Clinical Results of Mitral Valve Repair (승모판막질환의 판막재건술36례 성적)

  • From January 1962 to March 1987, 97 patients underwent operation for mitral valve disease. Of these patients, 61 [62.9%] required mitral valve replacement. Thirty-six patients [37.1%] had mitral valve repair. The mean age was 26.9*11.6 years [range 5 months to 48 years]. There were 32 [88.9%] cases of rheumatic valve disease, 4 [11.1%] cases of congenital mitral valve disease. Valve dysfunction was classified into three types: type I [normal leaflet motion], 6 patients; type II [prolapsed leaflet], 1 patient; type III [restricted leaflet motion], 29 patients. Twenty-nine patients [80.6%] had pure or predominant stenosis and 7[19.4%] had pure regurgitation. No patient was in NYHA functional class I. Three patients [16.7%] were in functional class II, 15[83.3%] were in functional class II. The techniques used included closed mitral commissurotomy [16 patients], open mitral commissurotomy [13 patients], localized Wooler type annuloplasty [4 patients], suture repair of leaflet defect [3 patients], chordal shortening [1 patient], Carpentier ring annuloplasty [1 patient], and fenestration of fused chordae [1 patient]. There were two perioperative deaths [5.6%], related to left ventricular failure and reoperation. The survivors were followed up for 94 patient-years [mean 4.68*5.54 years]. One late death [1.1*1.1% per patient-year] occurred and was valve related. Reoperation was required in 3 patients, of whom 2 were deaths. There was 1 case [1.1*1.1% per patient-year] of thromboembolism. No patient received anticoagulant after operation. At 10 year, 92*7.4% of the patients were still alive. The actuarial survival rate of patients free of valve-related complication was 79*6.4% at 6 years, 27*12.1% at 11 years. After surgery, 18 patients [88.9%] were in NYHA functional class I or II.

Evaluation about Contaminant Migration Near Abandoned Mine in Central Region (중부지역에 위치한 폐광산 주변의 오염물질 이동성 평가)

  • Several mines including Namil, Solim and Jungbong which are located in the Gyeonggi and Kangwon province have been abandoned and closed since 1980 due to "The promotion policy of mining industry". An enormous amount of mining wastes was disposed without proper treatment, which caused soil pollution in tailing dam and ore-dressing plant areas. However, any quantitative assessment was not performed about soil and water pollution by transporting mining wastes such as acid mine drainage, mine tailing, and rocky waste. In this research, heavy metals in mining wastes were analyzed according to leaching method which used 0.1 N HCl and total solution method which used Aqua-regia to recognize the ecological effect of distance from hot spot. We sampled tailings, rocky wastes and soils around the abandoned mine. Chemical and physical parameters such as pH, electrical conductivity (EC), total organic carbon (TOC), soil texture and heavy metal concentration were analyzed. The range of soil's pH is between 4.3 and 6.4 in the tailing dam and oredressing plant area due to mining activity. Total concentrations of As, Cu, and Pb in soil near ore dressing plant area are 250.9, 249.3 and 117.2 mg/kg respectively, which are higher than any other ones near tailing dam area. Arsenic concentration in tailing dams is 31.0 mg/kg, which is also considered as heavily polluted condition comparing with the remediation required level(RRL) in "Soil environment conservation Act".

Treatment of Acute Acromioclavicular Joint Dislocation: Kirschner's Wire Trans-acromial Fixation versus AO Locking Hook Plate Fixation

  • Background: The purpose of this study is to compare clinical and radiological outcomes between trans-acromial fixation with Kirschner's wire (K-wire) and AO locking hook plate fixation for acute acromioclavicular (AC) joint dislocation. Methods: This study included 61 patients who underwent either closed reduction and trans-acromial fixation with K-wire (group A, 23 patients) or open reduction and internal fixation with AO locking hook plate (group B, 38 patients). Pain on a visual analogue scale (VAS) score, the University of California Los Angeles (UCLA) shoulder score, the American Shoulder and Elbow Surgeons (ASES) score, and active range of motion (ROM) were used in the functional evaluation. For radiological evaluation, coracoclavicular distance (CCD) was measured on both clavicular anteroposterior view and compared between groups. Results: At one-year follow-up, no significant differences in VAS pain score, UCLA shoulder score, ASES score, and active ROM were observed between groups, despite five cases (22.7%, 5/23) of complication in group A. The side-to-side difference between normal and affected CCD was $2.4{\pm}2.2mm$ in group A and $0.2{\pm}0.7mm$ in group B. This difference showed a statistical significance between groups (p<0.001). Conclusions: For the treatment of acute AC joint dislocation, the K-wire trans-acromial fixation group showed a significantly greater CCD than the AO locking hook plate group. In addition, during the follow-up period, much higher incidence of complication related to implant was observed in the trans-acromial fixation group. Although clinical outcomes between groups were not significantly different, these results should be interpreted carefully.

