A Study on the Need of Clinical Nurses about the System of 12-Hour Work Shift (이부교대(二部交代) 근무제도(勤務制度)에 대한 임상간호사(臨床看護師)의 요구도(要求度)에 관한 연구(硏究))

  • The 12-hour work shift can be influenced on the nurse's job satisfaction, effective personnel administration, and quality of patient care. The purpose of this study was to explore the perception on the 12-hour work shift by nurses. A total of 516 nurses were selected by convenience sampling from six general hospitals in Seoul. Based on the literature review, the questionnaire on the need of the 12-hour work shift was designed by the investigator. The data were analyzed using descriptive statistics, ANOVA and Duncan's method as post-hoc test. The results of this study are as follows ; 1. The need of 12-hour work shift The mean needs of 12-hour work shift in the nursing practice, psychological, physical, socioeconomic, and environmental perspectives were 3.05, 2.72, 2.66, 3.08, and 4.22. 2. The need of 12-hour work shift by demographic data For the nursing practice, there was a statistically significant relationship between marital status(p=.021), satisfaction on the 8-hour work shift(p=.038), the perception on the 12-hour work shift(p=.001) and the need of 12-hour work shift. For the psychological perspectives, there was a statistically significant relationship between marital status(p=.016), the perception on the 12-hour work shift(p=.0001), and the need of 12-hour work shift. For the physical perspectives, there was a statistically significant relationship between age(p=.002), marital status(p=.005), the number of children(p=.005), the duration of job career(p=.014), the job position(p=.002), the work shift(p=.030), the perception on the 12-hour work shift(p=.0001) and the need of 12-hour work shift. For the socioeconomic perspectives, there was a statistically significant relationship between the job position(0=.002), the work shift(p=.006), the perception of the 12-hour work shift(p=.002) and the need of 12-hour work shift. For the environmental perspectives, there was a statisitically significant relationship between the traffic method(p=.05), the duration of the job career(p=.026), the perception on the 12-hour work shift(p=.309) and the need of 12-hour work shift. 3. The need of 12-hour work shift by the demographic data There was a statistically significant difference between marital status(p=.014), the number of children(p=.038), the job position(p=.007), the work shift(p=.037), and the perception on the 12-hour work shift(p=.0001) for the need of 12-hour work shift.

Risk factor analysis of additional administration of sedative agent and patient dissatisfaction in intravenous conscious sedation using midazolam for third molar extraction

  • Objectives: The primary purpose of this study was to investigate the factors related with additional administration of sedative agent during intravenous conscious sedation (IVS) using midazolam (MDZ). The secondary purpose was to analyze the factors affecting patient satisfaction. Materials and Methods: Clinical data for 124 patients who had undergone surgical extraction of mandibular third molar under IVS using MDZ were retrospectively investigated in this case-control study. The initial dose of MDZ was determined by body mass index (BMI) and weight. In the case of insufficient sedation at the beginning of surgery, additional doses were injected. During surgery, peripheral oxygen saturation, bispectral index score (BIS), heart rate, and blood pressure were monitored and recorded. The predictor variables were sex, age, BMI, sleeping time ratio, dental anxiety, Pederson scale, and initial dose of MDZ. The outcome variables were additional administration of MDZ, observer's assessment of alertness/sedation, intraoperative amnesia, and patient satisfaction. Descriptive statistics were computed, and the P-value was set at 0.05. Results: Most patients had an adequate level of sedation with only the initial dose of MDZ and were satisfied with the treatment under sedation; however, 19 patients needed additional administration, and 13 patients were unsatisfied. In multivariable logistic analysis, lower age (odds ratio [OR], 0.825; P=0.005) and higher dental anxiety (OR, 5.744; P=0.003) were related to additional administration; lower intraoperative amnesia (OR, 0.228; P=0.002) and higher BIS right before MDZ administration (OR, 1.379; P=0.029) had relevance to patient dissatisfaction. Conclusion: The preoperative consideration of age and dental anxiety is necessary for appropriate dose determination of MDZ in the minor oral surgery under IVS. The amnesia about the procedure affects patient satisfaction positively.

