Study on Clinical Diseases of Blood Deficiency Pattern (혈허증(血虛證)의 임상 질환 범위에 대한 고찰)

  • This article is a study on to which categories of modern diseases blood deficiency pattern types are assigned by reference to modern clinical papers. Clinical papers were searched in China National Knowledge Infrastructure(CNKI) from 1994 to 2013. Results are as follows. First, diverse diseases classified in qi-blood depletion pattern and pattern of blood deficiency and wind-dryness are reported and pattern types designated by the name of viscera are the minority. Second, among pattern types in Korean Standard Classification of Diseases(KCD), diseases classified in heart blood deficiency pattern, liver blood deficiency pattern and heart-liver blood deficiency pattern are a few. Third, the level of designation by the combined patterns such as qi deficiency, fluid deficiency, yin deficiency, kidney deficiency, essence deficiency, wind-cold, cold-dampness, dampness-heat, liver hyperactivity, liver depression and static blood is more specific than KCD, which makes pattern types more useful to clinical application. The detailed relation between modern diseases and pattern types can be an another topic.

A Clinical Case Study on Climacteric Woman with Numbness of Lower Limbs (하지마목을 동반한 갱년기 환자 치험 1 례)

  • Purpose : This study was performed to evaluate the efficacy of herb medicine on climacteric woman with numbness of lower limbs by using Digital Infrared Thermographic Imaging(DITI). Methods : The patient was 56-year-old female who was suffering from numbness, cold hypersensitivity of lower limbs and hot flush. We treated her using herb medicine, acupuncture and moxibustion. The progress of symptoms were evaluated by DITI, and then we compared DITI before treatment and after treatment. Results : After treatment, visual analog scale of numbness and cold hypersensitivity of lower limbs were decreased and frequency of hot flush was also decreased. The temperature of forehead got lower than that before treatment and the temperature of LR3 and ST32 got higher than those before treatment on DITI. Conclusion : This result suggest that DITI can be used for assessment after treatment on climacteric symptoms with numbness of lower limbs.

Recent Research Trends of Sayeok-tang Used in Ischemic Heart Disease (허혈성 심질환에 활용된 사역탕(四逆湯)의 최신 연구 동향)

  • Purpose : Sayeok-tang (Sini decoction, SND) is a cold-dispelling formula used for cold deficiency syndrome and is composed of Aconiti Lateralis Radix Preperata, Zingiberis Rhizoma, and Glycyrrhizae Radix et Rhizoma. It is used for diseases such as myocardial infarction, heart failure, acute and chronic gastroenteritis, and gastric effusion. This study proposes the possibility of expansion of basic research and clinical applications for ischemic heart disease (IHD) through systematic analysis of domestic and foreign studies on SND. Methods : We collected studies within the last 10 years on the use of SND in IHD and excluded those lacking relevance. Selected studies were classified by research method and the main themes of the studies were analyzed for each classification. Result: Out of 15 studies, there were 5 animal studies, 8 metabolite analyses in animals, 1 in vitro study, and 1 systematic review. Our review suggests that SND may be used as an adjuvant to nitroglycerin and percutaneous transluminal coronary angioplasty, and may improve symptoms and quality of life of patients with IHD. Myocardial protective effects through antioxidant, anti-inflammatory, anti-apoptotic, and anti-hypertensive actions were confirmed through these studies. Effects on carbohydrate, protein, and lipid metabolism were also reported. Conclusions : This study suggests that SND has potential as a treatment for IHD.