Clinical and Radiological Outcome after Surgical Treatment in Displaced Clavicular Midshaft Fracture

  • Background: The first purpose of this study is to compare the clinical and radiological outcomes of surgical treatment for displaced midshaft clavicle fracture (Robinson type 2B1 vs. 2B2) with 3.5-mm low profile clavicular locking compression plate. The second purpose is to evaluate the difference of the results depending on the presence of accompanying injuries. Methods: Forty-nine patients who underwent an operation for the fractures were reviewed retrospectively. Fracture patterns were classified according to group 2B1 and 2B2 using Robinson's classification. For radiological outcome, time to union after operation was evaluated and for clinical outcome, American Shoulder and Elbow Society (ASES) score, University of California in Los Angeles (UCLA) score, visual analogue scale (VAS), and range of motion (ROM) were evaluated from preoperative period to last follow-up period. Results: The mean time for union was not significantly different in the 2B1 group and 2B2 group (p=0.062). No statistically significant difference in ASES score, UCLA score, and VAS was observed between 2B1 and 2B2 (p=0.619, p=0.896, p=0.856, respectively). In ROM, significant higher mean forward flexion and abduction was observed in 2B2 (p=0.025, p=0.017, respectively) and there was no difference in external rotation and external rotation at shoulder $90^{\circ}$ abduction position (p=0.130, p=0.180, respectively). There was no significant difference in clinical outcomes according to the accompanying injuries. Conclusions: There was no difference in clinical and radiological outcome between Robinson 2B1 and 2B2 type fracture after the operation. Accompanying injuries may not affect the clinical result of displaced midshaft clavicle fractures.

Partial Relay Selection in Decode and Forward Cooperative Cognitive Radio Networks over Rayleigh Fading Channels

  • The performance of an partial relay selection on the decode-and-forward (DF) mode cognitive radio (CR) relay networks is studied, with some important factors, including the outage probability, the bit error ratio (BER), and the average channel capacity being analyzed. Different from the conventional relay selection schemes, the impact of spectrum sensing process as well as the spectrum utilization efficiency of primary users on the performance of DF-based CR relaying networks has been taken into consideration. In particular, the exact closed-form expressions for the figures of merit such as outage probability, BER, and average channel capacity over independent and identically distributed (i.i.d.) Rayleigh fading channels, have been derived in this paper. The validity of the proposed analysis is proven by simulation, which showed that the numerical results are consistent with the theoretical analysis in terms of the outage probability, the BER and the average channel capacity. It is also shown that the full spatial diversity order can always be obtained at the signal-to-noise ratio (SNR) range of [0dB, 15dB] in the presence of multiple potential relays.

Improvement of High-Availability Seamless Redundancy (HSR) Traffic Performance for Smart Grid Communications

  • High-availability seamless redundancy (HSR) is a redundancy protocol for Ethernet networks that provides two frame copies for each frame sent. Each copy will pass through separate physical paths, pursuing zero fault recovery time. This means that even in the case of a node or a link failure, there is no stoppage of network operations whatsoever. HSR is a potential candidate for the communications of a smart grid, but its main drawback is the unnecessary traffic created due to the duplicated copies of each sent frame, which are generated and circulated inside the network. This downside will degrade network performance and might cause network congestion or even stoppage. In this paper, we present two approaches to solve the above-mentioned problem. The first approach is called quick removing (QR), and is suited to ring or connected ring topologies. The idea is to remove the duplicated frame copies from the network when all the nodes have received one copy of the sent frame and begin to receive the second copy. Therefore, the forwarding of those frame copies until they reach the source node, as occurs in standard HSR, is not needed in QR. Our example shows a traffic reduction of 37.5%compared to the standard HSR protocol. The second approach is called the virtual ring (VRing), which divides any closed-loop HSR network into several VRings. Each VRing will circulate the traffic of a corresponding group of nodes within it. Therefore, the traffic in that group will not affect any of the other network links or nodes, which results in an enhancement of traffic performance. For our sample network, the VRing approach shows a network traffic reduction in the range of 67.7 to 48.4%in a healthy network case and 89.7 to 44.8%in a faulty network case, compared to standard HSR.

A Surgical Treatment of the Esophageal Foreign Body (10 cases report) (수술적 처치가 필요했던 식도이물에 관한 임상적 고찰)

  • Ten cases with esophageal foreign body were treated surgically from July 1980 to October 1995 at the Departme t of Thoracic and Cardiovascular Surgery, Chungnam National University Hospital. The mean age was 45.3 years, with a range from 25 to 71. Out of ten cases, 6 were female and four were male. Common symptoms were dysphagia, fever, foreign body sensation and neck pain. Three cases of foreign bodies were of fish bones, two of bubble paclcage of drugs, one case of a Eeer bottle cap, one of a piece glass, one of a bathtub plug, one of chicken and one of a bean. The diagnosis was established by esophagography using a water soluble contrast material and esophagoscopy. Among of ten cases, two had esophageal stricture due to the ingestion of Aye at a young age. One case had experienced psychological problems. All foreign bodies were removed by surgical procedures. Five cases were treated by cervical esophagostomy, one case by right thoracotomy, one case by retrograde bougienation through gastrostomy and two cases by cervical incision and drainage for cervical abscess. Three cases developed pos operative esophageal leaks which healed spontaneously and transient hoarseness developed in one case. One case developed traumatic pneumothorax and subcutaneous emphysema which was treated by closed thoracostomy. There were no operative deaths. C(ocean J Thorac Cardiovasc Surg 1997;30:1117-20)