The Difference of Health Related Quality of Life According to Ankylosing Spondylitis Patient's General Characteristics (강직척추염 환자의 일반적 특성에 따른 건강관련 삶의 질 차이)

  • This study was conducted to identify differences between qualities of health related to quality of life according to the general characteristics of ankylosing spondylitis patients. Overall, 275 patients who were older than 19 years, drug-maintained for over 4 weeks from Jan 13th to Feb 12th and diagnosed as ankylosing spondylitis at C university hospital participated in this study. Collected data were analyzed through descriptive statistics, t-tests, ANOVA, Scheffe's-test and multiple regression. The results of the present study revealed that Age (p=.008), Job (p<0.001), Number of comorbidity (p<0.001), Surgery for Ankylosing Spondylitis (p=0.002), Disease duration after onset symptom (p=0.010), Disease duration after diagnosis (p=0.027), Alcohol drinking (p=0.002) and Erythrocyte sedimentation rate (p=0.049) were meaningful factors to the quality of health-related life for ankylosing spondylitis patients, and multiple regression analysis showed that level of education, income, comorbidity and alcohol consumption was a significant variable. In conclusion, the results of this study indicated that, to improve quality of life, arbitration programs for improving general characteristics and considering demographic social characteristics and clinical features need to be provided. Therefore, it is expected that the results of this study would be used as actual grounds to develop integrated arbitration programs.

A Study on Using EAV (MERIDIAN) by Analysis of Cerebrovascular Disease Risk Factors (뇌혈관질환 위험요인과의 분석을 통한 EAV(MERIDIAN)활용에 관한 연구)

  • Objectives: Electroacupuncture according to Voll (EAV) has been used to quantify the skin's electrical resistance and conductance over acupuncture points that, based on traditional Chinese medicine, represent the state of health or disease of the organ or tissue. However, it doesn't have enough objective data yet, so the purpose of this study was to aid in the use of EAV in analysis of cerebrovascular disease risk factors. Methods: This study researched the clinical statistics of 216 cases: cerebrovascular attack (CVA) group 43, control group 173. We measured control meridian points (CMP) on hands and feet and the cardio ankle vascular index (CAVI) which represents atherosclerosis severity, and sex, age, hypertension, diabetes, dyslipidemia, and obesity. The data were then analyzed by t-test, chi-square test and logistic regression. Results: Between the CVA and control groups, there were not statistically significant differences in CMP. However, logistic regression analysis of the presence of CVA, mean of CMP heart and lung, sex, age, DBP (diastolic blood pressure), and HDL (high density lipoprotein) cholesterol showed that the risk of CVA was 1.1 times increased with CMP heart (p=0.002), in men was 4.12 times higher than in women (p=0.001), 1.09 times higher with age (p=0.000), 1.04 times higher with DBP (p=0.045), while was lower by 0.924 times with CMP lung (p=0.005) and 0.957 times with HDL cholesterol (p=0.006). Conclusion: Although there were not clear evidence or mechanisms about EAV, this study showed that if we study EAV further, we may be able to apply EAV as an objective instrument of oriental medicine in cerebrovascular disease.

A Clinical Trial to Assess the Efficacy of Acupuncture on Hot Flashes in Postmenopausal Women;Focusing on the comparison of the effects of Traditional Korean medical acupuncture (TKMA) and Minimal Acupuncture (MA)

  • Objective : In this study we wanted to confirm if proper stimulation and de-Qi of traditional Korean medical acupuncture could increase hot flash relief efficacy. Design : A randomized controlled, single blind study. We used two modalities of acupuncture, one with optimal stimulation [Study group; Korean medical acupuncture (TKMA)] and one with minimal stimulation [Control group; Minimal acupuncture (MA)]. Same acupoints [PC6(內關), HT8(少府), HT7(神門), LI4(合谷), ST36(足三里), SP6(三陰交), Ren4(關元)] were used in both groups. Fifty-two patients were treated twice a week for 8 weeks, and follow up was done after 4 weeks from the last treatment. Patients were checked hot flash VAS (visual analog scale), frequency and duration every time they visited. Results : Hot flash relief efficacy by 100mm hot flash VAS was obvious in both groups. Hot flash VAS scores of study group were smaller than the scores of control group at the early stage (3rd, $4^{th}$ and $8^{th}$ visit), but there wasn't a remarkable difference between study and control group at the end of the trial. Besides, diminution of hot flash VAS was faster and more even in the study group than control group by visualization using 'Box plot'. We compared frequency and duration of hot flash, 100mm sweating, palpitation, sleep disturbance VAS, and Kupperman Index, MENQOL, Patient's global assessment score. Both groups showed definite decrease from the baseline, but the difference was not statistically significant. There wasn't any adverse event. Hot flash relief efficacy was kept in most patients after 4 weeks' follow-up. Conclusion : Acupoint combination by Traditional Korean medical theory is effective on hot flashes and hot flash relief efficacy was faster and more even in optimal stimulation than minimal stimulation.

