Benefit of chuna for managing symptoms in chronic fatigue syndrome patients: A systematic review and meta-analysis (만성피로증후군의 증상관리를 위한 추나요법의 유용성: 체계적인 문헌고찰과 메타분석)

  • Objectives: The aim of this review is to assess the clinical benefit of chuna for managing symptoms in chronic fatigue syndrome (CFS) Methods: We searched eligible studies from the following 11 databases with no language restriction: Pubmed, CENTRAL, Embase, CNKI, CQVIP, Wanfang, CiNii, OASIS, RISS and Koreamed. We selected randomized controlled trial (RCT), quasi-randomized controlled trial (Quasi-RCT) and controlled clinical trial (CCT) in which patients with chronic fatigue syndrome were treated with chuna. The results of the included studies were synthesized through meta-analysis, and their risk of bias were also assessed. Results: We searched 914 potentially relevant studies, and only 20 studies were selected for this systematic review. Meta-analysis showed that chuna was more effective in improving general symptoms (n=291, RR 0.19 [95% CI 0.11 to 0.32], Z=6.08, p<0.00001, I2=0%), fatigue (n=487, SMD -0.95 [95% CI -1.19 to -0.71], Z=7.76, p<0.00001, I2=37%) and sleep disorder (n=139, SMD -0.58 [95% CI -0.92 to -0.24], Z=3.36, p=0.0008, I2=0%) compared to health supplements or usual care. When chuna was used as an added intervention, it was more effective in improving general symptoms(n=729, RR 0.25 [95% CI 0.15 to 0.42], Z=5.19, p<0.00001, I2=0%) and fatigue severity alone(n=217, SMD -1.21 [95% CI -1.53 to -0.89], Z=7.45, p<0.00001, I2=15%) compared to control. Conclusion: We found that chuna was beneficial for improving severity of general symptoms and fatigue at post treatment in CFS patients.

Correlation Study between Cervical X-ray Sagittal Parameters and Chuna Posture Analysis Results (경추 X-ray 시상면 지표와 추나체형분석 결과와의 상관성 연구)

  • Objectives The purpose of this study was to examine how changes in the cervical spine correlate with body posture changes in the whole body, and to find out the significance of complementary clinical application X-ray and Chuna posture analysis. Methods From January 1, 2019 to October 31, 2019, the results of 27 patients with pain in the vertebral region were analysed in accordance with the results of cervical X-ray and Chuna posture analysis. In order to confirm the significance of Chuna posture analysis results, the survey of 187 Chuna standard curriculum instruction qualification certifiers was conducted and the responses of 47 of them were analyzed. Results The occiput-atlas cline angle increases in both hypolordosis/hyperlordosis groups based on cervical lordosis angle, and increases further than in the hyperlordosis group. There were significant correlations between the changes in the cervical spine and the body posture changes in the whole body. There were no significant differences between cervical X-ray sagittal parameters and the body posture analysis parameters based on the patient's major disease codes. Conclusions As a result of conducting a survey on the clinical importance of the body posture analysis parameters, the importance of cervical parameters was verified. Changes in the cervical spine may not only cause other changes in the cervical region, but also affect the body posture changes in the whole body. The complementary application of X-ray and Chuna posture analysis results is helpful in clinical diagnosis and treatment of musculoskeletal disorders.

  • Objective : The purpose of this study is to evaluate the conservative treatment for FBSS patient. Methods : We used Herbal medication, Acupuncture, burning Acupuncture, Physical Therapy for this patient. And we measured of VAS and ODI score to evaluate treatment effects. Results : Patient's sciatica and sensory test has improved. VAS and ODI score were also decreased.

  • Objectives : We evaluated the effect of Mckenzie method treatment by participating two patients suffering from Tortipelvis by lumbar disc herniation. Methods : Two patients were diagnosed as Tortipelvis by lumbar disc herniation through CT scan and X-ray examination. We used Mckenzie method and measured VAS(Visual analogue scale). Results and Conclusions : After using Mckenzie method treatment, We found out a recovery from both patients suffering from Tortipelvis by lumbar disc herniation. The result, through our study, suggests that Mckenzie method treatment was effective to cure the Tortipelvis by lumbar disc herniation. It also was effective to reduce the low back pain and to stabilize the lumbar disc herniation.

