Purpose : The etiology of hemolytic anemia can be classified as either cellular or extracellular defects of red blood cells. The aim of this study was to investigate the clinical and laboratory findings of hemolytic anemia concerning its etiological classification. Methods : Clinical and laboratory findings of the patients with hemolytic anemia treated from January 1987 to May 2002 at Severance Hospital were analyzed retrospectively. They were divided into two groups based on the types of red cell defects(group I : erythrocytic defect, group II : extraerythrocytic defect). Results : Twenty one cases were included in group I, thirty four cases in group II, and three cases were unclassified. In group I, nineteen cases(90.5%) were diagnosed as hereditary spherocytosis and were proved to have red cell membrane disorders while two cases(9.5%) were shown to have red cell enzyme deficiencies. In group II, thirteen cases(38.2%) were noted as autoimmune hemolytic anemia, eleven cases(32.4%) as traumatic or microangiopathic hemolytic anemia, four cases(11.8%) as drug induced hemolytic anemia, two cases(5.9%) were related with systemic lupus erythematosus and one case(2.9%) with malignancy. Hemoglobin at the time of diagnosis(7.5 g/dL vs. 6.2 g/dL, P<0.05) and the incidence of splenomegaly(85.7% vs. 18.2%, P<0.05) were higher in group I though blood urea nitrogen(9.0/0.4 mg/dL vs. 27.8/1.6 mg/dL, P<0.05) was higher in group II. Conclusion : Comparing the clinical features of pediatric hemolytic anemia, we concluded as following : In cases associated with extraerythrocytic defect, blood tests revealed significant initial lower hematocrit with higher level of BUN and Cr while cases with erythrocytic defect, splenomegaly were more common noted.
Background: Endobronchial tuberculosis is one of the serious complications of pulmonary tuberculosis. Without early diagnosis and proper treatment of endobronchial tuberculosis, bronchostenosis can leave and lead to the collapse of distal lung parenchyme, bronchiectasis, and secondary pneumonia accompanied with moderate to severe dyspnea, cough, hemoptysis, and localized wheezing. Therefore steroid therapy has been tried to prevent bronchostenosis. But the effect of steroid therapy on the endobronchial tuberculosis is not definite at present. We tried to elucidate the effect of steroid on the treatment of endobronchial tuberculosis for prevention of bronchostenosis. Methods: We observed the initial and sequential bronchoscopic findings, pulmonary function tests and simple chest roentgenograms in 58 patients diagnosed as endobronchial tuberculosis and admitted to Chung-Ang university hospital from 1988 to 1992. The patients in nonsteroid group (n=39) were treated with anti-tuberculosis chemotherapy only and steroid group(n=17) with combined steroid therapy. Sequential bronchoscopic findings, pulmonary function tests, and chest roentgenograms were comparatively analyzed between the two groups. Results: 1) The endobronchial tuberculosis was highly prevalent in young females especially in third decade. 2) Both actively caseating type and the stenotic type without fibrosis was the most common in the bronchoscopic classification. 3) The sequential bronchoscopic findings in steroid group 2 months after treatment showed no significant improvements compared with nonsteroid group. 4) There was no significant difference between the two groups in the sequential bronchoscopic improvements according to bronchoscopic types. 5) We did not find any significant difference in improvements on follow-up pulmonary function tests and simple chest roentgenograms between the two groups 2 month after treatment. 6) There was no significant adverse effect of steroid during the treatment. Conclusion: Combined steroid therapy provably would not influence outcome of the treatment of endobronchial tuberculosis.
