Effect of aquatic exercise on gait in persons with chronic stroke: a meta-analysis study in Korea

  • Objective: Based on the results of previous studies, it is necessary to analyze gait and discuss and present the effects of aquatic exercise for chronic stroke. The purpose of this study was to present objective data on the effect of aquatic exercise on the gait of persons with stroke by performing a meta-analysis. Design: A systematic review and meta-analysis. Methods: We performed a meta-analysis of 23 studies that investigated the effects of aquatic exercise performed between 2006 and 2017. The studies were searched on the basis of the participants, intervention, comparison, outcomes standard. The quality of the research method was assessed using a tool that can assess the risks posed by each study design. A meta-analysis software program was used to calculate the mean effect size, effect size by intervention, and effect size by outcome. We also performed a meta-regression analysis and an analysis of publication bias. Results: The mean effect size of the patients' gait was 0.65 (p<0.05). The largest effect size by outcome was observed at the 6-m walk test, followed by the 6-minutes walk test, 10-m walk test, and the walking equipment test (p<0.05). The meta-regression analysis showed that the effect size increased with increased duration, number, and length of sessions. Conclusions: Aquatic exercise appears to show a moderate effect on the gait of chronic stroke survivors. Meta-analyses on the effects of aquatic exercise in other patient populations are needed. This study suggests standard criteria establishments for the effect of aquatic exercise on the walking ability of persons with chronic stroke.

Study on the Effects of Tetrax®-based Combined Rehabilitation Exercise on Chronic Back Pain Cases

  • The purpose of this research is to utilizing the Tetrax$^{(R)}$ balance measuring instrument in order to analyze the postural balance of males and females in their 30 s diagnosed with chronic lower back pain who have followed a 12-week rehabilitation exercise program. The research also examines the effects on any change in back pain level. In terms of the variables in this research, postural balance (left/right, front/back, postural balance) and pain level change (0~100 mm) were measured. Pre-/post-experimental differences were assessed using the paired-t test. In addition, to identify any gender gap, we set the preliminary scores as a covariate and ran the Analysis of Covariance. Statistical significance (a) herein was set at 0.05. As a result of this research experiment, the left/right, front/back, and overall postural balance were found to increase in both the male and female cases, but with no statistical significance or gender gap. However, both males and females showed a significant decrease in their back pain levels. These findings demonstrate the necessity of continuing clinical research based on the Tetrax$^{(R)}$ equipment for scientific evaluation of the effects of rehabilitation exercises on chronic lower back pain patients and their balancing ability.

만성 폐색성 폐질환자를 위한 호흡재활 프로그램 개발 및 효과에 관한 연구 (The Study on the Effects of a Respiratory Rehabilitation Program for COPD Patients)

  • It is known that a pulmonary rehabilitation program improves dyspnea and exercise tolerence in patients with chronic obstructive pulmonary disease. However, it is also known that although it does not improve pulmonary function. This study was performed to evaluate the effect of a 4 week pulmonary rehabilitation on pulmonary function, gas exchange, and exercise tolerance in patients with chronic obstructive pulmonary disease. The pulmonary rehabilitation programs included breathing exercises, such as pursed-lip breathing and diaphragmatic breathing, upper-limb exercises, and inspiratory muscle training. These activities were performed for 4 weeks in twenty one patients with chronic obstructive pulmonary disease. Pre and post-rehabilitation pulmonary function and exercise capacities were compared after the 4 week period. Results are as follows: 1) Before the rehabilitation, the predicted value of FVC and FEV1 of the patients were 70.3$\pm$16.7% and 41.1$\pm$11.9% respectively. These pulmonary functions did not change after pulmonary rehabilitation. 2) Aloility of walking a 6 minute distance (325.29$\pm$122.24 vs 363.03$\pm$120.01 p=.01) and dyspnea (p=.00) were significantly improved after rehabilitation. Thus showing that pulmonary rehabilitation for 4 weeks can improve exercise performance and dyspnea in patients with chronic obstructive pulmonary disease.

