국외 임상술기센터의 임상교육 현황과 시사점 (A review on clinical education in clinical skills centers worldwide and its implications)

  • Purpose : The establishment of clinical skills centers(CSCs) to facilitate the teaching and assessment of clinical skills is one of the more recent developments occurring in medical schools worldwide. The aim of this study is to review experiences of CSCs in other medical schools and learn how to design a CSC in our school. Methods : This study was undertaken in two steps. In the first step, educational activities of CSCs in 6 medical schools were reviewed. In the second step, a search for articles of journals regarding clinical skills education in CSCs was conducted. Results : The review of CSCs programs reveals variations among centers in teaching and assessment activities. However there are increasing trends of utilizing CSCs in teaching and learning in CSCs. The delivery of clinical skills is expanded by an increasing use of simulated patients and realistic simulators. Through an audio/video technology, availability of more detailed monitoring and feedback. CSCs also provide greater opportunity for assessment of communications skills, physical examination and practical procedures. Conclusions: CSCs contribute to the effectiveness in clinical teaching and assessment. Educational benefits of a CSC can be maximized by utilizing new delivery methods, implementing educational strategies and staff development programmes.

교육 현장에서 시행된 임상 술기 시험의 다면적 타당도 분석 (Multifaceted validity analysis of clinical skills test in the educational field setting)

  • Introduction: The importance of clinical skills training in traditional Korean medicine education is increasingly emphasized. Since the clinical skills tests are high-stakes tests that determine success in national licensing exams, it is essential to develop reliable multifaceted analysis methods for clinical skills tests in actual education settings. In this study, we applied the multifaceted validity evaluation methods to the evaluation results of the cardiopulmonary resuscitation module to confirm the applicability and effectiveness of the methods. Methods: In this study, we used internal consistency, factor analysis, generalizability theory G-study and D-study, ANOVA, Kendall's tau, descriptive statistics, and other statistical methods to analyze the multidimensional validity of a cardiopulmonary resuscitation test in clinical education settings over the past three years. Results: The factor analysis and internal consistency analysis showed that the evaluation rubric had an unstable structure and low concordance. The G-study showed that the error of the clinical skills assessment was large due to the evaluator and unexpected errors. The D-study showed that the variance error of the evaluator should be significantly reduced to validate the evaluation. The ANOVA and Kendall's tau confirmed that evaluator heterogeneity was a problem. Discussion and Conclusion: Clinical skills tests should be continuously evaluated and managed for validity in two steps of pre-production and actual implementation. This study has presented specific methods for analyzing the validity of clinical skills training and testing in actual education settings. This study would contribute to the foundation for competency-based evidence-based education in practical clinical training.

Urinary Biomarkers for the Noninvasive Detection of Gastric Cancer

  • Gastric cancer (GC) is associated with high morbidity and mortality rates. Thus, early diagnosis is important to improve disease prognosis. Endoscopic assessment represents the most reliable imaging method for GC diagnosis; however, it is semi-invasive and costly and heavily depends on the skills of the endoscopist, which limit its clinical applicability. Therefore, the search for new sensitive biomarkers for the early detection of GC using noninvasive sampling collection methods has attracted much attention among scientists. Urine is considered an ideal biofluid, as it is readily accessible, less complex, and relatively stable than plasma and serum. Over the years, substantial progress has been made in screening for potential urinary biomarkers for GC. This review explores the possible applications and limitations of urinary biomarkers in GC detection and diagnosis.

집단적 놀이치료가 발달장애아동의 사회성 기술 향상에 미치는 효과 (Effects of a Group Play Therapy of Social Skills in Children with Developmental Disorders)

  • Purpose. The purpose of this study is to investigate effects of group play therapy program on improvement of social skills of children with developmental disabilities. Methods. With 4-year children with developmental disabilities with developmental problems, group play therapy program with 16 sections, 40minutes for each section for 16 weeks was conducted, and by using Korean infant social skills test (K-SSRSP), social skills index (SSQ) between before and after applying group play therapy program on children with developmental disabilities were compared. Results. When group play therapy are applied on children with developmental disabilities, it was found that there was improvement of social skills including problem solving, emotional expression, sense of order and self-confidence. Conclusions. According to the result of this study, if more specific studies regarding group play therapy program are conducted and they can reflect on other integrated programs, then it is expected that there will be greater synergy effects on improving not only children with developmental disabilities but also social skills of them.

The Mediating Effects of Learning Motivation on the Association between Perceived Stress and Positive-Deactivating Academic Emotions in Nursing Students Undergoing Skills Training

  • Purpose: Nursing students experience a high degree of perceived stress during skills training. The resulting academic sentiment is worthy of research. This study examined the learning motivation as a mediator in the association between perceived stress and positive deactivating academic emotions in nursing students undergoing skills training. Methods: A survey was conducted on 386 third-year undergraduate nursing students at a university in Changchun, China, in 2017. The survey included the items on perceived stress, learning motivation during nursing skill training, and general academic emotion. There were 381 valid responses (response rate=98.7%). Based on the results of partial correlation and stepwise multiple regression equations, the study examined the mediation model between perceived stress, learning motivation and positive-deactivating academic emotions using process 2.16 (a plug-in specifically used to test mediation or moderation effect in SPSS). Results: There was a significant negative correlation between students' perceived stress and learning motivation during nursing skills training and positive-deactivating academic emotions. Nervousness, loss of control, and interest in developing reputation had significant predictive effects on positive-deactivating academic emotions. The mediating model was well supported. Conclusion: Learning motivation during nursing skills training lessened the damage of perceived stress on positive-deactivating academic emotions. Improving students' motivation to learn could reduce their perceived stress and build more positive emotions. Positive emotions during learning played an important role in helping nursing students improve skills and enhance their nursing competence.