Design and Fabrication of Thin Microwave Absorbers of ITO/Dielectric Structures Used for Mobile Telecommunication Frequency Bands (ITO박막/세라믹유전체 구조의 이동통신 주파수대역용 박형 전파흡수체의 설계 및 제조)

  • For the aim of thin microwave absorbers used in mobile telecommunication frequency band, this study proposed a high permittivity dielectrics(λ/4 spacer) coated with ITO thin films of 377 $\Omega$/sq(impedance transformer). High frequency dielectric properties of ferroelectric ceramics, electrical properties of ITO thin films and microwave absorbing properties of ITO/dielectrics were investigated. Ferroelectric materials including $BaTiO_3$(BT), 0.9Pb($Mg_{1}$3/Nb$_{2}$3/)$O_3$-0.1 $PbTiO_3$(PMN-PT), 0.8 Pb (Mg$_{1}$3/$Nb_{2}$3/)$O_3$-0.2 Pb($Zn_{1}$3$_Nb{2}$3/)$O_3$(PMN-PZN) were prepared by ceramic processing for high permittivity dielectrics,. The ferroelectric materials show high dielectric constant and dielectric loss in the microwave frequency range. The microwave absorbance (at 2 ㎓) of BT, 0.9PMN-0.1PT, and 0.8PMN-0.2PZN were found to be 60%(at a thickness of 3.5 mm), 20% (2.5 mm), and 30% (2.5 mm), respectively. By coating the ITO thin films on the ferroelectric substrates with λ/4 thickness, the microwave absorbance is greatly improved. Particularly, when the surface resistance of ITO films is closed of 377 $\Omega$/sq, the reflection loss is reduced to -20 ㏈(99% absorbance). This is attributed to the wave impedance matching controlled by ITO thin films at a given thickness of high permittivity dielectrics of λ/4 (3.5 mm for BT, 2.5 mm for PMN-PT and PMN-PZN at 2 ㎓). It is, therefore, successfully proposed that the ITO/ferroelectric materials with controlled surface resistance and high dielectric constant can be useful as a thin microwave absorbers in mobile telecommunication frequency band.

  • A semi-solid alloy in which solid and liquid phase are co-existing is obtained by strirring of Al7075 molten metal. A semi-solid alloy is dependent on the corresponding temperature within the solid-liquid range, and the process parameters should be controlled accurately to obtain the homogeneous semi-solid alloy. The possibility o homogeneous fiber-reinforce aluminum alloy by addition of $Al_{2}$O$_{3}$ short fibers with vigorous agitation was investigated. The billet of composite materials was fabricated by squeeze casting, and homogeneous dipersion state of fibers in billet of fabricated metal matrix composites was observed. A slurry of semi-solid short fiber metal matrix composites is used in the direct rolling process, and this process showed the fabrication possibility of metal matrix composite sheets. The fabricated sheet was tested regarding vickers hardness, elongation and micro-structure. It has become clear that mashy state processing and working are very useful to obtain parts of composites material closed to near net shape.

  • The annual variations of some heavy metal concentrations, Al, Mn, Fe, Cu, Pb, Zn, Cd, Hg were determined on tile day of spring tides in every month from May 1977 to April 1978 at eight stations in the Nagdong River downstream. Samples were taken at intervals of one or two hours from 7 a.m. to 7 p.m. at each station. Annual ranges and means of concentrations of the heavy metals are as follows : aluminium 40.7-3700 ppb, 286 ppb; manganese 10.3-261.5 ppb, 80.8 ppb; iron ND-1237 ppb, 147.7 ppb; copper ND-30.9 ppb, 2.49 ppb; lead ND-29.9 ppb, 1.10 ppb; zinc ND-156.8 ppb, 5.61 ppb; cadmium ND-1.22 ppb, 0.09 ppb; mercury ND-0.37 ppb, 0.02 ppb respectively. In general, the contents of heavy metals except managanese and mercury were higher at the stations above station one, Kupo, though the ranges of the contents showed remarkable difference according to the sampling stations. Annual means of the concentrations of iron and manganese were exceeded already the desirable standards for industrial water and closed to the criteria of raw water for public supply. The values that over the criteria of raw water were sometimes found. Mercury concentration showed comparative high level such as ND-0.37 ppb in annual range but it was not over the environment criterion, 0.5 ppb. Aluminium, iron and mercury showed generally high values in summer period and other metals in winter period, through the patterns of seasonal variations were difference depending on the sampling stations and the kind of metals.