  • 1. Objectives This study is purposed to classify deficiency syndrome(虛證) and Excess syndrome(實證) of chief lung-disorder diseases. 2. Methods It was researched on the comparative and literal study about the relation to Yeong-Chu Gyeong-maek Chapter (${\ulcorner}$靈樞 經脈編${\Ircorner}$) and Dongui-Bogam (${\ulcorner}$東醫寶鑑${\Ircorner}$) and Dongui-Susebowon (${\ulcorner}$東醫壽世保元${\Ircorner}$) in chief lung-disorder symptoms or diseases. 3. Results and Conclusions (1) The chief lung-disorder diseases of Yeong-Chu Gyeong-maek Chapter (${\ulcorner}$靈樞 經脈編${\Ircorner}$), Dongui-Bogam (${\ulcorner}$東醫寶鑑${\Ircorner}$) and Dongui-Susebowon (${\ulcorner}$東醫壽世保元${\Ircorner}$) are asthma(喘), coughing(咳), fullness in the chest(胸滿), sweating symptoms(汗出), cold-heat symptoms(寒熱), dysphoria(煩躁) and sneezing. (2) Asthma symptom(喘) was shown to Soeumin's 4 exterior deficiency diseases, Taeumin's 1 exterior deficiency disease and 1 interior excess disease, and Soyangin's 1 interior excess disease. (3) Coughing symptom(咳) was shown to Soeumin's 1 extrerior deficiency disease and Taeumin's 1 interior excess disease. (4) Fullness in the chest(胸滿) was shown co Soyangin's 4 exterior excess diseases. (5) Sweating symptoms(汗出) was shown to Soeumin's 10 exterior deficiency diseases, Taeumins's 1 exterior deficiency disease, and Soyangin's 1 exterior defecieny disease. (6) Cold-heat symptoms(寒熱) was shown to Soyangin's 4 exterior excess diseases. (7) Dysphoria(煩躁) was shown co Soeumin's 1 exterior deficiency disease and 8 interior deficiency diseases, Taeumin's 1 interior excess disease, and Soyangin's 4 exterior excess and interior excess diseases. (8) Sneezing symptom was shown to Taeumin's 1 exterior excess disease.

The Analysis on Characteristics according to Sasang Constitution of Patients with Parkinson's Disease in a Korean Medicine Hospital (일개 한방병원에 파킨슨병으로 내원한 환자의 사상의학적 특성에 관한 고찰)

  • Objectives The aim of this study was to make an analysis of clinical characteristics and status of Sasang constitutional medicine of patients with Parkinson's Disease who visited a Korean medicine hospital. Methods We studied 129 patients with Parkinson's Disease who visited a Korean medicine hospital from September 1, 2012 to June 30, 2018. We retrospectively collected medical history and characteristics of subjects using data of EMR (electronic medical record). Results The proportion of female was higher than male. The average age was 60s. The patients visited a hospital more than one year after the occurrence of symptoms. And they were mostly under western medicine treatment. Among the 129 patients, 56 persons(43.4%) were Taeeumin, 45 persons(34.9%) Soyangin and 28 persons Soeumin(21.7%). In the distribution of prescription according Sasang Constitutional Medicine symptomatology, there was no significant difference in the proportion of exterior cold disease(48.9%) and interior heat disease(51.1%) in Soyangin. Meanwhile the proportion of heat disease's prescription was higher than cold disease in Taeeumin(73.2%) and Soeumin(84.1%). The majority of disease were Dry-febrile symptom(73.2%) of heat disease in Taeeumin, Yang depletion-syndrome(64.3%) of heat disease in Soeumin, and Gyolhyung-symptom(33.3%) of exterior cold disease and Wasting thirst-syndrome(22.2%) of interior heat disease in Soyangin, Conclusions By Developing of guideline for treatment of Parkinson's Disease through analyzing on clinical characteristics and distribution of Sasang Constitutional Medicine symptomatology could contribute to the improvement of the cure rate and accuracy of constitution pattern.

  • Objectives & Methods : I investigated 45 literature of Oriental and Western medicine about the treatment of pain in shoulder and arm. Result and Conclusion : 1. The etiological causes of Pain in Shoulder and Arm based on literatures of Oriental medicine are attack of wind-heat on the lung, wind cold, damp-heat struggle between the vital energy and pathogenic factor and six pathogenic factors. And all these causes are the conception of blockage syndrome, Qi and blood stagnating in meridian system. 2. The treatment of Pain in Shoulder and Arm based on Oriental medicine is mainly composed of both medical therapy for Bi syndrome due to pathogenic wind, deficiency of both Qi and blood, consumption of the liver and the spleen, and also acupuncture and moxibustion treatment by selection for acupoint. And those treatments are for treating etiology. And also there are treatments using the meridian system and Twelve Muscle Region and Ashihyeol for the purpose of treating the symptoms. 3. The etiological causes of Pain in Shoulder and Arm based on literatures of Western medicine are degenerative cut of tendon and nerve symptoms caused by tendonitis, bursitis, calcification, ruptured cervical disc and thoracic outlet syndrome. 4. The treatment of Pain in Shoulder and Arm based on Western medicine is for alleviation of pain, such as giving an anodyne, steroid products, local anesthetic injection and stretching and strengthening the muscles.

