Five Cases of Nonstructural Scoliosis with Lumbar Herniated Intervertebral Disc Treated by Complex Korean Medicine Treatment with Chuna Manual Therapy: Case Report (추나요법을 적용한 한방복합치료로 호전된 요추 추간판 탈출증으로 인한 비구조적 척추측만증 5례: 증례보고)

  • Objectives : This case report describes the management of five patients suffering from nonstructural scoliosis with lumbar herniated intervertebral disc all treated with complex Korean medicine treatment. Methods : Five patients were hospitalized and treated with herbal medicine, acupuncture, pharmacopuncture, cupping, and Chuna manual therapy. Patients were assessed for Numerical Rating Scale (NRS), Oswestry Disability Index (ODI), EuroQol-FiveDimensions (EQ-5D), and Cobb's angle. Results : In all patients, NRS, ODI, and Cobb's angle decreased, while the EQ-5D score increased. Conclusions : As seen in these five cases, complex Korean medicine treatment with Chuna manual therapy has effects on the management of nonstructural scoliosis.

A Thoracic Spine Segmentation Technique for Automatic Extraction of VHS and Cobb Angle from X-ray Images (X-ray 영상에서 VHS와 콥 각도 자동 추출을 위한 흉추 분할 기법)

  • In this paper, we propose an organ segmentation technique for the automatic extraction of medical diagnostic indicators from X-ray images. In order to calculate diagnostic indicators of heart disease and spinal disease such as VHS(vertebral heart scale) and Cobb angle, it is necessary to accurately segment the thoracic spine, carina, and heart in a chest X-ray image. A deep neural network model in which the high-resolution representation of the image for each layer and the structure converted into a low-resolution feature map are connected in parallel was adopted. This structure enables the relative position information in the image to be effectively reflected in the segmentation process. It is shown that learning performance can be improved by combining the OCR module, in which pixel information and object information are mutually interacted in a multi-step process, and the channel attention module, which allows each channel of the network to be reflected as different weight values. In addition, a method of augmenting learning data is presented in order to provide robust performance against changes in the position, shape, and size of the subject in the X-ray image. The effectiveness of the proposed theory was evaluated through an experiment using 145 human chest X-ray images and 118 animal X-ray images.

Diurnal Variation in Hydration of the Cervical Intervertebral Disc Assessed Using T2 Mapping of Magnetic Resonance Imaging

  • Objective: The study aimed to investigate the diurnal variation in cervical disc hydration and its relationship with cervical degeneration. Materials and Methods: C3-C7 discs of 86 prospectively enrolled participants (37 males, 49 females; mean age ± standard deviation, 23.5 ± 2.5 years) were assessed using T2 mapping in the morning and evening. All discs were stratified by Miyazaki grade or C2-C7 Cobb angle and T2 values (T2). The degree of diurnal T2 variation (T2-DDV), defined as (morning T2 - evening T2)/morning T2 x 100%, was measured for the entire disc, annulus fibrosus (AF), nucleus pulposus (NP), and endplate zones. Results: T2 of the entire disc decreased significantly after the daytime load (p < 0.001), with a T2-DDV of 13.3% for all discs and 16.0%, 12.2%, and 13.0% for healthy (grade I), mild degenerative (grade II), and advanced degenerative (grade III/IV) discs, respectively. T2 of regional NPs and AFs decreased significantly from morning to evening (p ≤ 0.049) except in the healthy anterior inner AF (p = 0.092). Compared with healthy discs, mild degenerative discs displayed lower T2 and T2-DDV in regional NPs (p < 0.001). Advanced degenerative discs showed higher T2-DDV in the anterior inner AF compared with healthy discs (p = 0.050). Significant diurnal T2 changes in the endplate zones were observed only in healthy discs (p = 0.013). Cervical discs in the low Cobb angle group showed higher T2-DDV in the anterior AFs and anterior NP and lower T2-DDV in the posterior AF than those in the high Cobb angle group (p ≤ 0.041). Conclusion: This study characterized the diurnal variation in hydration of the cervical discs as assessed using T2 mapping and revealed early chemo-mechanical coupling dysfunction in degenerating discs. Cervical sagittal alignment on MRI can affect the diurnal stress patterns of the cervical discs. T2 mapping is sensitive to disc biomechanical dysfunction and offers translational potential from biomechanical research to clinical application.

