One Year Follow up for Severe Adult Atopic Dermatitis of 15 Patients After Sasang Constitutional Therapy (중증 성인형 아토피 피부염을 사상처방으로 치료하여 호전된 환자 15례에 대한 1년간의 추적 관찰)

  • Background : Atopic Dermatitis is a chronic and recurrent skin disease. it is characterized as itch-scratch cycle and topical or systemic use of corticosteroids is required in western medicine. Though its effects on major symptoms are impressive, these treatments are prone to several side effects and tend to recur after months or years. So potential treatments have been actively studied in Estern medicine, especially Sasang constitutional therapy, which has fewer side effects. Objectives : The purpose of this study was to examine the effect of Sasang constitutional therapy on treating severe adult Atopic Dermatitis and preventing its recurrent symptoms. Methods : fifteen patients who had severe adult Atopic Dermatitis attended treatment of Sasang constitutional therapy and were followed up for one year. An oriental internal specialist first isolated Sasang constitution and a distinctive body type. Grading of Atopic dermatitis was measured by the guidelines offered by Rajka G. and Langeland T. Results : After treatment with Sasang constitutional therapy, the patients symptoms were improved and the mean Rajka and Langeland score reduced significantly from $7.73{\pm}0.79\;to\;3.4{\pm}10.91$. During the follow-up period of one year, the mean Rajka and Langeland score was $2.72{\pm}2.06$ which was lower than last treatment. Conclusions : The results of this study support Sasang constitutional therapy as an effective therapy for severe adult Atopic Dermatitis. Follow up data suggest that this Eastern medical approach is of greater clinical value than the western medical approach in the long term.

Epidemiological Study on Temporomandibular Disorders Using Research Diagnostic Criteria for TMD (RDC/TMD)

  • Purpose : This epidemiological research was conducted to investigate the relationship between the groups of TMD and the behavioral, psychological, and physical symptoms through RDC/TMD. Subjects and Methods : The subjects of this research were the 286 patients who had visited Seoul National University Bundang Hospital; their common chief complaint was temporomandibular discomfort. The mean age of the patients was 32.9 from 11 to 85, and the number of men and women was 67 and 219, respectively. The patients were examined through clinical and radiological methods and diagnosed by 1 investigator. They were divided into 3 groups: myogenous group (group 1), arthrogenous group (group 2), and combined group (group 3). The behavioral, psychological, and physical symptoms were evaluated through questionnaires on RDC/TMD. Specific items were selected to calculate the graded chronic pain (characteristic pain intensity, disability points), jaw disability, depression, and non-specific physical symptoms (pain items included/excluded) in the questionnaire. One-way ANOVA, Kruskal-Wallis test, and chi-square test were applied as statistical methods. Results : As a result of classifying temporomandibular disorder in this study, the patients were distributed as follows: 9.1% of group 1, 79.7% of group 2, and 11.2% of group 3. In the analysis of graded chronic pain (characteristic pain intensity, disability points), jaw disability, and non-specific physical symptoms (pain items included/excluded), group 3 had the highest score, and the difference was significant (p<0.001). Moreover, the depression score of group 3 was significantly higher than groups 1 and 2 (p<0.05). Note that that the second order of jaw disability score was group 2, on the other hand, those of the other groups were group 1. Conclusion : Myofascial pain could be assumed to be related closely to the behavioral, psychological, and physical symptoms except jaw disability compared to joint pain through RDC/TMD.

Review of the Prevalence of Chronic Fatigue Worldwide

  • Objectives: To obtain the features of prevalence of chronic fatigue and chronic fatigue syndrome worldwide. Methods: All research reports for prevalence of chronic fatigue and chronic fatigue syndrome were selected from PubMed, KMbase and KISS database, and the prevalence were analyzed according to the symptoms and countries. Results: A total of 39 articles from 13 other countries and 4 articles from Korea met the criteria of this study. The prevalence of chronic fatigue and chronic fatigue syndrome was about 10% and 1% respectively. The prevalence for Korean was 17.7% and 1.4% respectively, however the data have a limitation due to the lack of well-designed studies. Conclusions: This report can provide essential information to build a strategy for development of therapeutics against chronic fatigue and chronic fatigue syndrome using traditional Korean medicine.