  • The Dryness and the Dampness is the congential nature. the Water and Fire is the acquired action. the skin is related to the physiological function of the lung, so the skin is the domain of the lung. the lung belongs to the dryness-metal(燥金). And the Dryness and the Dampness in the six pathogenic factors exert a important pathogenic influence upon the skin. Psoriasis is a chronic skin condition charactered by red, silver scaly patches in the affected areas, it is caused by excessive growth of the top layer of the skin(epidermis). Scalp psoriasis affects at least two thirds people who have psoriasis. Generally, the scalp will have plaque psoriasis characterized by elevated, inflamed, lesions covered with silvery white scales. The lesion of psoriasis is head, elbow and knee that are yang-channels in Oriental medicine. The syndrome falls with in the purview of the dryness-metal(燥金). In time, psoriasis takes a turn for the worse in the winter season. In space, many of people in comparison to the average in the world have psoriasis in high latitude that is dry and cold, for instance, the Scandinavian peninsula. Deficiency of yin-fluid and blood course often the dryness-syndrome, for instance, psoriasis, etc. So psoriasis is one of the dryness-syndrome in Oriental medicine.

Hypersensitive Large Intestine Syndrome in Hyungsang Medicine (과민성대장증후군의 형상의학적 고찰 -동의보감(東醫寶鑑)을 중심으로-)

  • The writer reports the conclusions gained from study about the cause of the hypersensitive large intestine syndrome with Dongeuibogam as the central figure through researching the disharmony among Body Essence, Vital Energy, Mentality, and Blood, mutual action of five viscera and six bowels, and external shapes. The hypersensitive large intestine syndrome is generally chronic and recurred in many cases, so it is more efficacious than symptomatic to treat according to find the contradictions of individual shapes. The shapes and cases suffering frequently the hypersensitive large intestine syndrome are Gi-kwa and Sin-kwa, having a long nose, having a bruised spot on Triple warmer, man with inclined mouth, Taeeum type, man with congested fluids, man with colic symptoms. The hypersensitive large intestine syndrome in Oriental medicine is recognized of diarrhea, constipation, abdominal pain, abdominal distention and fullness caused by seven emotions. In Dongeuibogam it can be found out the similarity in depressive symptoms due to disorder of Gi, stagnation of Gi, dysphasia due to disorder of Gi, diarrhea due to disorder of Gi, fullness of due to Gi, diarrhea due to phlegm-retention, retention of undigested food, immoderate drinking, hypo-function of the spleen, or deficiency, abdominal pain from colic symptom, and difficulty in defecation and urination, internal injury, diarrhea due to weakness and fatigue. If the Jung, Gi, Sin, and Hyul composed the human body is broken harmony, the function of large intestinal transmission would be fallen, so similar symptoms like the hypersensitive large intestine syndrome are gotten. Especially Gi-kwa suffers diarrhea, constipation abdominal pain, and abdominal distention and fullness due to depressive symptoms from disorder of Seven emotions or Seven Gi. And Sin-kwa suffers from the hypersensitive large intestine syndrome due to emotional restlessness having an influence on rhythmic movement of abdomen. Examining between five viscera and six bowels and the hypersensitive large intestine syndrome, Liver cannot disperse well having influence on mutual relation of Liver-Large intestine, Heart reduces the function of defecation and urination not to control the seven emotions, Lung having exterior and interior relation with intestine has an influence on primordial energy and let the main symptoms occur, Spleen circulating the body fluid let the main symptoms occur due to malfunction of circulation, Kidney locating in lower part of the body has deep connection with intestine, so let the disorder. Urinary bladder is connected with intestine in moisture metabolism, Stomach is connected in receive and transmission, Small intestine is connected in absorption and excretion, from small intestine pain disturbing the abdominal movement, Samcho managing the catharsis of lower heater if declined its function causes the hypersensitive large intestine syndrome. The colic symptoms of Front private parts which disorder in lower abdomen give rise to abdominal pains, difficulty in defecation and urination due to Cold are similar to the hypersensitive large intestine syndrome. The treatments of applying the shapes of colic syndrome advocated by Master Park can be efficacious cure in clinic. Researching after the clinical cases of Master Park advocating Hyungsang medicine, we came to know that plenty of prescriptions of internal injury are applied and take good effects.