Prevalence of obesity, hypertension, and hyperlipidemia in Gunpo children of low economic status (군포시 저소득층 소아의 비만도, 혈압 및 지질검사)

  • Purpose : This study aimed to assess the prevalence of obesity, hypertension, and hyperlipidemia in children from low-income families in Gunpo and to evaluate whether economic status affects the prevalence of obesity. Methods : Between October 2007 and March 2008, 341 children (167 girls and 174 boys; age, 6 to 13 years) were enrolled in this study. All these children came from families who earned minimum wages and who were supported by government. We measured height, weight, and blood pressure and performed laboratory examinations, including total cholesterol, high-density lipoprotein cholesterol, and triglycerides. Body mass index (BMI) was calculated using weight and height. We compared the prevalence of obesity, hypertension, and hyperlipidemia in children from low-income families with the prevalence of these maladies found in other studies. Results : Prevalence of obesity ($BMI{\geq}95^{th}$) was 7.1%. Prevalence of hypertension and hyperlipidemia was 8.2% and 16.7%, respectively. In the obese group, prevalence of hypertension and hyperlipidemia was 25.0% and 45.8%, respectively. Obesity and hyperlipidemia were slightly more frequent in our study than those found in other reports, and the prevalence of hypertension in the obese group was very high compared with statistics from the Korean National Health and Nutrition Examination Survey (25% vs. 19.5%) and those from other reports. Conclusion : Low-income status was associated with an increased incidence of obesity, hyperlipidemia, and hypertension. Hypertension showed an especially strong association with economic status, which seemed to correlate with genetic, environmental, and dietary effects.

Study on Diagnosis by Facial Shapes and Signs as a Disease-Prediction Data for a Construction of the Ante-disease Pattern Diagno-Therapeutic System - Focusing on Gallbladder's versus Bladder's Body and Masculine versus Feminine Shape - (미병학(未病學) 체계구축을 위한 질병예측자(疾病豫側子)로서의 형상진단연구 - 담방광체(膽膀胱體)와 남녀형상(男女形象)을 중심으로 -)

  • There needs disease-predictable signs in order to enable preventive diagnosis and therapy. Then traditional Chinese medicine applies various medical diagnostic equipments used in western medicine to diagnosing sub-healthy state. But such data are not originated from inherent oriental medicine, and not obtained easily in ordinary clinical practice. This paper is to provide synopsis of the ante-disease diagno-therapeutics partly and to show predictable data based on the facial shapes and signs, especially of gall bladder's versus bladder's body and masculine versus feminine shape. Ante-disease means not only the complete healthy state, but also the state unseen any symptoms in macrographically in the course of outbreak of disease. It contains two stages, first one is the former state of disease and second one is untransmitted state of disease. The patterns of ante-disease consist of latent disease, pre-disease, transmission type like senescent syndrome, abnormal reactive syndrome(變證), syndrome of transmission and transmutation. The classification with gall bladder and bladder type manifests the differences of shape, color and size of each organ in comparison of the universal and standard figures of the human being. On the other hand, the classification with masculine and feminine shape contrasts the innate sexual difference and the shape, characteristics originated from in itself. These two classification theories have their own pathologic types and syndrome types with each disease so that disease-predictable data can be constructed based on such a relationship. In addition, this diagnostic method by facial shapes and signs is able to be applied to whole stages from prenatal to present state of disease even if the cause and inducement are not clear. Ante-disease diagno-theraputic system by Gall Bladder's versus Bladder's Body and Masculine versus Feminine Shape is getting more important in the chronic and internal disease in comparison of the acute and traumatic disease. So this study is able to make up for the limit of diagnosis on ante-disease in the field of oriental medicine clinic.