Effects of Whole Body Vibration Exercise on Lower Extremity Muscle Activity and Balance Ability in Football Player with Chronic Ankle Instability

  • Purpose: The purpose of this study was to determine the effects of whole body vibration exercise (WBVE) on lower extremity muscle activity and balance ability according to different methods of exercises in football player and use it as basic data for the rehabilitation training of chronic ankle instability. Methods: Thirty subjects were randomly divided into two groups: the two groups, which each group have 15 members, are WBVE group and neuromuscular training (NMT) group according to training method. The exercise program was conducted for six weeks. Subjects were measured on lower extremity muscle activity and balance ability. Results: The muscle activity increasement of the WBVE group was significantly higher than that of the NMT group (p<0.05) and the balance ability decreasement of the WBVE group was significantly higher than that of the NMT group (p<0.05). Conclusion: These findings of this study suggest that WBVE may have a beneficial effect on improvement of lower extremity muscle activity and balance ability in football player with chronic ankle instability.

Rh2-enriched Korean Ginseng Ameliorates Chronic Fatigue in a Forced Exercise mouse model

  • Objectives: We evaluated the anti-fatigue effects of Rh2-enriched Korean ginseng (Ginseng Rh2+) using a forced exercise-induced chronic fatigue mouse model. Methods: ICR male mice were subjected to running wheel for 1 h, 5 days/week during 4 weeks, and running velocity was gradually increased. Each running session was followed by oral administration of distilled water, Ginseng Rh2+ (150 or 300 mg/kg), or N-acetyl-L-cysteine (NAC, 100 mg/kg) 1 h later. The exercise tolerance and forced swimming test were performed to evaluate the fatigue condition. Results: Chronic forced exercise reduced the physical activity, as evidenced by the behavioral tests, which were notably ameliorated by Ginseng Rh2+ treatment. Ginseng Rh2+ treatment also attenuated the alterations of energy metabolism and oxidative stress in skeletal muscle tissues and/or sera, including malondialdehyde (MDA), lactate concentration and its related factors (lactate dehydrogenase, blood urea nitrogen, and glucose levels). Conclusion: These findings strongly suggest that Ginseng Rh2+ exerts a potent anti-fatigue effect through modulation of energy metabolism and oxidative response.

Effect of a Five-week Scapular Correction Exercise in Patients with Chronic Mechanical Neck Pain

  • Purpose: The purpose of this study was to compare the change in pain, Neck Disability Index score, and the craniovertebral angle by performing scapular correction exercise or general neck exercise for five weeks in participants with mechanical neck pain. Methods: A total of 31 participants were randomly assigned between the scapular correction exercise and the general neck exercise groups, and all participants performed intervention for 40 minutes each, three times a week for five weeks The effects were evaluated by measuring the Visual Analog Scale score, the Neck Disability Index score, and the craniovertebral angle, before and after the intervention. Independent t-tests were used to compare differences between two groups, and to compare differences between pre- and post-intervention, paired t-tests were used. Results: As measured before and after the intervention, the scapular correction exercise group showed significant improvement in all variables (p<0.05), while the general neck exercise group improved only in the neck disability index score. The differences between the two groups revealed further improvement in the scapular correction exercise group compared to the general neck exercise group (p<0.05). Conclusion: We found that five weeks of the scapular correction exercise to modify the position and movements of the scapula is clinically an important treatment tool for recovery from chronic mechanical neck pain symptoms and restoration of proper neck function.

  • Purpose: This research was conducted to compare the effects of drug therapy, physical therapy, and exercise on pain, disability, and depression in patients with chronic low back pain. Methods: The research design of this study was a nonequivalent control group pretest-posttest design. The subjects of this study were 28 patients for the drug therapy & physical therapy, 24 patients for the drug therapy & exercise, and 22 patients for the physical therapy & exercise. Data was collected by MVAS, Oswestry disability questionnaires, and questionnaires of depression. It was analyzed by paired t-test for effectiveness, ANOVA, and Scheffe for comparison of the effects of the 3 experimental treatments, using SPSS/WIN 12.0. Results: There were no effects of drug therapy & physical therapy on pain, disability, and depression. However, there were effects of drug therapy & exercise and the physical therapy & exercise on pain, disability, and depression. The effects of physical therapy & exercise on pain, disability, and depression were the greatest, but there was no statistically significant differences between the drug therapy & exercise and the physical therapy & exercise. Conclusions: Exercise is regarded as a more effective and easily accessible nursing intervention to apply alone than drug therapy or physical therapy simultaneously in reducing pain, disability and depression.

