Effects of Home-Visit Program with Cognitive-Stimulating Activities on Cognitive Function and Memory Self-efficacy of an Elderly with Dementia Special Rating : Single Subject (가정방문 인지자극 활동이 치매특별등급(5등급) 노인의 인지기능 및 기억 자기효능감에 미치는 효과 : 개별대상연구)

  • This study involved the effects of home-visit program with cognitive-stimulating activities on cognitive function and memory self-efficacy of an elderly with dementia special rating(5rates). The study was conducted from February 11 to April 26, 2019 using a 65-year old woman residing in G Metropolitan City. The individual subject study used an A-B-A design. The subject performed activities that required thinking and resolving in order to stimulate both her body and cognition. The activities chosen were easily accessible to those having a form of mild dementia, rates 5 dementia special rating, and repetitively stimulated the subject's intellectual capacity in an attempt to increase her degraded cognitive functioning. Specifically, the activities exercised both short-term memory and recollection using familiar letters, numbers, and bodily movements frequently encountered in daily life. The program positively affected the subject's memory and concentration. In addition, the program was found to be feasible for home use by those with mild dementia, obviating the need to travel to a location like a welfare center. The author presents specific activities for stimulating cognition that can effectively intervene to positively influence the cognitive functioning and memory of those with rates 5 dementia special rating.

Mediating Effect of Cognitive Function on the Relationship Between Vision, Hearing, and Activities of Daily Living (노인의 시각, 청각 및 일상생활활동의 관계에 대한 인지기능의 매개효과)

  • Objective : The typical decline in function due to aging includes limitations in vision, hearing, cognitive function, and activities of daily living. Therefore, this study evaluated the mediating effect of cognitive function on the relationship between vision and hearing and activities of daily living. Methods : The participants in this study were 36,767 persons from the long-term care database on older adults provided by the National Health Insurance Sharing Service. The extracted variables were: (i) independent variables: visual and hearing; (ii) dependent variables: basic and instrumental daily living activities; and (iii) mediator variables: cognitive function. Regression analysis was performed evaluate the mediating effects of the extracted variables. Results : Cognitive function showed a mediating effect of 30% on the relationship between vision and basic daily life activities and 42% in the relationship with instrumental daily life activities. The mediating effect was 50% in the relationship between hearing and basic activities of daily living, and 53% in the relationship with instrumental activities of daily living. Conclusion : Prevention programs for functional decline due to aging are in progress in Korea. Based on the results of this study, establishing a prevention program would help promote successful aging.

Effects of Coping on Physical and Psychosocial Adaptation by Illness Duration in Patients with Rheumatoid Arthritis (류마티스 관절염 환자의 질병 기간에 따른 대처 유형과 적응과의 관계)

  • The effects of coping on physical and psychosocial adaptation in the 297 patients with rheumatoid arthritis were investigated. The coping methods were divided into compliance of medical regimens, self-control activities, and cognitive control. The effects of these coping methods was analyzed with stepwise regression. The physical adaptation is found to be significantly affected by cognitive control, self-control activities, and compliance of medical regimens in order Compliance of the medical regimens affects negatively the physical adaptation. And psychosocial adaptation is significantly affected by the cognitive control. The analysis of these results by illness duration showed the distinct differences. That is in the patients who experienced the disease less than 48 months, the physical and the psychosocial adaptation were significantly affected only by the cognitive control. But in the patients who experienced the disease more than 96 months, the physical and the psychosocial adaptation were significantly affected by self-control activities. Based upon these results, it is recommended that the nurses who care the newly diagnosed patients emphasize on the positive side of the state. And the patients who are diagnosed long ago need the nursing programs which teach and promote self-control activities. Also it is suggested that the results of compliance must be considered rather than as the result variable.