일개 상급종합병원 간호사의 근거기반실무(Evidence-Based Practice) 촉진요인에 대한 인식 (Nurses' Perceptions regarding Evidence-Based Practice Facilitators in a Tertiary Hospital)

  • Purpose: The purpose of this study was to investigate facilitators of evidence-based practice (EBP) in Clinical Nurses. Method: The instruments used in study were the EBP facilitator scale developed by Nagy et al. and a questionnaire on EBP-related characteristics. Data were collected from 230 nurses at a tertiary hospital and analyzed by descriptive statistics. Results: Compared to previous studies, this study showed that nurses had more experience related to research courses and clinical research. However, the proportion of nurses who reviewed relevant articles still remained low. The respondents had positive perceptions of organizational supports for EBP and belief in the value of EBP, whereas they had negative perceptions of skills in locating and evaluating research reports, knowledge of research terms and statistics, and time to devote to EBP. Conclusion: The findings of the study provide important basic data to develop and implement an EBP programs. In future, EBP programs should cover the nurses' skills to search and review research literature as well as their knowledge of research terms and statistics. Furthermore, nurses will require help to ensure that there is adequate time to devote to EBP.

의과대학 임상실습에서의 학생평가방법: 과거, 현재 및 제언 (Assessment of Medical Students in Clinical Clerkships)

  • The clinical clerkship focuses students on developing their ability to perform comprehensive diagnosis and management of patients with common undifferentiated problems by the integration of knowledge and clinical reasoning. Therefore, the clerkship evaluation system should assess their actual problem solving and professional behavior. However, concern remains that clerkship evaluations are imprecise and highly variable. This review is designed to provide faculty members with concepts, options, and a methodology to actively teach and evaluate the clinical clerkship, as well as offer encouragement and inspiration to medical students. We reviewed past and current clinical clerkship evaluations and discuss several tips to improve clinical excellence such as continuity, transparency of the evaluation process, a faculty development program, practical examination of clinical skills, implementation of a checklist for recording exposure and skills, providing prompt and constructive feedback to students, self-evaluation of professional performance, varying multi-faceted assessment combinations, being outpatient clinic-centered, and having dedicated faculty members who give students one-on-one contact with a preceptor.

스마트폰 동영상과 형성적 피드백을 활용한 핵심기본간호술 교육의 효과 (Effectiveness of Education Program for Core Fundamental Nursing Skills using Recording Video with Smartphone and Formative Feedback)

  • 본 연구는 간호대학생을 대상으로 기관내 흡인실습교육을 중심으로 스마트폰 동영상과 형성적 피드백을 활용한 핵심기본간호술 교육이 대상자의 핵심기본간호술 지식, 핵심기본간호술 수행능력, 자기효능감, 학습동기에 미치는 효과를 파악하기 위해 시도되었다. 간호대학 2학년 학생 54명이 모집되었으며, 실험군과 대조군으로 각각 27명씩 구성되었다. 실험군에게는 핵심기본간호술 지도 시 개별적인 형성적 피드백을 제공하였고, 수행과정을 스마트폰 동영상으로 촬영한 교육을 실시하였다. 대조군에게는 일반적 실습교육인 4시간 자율실습을 실시하였다. 연구결과 실험군의 핵심기본간호술 지식, 핵심기본간호술 수행능력, 자기효능감, 학습동기가 대조군에 비해 유의하게 향상되었다. 본 연구결과를 바탕으로 향후 간호대학에서 핵심기본간호술 교육 시 스마트폰 동영상을 활용한 반복학습과 형성적 피드백을 통한 정교한 학습이 이뤄질 수 있도록 교육현장에서 활발하게 활용되기를 고대한다.

객관 구조화 임상시험을 활용한 봉약침 시술 전 과정에 대한 교육효과 (Training future Korean medicine doctors to perform bee venom acupuncture and obtain informed consent using an objective structured clinical examination)

  • Objectives: This study describes the implementation of a training course that aimed to increase the competency of undergraduate students of Korean medicine at a single college to obtain informed consent and perform bee venom acupuncture using an objective structured clinical examination (OSCE). Methods: We developed and implemented a short training course for 111 fourth-year students of Korean medicine during their clinical rotation at the Department of Acupuncture and Moxibustion Medicine. Lectures were conducted on the effects of and hypersensitivity reactions to bee venom acupuncture, obtaining informed consent, performing skin test and bee venom acupuncture. Due to the COVID-19 pandemic, some students were offered online lectures, and some could not experience the performance of OSCE in person. A survey with 16 closed and three open questions was used to investigate self-efficacy, opinions about the online lectures, and the OSCE. Results: Out of 111 questionnaires, 104 were completed (94%). Students were generally satisfied with the online lectures and OSCE. For the subjective questionnaire about what was helpful in the training of bee venom acupuncture techniques and OSCE, the authenticity of the teaching method was recorded the highest. Conclusions: This study showed that online lectures were effective in teaching clinical skills of Korean Medicine. It is necessary to produce standard educational materials for clinical skills of Korean Medicine, and studies to analyze the reliability and validity of OSCE are needed.