The Effect of Long-term Treatment with Clozapine on Cognitive Functions in Chronic Schizophrenic Patients (만성 정신분열증 환자의 인지기능에 미치는 Clozapine 장기치료의 효과)

  • It is not known whether negative symptoms and cognitive functions are dissociable or improvements in symptoms are reflected in improvements in cognitive functions in chronic schizophrenic patients. We administered clozapine to evaluate its effect on cognitive functions in chronic schizophrenic patients and to show correlations between improvement in psychotic symptoms and in cognitive functions. Neuropsychological tests such as Wisconsin Card Sorting Test, Digit Span test and Judgment of Line Orientation Test were applied to 16 chronic schizophrenic patients at baseline and after 9 months of treatment with clozapine. Using BPRS we assessed psychopathology before initiation of clozapine and at 9 months. Clozapine improved both positive and negative symptoms in chronic schizophrenic patients significantly. After nine months of clozapine treatment, significant improvements occurred in attention, short-term memory and visual perception ability. And interestingly we noted the trend of improvement in executive functions even though they were not statistical significant. Any significant correlations between the clinical improvement and change in congnitive functions were not observed. Long-term treatment with clozapine improved parts of cognitive functions of chronic schizophrenics. The results of the study suggest that deficits in simple cognitive functions as well as psychotic symptoms are improved after 3 month period of short-term treatment, but executive functions requiring more sophisticated processing of information could be improved after more than 9 months of long-term treatment.

Clinical study of Chronic Urticaria (만성두드러기에 대한 임상적 고찰)

  • Introduction Urticaria is widely spread disease. About 15-20 $\%$ of population has experienced at least once in a life time. The etiology of urticaria is uncertain till now, and the affcting factors are various chemical material, physical factors, alcohol, fever, exercise, and hormone, etc. The symptoms of urticaria are small or large wheal-erythema reaction and itching or tingling sense. Cause there is no probe to detect the urticaria clearly, the treatment of urticaria is symptomatic. And mostly urticaria can be chronic and very hard to treat it fundamentally. Nowadays, many trials to treat the urticaria in oriental medical way show good curability. In this paper I'd like to report the treatment rate and degree of urticaria patients mostly using anti-histamine medication. Subjects From the outpatients who visited Korean Hospital of Kyunghee Kangnam during 2000-4 and 2002-1, the 36 chronic urticaria patients, at least more than 4 weeks from onset and 2weeks of treatment period, were chosen. Methods I evaluated the results of treatment with the following scale. Very Good: The all symptoms are clearly disappeared or one third of symptoms remain and sometimes eruption is appeared. Good: Half of the symptoms are improved but most of symptoms remain. No Change: No change appears before and after treatment. Worse: The degree and duration of eruption get worse than pre-treatment state. Results For Acupuncture treatment, I chose the several Acu points like Hapgok(합곡), Taichung(대충), Gokji(곡지), Yanggok(양곡), Yanggea(양계), and lmeup(족임읍), and usually lasted it 15 minutes. With the acupuncture treatment and herb medicine, 2-3 times a week, the patients whose treatment period was 2 to 4 weeks were 17(47.2$\%$), 4 to 8 weeks were 11(30.5$\%$), 8 to 12 weeks were 3(8.3$\%$), 12 to 16 weeks were 3(8.3$\%$), longer than 16 weeks were 2(5.5$\%$) Collecting the statistics of the frequency of prescripted herb medicine, Hyangsosan(향소산) was prescripted 21 times(58.3$\%$), Hyangsapyungwisan(향사평위산) was 15 times(41.6$\%$), Hwapisan(화피산) was 9 times(25$\%$), Yangwitang(양위탕) was 6 times(16.6$\%$), Bojungikkitang(보중익기탕가미) was 4 times(11.1$\%$), Yongdamsagantang (용담사간탕가미) was 4 times(11.1$\%$). The result of the treatments, evaluating with mentioned rate scale, was 11 cases(30.5$\%$) were Very Good degree, 19 cases(52.7$\%$) were Good , 6 cases(16.6$\%$) were No Change. No cases were Worse degree. Conclusion According to this research, we could say that chronic urticaria can be treated with Oriental medical methods. But more precise probes in both Oriental and Western medicine to diagnose the chronic urticaria should be established and we need to make standards for testing and diagnosing the chronic urticaria.