Purpose: This study examined the relationship between graft appearance on follow-up magnetic resonance imaging (MRI) and knee stability after double bundle anterior cruciate ligament (ACL) reconstruction. Materials and Methods: For each patient, 1.5 tesla MRI's were obtained. The signal intensity of grafts was divided into 3 grades by Sononda's classification. The course of grafts was divided into two patterns: straight and curved. We assessed Lachman test, KT 2000 arthrometer and anterior drawer stress radiograph using Telos$^{(R)}$ in $30^{\circ}$ knee flexion for anterior stability and evaluated pivot shift test for rotatory stability. The correlation between graft appearance on MRI and the results of knee stability tests was evaluated. Results: The anteromedial (AM) graft was evaluated as being grade 1 in 66.7%, grade 2 in 26.7%, and grade 3 in 6.7% of the cases and the posterolateal (PL) graft was assessed as being grade 1 in 63.3%, grade 2 in 33.3%, and grade 3 in 3.3% of the cases according to the signal intensity. The AM graft was evaluated as being straight in 83.3% and curved in 16.7% of the cases, and the PL graft was assessed as being straight in 86.7% and curved in 13.3% of the cases according to the course. The course of AM graft was correlated with the results of anterior stability tests and the course of PL graft was correlated with the result of rotatory stability test. However, the signal intensity of grafts was not correlated with the results of anterior stability and rotatory stability tests. Conclusion: The course of AM is correlated with anterior stability and the course of PL is correlated with rotatory stability on follow-up MRI after double bundle ACL reconstruction.
The internal structure of a river embankment must be delineated as part of investigations to evaluate its safety. Geophysical methods can be most effective means for that purpose, if they are used together with geotechnical methods such as the cone penetration test (CPT) and drilling. Since the dyke body and subsoil in general consist of material with a wide range of grain size, the properties and stratification of the soil must be accurately estimated to predict the mechanical stability and water infiltration in the river embankment. The strength and water content of the levee soil are also parameters required for such prediction. These parameters are usually estimated from CPT data, drilled core samples and laboratory tests. In this study we attempt to utilise geophysical data to estimate these parameters more effectively for very long river embankments. S-wave velocity and resistivity of the levee soils obtained with geophysical surveys are used to classify the soils. The classification is based on a physical soil model, called the unconsolidated sand model. Using this model, a soil profile along the river embankment is constructed from S-wave velocity and resistivity profiles. The soil profile thus obtained has been verified by geotechnical logs, which proves its usefulness for investigation of a river embankment.
Purpose : This study analyzes prehospital care report of emergency service at prehospital stage, examines characteristics of activities of 119 paramedics and its users and aims to provide help for improving emergency system in future. Methods : Data collected were 119 prehospital care report and hospital records with 7,160 patients to emergency room by 119 ambulance from Jan. 1 to Dec. 31, 2006 and percentage and frequency of the data were obtained. Results : 1) Use of emergency room by 119 ambulance was increased in summer and autumn such as August(9.1%), September(11.2%) and October(13.5%) and it was more frequently used on Monday(17.3%), Saturday(17.2%) and Friday(16.1%) by telephone(98.6%). 2) Using emergency room was most in over sixties(51.8%), men(64.2%), community residents (78.3%), by report of family(50.3%) and at '09:01~12:00'(16.5%). 3) Symptoms of emergency room users included headache, chest pain, stomachache, lumbago and others as 40.6% and places where patients were found were at home(60.1%) due to chronic internal diseases at 49.2%. 4) Most of non-emergency patients(80.2%) arriving at hospital had normal pupil condition (88.4%) and clear consciousness(71.2%) and most of them left hospital after having first-aid treatment. 5) Physiological symptom tests evaluated by paramedics at prehospital stage included blood pressure(56.6%), pulse(22.9%), breathing(13.0%) and temperature(9.2%), and there was no SPo2 case. 6) Classification of severity by paramedics showed difference as emergency patients(18.0%) by paramedics and those(24.9%) by hospital. 7) First-aid treatments by paramedics at prehospital stage were promoting comfort(28.9%), hemostasis(7.7%), fixing cervical vertebrae(4.0%) and ensuring vein route(3.1%). 8) Selectors of medical agency were patients or guardians(86.2%) and emergency medical technicians(73.6%). Conclusion : To sum up the above research, it was found that percentage of using 119 ambulance by non-emergency patients was higher and paramedics performed basic first-aid treatment rather than professional first-aid treatment due to several conditions such as legal problems, range of allowance, etc. Therefore, it is considered that method to reduce frequency of ambulance by non-emergency patients and approaches to alleviate limitations of allowance of paramdeics to make them perform effective first-aid treatment at prehospital stage should be sought in the dimension of individual, organization and government.