An Ethnographic Study of Sanhubyung experienced by Women in Korean Postpartal Culture (한국의 산후 문화와 여성이 경험한 산후병에 관한 일상생활기술적 연구)

  • This ethnogrphic exploratory study sought to de-fine the meaning of Sanhubyung, as consequence from the perspective of the women who experienced it. A convenience sample of 9 elderly women in San Francisco. and 20 postpartal women and their 20 non-professional helpers during postpartum in Seoul, Korea were observed and interviewed for 23 months from January 1991 to December 1992 at the Human Development Center in San Francisco and at the hospital and their homes in Seoul, Korea. Sanhubyung was regarded as the consequence of "Doing a Sanhujori Wrongly," as a group of symptoms or sequelae which have two types of characteristics of symptoms . chronic and acute. It can be called a culture bound syndrome in the cultural context re-lated to childbearing phenomenon in Korea. If women violate the principles of Sanhujori, such symptoms can appear at various times : during the period of postpartum itself, at any time, periodically, especially at the anniversary of the child's birth, late forties, and in old age. Acute symptoms that can be classified into immediate and late types include painful and edematous gingiva, sensitive teeth, strange sensation and pain in the knees or backache. Besides, there is a localized sense of soreness and pain ; sense of being in a draft and cold, stomach upset, GI irritation, chilling, shivering, and tiredness, pain and dazzling in the eyes. Chronic symptoms occur in the head, neck, teeth, back, hands, knees, hands and feet, arms and legs, eyes, sinews and joints, bones, and in the body or as a whole. Generally these symptoms are pain, often accompanying a feeling of being cold and in a draft, regardless of actual weather conditions. In conclusion, this findings reflect the Oriental way of thought of causal relationship of women's health and illness based on the wholistic paradigm of harmony and balance of two forces, Yin(cold) -Yang(hot). It provides a challenge to the professional sector to rethink the effect of culture on health and illness. Finally, it suggests care providers use cultural assessment for the appropriateness of the intervention and quality of care for desirable health outcomes.

Anti-allodynic Efficacy of NMDA Antagonist Peptide and Noradrenaline Alone and in Combination in Rodent Neuropathic Pain Model

  • Background: The present experiment was conducted to identify the cooperative effect of serine histogranin (SHG) and noradrenaline in alleviating peripheral neuropathic pain. Methods: Chronic constriction injury of the right sciatic nerve was used to induce chronic neuropathic pain. For drug delivery, a PE10 tube was inserted into the subarachnoid space. Acetone drops and a $44^{\circ}C$ water bath were used to evaluate the cold and heat allodynia, respectively. Placing and grasping reflexes were used to assess the locomotor system. Results: SHG at 0.5 and $1{\mu}g$significantly (P < 0.05) decreased the thermal allodynia. The cold allodynia was also significantly reduced by intrathecal injections of 0.5 (P < 0.05) and $1{\mu}g$(P < 0.001) of SHG. $1{\mu}g$of noradrenaline, but not $0.5{\mu}g$, significantly alleviated the cold (P < 0.01) and thermal (P < 0.05) allodynia. The ameliorating effect of noradrenaline or SHG disappeared when the two compounds were administrated in equal concentrations. A significant difference (P < 0.01 in the acetone and P < 0.05 in the heat) was observed in the groups under equal doses of the two compounds, with a lower effectiveness of the combination therapy. Conclusions: Our findings suggest that the simultaneous administrations of noradrenaline and SHG do not result in synergistic analgesia, and combination therapy may not be a good approach to the treatment of chronic neuropathic pain syndrome.