Correlation of Anxiety or Depression Mood with HRV in Chronic Headache Patients (만성 두통 환자에서 불안·우울 정서와 HRV와의 상관성 연구)

  • Objectives: To investigate the correlation of anxiety or depression mood with physical stress of 59 participants (29 who complained of chronic headache and 30 who did not complain of headache) using a questionnaire on chronic headache symptoms, Beck Anxiety Inventory (BAI), Beck Depression Inventory (BDI), and Heart Rate Variability (HRV). Methods: In this study, medical records of 59 participants (29 who complained of chronic headache and 30 who did not complain of headache) who completed the BAI, BDI, HRV, and Questionnaire for chronic headache symptoms were evaluated. All data were analyzed using SPSS (Statistical Package for the Social Sciences, Version 26.0). Descriptive Analysis, Mann-Whitney U test, Chi square test (Fisher's exact test), and Pearson correlation coefficient were used for data analysis. Results: 1. The chronic headache group had significantly lower SDNN, RMSSD, TP, LF, and HF values on HRV than the general control group. 2. As BAI and BDI scores of the chronic headache group increased, the overall HRV value tended to decrease, indicating a deep association. 3. There was a negative association between BDI and HRV values in the chronic headache group, although such association was not statistically significant. Conclusions: BDI can be negatively correlated with HRV (SDNN, RMSSD, TP, LF, HF) in chronic headache patients.

Prevalence of Urinary Incontinence and Other Urologic Symptoms in a Community Residing Elderly People (일개지역 고령자의 요실금의 유병률, 지식 및 배뇨특성)

  • The purpose of this study was to measure the prevalence of urinary incontinence (UI), urologic symptoms, chronic health problems they have, and to explore whether the differences in incidence of UI ware by age, sex, voiding pattern, and chronic health problems. Method: 298 subject were selected, age range from 60 to 94 years residing in one city, in republic of Korea. Data was collected presence of UI, urologic symptoms, chronic health problems, knowledge, and the discomfort with incontinent. Collected data was analyzed with frequency, percentage, t-test, and $\chi$2-test. Result: The results of this study are as follows: 1. Mean age was 71.4 years. Prevalent rate of UI was 17.0%, woman showed more than man. 2. UI incidence was significant in age (t=7.84, p=.000), sex ($\chi$2 =9.47, p=.002), and voiding frequency ($\chi$2=18.34, p=.000). Also, UI incidence was significant relationship with chronic health problem of heart disease ($\chi$2 =10.65, p=0.001), hypertension ($\chi$2=4.04, p= 0.046) and respiratory problem ($\chi$2=9.67, p=0.002). 3. The UI was grouped into urgent incontinence (45%), stress incontinence (33%), and combined (22%). UI occurred during the daytime 48% and 17% at night. 4. Only 9.8 % of the UI seek advice and/or treatment for their symptoms, almost 90 % remained untreated due to lack of knowledge or improper information. 5. The discomforts due to their UI was no significant difference in their condition, the urgent use of the rest room, leaking urine, and nocturia. Conclusion: This study suggests that 1 year and 3 year follow-up study is needed to compare health status of UI. Also suggests intervention study for urologic discomfort of incontinent and behavioral education for the elderly are needed.