The sedimentary rocks exposed around Mt. Palgong were subjected to metamorphism due to a granitic magma intrusion at late Cretaceous, and they eventually metamorphosed to hornfels by the action of both hydrothermal solution and high temperature supplied from the magma. The hornfels zone around the granite body ranges from 2.0 to 3.5 km in width but the boundary between hornfels and sedimentary rocks is not obviously defined because the metamorphic grade gradually decreases with distance from the granite boundary. A series of laboratory tests on 350 core specimens made by 35 fresh rock blocks obtained from 5 selected locations around Mt. Palgong are performed to verify the variation of physical and mechanical properties related to metamorphic grade of the rock. Water content and absorption ratio of the hornfels linearly increase with distance to the granite boundary whereas dry unit weight, p-wave velocity, point load strength, and slake durability index linearly decrease with the distance. These results imply that the metamorphic grade of the hornfels also linearly decrease with the distance to granite boundary. Empirical equations for the variation of properties with the distance to granite boundary and relationship between a property and another one are deduced by regression analyses. And a criteria for classification of hornfels exposed in the study area based on the P-wave velocity and point load strength is proposed.
In order to find an alternative source of inoculum to caecal content for studying the fermentation activity of rabbit hindgut, caecal content and faeces of 25 hybrid Hyla rabbits were used as inocula for an in vitro gas production trial. About 1 g of three substrates (dehydrated alfalfa meal, dehydrated beet pulp, barley) was weighed, in quadruplicate per inoculum, in 120 ml bottles; 75 ml of anaerobic medium and 4 ml of reducing solution were added and bottles were placed at $39^{\circ}C$. Caecal content and faeces were diluted respectively 1:2 (CI) and 1:8 (FI) with anaerobic medium and were introduced in the respective bottles (10 ml). Gas production was recorded 20 times at 2-24 h intervals throughout fermentation (96 h). The fermentation characteristics (i.e. degraded organic matter, OMd; potential gas production, A; fermentation rate, Rmax; time at which it is reached, Tmax; pH, volatile fatty acid, VFA) were studied by inoculum and feedstuffs. The feedstuffs, according to their chemical composition, showed very different fermentation characteristics. In particular, OMd, A and Rmax allowed feedstuff classification as follows: barley>beet pulp>alfalfa. The inocula differ (p<0.05) in Tmax, were higher for CI (15.53 vs. 11.96 h) and in VFA production. In particular, CI produced higher levels of acetate (38.9 vs. 33.4 mM/g OM incubated, p<0.01) and isobutyrate (0.72 vs. 0.42, p<0.01) but less propionate (7.1 vs. 10.3, p<0.01) and butyrate (11.3 vs. 14.0, p<0.01). However, the trend of gas production, similar for the inocula according to the fermented substrate, and the good regression equation to estimate some caecal fermentation parameters from faeces suggest that, after standardisation, the faeces could be used as an alternative inoculum for gas tests in rabbit.

