Loop-Mediated Isothermal Amplification Assay Targeting the femA Gene for Rapid Detection of Staphylococcus aureus from Clinical and Food Samples

  • In this study, a loop-mediated isothermal amplification (LAMP) method to rapidly detect Staphylococcus aureus strains was developed and evaluated by extensively applying a large number of S. aureus isolates from clinical and food samples. Six primers were specially designed for recognizing eight distinct sequences on the species-specific femA gene of S. aureus. The detection limits were 100 fg DNA/tube and $10^4$ CFU/ml. The LAMP assay was applied to 432 S. aureus strains isolated from 118 clinical and 314 food samples. Total detection rates for the LAMP and polymerase chain reaction assays were 98.4% (306/311) and 89.4% (278/311), respectively.

위점막에서 H. pylori 검출을 위한 조직염색법의 비교 (Comparative Analysis of Histochemical Stains about Detection of H. pylori in Gastric Mucosa)

  • Several methods have been used in the detection of Helicobacter pylori (H. pylori) which was believed to be a pathogenic organism causing chronic gastritis, benign peptic ulcer, gastric carcinoma or malignant lymphoma. Even though several methods were introduced for detection of H. pylori in stomach, there were controversies in their sensitivities and specificities. This experiment were designed to study the comparative analysis of staining methods (hematoxylin and eosin (H&E), Giemsa, Warthin-Starry and immunohistochemical stain) to dectect H. pylori in the gastric mucosa. The results were as follows. Average density score of H. pylori classified by Genta were 2.29 in Warthin-Starry stain, 2.19 in Giemsa stain, 1.34 in immunohistochemical stain and 0.98 in H&E stain. By comparison between inflammatory degree by Sydney system and result of Warthin-Starry stain, the detection rate and densities of H. pylori were increased from mild (61.5% and 0.8), moderate (90.4 and 2.1), and severe (100% and 3.2). From the above findings, Warthin-Starry stain is useful method for detection of H. pylori in gastric mucosa.

Detection of Mycobacterium leprae by Real-time PCR Targeting Mycobacterium leprae-Specific Repetitive Element Sequence

  • Mycobacterium leprae detection is difficult even with molecular biological techniques due to the low sensitivity of current methodologies. In this report, real-time PCR targeting the M. leprae-specific repetitive element (RLEP) sequence was developed as a new diagnostic tool and evaluated using clinical specimens. For this, M. leprae DNAs were extracted from skin biopsy specimens from 80 patients and analyzed by real-time PCR using TaqMan probe. Then, the detection efficiency of the real-time PCR was compared with that of standard PCR. In brief, the rate of positive detection by the standard PCR and real-time PCR was 32.50% and 66.25%, respectively. The results seemed to clearly show that the TaqMan real-time PCR developed in this study may be a useful tool for sensitive detection of M. leprae from clinical specimens.

  • The leading paradigm of dentistry had been focused on the rehabilitation treatment that identifies active caries, manages them surgically, and restores their original functions. However, changes in the external environment including the current disease prevalence require dentistry to have a paradigm shift. The new paradigm suggests the detection of caries in their earlier stages over the visual diagnosis of cavities, and the reversal of the incipient caries by non-surgical approach. For this to be achieved, a high-technology detection device recognizing changes in the earlier stages which can not he visually observed is needed. Development of early caries detection device has recently become a major issue in preventive dentistry on account of this need, and QLF(Quantitified Light induced Fluorescence) conspicuously stands out among the newly released devices. In this study, the fundamental concept of QLF(Quantitified Light induced Fluorescence) and the possible clinical applications of the earlier intraoral camera model as well as the recently designed digital camera model will be discussed.

  • The results of this study suggest the use of a Back Propagation Network (BPN) algorithm for the detection of esophageal erosions or abnormalities - which are the important signs of esophagitis - in the analysis of the color and textural aspects of clinical images obtained by endoscopy. The authors have investigated the optimization of the learning condition by the number of neurons in the hidden layer within the structure of the neural network. By optimizing learning parameters, we learned and have validated esophageal erosion images and/or ulcers functioning as the critical diagnostic criteria for esophagitis and associated abnormalities. Validation was established by using twenty clinical images. The success rates for detection of esophagitis during calibration and during validation were 97.91% and 96.83%, respectively.

