Assessment of Gestational Age based on Newborn Maturity Rating ; Ballard Examination (신생아의 성숙도 평정에 의한 재태기간 사정)

  • Newborn period is a transitional stage for independent adaptation from intrauterine to extrauterine life by maintaining respiration, temperature and nutrition. In general, the adaptability of the newborn is proportional to the gestational age(GA), so knowing the accurate GA is critical to develop nursing process in the newborn nursery. A newborn ma turity rating, a Ballard examination, has been used to measure GA by assessing the maturity of new-born. It consists a total of 12 items, which is the 6 items for the neuromuscular maturity and the 6 items for the physical maturity A total of 75 new-born were assessed for the maturity and GA using the Ballard examination. The results are follows : 1) The score of each item of Ballard examination is propotional to GA using the Ballard examination as well as LMP. 2) There was a greater positive relationship between neuromuscular, physical and total maturity, and the GA measured by Ballard examination, than the GA measured by LMP. 3) Any stressful environment to the newborn could influence to the maturity of newborn. In summary, the study showed the Ballard examination Is more reliable and clincially feasible method to measure the accurate G4 compared to the GA by LMP. Therefore, it suggests the application of Ballard examination to measure the new born maturity and GA is beneficial in developing nursing process. The expansion of the study with the variety of the subject characteristics nil on hance the clinical applicability of the examination.

A pilot study of Korean Medical Examination

  • Objectives: This study aimed to evaluate the feasibility of the Korean medical examination protocol, which included 14 questionnaires and 20 medical examination devices. Methods: We conducted a pilot observational study of 90 subjects to measure the time required to fulfill each item of the Korean medical examination, to evaluate patient satisfaction, and to report improvements that can be made to the Korean medical examination protocol based on clinical coordinator and subject feedback. Results: Among the 90 subjects included in the study (59 women and 31 men; mean [standard deviation] age, 37.2 [12.3] years), over 80% intended to receive a Korean medical examination if hospitals provided it and would recommend a Korean medical examination to others. The average time spent on the overall Korean medical examination was approximately 88.0 (21.4) minutes. Three areas for improvements were reported: survey issues, including the number of items, understanding, and readability; error issues in device measurements; and environmental issues affecting the sequence of medical examinations and temperature. Conclusions: Most subjects were satisfied with the Korean medical examination. Future studies should be conducted with larger samples to collect data continuously.

Literature Review of Clinical Studies for the Relationship between Ultrasonographic Examination and Syndrome Differentiation Classification in Chinese Medicine (초음파영상검사와 한의변증분류와의 관계와 관련된 중의학 임상연구에 대한 문헌고찰)

  • This study was to investigate the relationship between ultrasonographic examination and pattern identification classification on cinical studies in chinese medicine. We searched clinical studies related correlation between ultrasonographic examination and pattern identification classification in chinese medicine, that published from 2013 to 2016 in China National Knowledge Infrastructure (CNKI) databases by keywords, 'ultrasound(超?)', 'chinese medicine(中?)', 'syndrome differentiation (辨?)'. Seventeen studies were found. There were 7 studies of gynecological diseases including polycystic ovary syndrome and uterine myoma, 5 studies of fatty liver, 3 studies of arthritis, and 1 studie of thyroid nodule and lymphadenopathy respectively. As a result, ii is thought that there was a certain degree of correlation between the change of the ultrasonographic image and the pathological types according to traditional chinese medicine (TCM) syndrome differentiation and ultrasonographic examination could be used as secondary means for the TCM syndrome differentiation classification. In conclusion, by using ultrasonograph device in a medicinal way of TCM and traditional korean medicine (TKM), it is thought that more detailed and accurate diagnosis and treatment are possible and the evidence for reasonableness of syndrome differentiation in TCM and TKM its validity can be secured.

Review for the Curriculum and License Exam of Physical Therapists in the United States

  • Purpose: This study was conducted to review the curriculum and license examination of physical therapists in the United States. Methods: The doctor of physical therapy (DPT) curriculum was evaluated by a review Course Work Tool version 6 (CWT6) data and current physical therapy licensure examination (National Physical Therapy Exam, NPTE) category in the United States. Results: The results indicated that they were required to meet the doctor of physical therapist degree based on 'Guide to Physical Therapist Practice' of American Physical Therapy Association (APTA). This includes general education in the areas of communications and humanities, physical science, biological science, social and behavioral science, and mathematics. A minimum of one course must be completed successfully in each area of general education. Moreover, there should be at least 68 didactic credits of professional education and 22 clinical education credits, which is a minimum of two full-time clinical internships with no less than 1050 hours in total, which were supervised by a physical therapist. Regarding the physical therapy licensure examination, National Physical Therapy Exam (NPTE, 2016) consisted of a physical therapy examination (26.5%), evaluation, differential diagnosis, baseline of prognosis (32.5%), intervention (28.5%), protection, responsibility, and research (6.5%) based on the 'Guide to Physical Therapist Practice'. Conclusion: Based on the study results provided above, it is considered a standard to meet domestic reality as the Guide to Physical Therapist Practice of APTA for South Korean physical therapists.

