A Patient with Cleft Lip Combined with Branchial Cleft Cyst : A Rare Case Report (새열낭종을 동반한 구순열 환아의 증례보고)

  • 구순구개열은 두경부에서 가장 흔히 발생하는 선천적 기형 중 하나로 554명 중 1명의 빈도로 나타나며 인종에 따라 다양하다. 구순구개열 환아들은 다른 선천적 기형을 동반하여 나타나는 경우가 흔하며, 그 빈도는 문헌에 따라 다르지만 1.5~63.4%로 나타난다. 새열낭종은 두번째 인두강의 폐쇄부전으로 나타나는 선천적 결손으로, 주로 흉쇄유돌근 전방에 나타난다. 구순구개열 환자에 있어 새열낭종을 동반하는 경우는 매우 드물다. 전북대학교 구강악안면외과학 교실에서는 새열낭종을 동반한 구순열 환아 1례를 경험하였다. 환아는 우측 불완전 편측성 구순열로 내원하여, 구순성형술과 함께 우측 목에 존재하던 새열낭종에 대한 제거 수술을 시행하였다. 환아는 출생 당시부터 심실중격결손 및 동맥관개존증 등의 선천적 심장질환 및 갑상선 기능저하증을 가지고 있던 환아로 다양한 선천적인 결손을 동반한 본 환아의 증례를 문헌고찰과 함께 보고하는 바이다.

Prognosis of Alveolar Bone Graft Alveolus in Cleft Patients : the preliminary report (치조열환자에서 치조골 이식술의 예후)

  • The case records of 24 patients in Seoul National University Hospital who had bone grafting of 29 alveolar clefts between 2001 and 2004 were examined. Details were recorded of age, sex, preoperative orthodontictreatment, the time of bone grafting, the type of donor site, cleft width, functional load applicationand the success of grafting as established by lowest marginal bone levels. Using this results and review of literature, we concluded that secondary bone graft with iliac bone before canine eruption with root development of 1/2 to 1/3 provide more favorable results and the functional load introduced to the grafted bone lower the resorption rates.

Personal technique for definite repair of complete unilateral cleft lip: modified Millard technique

  • Background: Millard's rotation-advancement repair, which is used by many surgeons, can make a natural philtral column, but most surgeons use a modification of the rotation-advancement flap. The purpose of this article is to introduce a modification utilized by the authors and to provide detailed surgical procedure. Methods: We retrospectively reviewed 82 patients' medical records and presented surgical technique and outcomes. The main features of the authors' strategy are emphasizing horizontal length of the lip, orbicularis oris muscle duplication for improving the definition of the philtral column, overcorrection of domal portion than the non-cleft side in order to compensate for the recurrence during growth. Two judges rated two times the appearance of the patients' nose and lip using Asher-McDade aesthetic index. Intra- and interobserver reliabilities were determined using Cohen's kappa statistics. Results: All patients recovered eventually after surgery; however, two patients have a minor complications (wound infection in one patient, wound disruption due to trauma in the other patient). The improvement of the aesthetic results can be achieved with this modified Millard technique. Total mean scores of the Asher-McDade index was 2.08, fair to good appearance. The intraobserver reliabilities were substantial to almost perfect agreement and the interobserver reliabilities were moderate to almost perfect agreement. Conclusion: We modified Millard method for repair of complete unilateral cleft lip. The surgical outcomes were favorable in long-term follow-up. We hope our technique will serve as a guide for those new to the procedure.

Amniotic constriction band: a report of two cases with unique clinical presentations

  • Amniotic constriction band is a rare clinical entity with varied manifestations that range from a combination of congenital malformations to isolated malformations that are unique to each patient. The etiology of this entity remains unknown. Herein, we highlight two cases of amniotic constriction band that presented to our unit with unique clinical characteristics. To the best of our knowledge, an isolated circumferential band of scarring on the face with ocular involvement, as demonstrated by the first case, and a combination of bilateral complete cleft lip and palate with bilateral microphthalmia, auto-amputation of the right thumb, and a constriction band on the left thumb, as demonstrated by the second case, are extremely rare presentations of amniotic constriction band that were not previously reported in the literature and therefore necessitate a special mention. We discuss potential etiologies for these cases and review the existing literature on this entity.

  • Distraction osteogenesis and orthognathic surgery are the widely used surgical methods for treating hemifacial microsomia and cleft lip and palate, the representative forms of congenital deformity. Distraction osteogenesis is an outstanding treatment of choice when more traction is needed than what can be achieved by general orthognathic surgery. However, the stability of distraction osteogenesis has not yet been established, and in most of the cases, additional orthognathic surgery is mandatory. Moreover, the difficulty in precise control of the traction directions is another disadvantage of distraction osteogenesis. Therefore, it would be desirable not to conduct distraction osteogenesis when the patient is suitable for an orthognathic surgery. Also, distraction osteogenesis should be recognized as an accessorial method of treatment, and be used restrictively.

  • In order to obtain the basic data of the movements of the mandible for orthodontic treatment and gnathosurgery of cleft individuals, the ranges and shapes of the movements of the mandible were measured in frontal, sagittal, and horizontal view with Saphon Visi-Trainer CII, in 19 adult cleft lip and palate individuals. The subjects included 5 BCLP, 9 UCLP, and 5 CLA patients. The measurements were compared with the values of normal individuals reported by Kang 1. Frontal view. The mean values for maximal laterotrusion were almost the same as those previously reported for the normal individuals. Mandibular deviation in maximal opening was usually toward the right, while it was mainly toward the left in normal individuals. Typical shield was formed only in the CLA group. 2. Sagittal view; The mean value for maximal protrusion was not different from that of normal group and antero-posterior deviation showed a significant difference. The angle of maximal protrusion and horizontal plane was less than that of normal group. 3. Horizontal view; The mean values for maximal laterotrusion and protrusion were not different from those of the normal group. The angle of the laterotrusion and horizontal plane was larger in the left and smaller in the right.