  • Human lymphotropic herpesvirus is known to be a major pathogen associated with various diseases in bone marrow transplantation (BMT) recipients. A multiplex nested-polymerase chain reaction (PCR) method was developed for the simultaneous detection of human lymphotropic herpesviruses, including Ebstein-Barr virus (EBV), cytomegalovirus (CMV), and human herpesvirus 6 variants A and B (HHV6-A, HHV6-B). To demonstrate the usefulness of multiplex PCR for the analysis of clinical samples, peripheral blood mononuclear cells and serum from BMT recipients were analysed. The results skewed that a clear detection could be made between EBV, HCMV and HHV-6. This multiplex PCR assay is an efficient and cost-effective approach to the analysis of large numbers of samples to determine the epidemiological importance of EBV HCMV and HHV-6.

  • Colonoscopy plays an important role in reducing the incidence and mortality of colorectal cancer by detecting adenomas and other precancerous lesions. Image-enhanced endoscopy (IEE) increases lesion visibility by enhancing the microstructure, blood vessels, and mucosal surface color, resulting in the detection of colorectal lesions. In recent years, various IEE techniques have been used in clinical practice, each with its unique characteristics. Numerous studies have reported the effectiveness of IEE in the detection of colorectal lesions. IEEs can be divided into two broad categories according to the nature of the image: images constructed using narrow-band wavelength light, such as narrow-band imaging and blue laser imaging/blue light imaging, or color images based on white light, such as linked color imaging, texture and color enhancement imaging, and i-scan. Conversely, artificial intelligence (AI) systems, such as computer-aided diagnosis systems, have recently been developed to assist endoscopists in detecting colorectal lesions during colonoscopy. To gain a better understanding of the features of each IEE, this review presents the effectiveness of each type of IEE and their combination with AI for colorectal lesion detection by referencing the latest research data.

Kabuki syndrome: clinical and molecular characteristics

  • Kabuki syndrome (KS) is a rare syndrome characterized by multiple congenital anomalies and mental retardation. Other characteristics include a peculiar facial gestalt, short stature, skeletal and visceral abnormalities, cardiac anomalies, and immunological defects. Whole exome sequencing has uncovered the genetic basis of KS. Prior to 2013, there was no molecular genetic information about KS in Korean patients. More recently, direct Sanger sequencing and exome sequencing revealed KMT2D variants in 11 Korean patients and a KDM6A variant in one Korean patient. The high detection rate of KMT2D and KDM6A mutations (92.3%) is expected owing to the strict criteria used to establish a clinical diagnosis. Increased awareness and understanding of KS among clinicians is important for diagnosis and management of KS and for primary care of KS patients. Because mutation detection rates rely on the accuracy of the clinical diagnosis and the inclusion or exclusion of atypical cases, recognition of KS will facilitate the identification of novel mutations. A brief review of KS is provided, highlighting the clinical and genetic characteristics of patients with KS.

Rapid PCR Method for Detecting Candida albicans Using Primers Derived from the Integrin-like Protein Gene $\alpha$INT1 of Candida albicans

  • Oligonucleotide primers amplifying a 344 bp fragment on the integrin-like protein alpha-INT1p gene (${\alpha}$INT1) of Candida albicans were synthesized for screenign of C. albicans from clinicalsamples by the polymerase chain reaction (PCR). The PCR specifically amplified DNA from C. albicans and none from any other Candida, fungal, or human DNA in standard used here. The PCR assay showed that the primers (LH1 and LH2) were specific for 26 isolates of C. albicans from clinical smaples, whereas the positive fragment, 344 bp, was not amplified from 15 clinical isolates including 14 other medically important Candida species and an isolate of Saccharomyces cerevisiae. PCR was conducted on the urine samples of 20 patients and 4 samples were C. albicans positive. The detection limit of the PCR assay for C. albicans was shown to be approximately 10 cells/ml saline. The PCR system using 344 bp ${\alpha}$INT1 as a target is more specific and rapid than the conventional culture method, and the sensitive detection method is applicable to clinical diagnosis of C. albicans infections.
