Chronic Obstructive Pulmonary Disease Combined with Interstitial Lung Disease

  • Although chronic obstructive pulmonary disease (COPD) and interstitial lung disease (ILD) have distinct clinical features, both diseases may coexist in a patient because they share similar risk factors such as smoking, male sex, and old age. Patients with both emphysema in upper lung fields and diffuse ILD are diagnosed with combined pulmonary fibrosis and emphysema (CPFE), which causes substantial clinical deterioration. Patients with CPFE have higher mortality compared with patients who have COPD alone, but results have been inconclusive compared with patients who have idiopathic pulmonary fibrosis (IPF). Poor prognostic factors for CPFE include exacerbation, lung cancer, and pulmonary hypertension. The presence of interstitial lung abnormalities, which may be an early or mild form of ILD, is notable among patients with COPD, and is associated with poor prognosis. Various theories have been proposed regarding the pathophysiology of CPFE. Biomarker analyses have implied that this pathophysiology may be more closely associated with IPF development, rather than COPD or emphysema. Patients with CPFE should be advised to quit smoking and undergo routine lung function tests, and pulmonary rehabilitation may be helpful. Various pharmacologic agents and surgical approaches may be beneficial in patients with CPFE, but further studies are needed.

The Impact of Air Pollution, Including Asian Sand Dust, on Respiratory Symptoms and Health-related Quality of Life in Outpatients With Chronic Respiratory Disease in Korea: A Panel Study

  • Objectives: Air pollution is a growing concern in Korea because of transboundary air pollution from mainland China. A panel study was conducted to clarify the effects of air pollution on respiratory symptoms and health-related quality of life (HR-QoL) in outpatients with and without chronic obstructive pulmonary disease (COPD) in Korea. Methods: Patients filled out a questionnaire including self-reported HR-QoL in February and were followed up in May and July. The study was conducted from 2013 to 2015, with different participants each year. Air quality parameters were applied in a generalized estimating equation as independent variables to predict factors affecting HR-QoL. Results: Lower physical fitness scores were associated with Asian sand dust events. Daily activity scores were worse when there were high concentrations of particulate matter (PM) less than $10{\mu}m$ in diameter ($PM_{10}$). Lower social functioning scores were associated with high PM less than $2.5{\mu}m$ in diameter and nitrogen dioxide ($NO_2$) concentrations. High $NO_2$ concentrations also showed a significant association with mental health scores. Weather-related cough was prevalent when $PM_{10}$, $NO_2$, or ozone ($O_3$) concentrations were high, regardless of COPD severity. High $PM_{10}$ concentrations were associated with worsened wheezing, particularly in COPD patients. Conclusions: The results suggest that PM, $NO_2$, and $O_3$ cause respiratory symptoms leading to HR-QoL deterioration. While some adverse effects of air pollution appeared to occur regardless of COPD, others occurred more often and more intensely in COPD patients. The public sector, therefore, needs to consider tailoring air pollution countermeasures to people with different conditions to minimize adverse health effects.

Sputum Inflammometry to Manage Chronic Obstructive Pulmonary Disease Exacerbations: Beyond Guidelines

  • Quantitative sputum cytometry facilitates in assessing the nature of bronchitis associated with exacerbations of chronic obstructive pulmonary disease (COPD). This is not assessed in most clinical trials that evaluate the effectiveness of strategies to prevent or to treat exacerbations. While up to a quarter of exacerbations may be associated with raised eosinophil numbers, the vast majority of exacerbations are associated with neutrophilic bronchitis that may indicate airway infections. While eosinophilia may be a predictor of response to corticosteroids (oral and inhaled), the limited efficacy of anti-interleukin 5 therapies would suggest that eosinophils may not directly contribute to those exacerbations. However, they may contribute to airspace enlargement in patients with COPD through various mechanisms involving the interleukin 13 and matrix metalloprotease pathways. The absence of eosinophils may facilitate in limiting the unnecessary use of corticosteroids. The presence of neutrophiia could prompt an investigation for the specific pathogens in the airway. Additionally, sputum measurements may also provide insight into the mechanisms of susceptibility to airway infections. Iron within sputum macrophages, identified by hemosiderin staining (and by more direct quantification) may impair macrophage functions while the low levels of immunoglobulins in sputum may also contribute to airway infections. The assessment of sputum at the time of exacerbations thus would facilitate in customizing treatment and treat current exacerbations and reduce future risk of exacerbations.

