis of a prospective cohort multicenter study including neonates, consecutively enrolled at birth, and followed up for one year. At birth all infants were classified by birth weight as extremely low (ELBW), very low, or low when <1,000, <1,500, and <2,500 g, respectively, and by birth weight for gestational age as appropriate (AGA, weight in the 10-90th percentile), small (SGA, weight <10th percentile), and large (LGA, weight >90th percentile) for gestational age. FGIDs were classified according to the Rome III criteria and assessed at 1, 3, 6, and 12 months of life. Results: Among 1,152 newborns enrolled, 934 (81.1%) completed the study: 302 (32.3%) were preterm, 35 (3.7%) were ELBW, 104 (11.1%) were SGA, 782 (83.7%) were AGA, and 48 (5.1%) were LGA infants. Overall, throughout the first year of life, 718 (76.9%) reported at least one FGID. The proportion of infants presenting with at least one FGID was significantly higher in ELBW (97%) compared to LBW (74%) (p=0.01) and in LGA (85.4%) and SGA (85.6%) compared to AGA (75.2%) (p=0.0001). On multivariate analysis, SGA was significantly associated with infantile colic. Conclusion: We observed an increased risk of FGIDs in ELBW, SGA, and LGA neonates. Our results suggest that prenatal factors determining birth weight may influence the development of FGIDs in infants. Understanding the role of all potential risk factors may provide new insights and targeted approaches for FGIDs.

대장악성종양수술 후 배변장애에 대한 치험 1례 (A Clinical Review of Bowel Disorders following Colorectal Cancer Surgery)

  • In July, 2000, A 71-year-old man complained of these symptoms: frequent defecation, urgent need to defecate, fecal leakage, anal pain, flatus, and diarrhea after surgery for colorectal cancer. The symptoms are caused specifically by injury to the bowel and sphincter function. The patient suffered physically and psychologically; but, there was no special treatment in western medicine. Therefore, we treated him with electrical acupuncture for the management of sphincter tone and the alleviation of anal pain. We also treated him with herbal medicine for the recovery of strength and gastro-colic function. As a consequence of these treatments, the bowel disorders improved immediately and successfully, even though there were some limitations. So, We report this case with a brief review of related literatures.