Association between Prognosis of Low Back Pain Patients with Severity of Abnormal Lumbar Lordosis (한방병원에 입원한 요통 환자의 요추 전만각, 요천각 변화의 심각도에 따른 예후 분석 연구)

  • Objectives To analyze the prognosis of patients with low back pain according to the severity of abnormal lumbar lordotic angle and Ferguson's angle. Methods Data from electronic medical record of a total of 199 patients hospitalized in Korean medicine hospital were analyzed. With the sagittal view of lumbar spine X-ray, lumbar lordotic angle was defined as the angle between the superior endplate of L1 with the inferior endplate of L5. Ferguson's angle was defined as the angle between the superior surfaces of the sacral with a horizontal line. 'Measure Cobb's Angle Tool' of 'INFINITT PACS' was used to measure both angles. Lumbar lordotic angle and Ferguson's angle were subdivided into five sections. The number of days from onset and the duration of hospitalization were analyzed in each section. Results Severe hypolordotic lumbar lordotic angle group were found to have longer medical history and hospitalization duration than the control group. The ratio of long-term hospitalization (over 15 days) and chronic medical history (over 180 days) of patients with severe hypolordotic Ferguson's angle were significantly higher than the control group. The ratio of patients with chronic medical history (over 180 days) was significantly higher in severe hyperlordotic Ferguson's angle group than the control group. Conclusions Severe hypolordotic lumbar lordotic angle was related to patient's prognosis. Both severe hyperlordotic and severe hypolordotic Ferguson's angle were found to be related to patient's prognosis. However, a moderate loss of lumbar lordotic angle and Ferguson's angle was not related to prognosis of low back pain patients.

The Study on the Factors Related to the Existence of Neck Pain in Female Office Workers (사무직 여성 근로자의 경부 통증 유무와 관련된 요인 연구)

  • Objectives : The purpose of this study was to investigate the factors related on pain in female office workers. Methods : Neck pain group of 31 female subjects complained of neck and arm discomfort. Normal group of 20 female subjects had no complaints or minimal discomfort. Cervical curvature and muscle tone were assessed by whole spine x-ray, meridian-electromyography(MEMG), craniovertebral angle, and Moire. Neck pain was evaluated by Neck Disability Index(NDI) and Visual Analog Scale(VAS). The emotional and other physical factors that could effect neck pain were checked by questionnaires including Beck Depression Index(BDI), Stress Reaction Index(SRI), Holmes & Rahe Social Readjustment Rating Scale(SRRS), International Physical Activity Questionnaire(IPAQ), and Gastrointestinal Symptom Rating Scale(GSRS). Results : The contraction and fatigue of upper trapezius by MEMG was significantly higher in the neck pain group. And BDI, SRI, SRRS, and GSRS were significantly higher in the neck pain group (p<0.05). However, there was no significant difference in the Jackson's angle, Cobb's method, craniovertebral angle, and moire between two groups. Conclusions : The results suggest that neck pain is related to mental stress rather than physical stress and physical stress does not change cervical curvature significantly.

The Change of the Cervical Spine Curvature after Whiplash Injury by Traffic Accident (교통사고로 인한 편타손상 후 경추 만곡의 변화 연구)

  • Objectives: To investigate the change of the cervical spine curvature after whiplash injury. Methods: Clinical study carried out in 46 patients who already had neck pain and took a cervical lateral X-Ray in Jaseng Oriental hospital. these patients had whiplash injury whitin one year. We followed up the cervical lateral X-Ray after that. Cervical spine curvature was measured using four measuring method. Type of cervical spine curvature was analyzed by Jochumsen method, Ishihara Index. T-test was used. Results: By Jackson's Angle and Cobb's angle, it is tend to be more Iordotic curvature after whiplash injury. In Jochumsen method, Ishihara Index the cervical spine curvature was significantly increased their Iordotic angle(P<0.05). Conclusion: The results suggest that after whiplash injury, patients cervical curvature change more Iordortic curve.