Review of a Case of Chronic Obstructive Pulmonary Disease in Workers Exposed to Synthetic Fibers

  • Objectives: Objectives of this study were: 1) to introduce industrial situation and health hazards of synthetic fiber, 2) to review a case of chronic obstructive pulmonary disease in a worker exposed to synthetic fiber reported to the Korea Occupational Disease Surveillance Center, and 3) to suggest supplementary measures for the occupational health system for workers exposed to synthetic fibers. Methods: Respiratory exposure, health hazards, and exposure standards for synthetic fiber dust in Korea and other countries were reviewed. In addition, a case of chronic obstructive pulmonary disease due to exposure to nylon dust reported to the Korea Occupational Disease Surveillance Center was reviewed and summarized. Results: The worker was a 53-year-old non-smoking male who had been involved in the nylon weaving process for 26 years. He had shortness of breath from three years ago. He was diagnosed with chronic obstructive pulmonary disease. PM1.0, PM2.5, and PM10 were measured at 26.6 ㎍/m3, 48.2 ㎍/m3, and 91.7 ㎍/m3, respectively. Fiber components estimated as nylon fiber were detected in the microscopic examination of a solid sample. Conclusions: For workers exposed to synthetic fiber dust, special health examinations of the respiratory system, regular work environment measurement, and work environment management through workplace health management should be performed. It is necessary to research on health effects of synthetic fibers.

의료보험 통계자료를 이용한 최근 우리나라 질병구조 변화관찰 - 의료보험관리공단 자료를 중심으로 - (The study for recent changes of disease-mix in health insurance data)

  • Accumulated data on medical care utilization among the insured in Korea Medical Insurance Corporation can explain the health status of the population. The purpose of this study was to analyze a change of the disease-mix and utilization pattern by controlling the size of the population enrollment. Major findings of the study are as follows : 1. The changes of inpatient disease-mix a. Utilization rate was 139.2% in 1988 against 1980. b. Disease groups higher than the average utilization rate included neoplasms, endocrine, nutritional and metabolic diseases and immunity disorders, mental disorders etc. Meanwhile, disease groups seen less often were infections and parasistic diseases, diseases of blood and bloodforming, diseases of the digestive system etc. c. Utilization rate was up 106.3% in 1988 compared to 1985, and diseases above that average level were ill-defined intestinal infections, chronic liver disease and cirrhosis, diabetes mellitus, essential hypertension, etc. d. The disease-mix by institution in 1988 compared to 1985 shows that chronic disorders rank high in general hospitals whereas opthalmologic, obstetric, and orthopedic diseases rank high in private clinics. 2. The changes of outpatient disease-mix a. Utilization rate was up 175.2% in 1988 compared to 1980. b. Disease groups higher than the average utilization rate included neoplasms, endocrine, nutritional and metabolic diseases and immunity disorders, mental disorders etc. And disease groups seen less often were infections and parasistic diseases, diseases of the respiratory system, diseases of the genitourinary system. etc. c. Utilization rate was up 104.0% in 1988 compared to 1985, and diseases above that average level were gastric ulcer, diseases of hard tissues of teeth, etc. And diseases seen below that average level were acute nasopharyngitis(common cold). acute upper respiratory infections of multiple or unspecified sites, etc. It was concluded that medical care utilization level was increased, and that, from 1980 to 1988, disease-mix shifted to the chronic disorders. Chronic disorders accounted for more medical care utilization in general hospitals.

만성 폐쇄성 폐질환 평가 테스트의 유용성 (The Usefulness of the Chronic Obstructive Pulmonary Disease Assessment Test)

  • Background: A chronic obstructive pulmonary disease (COPD) assessment test (CAT) has recently been developed as a short and simple method for assessing the quality of life in COPD patients. The object of this study was to assess the usefulness of the Korean version of the CAT for assessing COPD patients in an outpatient clinic. Methods: The study included 60 COPD patients in a stable state from an outpatient clinic. The authors investigated the frequency of acute exacerbation during aprevious year through reviewing medical records. We evaluated the spirometry test, a 6-min walk distance test, and obtained the MMRC dyspnea scale, the Korean version of the CAT, and the BODE index at the time of visit. To assess the usefulness of the CAT, correlations between the CAT and other methods were evaluated. Results: The mean age of patients was $68.3{\pm}8.6$ years and 95% of patients were male. There was a significant correlation between the CAT score and $FEV_1%$ (r=-0.323, p=0.012), the frequency of acute exacerbation (r=0.292, p=0.024), the MMRC dyspnea scale (r=0.554, p<0.001), the BODE index (r=0.380, p=0.003), and 6 MWD (r=-0.372, p=0.004). The mean CAT score increased according to the GOLD stage (stage 1, $10.7{\pm}4.5$; stage 2, $13.1{\pm}7.9$; stage 3, $16.3{\pm}6.2$; stage 4, $16.5{\pm}14.8$; p=0.746). Conclusion: The CAT was shown to be useful for the assessment of COPD severity. Therefore, the CAT is an easily applied and simple method for assessing COPD severity in an outpatient clinic.