A Case of Chronic Allergic Contact Dermatitis Treated with Oryeong-san-Based Korean Medicine (오령산(五苓散) 위주의 한방 치료로 호전된 만성 알레르기성 접촉피부염 치험 1례)

  • Objectives : This study is to report a case of chronic allergic contact dermatitis, treated with Korean medicine, primarily focused on Oryeong-san. Methods : A 43-year-old female has suffered from dermatitis in axillary, lower abdominal, inguinal and popliteal area for more than 10 years. She was diagnosed with chronic allergic contact dermatitis accompanied by skin symptoms such as severe pruritus, erythema, skin pigmentation, papules and lichenification, and by decreased quality-of-life. She received Korean medicine treatment including herbal medicine, acupuncture and herbal external medication for 4 months. Results : The skin symptoms such as pruritus, erythema, skin pigmentation, papules and lichenification were remarkably improved. Also, the NRS of pruritus changed from 10 to under 1, and the DLQI score changed from 29 to 6. Conclusions : This case report suggests that Korean medicine, especially Oryeong-san may have a role in treating skin symptoms such as pruritus, erythema, skin pigmentation, papules and lichenification in the patients diagnosed with chronic allergic contact dermatitis. Also, the improvement of skin symptoms treated with Korean medicine may result in quality-of-life improvement in those patients.

Daily Functioning in Chronic Pain: Study of Structural Relations with Posttraumatic Stress Disorder Symptoms, Pain Intensity, and Pain Avoidance

  • Background: This study aimed to evaluate processes from the mutual maintenance model in relation to daily functioning in patients with both chronic pain and a history of a traumatic experience. The mechanism illustrated the structural relations for daily functioning among pain intensity, hyperarousal, re-experiencing, trauma avoidance, and pain avoidance. Methods: Archival data (N = 214) was used for this study and data were analyzed for 142 chronic pain patients reporting a traumatic experience and seeking treatment at a tertiary pain clinic in Korea. Results: The results indicated that pain intensity, hyperarousal, and pain avoidance had significant direct effects on daily functioning. Also, pain intensity showed significant indirect effects on daily functioning through hyperarousal and pain avoidance; and hyperarousal through pain avoidance. Conclusions: Results suggest a direct contribution of high levels of pain, hyperarousal symptoms of PTSD, and pain avoidance behaviors to reduced daily functioning. Also, elevated pain as reminders of the trauma may trigger high levels of hyperarousal symptoms of PTSD. Subsequently, avoidant coping strategies may be used to minimize pain so that the trauma would not be re-experienced, thus inhibiting the activation of hyperarousal symptoms of PTSD. However, prolonged use of such strategies may contribute to decline in daily functioning.

A Case Report of Atopic Dermatitis with Otitis Media with Effusion and Chronic Rhinitis (삼출성 중이염과 만성 비염을 동반한 아토피 피부염 1례)

  • Objectives: This study aims to report a case of atopic dermatitis with otitis media with effusion and chronic rhinitis improved by oral administration of Galgeungabanha-tang. Methods: The patient of this case had symptoms such as erythema, abrasions, scales, and itching. She also complained of rhinorrhea, nasal obstruction, cough, sputum, and ear fullness. Considering the fact that she was sensitive to heat, but did not sweat well, and respiratory symptoms often occurred, Galgeungabanha-tang was prescribed. Results: After medication, the symptoms of atopic dermatitis significantly improved from 10 to 2 point. The subjective symptoms of chronic rhinitis and otitis media with effusion also improved considerably. In addition, nasal swelling and purulent exudate almost disappeared. The tympanic membrane, which was swollen due to amber exudate, also changed to a normal gray-white color. Conclusions: Atopic dermatitis accompanied by chronic rhinitis and otitis media was improved after 45 days of medication.
