  • Chronic obstructive pulmonary disease (COPD) is characterized by a diverse array of pulmonary and nonpulmonary manifestations, but our understanding of COPD pathogenesis and the factors that influence its heterogeneity in disease presentation is poor. Despite this heterogeneity, treatment algorithms are primarily driven by a single measurement, forced expiratory volume in 1 second ($FEV_1$) as a percentage of its predicted value ($FEV_1%$). In 2011, a major shift in Global Initiative for Chronic Obstructive Lung Disease (GOLD) treatment recommendations was proposed that stratifies patients with COPD on the basis of symptoms and exacerbation history. This article reviews the work reported in 2013 that enlightens our understanding of COPD with respect to COPD classification systems, phenotype, biomarker, exacerbation, and management for patients with COPD.

Methods: 121 patients (75 male and 46 female) who visited a fatigue-care center were enrolled in this study, and the main complaints besides fatigue feeling, symptom differentiation, and health-related factors were analyzed. Results: On a visual analogue scale (VAS), average severity of fatigue was 7.0 (male 6.77 and female 7.4; 0 indicates no fatigue vs. 10 indicates intolerable fatigue), and the main complaints included headache (male and female) and dyspepsia (female). Patients generally slept about 7 hrs and caught a cold 2.4 times per year (same in both male and female). 54.6 % of patients used functional supplements, and red ginseng, vitamin and herbal medicine were chosen in that order. Conclusions: Chronic fatigue is a main target of traditional Korean medicine (TKM). This study can be helpful to design TKM-based treatment for chronic fatigue-related symptoms.

  • Acute ankle sprain is the most common injury in the lower extremities, and approximately 10% to 40% of acute lateral ankle ligament injury causes chronic pain or instability. For chronic symptoms lasting after an acute sprain, the possibility of joint damage, such as bony structures, ligaments, cartilage, and nerves around the ankle joint, should be considered. Patients with chronic lateral ankle instability usually complain of repeated sprains or giving way sensations. There has been steady progress in the treatment options until recently, however new treatments are still being attempted. This paper describes the causes, diagnosis, and recent trends in the conservative and operative treatment of chronic lateral ankle instability.

  • Chronic lateral ankle instability occurs in 10% to 20% of individuals after acute ankle sprain. The management of chronic lateral ankle instability is traditionally conservative treatment in the acute phase. On the other hand, surgical intervention is considered if conservative treatment fails and the symptoms are ongoing. This review focuses on the surgical approaches to treatment of chronic lateral ankle instability, including $Brostr{\ddot{o}}m$ surgical techniques, with a review of the traditional procedure and newer techniques.
