2예의 Cervicogenic Headache의 치료 경험 -증례보고- (C2 Ganglion Block for Treatment of Cervicogenic Headache -A Case Report-)

  • The cervicogenic headache is not well defined for its characteristics and development of disease. It is developed by many causes including trauma principally. But, most cases have no radiologically specific clue for diagnosis. So, past hisory and clinical symptoms and signs are important for clinician to detect and diagnose the disease. Our clinic had treated the 2 cases of cervicogenic headache with different methods, and the patients were well respond the therapy.

  • We treated a patient who experienced motor weakness and sensory change on left lower extremity after thoracic sympathetic ganglion block with pure alcohol. The following factors were suspected of contributing to neurologic complication: (1) ischemia of spinal cord, (2) infection, (3) re-expression and aggravation of pre-existing neurologic disease, (4) improper position. Patient spontaneously recovered from neurologic complication with conservative therapy.

  • Meralgia paresthetica is a disorder characterized by a pain or dysaesthesia, or both, in the anterolateral aspect of the thigh caused by entrapment or neurinoma formation of the lateral femoral cutaneous nerve. Currently available modes of therapy include conservative treatment, lateral femoral cutaneous nerve block with steroids and local anesthetics, and surgery. At our neuro-pain clinic, w recently encountered three cases of meralgia paresthetica, all of which were treated by lateral femoral cutaneous nerve block. In which of them, two cases were successfully treated but one case was associate with pain relapse due to entrapment of lateral femoral cutaneous nerve by a retroperitoneal mass, schwannoma. In this paper we report our experience along with a review of the current literatures.

신경통증과 환자의 임상통계 고찰 (A Clinical Survey of Patients of Neuro-Pain Clinic)

  • Retrospective analysis, of 1,734 patients treated for nerve block from October 1991 to March 1994. Largest percentage of patients were in the 50 year old range, with a distribution of 44.9% male and 55.1% female. Treatments were for ailments of: Low Back Pain 17.3%, Multiple Contusion 10.5%, and Cancer 10%. Most common nerve block was epidural block 38.6%, followed by stellate ganglion block 38.4%, intercostal block 5.4%, and suprascapular nerve block 5.2%. Nerve block under fluorscopic guide were as follows: facet joint block 34.1%, lumbar sympathetic ganglion block 13.6%, and celiac plexus block 12.9%.

Novel Repair of Clamshell Thoracotomy Sternal Dehiscence after Lung Transplant: A Case Report

  • Bilateral transverse thoracosternotomy, or "clamshell" thoracotomy, can be complicated by dehiscence. A 65-year-old male underwent lung transplantation via clamshell thoracotomy, with subsequent sternal dehiscence on postoperative day 11. Upon repair, the previous sternal wires had pulled through, so a Sternal Talon connected to a Recon Talon was utilized to re-approximate the inferior sternum. On follow-up at 3 months, the patient recovered well. Use of the Sternal Talon provides an effective technique for repairing transverse sternal dehiscence.

  • The presence of epidermal growth factor receptor (EGFR ) mutation is a prognostic and predictive marker for EGFR-tyrosine kinase inhibitor (TKI) therapy. However, inevitably, relapse occurs due to the development of acquired resistance, such as T790M mutation. We report a case of repeated responses to EGFR-TKIs in a never-smoked woman with adenocarcinoma. After six cycles of gemcitabine and cisplatin, the patient was treated by gefitinib for 4 months until progression. Following the six cycles of third-line pemetrexed, gefitinib retreatment was initiated and continued with a partial response for 6 months. After progression, she was recruited for an irreversible EGFR inhibitor trial, and the time to progression was 11 months. Although EGFR direct sequencing on the initial diagnostic specimen revealed a wild-type, we performed a rebiopsy from the progressed subcarinal node at the end of the trial. The result of peptide nucleic acid clamping showed L858R/L861Q.

한방의료기관에서의 감기 진료에 대한 일반인의 인식 조사 (A Survey in the General Population on the Perception of the Common Cold Treatment at the Korean Medical Clinic)

  • Objective: This study investigated the perception, utilization, and satisfaction (in the general population), of the common cold treatment at the Korean Medical Clinic, to provide basic information for the development of Korean medicine clinical practice guidelines for the common cold. Method: A questionnaire was developed that consisted of questions about the general perception, utilization status, degree of satisfaction, willingness to use, and the improvement of the common cold treatment at the Korean Medical Clinic. An online survey was conducted using this questionnaire. Results: Three-hundred subjects responded to the survey. The results of the analysis were as follows. 1. 73.7% of the subjects recognized the common cold treatment at the Korean Medical Clinic. 2. 72% of the subjects had a positive perception of Korean medicine for common cold treatment. The major reason for the positive perception was the expectation about improving immunity and preventing recurrence. 3. Only 20% of the subjects had visited the Korean Medical Clinic for common cold treatment. The expensive cost was the major reason for not visiting the Korean Medical Clinic for common cold treatment. 4. The ratio of subjects who were willing to visit the Korean Medical Clinic for common cold treatment was 70%. 5. The expansion of health insurance coverage (67.7%), the activation of public relations (54.7%), and the development of a new herb medicine preparation that was easy to take (43.3%) were found to be necessary for improving the Korean medical service for common cold treatment. Conclusion: In spite of high recognition and positive perception, actual utilization of common cold treatment at the Korean Medical Clinic was relatively low. Multifaceted efforts are necessary to enhance the competitiveness of Korean medical service for treatment of the common cold.
