Sympathetic Ganglion Block for the Complication of Frostbite -A case report- (교감신경절 차단에 의한 동상합병증 환자의 치료 경험 -증례 보고-)

  • Frostbite involves freezing of tissues and usually affects the distal aspects of the extremities or exposed parts of the face. such as the ears, nose, chin, and cheeks. It produces tissue injury by ice crystal formation between the cells, cellular dehydration, and microvascular occulsion. There are four degrees of frostbite. First degree is accompanied by erythema and edema; second degree, by vesiculation, blistering, and eschar formation; third degree, by hemorrhagic blistering and bluish gray discoloration; and fourth degree, by injury to subcutaneous tissue, muscle, tendon, and bone leading to mottled, dry, black, and necrotic changes. We successfully treated 2 patients suffering from frostbite by performing sympathetic ganglion block with pure alcohol. We concluded sympathetic ganglion block is one of the most effective treatments for frostbite.

Quadriplegia due to Epidural Abscess following Continuous Cervical Epidural Block -A case report- (지속적 경부 경막외 차단 중 발생한 경막외 농양에 의한 사지마비 -증례 보고-)

  • A 45-year-old male received cervical continuous epidural block for posterior neck pain radiating to right upper extremity secondary to cervical herniated nucleus pulposus. Three days after epidural catheterization, fever, radicular pain and weakness of both upper extremities were developed. On admission, his temperature was $38.3^{\circ}C$ and showed progressive weakness and numbness in both upper and lower extremities. Cervical epidural abscess was suspected; MRI showed an epidural abscess from C4 to C7 level. Within 24 hours of admission, surgical decompression and drainage was effected. Culture of pus obtained at the lesion yielded Staphylococcus aureus. He was treated with intravenous antibiotics for 7 weeks resulting marked improvement of neurologic signs and symptoms.

  • Caudal epiduroscopy has been introduced as an alternative technique for direct injection of epidural steroid and lysis of adhesion. Futher, it gives a better understanding of the role of epidural adhesion in recurrence of sciatica and low back disorder after surgery. We experienced a clinical application of flexible fiberoptic epiduroscope inserted through the sacral canal. A 37-year-old woman was suffering from right lumbar radiculopathy after an operation for a herniated disc. A series of volumetric caudal steroid injections and physical therapy had little effect on her symptoms. Patient was thought to be a good candidate for epiduroscopy. Flexible flberoptic epiduroscopy was as follows: 60 ml of normal saline irrigation and epidurogram, 40 mg of triamcinolone in 10m1 of normal saline was directed around right L5 nerve root. The following morning, patient reported reduced pain in her leg.

The Effect of the Stellate Ganglion Block on Palmar Hyperhidrosis (수장 다한증 환자에서 성상신경절 차단의 효과)

  • We evaluated the effects of the stellate ganglion block(SGB) on the palmar hyperhidrosis. Ten patients of the palmar hyperhidrosis were taken right and left SGBs, 15 times on each side, total of 30 times, with 1% mepivacaine HCl 5 ml, with no discrimination on sex and age. Although there was a little decrease in the frequency of perspirations on 2 patients after the 15th block, no difference was noted after the overall 30th block at them. None of all 10 patients was satisfied symptomatically and no evidence of decreasing perspiration was found. Conclusively it seems that SGB with 1% mepivacaine HCl 5 ml is not an adequate therapy on the palmar hyperhidrosis even though it diminishes perspiration transiently.

  • This report is a case of 62-year-old man with anterior chest pain and pin pricking pain with allodynia affecting left T5 sensory dermatome for 3 months without history of vesicular skin eruption. He had a history of diabetes mellitus for 10 years and insulin therapy for recent 1 year. EKG, chest PA and rib series were normal. Serologic evaluation of IgG antibody to varicella-zoster virus was positive and was diagnosed as post herpetic neuralgia after zoster sine herpete. He was treated with left T5 nerve root block followed by thoracic epidural blockade and intercostal nerve block for 2 weeks. His VAS score decreased from 10 to 2 after 2 weeks of treatment. After 3 months, he revisited our clinic complaining right side chest pain followed by vesicular skin eruption 8 days after the onset of pain. He was treated as herpes zoster and tolerates well after 4 months.

