A 7.1 kg, seven-year old, castrated male, Shih-Tzu with severe pruritus, chronic otitis externa and Malassezia infection was referred to Veterinary Medical Teaching Hospital of Chungnam National University. In local animal hospital, steroid therapy was used to treat uncontrollable pruritus, but the clinical signs were recurrent when steroid therapy was discontinued. On physical examination, generalized alopecia, erythema, papules, severe crust and diffuse lichenification were presented. Tape strip test of skin lesions revealed cocci and Malassezia infections. Based on the result of history, clinical signs and examination described above, canine atopic dermatitis with secondary superficial pyoderma and Malassezia dermatitis was diagnosed. Oral challenge with cyclosporine and antibiotics had good results in clinical signs. Clinical sign scores were evaluated by investigator with CADESI at 2weeks, 4weeks, 6weeks, 8weeks and 10weeks after cyclosporine administration. And in the result of comparing of allergen-specific IgE value, the level of allergen-specific IgE to general causative allergen after 10 weeks of cyclosporine therapy was higher than that before cyclosporine therapy.

Skull base osteomyelitis is a very rare disease that affects the bone marrow of the temporal bone, sphenoid bone, occipital bone. It occurs typically chronic ear canal infections by malignant otitis externa, but some of atypical osteomyelitis have been reported. It most commonly presents old diabetic patients, and have high morbidity and mortality rate if diagnosis and treatment are delayed. However with respect to pain or dysfunction, it appeared similar to the initial symptoms of temporomandibular joint disorder. So frequently, definitive diagnosis is tend to delayed. We have clinical experience that a patient who presented with symptom similar to temporomandibular disorder, and differential diagnosised by skull base osteomyelitis. We will report this case with literature review.

A 13-year-old spayed female American cocker spaniel dog was submitted to an animal hospital with a mass from left ear canal and enlarged left parotid lymph node. Given one-year history of odorous exudates and chronic otitis externa, total ear canal ablation was performed. Grossly, because of the neoplastic mass in both inner and outside of annular cartilage in external ear, vertical ear canal was severely narrowed. Histologically, there were numerous proliferated glands in the ear canal mass. Many neoplastic glands contained secretory vesicles on the free margin and necrotic cellular debris. Severe multifocal necrosis and strong invasion were also observed throughout the mass. Massive metastatic foci of glandular structures originated from ceruminous gland were presented in the enlarged parotid lymph node. Neoplastic glandular epithelia contained PAS-positive diastase-resistant eosinophilic cytoplasmic granules. Immunohistochemically, the neoplastic cells showed positive reactions for cytokeratin (CK) 7 and negative for CK 5/6. Based on the clinical implication and gross findings, histopathology and immunohistochemistry, this case was diagnosed as metastatic ceruminous gland adenocarcinoma in the American cocker spaniel.
