Treatment of Tooth Discoloration using Microabrasion Technique : Case Report (Microabrasion Techinique을 이용한 치아변색의 치료증례)

  • Tooth discoloration detracts from one's appearance and influences self-image and it is particularly true in children. Therefore, pediatric dentists are required to treat tooth discoloration manifested in children for the normal development of their psycosocial health. Three treatment modalities are currently availabler for the removal of a variety of intrinsic stains from vital teeth. These are enamel microabrasion technique using hydrochloric acid, office bleaching and home bleaching technique with carbamide. Microabrasion technique has several advantages over bleaching in that it is easy to accomplish and does not require multiple office visits or the expensive instruments and the color change seems to be permanent after treatment. The process relies on decalcification, a softening with HCl and then removal of the enamel containing the stain with rubbing. Due to the mechanism of stain removal, this method is indicated for the removal of superficial enamel stains or disc oloration only. We report four successfully treated cases by enamel microabrasion using 15% HCl and pumice. Entire clinical steps are described in detail with some discussions on the outcome.

The analysis of complementary/alternative medicine-related study trend registered in the Korean Journal -focusing on the treatises listed at DBpia (한국 저널에 수록된 보완대체의학 관련 연구 동향 분석 -DBpia에 수록된 논문을 중심으로-)

  • Complementary/alternative medicine has been developed in extremely diverse forms reflecting inter-state historical, cultural differences. As Korean Medicine is institutionally accepted to the same status of Western Medicine, the concept of complementary/alternative medicine needs to be realigned. Data-basing academic level Web Service DBpia, the trend of research in the field of complementary/alternative medicine was analyzed by focusing on 144 treatises listed in the Korean Journal. As the result, we could subgroup treatises as those of concept, category, current status, institutions, details, and miscellany. The treatises related to the answers to "What is Complementary/alternative medicine" apply to the category of 'concept' Likewise in category are listed the treatises covering range and kinds of medicine. In 'category', 'current status', the discussions about the direction of Complementary/alternative medicine pending and treatises dealing with clinical applications are covered. In 'institutions', treatises on policy-making are enlisted. In 'details', treatises applied on specific diseases were sub-grouped. Others not otherwise covered enlisted in the list of "Miscellany".

Connect Attachment of Fixed Segmented Bridge (고정성 분할 브릿지의 연결 어태치먼트)

  • There are some cases that dental prosthesis does not operate as properly as expected in oral mouth. The reasons are such as a distortion of the mandibular, a fault of impression taking system or an extrusion of remaining teeth. One of dental prostheses to consider in the situations is the attachment which connects segment bridge. Active discussions are managed on theoretical side of this field but few on clinical side of it, which must be considered first. Accordingly I'd like to suggest a theoretical background for connect attachment of fixed segmented bridge. 1. As a bridge gets longer, burden on dental ligament is increased and the hardness of a bridge is lessened. 2. The flexibility of a bridge increases in ratio to 3 multiplication of the length and decreases in ratio to 3 multiplication of the width of occlusal surface and base of pontic. 3. Precision rest is needed to cope with the shake of teeth and the difference of axis direction among abutments. 4. Female part of the precision rest should be on middle abutment distal and male one on mesial of pontic. 5. Segmented attachment can be efficiently used to cope with long span bridgework and also in case that one piece casting can't be done because of slant of abutment.

Knowledge regarding factors that influence fertility in Thai reproductive-age population living in urban area: A cross-sectional study

  • Objective: To survey knowledge about the factors that influence fertility in a reproductive-age population living in an urban area. Methods: A cross-sectional study was conducted using a questionnaire-based survey among both males and females aged 18-45 years living in the Bangkok metropolitan area. Results: The mean age of the participants was $26.8{\pm}7.2years$ (male, 41.9%; female, 58.1%). Of the participants, 53.1% had an undergraduate degree and 57.1% were single. Only one-fifth of the participants correctly identified the age when fecundity declines in male and female, the definition of infertility, and the period during the menstrual cycle with the highest chance of pregnancy. Approximately three-fourths of the participants correctly identified that cigarette smoking, alcohol consumption, and sexually transmitted infections affect fertility. Conclusion: A considerable knowledge gap about the factors that influence fertility was identified in reproductive-age individuals in an urban area of Thailand. This issue should be urgently addressed by promoting fertility awareness through education, discussions about social perceptions regarding fertility, and reliable sources of knowledge.

  • The international government committee is progressing their agreements about intellectual property protection of traditional knowledge(TK), gene resource(GR), folklore(FL) in WIPO. It is in the course of selection with precedence of TK, GR, FL in WIPO, focused on discussions about listing of TK documents, standardization of DB construction, sharing and profit distribution of GR. We made some plans to deal with agreements in WIPO. These plans consist of 5 subjects which are investigation about intellectual property related laws and systems, study of examination and finding, construction of DB information, development of value evaluation system, development of industrialization system with TK, GR, We suggest preceding details about 5 subjects respectively. It is the character of this plan that patent information DB system of TK, GR is the axis. It is the most important thing in these plans to succeed that we have to have connections with expert groups in order parts and government officials, taking whole supports of government because our traditional medical related study basis is weak.