  • The aim of this study is to estimate clinical outcome of Conservative treatment for patients with Herniated Intervertebral Disc(HIVD) of lumbar spine. We investigated the 33 cases of Inpatients who were diagnosed as the HIVD of L-Spine and were treated Acupuncture therapy, Herb-medicine, Bee-vom treatment in Oriental Medicine Hospital from March 1, 2005 to November 31, 2005. We evaluated the subjective symptoms by Visual Analogue Scale, Hudgin's classification and follow-up of Activity Daily Living status. The improvement index showed that the oriental medical conservative therapy improved the symptom of HIVD effectively.

  • Objectives : The purpose of this study is to evaluate the korean medical treatment for carpal tunnel syndrome, especially taping therapy for carpal space expansion. Methods : Six patients were treated with acupuncture, moxibustion and kinesio taping therapy. To evaluate the pain and numbness of the palm and finger, visual analogue scale(VAS) and Phalen's test were used. Results : After about 10 to 20 times treatment, the pain and numbness of almost cases were remarkably removed. Also the VAS of post-treatment was reduced by 1.33(means). And the time being required to response to Phalen's test was signally prolonged, as 5 cases took over one minute. Conclusions : Korean medical treatment including expansion taping of carpal space as kinesio correction therapy is remarkably effective for carpal tunnel syndrome. But further studies are required to concretely prove the effectiveness of correction taping on other compressive neuropathy.

  • This study was performed to report the effect of oriental medical treatment and general biomechanic manipulation on a patient with upper limb palsy caused by postural brachial plexus injury. The patient was treated with acupuncture, herb medicine and general biomechanic manipulation. The effectiveness of treatment was evaluated with range of motion, visual analogue scale and manual muscle test. After 11 times treatment, motion and muscular force were progressed, pain was decreased. This result suggests that oriental medical treatment and general coordinative manipulation are effective to care the postural brachial plexus injury.

  • Objectives: This study is to report three cases those have an meaning for rheumatoid arthritis treatment by oriental medicine. In this case rheumatoid arthritis patients have phalangeal and carpal joints disorder mainly. Methods: These patients were treated by acupuncture and herb medication. Results: After treatment, the conditions of patients were improved with oriental medicine, so in two cases rheumatoid arthritis disappeared, but in one case rheumatoid arthritis not appeared. Conclusions: These results suggested that oriental medical treatment was effective to improve the condition of rheumatoid arthritis patients.

  • Objectives : The purpose of this study was to report clinical effects of Lumbar flexion-distraction manipulation and Sitting position thoracic and lumbar vertebra adjustment manipulation on patient with Juvenile idiopathic scoliosis Methods : The patients have been treated with the above two Chuna manual manipulations. We measured Cobb's angle, VNRS before and after treatment Results : After treatment, we confirmed these transition : In first case, Cobb's angle of T-spine changed from 27 to 23 : Cobb's angle of L-spine changed from 17 to 10 : VNRS changed from 6 to 2. In second case Cobb's angle of T-spine changed from 30 to 21 : Cobb's angle of L-spine changed from 16 to 16 : VNRS chagned from 6 to 3 Conclusion : The result suggests that these two Chuna manual manipulations were effective treatment on patients with Juvenile idiopathic scoliosis.

  • The study reports the clinical case of a patient with lower extremity weakness and gait disturbance treated with Korean medicine treatments including Chuna therapy. The patient suffered lower extremity weakness and gait disturbance with diagnosis of lumbar disk herniation and spinal stenosis. As a treatment, the doctor applied Chuna therapy, herbal medicine, acupuncture, pharmacopuncture, cupping and moxa. The effect of treatment was evaluated by numeral rating scale (NRS), self-walkable distance, functional independence measure (FIM), Oswestry disability index (ODI) and manual muscle test (MMT). NRS decreased from 6 to 4 at the leg. Self-walkable distance increased from 0 m to 10 m, FIM increased 85 to 96 points while ODI decreased 64% to 54%. MMT of hip flex and knee extension improved from grade 3+, grade 3 to grade 4, respectively. Korean medicine treatment can be effective for patients who suffer lower extremity weakness and gait disturbance due to lumbar disk herniation and spinal stenosis. Further clinical studies are required to verify these findings.