Different Pattern of Chronic Obstructive Pulmonary Disease Assessment Test Score between Chronic Bronchitis and Non-chronic Bronchitis Patients

  • Background: Chronic bronchitis (CB) is an important phenotype in chronic obstructive pulmonary disease (COPD). The purpose of this study is to evaluate different pattern of COPD assessment test (CAT) score between CB and non-CB patients. Methods: Patients were recruited from 45 centers in Korea, as part of the Korean COPD Subgroup Study cohort. CB was defined when sputum continued for at least 3 months. Results: Total 958 patients with COPD were eligible for analysis. Among enrolled patients, 328 (34.2%) were compatible with CB. The CAT score was significantly higher in patients with CB than non-CB, and each component of CAT score showed a similar result. CB was significantly associated with CAT score when adjusted with age, sex, modified Medical Research Council, and post-bronchodilator forced expiratory volume in 1 second. Each component of CAT score between patients with CB and non-CB showed different pattern according to Global Initiative for Chronic Obstructive Lung Disease grade. Conclusion: CAT score is significantly higher in patients with CB than non-CB. Each component of CAT score was significantly different between two groups.

한국어판 세인트조지 호흡기설문의 타당도와 신뢰도 검정 (Validation of the Korean Version of the St. George's Respiratory Questionnaire for Patients with Chronic Respiratory Disease)

  • 연구배경: 만성폐질환 환자에서 삶의 질을 측정하는 도구로서 세인트조지 호흡기 설문(St George's Respiratory Questionnaire;SGRQ)이 개발되어 치료효과를 판정하는 측정지표로 널리 사용되고 있으나 영어권국가에서 만들어 졌기 때문에 우리나라에서 동일하게 사용하는데 제한점을 가지고 있다. 따라서 이의 정확한 적용을 위해서 언어, 기술, 개념적인 관점을 고려하여 한국어 설문지를 만들고 이들의 타당도와 신뢰도를 평가하였다. 방 법: 한국판 세인트조지 호흡기 설문은 번역, 역번역, 전문가 패널의 수정을 통해 완성되었고, 세브란스병원 호흡기내과를 방문한 환자를 대상으로 하여 이 설문을 시행하였다. 신뢰도 검정은 Cronbach's $\alpha$를 구하여 측정하였고 타당도는 폐기능검사, 6분보행검사, 호흡곤란지수와 산소포화도 검사를 동시에 시행하여 설문 검사결과와의 상관관계를 알아보았다. 결 과: 74명의 환자를 대상으로 시행하였으며, 남녀 비는 2.2:1, 평균연령은 $63.8{\pm}10.3$세였다. 이 중 천식환자는 27명(36.5%), 만성폐쇄성폐질환 환자는 39명 (52.7%)이었고, 결핵에 의한 파괴 폐환자는 8명(10.8%)이었다. Cronbach's ${\alpha}$ 값은 증상영역, 활동영역, 영향영역 및 총 점수에서 각각 0.63, 0.87, 0.89, 0.92 이였다. 총점수와 $FEV_1$, FVC, 6분보행거리, 산소포화도, 호흡곤란지수와 유의한 상관관계를 보였다. 결 론: 한국어판 세인트조지 호흡기 설문은 만성폐질환 및 천식과 같은 만성폐질환 환자에서 신뢰도가 높았고 총점수와의 상관관계는 6분보행검사 및 산소포화도에서 높음을 알수 있었고 폐기능 검사와의 상관계수는 원저인 세인트조지 호흡기설문 및 타 국가의 번역본 호흡기설문에서와 같이 높지 않음을 알 수 있었다. 따라서 우리나라에서도 호흡기질환의 삶의 질에 대한 치료효과 판정에 활용할 수 있을 것으로 기대된다.