Changes in Acoustic Parameters According to Intensity Increase in Voice Assessment (음성질환자의 음성검사 시 강도 증가에 따른 음향학적 지표의 변화)

  • Background and Objectives : Clinically, as a tool for voice assessment before and after the operation or the voice treatment, acoustic analysis is widely used. However, in clinical situations, acoustic parameters vary according to how the assessment is made. Thus, with voice disease patients as subjects, we are to investigate what influence intensity increase exerts on acoustic parameters and how to reduce variation according to the way of assessing. Material and Method : At the voice clinic of the department of otorhinolaryngology in Gangnam Severance Hospital, with 30 female voice-disease patients (40.6 years old on the average) and 23 male voice-disease patients (40.1 years old on the average) as subjects, using the Dr Speech vocal-assessment program, we statistically tested the significance of the difference in each of acoustic parameters between when the "Ah" vowel is produced with a normal voice and when the "Ah" vowel is produced with a loud voice. Results : Acoustic parameters that showed a statistically significant difference according to intensity increase were Jitter, SD F0, and NNE for females, and Jitter, SD F0, HNR, SNR, and NNE for males. Voice quality estimates showed a statistically significant difference according to intensity increase in female hoarse voice, female breathy voice, and male breathy voice. Conclusion : In this research, acoustic analysis, which is generally used for voice assessment before and after the operation or the voice treatment, showed a tendency that acoustic parameters became better under the influence of intensity increase except for the cases where a voice disease was severe. Thus, to raise the reliability of voice assessment, the range of intensity needs to be set up. This should be the topic for the future research.

Perception of Artificial Hydration for Terminally Ill Cancer Patients: Patients, Families and General Public (말기 암 환자에서의 정맥 내 수액요법에 대한 인식도: 환자, 보호자 및 일반인)

  • Purpose: The purpose of the study was to investigate how much understand about artificial hydration in patients with terminal cancer, according to the subject groups, including patients, families, and general public. Methods: Data were collected from June 2007 to December 2007 and the participants included 22 hospitalized patients in the hospice unit of S Hospital, 100 families, and 101 participants who participated in a hospice education program for the general public. The questionnaire was developed through literature review, interview with patients' families, and expertise consultation. Data were analyzed using descriptive statistics with an SAS program. Results: Understanding of artificial hydration among patients, families and general public was examined from three perspectives. From an ethical perspective, 'if you receive artificial hydration, you can live longer', 45.5%, 63%, and 52.4% of the above three groups, respectively, answered "yes". From an emotional perspective, 'artificial hydration must be provided', 81.8%, 70% and 58.4%, respectively, agreed. From a cultural perspective, 'if artificial hydration is not provided for the patient, the families will feel painful', 95.5%, 83%, and 88.2%, respectively, answered "yes". Conclusion: This study found the differences in understanding of artificial hydration among patients, families and general public, and also found that less than 50 percent of the participants understood artificial hydration appropriately. We suggest, therefore, that patients' understanding about artificial hydration should be determined in the clinical setting and then followed by individualized education according to given medical situations.

Nursing Students' Awareness of Biomedical Ethics and Attitudes toward Death of Terminal Patients (간호대학생의 말기환자에 대한 생명의료윤리 인식과 죽음에 대한 태도)

  • Purpose: The aim of this study was to investigate nursing students' awareness of biomedical ethics and attitudes toward death of terminal patients. Methods: A structured questionnaire was developed to examine nursing students' biomedical ethics. Their attitudes toward terminal patients' death were measured by using the Collett-Lester Fear of Death Scale. Surveys were conducted with 660 nursing students enrolled at a three-year college located in Daejeon, Korea. Data were analyzed using descriptive statistics, Wilcoxon rank sum test and Kruskall Waills test. Results: Students who have experienced biomedical ethics conflicts, agreed to prohibition of cardiopulmonary resuscitation (CPR) and have no religion exhibited more negative attitudes toward death compared to students without the above characteristics. Of the participants, 81.2% answered that life sustaining treatment for terminal patients should be discontinued and 76.4% replied that CPR on terminal patients should be prohibited. The majority of the correspondents stated that the two measures above are necessary "for patients' peaceful and dignified death". Conclusion: Study results indicate the need to establish a firm biomedical ethics value to help nursing students form a positive attitude toward death. It also seems necessary to offer students related training before going into clinical practice, if possible. The training program should be developed by considering students' religion, school year, experience with biomedical ethics conflicts and opinion about CPR on terminal patients. The program should also include an opportunity for students to experience terminal patient care in advance via simulation practice on standardized patients.