A study on the effect of negative word-of-mouth of dental clinic patients (치과의료기관 내원환자의 부정적 입소문 전파에 관한 연구)

  • This study was done to investigate the negative word-of-mouth style and effect of communication with negative word-of-mouth from dental clinic patients. Data were collected from 223 dental clinic patients living in Seoul and GyeongGi-Do. The study was collected from October 15th to October 29th, 2007 with self-recording questionnaires. The results of this study were as follows. First of all, in the characteristic of relationships category, subject who were negative word-of-mouth was more 'persuader person' than others. The results showed that the general characteristics of subjects was effective factor of word-of-mouth. Secondly, the behavior scale which was based of negative word-of-mouth was not suitable of the satisfaction of dental clinic service. This results meant the low satisfaction of dental services haven't relation with negative word-of-mouth. Thirdly, 33% of people who have complaints spread negative word-of-mouth. Finally, the main reason of dissatisfaction was long-waiting time for dental clinic service. The results showed the adjustment of dental clinic system and staffs service will prevent negative word-of-mouth spread.

A Study on the Development and Validation of the Local Clinic Medical Doctor Role Stress Scale: Focusing on Local Clinic Medical Doctor (개원의 역할스트레스 척도 개발 및 타당화 연구: 1차 의료기관 개원의를 중심으로)

  • Objectives : This study developed and validated the local clinic Medical Doctor Role Stress Scale(MDRS). Methods : The interviews were conducted with 12 local clinic medical doctors to develop final preliminary questions. The scale was developed using statistical item analysis, exploratory and confirmatory factor analysis surveys with local clinic medical doctors in Seoul, Busan, Daejeon, and Gyeonggi province. Results : The MDRS developed in this study consisted of 14 items and three factors. The factors were role conflict, role ambiguity and role overload. The three factors explained 56.71% of the total variance, and the internal consistency of this scale was .90. The internal consistency for each factor was .81 ~ .90. Confirmatory factor analysis through a separate sampling met the fit criteria (CFI = .941, TLI = .927, RMSEA = .079, SRMR = .069). Conclusions : The results indicate that this scale is a reliable instrument for assessing local clinic medical doctor role stress.

A Clinical Experience of Facial Nerve Palsy Treated by Magnetic Resonance Analyser and Drug Therapy -A case of facial palsy- (자기공명분석기와 약물요법을 이용한 안면신경마비의 치험 -1예 보고-)

  • A 43 year old man who suffered from right facial palsy was treated successfully with the application of both magnetic resonance diagnostic analyser(MRA) and drug therapy. Treatment of facial palsy is generally composed of stellate ganglion block(SGB), drug therapy and operative intervention. Short periods of exposure to appropriate magnetic resonance can beneficially modulate the balance of autonomic nervous system that are responsible for sympathetic overflow. It was concluded that recovery of facial palsy by application of both MRA and drug therapy was effective in patient who refused SGB.

  • Hemifacial spasm is a distressing condition characterized by involuntary, intermittent, unilateral twitching of all or parts of the muscles innervated by the facial nerve. This occurrence is most common in middle-aged women. Because etiology of idiopathic hemifacial spasm has remained undefined, no causative agent nor reliable treatment has been established. This report describes a case of CT guided chemical facial nerve block for the treatment of hemifacial spasm. An injection of small amount(0.1 ml) of alcohol(95%) provided relief of the facial spasms.

  • Percutaneous retrogasserian gangliolysis is suitable for treatment of trigeminal neuralgia because it is simple and effective. Further, it can avoid severe complications such as anesthesia dolorosa or dysesthesia that may develop after surgical procedure. However we have experienced an anesthesia dolorosa(pain in an area or region that is anesthetic) after percutaneous retrogasserian glycerol injection. This deafferentation pain is characterized by scratching, burning or itching sensation; and has shown to be of higher incidence in older and inemotionally unstable patients. Anesthesia dolorosa is relatively rare but difficult to treat. Consideration should be made for the complication which may develop during the procedure.