The Tasks of Medical Education to Support the Formation of Medical Professional Identity (전문직 정체성 형성을 위한 의학교육 현장의 과제)

  • Building professional identity is the most basic purpose of medical education. Students who enter medical schools do not have an identity rooted in the medical profession, and universities should therefore take steps to help students form their identity as doctors, attitudes, beliefs, and values through the curriculum. However, while medical knowledge and clinical skills are fully reflected in basic medical education, issues persist regarding education on values, attitudes, and beliefs that are important for professional identity. Regarding the process of professional identity formation, it is important to keep in mind that rapid changes in modern society lead to corresponding changes in socio-cultural expectations and demands related to professional identity, resulting in discrepancies between the reality of medical education and the actual field of medicine. Medical schools need to prepare students for these discrepancies, and in-depth discussions should address what is important and what should be solved first at medical education sites. However, it is difficult to generalize the tasks of professional identity formation in the field of medical education because each medical school may have unique circumstances. This article discusses the tasks that medical education should solve for professional identity formation education in terms of five aspects: establishing learning outcomes, training educational experts, introducing transformative learning, utilizing self-directed learning, and developing evaluation methods.

A Brief Note on Medical Ethics in Korean Medicine for Children and Adolescents (소아 청소년 한의 의료 윤리에 관한 소고(小考))

  • Purpose As modern science and medicine develop, the concept of life is changing, and the importance of patients' rights is emphasized, making it essential for medical professionals to think ethically. However, there is currently a lack of medical ethics research in the field of Oriental medicine, especially for pediatric and adolescent patients, we would like to take a look at this. Methods Through a literature review, we aim to discuss various ethical issues and human rights of children and adolescents that arise when Oriental medical doctors treat children and adolescents as medical professionals and use these as basic data for future research and education on Oriental medical ethics for children and adolescents. Results Medical ethics include the principles of autonomy, prohibition of evildoing, beneficence, and justice, and medical staff must make ethical judgments based on these principles. Ethical issues regarding children and adolescents arise in various clinical situations, and education on medical ethics is essential. Conclusions Discussions on the rights of children and adolescents are becoming active, and their importance is increasing. Therefore, sufficient explanation and consent must be provided to guardians, children, and adolescents with legal rightsand the opinions of the participants must be respected as far as possible.

A Study on Cold Damage(傷寒) in the Compendium of Prescription from the Countryside(鄕藥集成方) - Focusing on citation, medical theory, prescription, medicinal herbs - (조선 의서 『향약집성방』 중에 실린 상한(傷寒) 논의 연구 - 인용 문헌, 의론(醫論), 처방, 본초 등을 중심으로 -)

  • The purpose of this paper is to derive the features of cold damage clinical medicine during the early days of the Chosun(朝鮮) period by analyzing discussions on cold damage published in the official medical book of the Chosun period, Compendium of Prescription from the Countryside(鄕藥集成方, CPC). Cold damage was one of the typical diseases in East Asia where there was constant seeking of the utilization of prescriptions, ways of preparations, and awareness regarding cold damage as shown in Zhang, Zhongjing(張仲景)'s Treatise on Cold Damage Disease(傷寒論, TCDD) below. Traditional Korean medicine which possessed the medical universality of East Asia also was no exception and through an analysis of the part on cold damage in CPC, it is expected that medical features of cold damage in Korea passed down from the Koryo(高麗) Dynasty to the early Chosun period will be revealed. For this, first there needs to be an organization of past discussions on cold damage surrounding the existence of infection and after checking the issues, exploring which of the writings related to TCDD and editions are being utilized through an analysis on citing literature of Cold Damage Disease Literature(傷寒門) and Heat Pathogen Disease Literature(熱病門) which have developed discussions on cold damage in CPC. In addition, by comparing Peaceful Holy Benevolent Prescription(太平聖惠方, PHBP) and Complete Record of Sacred Benevolence(聖濟總錄, CRSB), known to have greatly influenced CPC and Cold Damage Literature and Heat Pathogen Disease Literature, features of form and content used by CPC were analyzed. Features of form were examined through pattern of organization and number of citing literature were examined and for features of content, cold damage infection, classification, syndrome differentiation method, and utilization of materia medica among prescriptions were examined. Discussions on cold damage as being uninfectious as stated in Treatise on the Pathogenesis and Manifestations of All Diseases(諸病源候論) unlike pestilence, epidemic pathogen(時氣), warm pathogen disease(溫病), and heat pathogen disease were excluded in PHBP. PHBP opened the possibility of cold damage infection and later writings, CRSB and CPC also follow this. As a result of analyzing citing literature of the part on cold damage in CPC, it is uncertain which edition of TCDD is being utilized; however, the most distinctive feature was that Classified Emergency Materia Medica(證類本草) and not writings specializing in cold damage are in use. In general, although CPC in terms of form is similar to CRSB, content creation predominantly depended on PHBP. More specifically; first, in terms of the existence of cold damage infection, arguments of PHBP and CRSB are maintained. Second, in terms of cold damage classification, although CRSB is followed, heat pathogen disease is classified separately developing PHBP as is. Third, in terms of method, as Book of Keep Healthy(南陽活人書) and CRSB compiled in later times are cited, it is deemed that arguments were raised to a certain extent regarding six-meridian syndrome differentiation(六經辨證). Fourth, although the majority of utilized materia medica among cold damage prescriptions utilize Materia Medica from the Countryside(鄕藥本草) in CPC and materia medica from Korean Peninsula, this is due to the desire for the compilation performance of CPC to be propagated to ordinary citizens and not the ruling class. CPC as the official medical book compiled in the early days of the Chosun period was greatly influenced by the Song(宋) Dynasty's medical books, PHBP and CRSB shows that cold damage medicine in the early Chosun Period indeed possesses the medical universality of East Asia. Furthermore, the features of published medical theory and prescriptions reveal the existence of the cold damage medical tradition of the Chosun period serving as clues for cold damage research tradition among Korea's medical history.