Development of cognitive-behavioral group counseling program for elementary children with speech anxiety and its effects (초등학생의 발표 불안 감소를 위한 인지적 행동주의 집단상담의 효과)

  • The purpose of this study was to develop the cognitive behavioral group counseling program to relieve elementary children of speech anxiety and to examine its effects. The program was developed on the basis of the cognitive, affective, and behavioral activities. The cognitive activities were based on the REBT procedures, and affective activities included making child's nickname, finding out his own strengths and exchanging positive feedback each other and behavioral activities included training assertiveness, coaching and practicing speech behavior etc, Subjects were 14 elementary children from M elementary school in Gwangju. They had the highest scores at the speech anxiety test which was administrated to all the sixth graders of M elementary school. Seven subjects were randomly allocated to experimental group and control group respectively. Two speech anxiety tests and one speech behavior checklist were administrated as pre- and post-tests. The collected data was analyzed by ANCOVA. Research results demonstrated that the experimental group showed statistically significant changes in the scores of the speech anxiety test and the speech behavior checklist comparing with the control group. Then it was accepted that the program which was developed in this study could make effects on the reduction of elementary children's speech anxiety.

Cognitive Strategy Intervention Affects Performance Skills of Daily Living in Individuals with Stroke: A Randomized Controlled Trial (뇌졸중 환자의 인지적 전략개발 중심 중재가 수행기술에 미치는 효과 비교 : 무작위 비교연구)

  • Objectives : The aim of this study was to compare a treatment focusing on the physical function to an intervention focusing on a cognitive strategy in stroke patients which improves their performance skills of daily activities. Methods : This study design was a randomized control trial selecting 43 people with stroke patients. This study consisted of a control group, which received conventional occupational therapy focusing on physical function, and an experimental group which was trained to develop a cognitive strategy by themselves. Both groups each received 10 sessions of the treatment. This study compared the skills for performing daily activities before and after the intervention and analyzed the data with SPSS Ver. 18.0. Results : This study showed a significant improvement in all performance skills in the experimental group (p<.05). There was no statistically significant difference in the performance skills before and after the intervention in the control group (p>.05). Conclusions : It was verified that stroke patients to develop a cognitive strategy by themselves is more effective than to improve the physical function in performance skills for daily activities.

Effects of a Cognitive Improvement Program on Cognition, Activities of Daily Living (ADL), Depression, Life Satisfaction, and Grasping Power in Small Groups (소그룹 인지기능강화 프로그램이 인지기능, 일상생활활동, 우울, 생활만족도, 악력에 미치는 영향)

  • Purpose: The purpose of this study was to evaluate effects of a cognitive improvement program on cognition, activities of daily living (ADL), depression, life satisfaction, and grasping power in small groups. Methods: A nonequivalent control group pretest-posttest design was used. Participants were 127 elders who were never diagnosed with dementia and could read (experimental: 61, control: 66). The program was consisted of 8 sessions to improve vocabulary and writing skills and fine motor skills. Each session was applied once a week for 8 weeks. A group was consisted of less than five members. Results: Cognition (t=3.82, p<.001), depression (t=-2.24, p=.027), life satisfaction (t=2.25, p=.027), and grasping power (Rt: t=2.44, p=.016, Lt: t=2.63, p=.010) except ADL (t=-0.49, p=.622) were significantly increased in the experimental group after the cognitive improvement program compared to the control group. Conclusion: This study indicates that the cognitive improvement program was effective to improve cognition, depression, life satisfaction and grasping power. Further study is needed in order to identify continuous effects of this cognitive improvement program.

How Do Oroxylin A and Spinosin Exert Their Activities on Cognitive Function?

  • Flavonoids are mainly contained in the vegetables and medicinal herbs. Until now, over 5,000 kinds of flavonoid have been identified and their biological activities have been reported. Among them, we are interested in oroxylin A and spinosin because of their specific structures having bulky group at C-6 of ring A. Oroxylin A is contained in the Scutellaria baicalensis and exhibits cognitive enhancing activity as a GABAA receptor antagonist, which is different from those of mainly contained in the S. baicalenis, baicalein or wogonin. Spinosin is isolated from Zizyphus jujuba var. spinosa and mainly studied as a hypnotic or anxiolytic agent because of traditional knowledge about its original herb. As far as we know, the cognitive function of spinosin was first identified by our group. In this review, we discuss how such flavonoids exert their pharmacological activities associated with cognitive function based on the receptor binding study and behavioral studies. Traditional knowledge and reverse pharmacology may be addressed in the research field of phytochemical pharmacology and useful to unveil the secret of phytochemicals.