만성 경부통을 가진 시내버스 운전기사에게 복합 운동프로그램의 효과 : 사례연구 (The Effects of Combined Exercise Program to Intra-City Bus Driver With Chronic Neck Pain : Case Report)

  • Background: Although many of those who drive for a living are exposed to repetitive and awkward posture and whole body vibration which cause them cumulative trauma disorder, therapeutic studies related to the problems from the transport unit are still somewhat inadequate. The purpose of this case report is to identify the effect of combined exercise program on patients suffering chronic neck pain caused by long hour of intra-city bus driving. Method: A total of 4 subjects are selected as patients, all of whom have more than 10 years of experience in intra-city bus transportation services. These people have suffered from neck pain for years. We ran the combined exercise program 3 times a week for 4 weeks and respectively evaluated the results after the 2nd and 4th week. We implemented conventional physiotherapy for 40 minutes, another 40 minutes of combined exercise program, and then educated the patients to enable themselves to do active stretching program as a home program. The combined exercise program contained 3 different stages. 1st stage: active stretching program, 2nd stage combined stabilization exercise and strengthening exercise, 3rd stage: proprioceptive exercise Result: After 4 weeks of intervention, there was enhance in the range of motion. $12^{\circ}$ increase in flexion, $10^{\circ}$ in extension, $6^{\circ}$ in lateral flexion respectively, and $10^{\circ}$ in rotation on average. VAS(visual analgue scale) decreased by 33% on average, NDI(neck diability index) by 28% and fatigability by 23%. Conclusion: There were improved results in the range of motion, NDI, VAS, and in fatigability after applying combined exercise program to intra-city bus drivers exposed to whole body vibration and cumulative trauma disorder.

Effects of Farinelli Breating Exercise on Respiratory Function and Symptoms in Patients with Chronic Obstructive Pulmonary Disease

  • Background: Farinelli breathing (FB) exercise is a typical breathing exercise used by singers. This study aimed to compare effects of FB exercise and diaphragmatic breathing (DB) exercise on respiratory function and symptoms in patients with chronic obstructive pulmonary disease (COPD). Methods: Sixteen patients aged 51-80 years with mild or moderate COPD were recruited for this study. They were divided into two groups: DB group (n=8) and FB group (n=8). Both groups received complete breathing exercise training five times per week for 8 weeks. Their respiratory functions, COPD symptoms, cytokine levels, and oxidative stress variables were analyzed during pre- and post-tests. Dependent variables were compared between pre- and post-tests using paired t-tests. An independent t-test was used to compare variables between the groups. Differences were considered significant at p<0.05. Results: The maximal expiratory pressure (MEP), maximum oxygen consumption (VO2max), and COPD Assessment Test (CAT) scores changed significantly in the DB group after the 8-week intervention compared to those at pre-test, whereas force vital capacity, forced expiratory volume in the first second, maximum voluntary ventilation, maximal inspiratory pressure (MIP), MEP, VO2max, CAT score, tumor necrosis factor-α, and malondialdehyde level changed significantly in the FB group at post-test compared to those at pre-test. Moreover, both MIP and MEP in the FB group were significantly higher than those in the DB group. Conclusion: FB exercise improved respiratory functions and COPD symptoms of patients with COPD. It might be an alternative breathing exercise in pulmonary rehabilitation programs for patients with COPD.

Effect of Hamstring Eccentric Exercise on Pain and Functional Activity in Patients with Chronic Low Back Pain

  • Objective: The aim of this study is to investigate the effect of hamstring eccentric exercise on pain and functional activity in patients with chronic low back pain. Design: Randomized controlled trial Methods: Participants comprised 27 subjects with back pain persisting for 3 months. They were randomly assigned to the experimental group (Eccentric exercise: n=13) and the control group (Stretching: n=14). The experimental group performed gastrocnemius, iliopsoas, and quadratus lumborum stretching along with hamstring eccentric exercise, while the control group engaged in the same stretches along with hamstring stretching. The eccentric exercise for the hamstrings was Nordic curl exercise. Each group practiced for 1 hour a day, 3 times a week, for 4 weeks. Pain, disability index, balance, and hamstring length were measured. Results: Significant differences were observed in the comparison of changes in back pain, back disability index, and hamstring length before and after exercise within each group (p<0.05). The balance ability of the experimental group showed a difference before and after exercise (p<0.05), whereas no significant difference in the control group. There were no significant differences in back pain and hamstring length between the groups. Changes in back disability index and balance ability significantly increased in the experimental group compared to the control group (p<0.05). Conclusion: Hamstring eccentric exercise using Nordic curls has a positive effect on back pain, back disability index, balance ability, and hamstring length changes.