(Z)-9-tetradecenyl acetate (Z9-14:Ac), (Z)-11-tetradecenyl acetate (Z11-14:Ac), (E)-11-tetradecenyl acetate (E11-14:Ac) and 10-methyldodecyl acetate (10me-12:Ac) have been reported as the sex pheromone components of the genus Adoxophyes. Our objective was to determine the difference in sex pheromone composition of three different A. spp. populations each from Cheonan, Sangju, and Naju area orchards in Korea. Gas chromatography (GC) analyses of pheromone gland extracts of virgin females confirmed the presence of two compounds (Z9-14:Ac and Z11-14:Ac) in Cheonan and Sangju populations, and four compounds (Z9-14:Ac, Z11-14:Ac, E11-14:Ac and tome-12:Ac) in Naju population. The Z9-14:Ac and Z11-14:Ac were detected in the ratio of 80:20 in the Cheonan population and 3:97 in the Sangju population. Females of Naju population produced sex pheromone blend consisting of Z9-14:Ac, Z11-14:Ac, E11-14:Ac and 10me-12:Ac at a ratio of 31:62:6:1. Field trapping tests in pear orchards with Z9-14:Ac and Z11-14:Ac indicated that maximum captures of the male were obtained with traps baited by 80:20 in Cheonan, 10:90 in Sangju, and 30:70 in Naju. These results suggest that there are remarkable geographical variations in the sex pheromone composition of A. spp. in pear orchards in Korea, and taxonomic classification of these species must be carefully assessed.

Objectives: Acquired immunodeficiency syndrome (AIDS) is a disease of the human immune system caused by the human immunodeficiency virus (HIV). People with AIDS are much more vulnerable to infections, including opportunistic infections and tumors, than people with a healthy immune system. The objective of this study was to correlate oral lesions associated with HIV/AIDS and immunosuppression levels by measuring clusters of differentiation 4 (CD4) cell counts among patients living in the middle western regions of Ghana. Materials and Methods: A total of 120 patients who visited the HIV clinic at the Komfo Anokye Teaching Hospital and the Regional Hospital Sunyani of Ghana were consecutively enrolled in this prospective and cross-sectional study. Referred patients' baseline CD4 counts were obtained from medical records and each patient received an initial physician assessment. Intraoral diagnoses were based on the classification and diagnostic criteria of the EEC Clearinghouse, 1993. After the initial assessment, extra- and intraoral tissues from each enrolled patient were examined. Data analyses were carried out using simple proportions, frequencies and chi-square tests of significance. Results: Our study included 120 patients, and was comprised of 42 (35.0%) males and 78 (65.0%) females, ranging in age from 21 to 67 years with sex-specific mean ages of 39.31 years (males) and 39.28 years (females). Patient CD4 count values ranged from 3 to 985 cells/mL with a mean baseline CD4 count of 291.29 cells/mL for males and 325.92 cells/mL for females. The mean baseline CD4 count for the entire sample was 313.80 cells/mL. Of the 120 patients we examined, 99 (82.5%) were observed to have at least one HIV-associated intraoral lesion while 21 (17.5%) had no intraoral lesions. Oral candidiasis, periodontitis, melanotic hyperpigmentation, gingivitis and xerostomia were the most common oral lesions. Conclusion: From a total of nine oral lesions, six lesions that included oral candidiasis, periodontitis, melanotic hyperpigmentation, gingivitis, xerostomia and oral hairy leukoplakia were significantly correlated with declining CD4 counts.

This study was conducted to set up ecological database for effective forest management and conservation of KNU Research Forest in Mt. Myeonbong with the characteristic analysis of stand structures. Following the results of clustering analysis, they were classified under 6 communities (Quercus mongolica-Pinus densiflora, Pinus densiflora, Carpinus cordata, Fraxinus rhynchophylla-Acer pseudosieboldianum-Acer pictum subsp. Mono, Quercus mongolica-Quercus variabilis, Quercus mongolica). Importance value tests were estimated that on ridge; Pinus densiflora, valley; Carpinus cordata, Fraxinus rhynchophylla-Acer pseudosieboldianum-Acer pictum subsp. Mono were recorded dominant species. Carpinus cordata and Fraxinus rhynchophylla-Acer pseudosieboldianum-Acer pictum subsp. Mono community, north aspect and valley were investigated high species richness value. It was showed decreasing tendency as altitude and degree of slope were high. Results of NMS, upper & middle layers and shrub & herbal layers were influenced by species richness and the case of species association.