  • In this study, we designed a device for consecutively observing position, utilizing 3-axises acceleration sensor. This method offer to check his or her wrong position and developed could to help derived a position appliance. And, we developed a Cobb's angle value in three dimensional using 3-axises acceleration sensor. A proposed device with integrated accelerometers, which can detect postural changes in terms of curvature variation of the spine in the sagittal and coronal planes, has been developed with intention to facilitate posture training. The proposed device was evaluated with 3 normal subjects daily activities. We evaluated the performance of our designed device as calculating the correlation coefficients and mean errors between the angle measured by an electro-goniometer and that estimated by a gravity accelerometer and verified the accuracy and sensitivity. The results showed that the angle obtained from the proposed device revealed a linear characteristic at the range of $\pm60^{\circ}$(correlation coefficient 0.99, error range $\pm2^{\circ}$). We demonstrated that our device could detect the changes of the motion in upper trunk accurately. Also, our device showed good potential for treatment of the patients with scoliosis and prevention of the unbalance position during a daily life.

Lumbar Lordotic Angle Improvement by Combining Korean Medicine Treatment with Chuna Manual Therapy: Case Report (추나요법을 동반한 한방복합치료로 호전된 요추 전만각 1례: 증례보고)

  • Objectives To introduce a case of a traffic accident trauma in a patient with lumbar kyphosis and straightened lumbar curvature, where the Korean medicine treatment with Chuna manual therapy (CMT) improved lumbar lordotic angle(LLA) and lower back pain. Methods Chuna manual therapy and other Korean medicine treatments; were provided to patient with a flat back posture and low back pain due to the trauma from a traffic accident. Cobb's angle to evaluate LLA and numeric rating scale (NRS) to evaluate pain were compared before and after the treatment. Results There was a significant increase in LLA to reach the normal range and a decrease in low back pain NRS after Korean medicine treatment with CMT for 9 days of admission treatment and 3 weeks of outpatient treatment. Conclusions This study suggests that Korean medicine treatment, including CMT, may improve low back pain as well as LLA of straightened lumbar curvatures in patients.

Effect on Morphological Change of Cervical Spine to Apply to Neck Retraction and Extension Regarding the Straight Deformity of Cervical Spine: Case Study (경추 일자목 변형에 대한 경추 후인과 신전 움직임이 경추 시상면의 형태학적 변화에 미치는 영향: 사례연구)

  • Background: The purpose of this study was to identify the effects of morphological Change of cervical spine to apply to neck retraction and extension regarding the straight deformity of cervical spine. Methods: A 40-year male subject with straight deformity of cervical spine participated in this study. The study subject underwent a cervical lateral radiography on the static position, neck retraction and neck extension on standing. Measurement method were using computer-based digital radiogram on a picture achieving computer system forthe centroid method, Cobb's angle and Jackson's angle. Results: Neck retraction was increased kyphosis on the C2-4 with lordosis on the C5-7. Neck extension was increased lordosis on the C2-7. Conclusions: These findings suggest that neck retraction was increased kyphosis and neck extension was increased lordosis based on the straight deformity of cervical spine. Therefore, we should be consider that neck extension exercise when increased lordosis for the patients of straight deformity of cervical spine.

  • Cervical dystonia (CD) is a disorder characterized by sustained or intermittent, involuntary muscle contractions which cause twisting, repetitive movements and abnormal postures. In this case report, a CD patients was treated with transfer energy capacitive and resistive (TECAR) therapy and conventional Korean medicine, which consists of acupuncture, Chuna manual medicine, pharmacopuncture and herbal medicine. For outcome measures, this study evaluated Cobb's angle, EuroQol five dimension scale (EQ-5D), numerical rating scale (NRS), neck disabillity index (NDI) and cervical range of motion. As a result, improvements were found in Cobb's angle ($18.65^{\circ}$ to $15.90^{\circ}$), EQ-5D score (0.808 to 0.862), NRS (5 to 3), NDI (16 to 8) and angle of cervical lateral bending ($15^{\circ}$ to $25^{\circ}$). In conclusion, this study shows that combined therapy of conventional Korean medicine treatment and TECAR therapy can be effective for CD patients.