Influence of Cognitive Function and Depressive Symptoms on Instrumental Activities of Daily Living in Community-dwelling Older Adults (지역사회 노인의 인지기능과 우울감이 도구적 일상생활동작에 미치는 영향)

  • Purpose: The purpose of this study was to explore the influence of cognitive function and depressive symptoms on instrumental activities of daily living (IADL) in addition to identify the factors associated with IADL in community-dwelling older adults. Methods: This was a descriptive study with cross-sectional design. Data were collected from July 2013 to June 2014. A total of 143 participants without dementia, depression and disability were enrolled in this study. Cognitive function was measured using Seoul verbal learning test (SVLT), digital span (forward/backward) and fist-edge-palm test. The Korean-IADL and Short Geriatric Depression Scale (S-GDS) were used. Data analysis was performed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and hierarchical regression. Results: Overall, a multiple regression model revealed that approximately 27.4% of total variability in IADL in the sample of community-dwelling older adults could be explained by the significant 12 variables in this model ($R^2=0.274$, F=5.467, p<.001). Age, having religion and cognitive function were the predictors of IADL in community-dwelling older adults. Conclusion: This study suggest that we need to monitor cognitive function in older to maintain the ability for IADL in older adults. Also, individualized program for improving older adults' IADL should be included in nursing intervention.

Effect of Obstacle Walking Training Combined with Cognitive Tasks on Balance, Gait, and Activities of Daily Living in Patients with Stroke: A Single Case Study (인지과제를 결합한 장애물 보행훈련이 뇌졸중 환자의 균형과 보행능력 및 일상생활동작에 미치는 효과 -단일사례연구-)

  • Purpose: This study aims to compare the effects of obstacle walking training combined with cognitive tasks on balance, gait, and activities of daily living in patients with stroke. Methods: A single-subject design was used, where one stroke patient participated. Obstacle walking training combined with cognitive tasks was performed for 1 hour per day for a total of 10 times during the intervention phase. The subjects were measured five times in the baseline phase, 10 times in the intervention phase, and five times in the follow-up phase. The outcome measurements included the Berg balance scale (BBS), the 10-meter walk test (10 MWT), and the Korean modified Barthel index (K-MBI). Results: In this study, the results showed that the 10 MWT scores during the intervention period improved and that this improvement remained, even during the post-period. In addition, BBS and K-MBI values for stroke patients increased significantly after training. Conclusion: The results of this study revealed that obstacle walking training combined with cognitive task training may be helpful to improve balance, gait, and activities of daily living in stroke patients. Therefore, obstacle walking training combined with cognitive tasks is recommended for stroke patients.

The Effect of Cognitive Decline of Elderly on Activity Participation and Social Network (노년기의 인지 저하가 활동참여와 사회적 관계망에 미치는 영향)

  • Objective : The purpose of this study was to investigate the effect of cognitive level of mild cognitive impaired elderly living in the community on participation in activities and social networks as well as leisure and daily life activities. Methods : The elderly people aged 65 years or older living in the community were selected for the mild cognitive impairment with a score of MMSE-K of 15 or more and 23 or less. MoCA-K was applied in order to examine the cognitive abilities of the subjects and K-ACS and LSNS-18 were applied to confirm the activity participation level and social network index. Results : As a result of examining the relationship between cognition and activity participation, various sub-abilities(spatio-temporal abilities, vocabulary abilities, attention, abstract thinking) of cognition had a significant influence on instrumental activities of daily living, leisure activities and social activities. In addition, all of the sub - abilities of cognition showed a significant correlation with sub-elements of social network. Conclusion : Through this study, it was found that the general elements of cognition influenced activity participation and social network of various areas of mild cognitive impaired elderly people. We suggest that measures for enhancing participation and social participation of the elderly with mild cognitive impairment are provided through follow up studies.