Clinical Case Study on Piriformis Syndrome after Traffic Accident (교통사고 후 발생한 이상근 증후군 치험 1례)

  • This study is performed to report that oriental medical treatment was effective to the patient with piriformis syndrome after traffic accident. The patient was diagnosed as piriformis syndrome by considering clinical symptom, clinical history, physical examination, radiologic study and NCV EMG study. We applied acupuncture, herbal medicine, cupping, Chuna manipulation and exercise to the patient. After treatment, VAS decreased, and physical examination sign was disappeared. This result suggest that oriental medical treatment can be effective to piriformis syndrome.

  • Objectives: The purpose of this study was to design a standardized questionnaire for patients with atopic dermatitis in oriental dermatology. Methods: We selected atopic dermatitis from the evaluation of students' satisfaction conducted by the Department of Oriental Dermatology, Dong-guk University College of Oriental Medicine in 2011. To design the questionnaire for atopic dermatitis, we made up descriptive materials consisting of history of the present illness, patient risk factors, past medical history, personal history, and physical examination. Results: Standardized patients (SP) were used in teaching and practicing interviewing and physical examination skills for students. The SP offers a readily available tool for teaching and evaluating the clinical performance and interpersonal skills of medical students. However, the SP program needs to be more thoroughly researched and developed. Research of pattern identification in atopic dermatitis is needed to design the questionnaire. Conclusions: Using an SP program is strongly recommended to teach and evaluate in oriental medicine clinical clerkships.

  • Radiographic examinations are used to discover and define the type and extent of disease in many clinical situations. Currently, it is common for dental patients to receive dental radiographic examinations on a routine schedule with little variation in spite of different patient signs and symptoms. Such routine scheduling of radiographic examinations may result in unnecessary patient exposure. There is also the possibility of under-utilization of radiography; this can result in inadequate or excessively delayed diagnosis. Patient selection criteria are descriptions of clinical conditions derived from patient signs, symptoms and history that identify patients who are likely to benefit from a particular radiographic examination. It makes possible making individual decisions based on history, clinical examination and risk factors. The radiographic selection criteria of dentistry have been revised in accordance with recent guidelines and peer-reviewed research in USA and Europe, but the studies of these subjects are not sufficient in our nation. In this review article, the relating factors and update necessity of radiographic selection criteria are discussed.

Intra-Articular Injection of High-Dose ELHLD Peptide for Managing Canine Stifle Osteoarthritis: Kinetic Gait Analysis

  • Intra-articular injection of ELHLD peptide is considered to have a therapeutic effect in osteoarthritis (OA) through the inhibition of transforming growth factor-β1. This study aimed to assess the efficacy of intra-articular injections of high-dose ELHLD peptide (100 ㎍/kg) in canine stifle OA. Six client-owned dogs diagnosed with stifle OA were included. Selected dogs were treated with an intra-articular injection of high-dose ELHLD peptide (100 ㎍/kg). Outcome measures, including orthopedic examination, gait analysis, and Canine Brief Pain Inventory (CBPI) score, were evaluated four times after injection. Orthopedic examination, gait analysis, and owner's assessment (CBPI) improved significantly from 4 weeks after injection. In conclusion, we obtained sufficient evidence from this small sample that high-dose ELHLD peptide improves clinical signs of canine OA not only through subjective assessment but also through objective evaluation.

Therapeutic Effect of Nangan-geon for an Elderly Patient with Refractory Abdominal Pain: a Case Report

  • Objectve: To inform a clinical usefulness of Nangan-geon (暖肝煎) decoction to treat a refractory abdominal pain especially in eldery. Methods: This case report presents an 85-year old male patient who had been suffered with a refractory abdominal pain without any abnormality in laboratory test and radiological examination for 5-years. The clinical outcome was observed by numerical rating scale (NRS) of self-reporting method. Results: Based on his previous multiple normal examination results from Western hospitals and clinical features including thin body (19.6 BMI), nocturnal pain around lower abdomen and cold-sensitivity, author diagnosed him as "deficiency and cold of liver-Qi (肝氣虛寒)" similar to intestinal spasm. After prescription with Nangan-geon (暖肝煎) for one month, all symptoms became improved notably (NRS 5), and almost completely recovered as a level of normal condition in 3 months (NRS 1). Conclusion: This study would show the potential of traditional Korean medicine (TKM) therapies for a refractory functional abdominal pain, and especially efficacy of Nangan-geon against cold-pattern of nonspecific abdominal pain in elderly patients.