The Reliability and Validity Test of Sasangin Diagnostic Questionnaire for Specialist(SDQ_S) (전문가용 사상체질진단지의 신뢰도와 타당도 검사)

  • Objectives The purpose of this study was to develop the Sasangin Diagnostic Questionnaire for Specialist(SDQ_S) and to evaluate the reliability and validity of SDQ_S for standardization of sasang constitutional diagnosis by korean medicine Specialist in clinical practice.Methods SDQ_S was developed through discussions of three sasang constitutional medical specialist. We recruited 210 subjects aged from 20 to 70, and conducted the evaluation of SDQ_S and sasang constitutional diagnosis. For the reliability test, internal consistency was examined by calculating the cronbach's α coefficient on 88 items excepted measurements of trunk, and independence and agreement was examined by chi-square test and McNemar test for 84 subjects which retested SDQ_S. For the validity test, discriminant functions was derived from the sample group(105 subjects), and they were applied to the test group(105 subjects). We identified the hit-ratio of SDQ_S in the test group.Results and Conclusions In the reliability test, the cronbach's α coefficient was 0.664 and 87 items(98.9%) have dependent relationships between 1st test and 2nd test. And 39 items(44.3%) have high agreements(0.8≤McNemar), 26 items(29.5%) have common agreements(0.4≤McNemar<0.8). In the validity test, four discriminant variables(Soeum Index, Taeeum Index, Torso Ratio I, Taeyang Index) were selected, and two discriminant functions were derived. Total hit-ratio was 74.2%, and hit-ratio of Taeeumin(76.7%) was higher than that of Soeumin(75.9%) and Soyangin(69.7%). SDQ_S has good reliability and validity for sasang constitutional diagnosis.

Flow Diverter Devices for the Treatment of Unruptured Vertebral Artery Dissecting Aneurysm

  • Objective : Vertebral artery dissecting aneurysm (VADA) is a very rare subtype of intracranial aneurysms; when ruptured, it is associated with significantly high rates of morbidity and mortality. Despite several discussions and debates, the optimal treatment for VADA has not yet been established. In the last 10 years, flow diverter devices (FDD) have emerged as a challenging and new treatment method, and various clinical and radiological results have been reported about their safety and effectiveness. The aim of our study was to evaluate the clinical and radiological results with the use of FDD in the treatment of unruptured VADA. Methods : We retrospectively evaluated the data of all patients with unruptured VADA treated with FDD between January 2018 and February 2021 at our hybrid operating room. Nine patients with unruptured VADA, deemed hemodynamically unstable, were treated with FDD. Among other parameters, the technical feasibility of the procedure, procedure-related complications, angiographic results, and clinical outcomes were evaluated. Results : Successful FDD deployment was achieved in all cases, and the immediate follow-up angiography showed intra-aneurysmal contrast stasis with parent artery preservation. A temporary episode of facial numbness and palsy was noted in one patient; however, the symptoms had completely disappeared when followed up at the outpatient clinic 2 weeks after the procedure. The 3-6 months follow-up angiography (n=9) demonstrated complete/near-complete obliteration of the aneurysm in seven patients, and partial obliteration and segmental occlusion in one patient each. In the patient who achieved only partial obliteration, there was a sac 13 mm in size, and there was no change in the 1-year follow-up angiography. In the patient with segmental occlusion, the cause could not be determined. The clinical outcome was modified Rankin Scale 0 in all patients. Conclusion : Our preliminary study using FDD to treat hemodynamically unstable unruptured VADA showed that FDD is safe and effective. Our study has limitations in that the number of cases is small, and it is not a prospective study. However, we believe that the study contributes to evidence regarding the safety and effectiveness of FDD in the treatment of unruptured VADA.
