Provisional National Nutrition Targets and Strategies for Health Promotion of Koreans (국민건강증진을 위한 영양 부문의 2000년 잠정목표와 전략)

  • This study attempts to propose a priority of national nutrition targets and strategies for health promotion by the year 2000 in Korea, as a part of the task set for national health promotion objectives and strategies. Among all of the important health issues raised, ten were chosen, nutrition was one priority area. In the first part, the current status of the nutrition-related health problems and risk factors are reviewed, in conjunction with the newly arisen health phenomena, such as changes in prevalence of lifestyle disease and causes of death, changes of food consumption patterns in our country. In the second section this study suggests six feasible national nutrition targets, eight implementing strategies and current major tasks on the basis of the assessment of present status and in consideration of the other health promotion goals and strategies, with reference to that of other developed countries. The main targets and strategies are suggested as follows ; Firstly, the national nutrition monitoring and surveillance system should be established for identifying the nutritional problems for our people, and current National Nutrition Survey is a strong need for improvement to a more comprehensive and reliable one. Secondly, effective administrative mechanism should be operation at national level for the development of nutrition policy. Ministry of Health and Welfare (MOHW) as well as local health department must be remarkably renewed and strengthened the nutrition section. And it is recommended that MOHW organize and operate “The Council of Nutrition”, in which all government authorities related with foodstuffs and nutrition would incorporated. The Council of Nutrition would act as an adjustor as well as a coordinator in nutrition related policy-making. Thirdly, healthy eating pattern will be supported by activities of introducing a nutrition labeling for providing consumers with the necessary information and skills for food selection. Fourthly, nutrition education, and nutrition intervention programs will be carried out in various settings such as health centers, schools, and clinical fields and workplace. Fifthly, the current dietary guidelines shall be continuously improved in detail, and publicly circulated to particular levels of people by age group and by health condition. And finally, researches and epidemiological studies particularly in regard to diet for development of chronic diseases are needed for more investigation and up-to-date national health and nutrition data should be collected with the support and cooperation from the various medical professional teams . A Survey of the Nutrients and a Price Comparison of Korean Style Full Course Meals - Based on Korean Restaurants in Changwon City and in Luxurious Hotels in Seoul - (한정식업소에서 제공되는 1인분 영양소와 가격 비교조사 - 서울시 특급호텔과 창원시를 중심으로 -)

  • The objectives of this study were to conduct preliminary research to investigate full course meals in Korean style restaurants in order: i) to analyze the nutrient contents of full course meals per servings, and ii ) by analyzing the above, to examine the amount and the quality of the foods served in the full course meals by two different types of Korean style restaurants. A total of 27 restaurants participated in this study, 7 restaurants from the luxurious hotels in Seoul and 20 restaurants from the City of Changwon. A key finding was that restaurant patrons tended to overconsume nutrients as compared to the Korean Recommended Daily Allowance (RDA), and restaurants generated large amounts of food wastes due to the excess food served. Other findings were as follows: 1) Compared with 1/3 or the Korean RDA, all the nutrients were oversupplied. The average nutrient ratios were about 3.7 times higher than 113 of the Korean RDA in calories, about 9 times higher in proteins, 10 times higher in phosphorus, 7 times higher in Vitamin Bl, and 12 times higher in Vitamin E. 2) Seventy-five percent (n = 20) of the restaurants served within the range of thirty to forty dishes, whereas most of the hotel restaurants (70%) served twenty dishes or fewer. 3) The average carbohydrate: protein: fat (CPF) ratio of caloric nutrients was 40:26 : 34. This study concluded that: i) Korean style full course meals provide too much food, nutrients, and calories, resulting in an over- or unbalanced nutrient intake, and ii ) the Korean style full course meals consisted of a high-protein, high-fat and high-caloric intake, which is similar to a westernized caloric nutrient pattern. Such over -or unbalanced nutrient intake could cause chronic degenerative problems such as cancer and cardiovascular disease. These findings indicate that restaurants serving Korean style full course meals should carefully plan their menus in order to provide their customers with balanced meals. They should also be strongly encouraged to play an active role in improving their customers' nutritional status, as well as reducing the restaurants wastage of food. A Case of Congenital Nephrogenic Diabetes Insipidus Confirmed by Gene Analysis (유전자 분석검사로 확진된 선천성 신성 요붕증 1례)

  • Nephrogenic diabetes insipidus (NDI) is a disorder in which the secretion of antidiuretic hormone is normal, but the response of the renal collecting tubules to vasopressin is impaired. Compared with acquired NDI (a-NDI), which is secondary to chronic bilateral incomplete urinary tract obstruction with hydronephrosis, congenital NDI (c-NDI) is a very rare heritable disorder that usually follows the X- linked recessive pattern. Clinical symptoms of c-NDI can be non specific, and often the disease ultimately results in failure to thrive, or mental retardation. Recently, the diagnosis can be confirmed by direct sequencing analysis of the peripheral blood specimens. The long-term results of treatment for c-NDI are not satisfactory. Reports on the follow up of c-NDI cases are rare and there is no report on the cases treated with combinations of three drugs. We report herein a case of severe c-NDI in an 8 year-old-boy with a severely dysconfigurated urinary tract system. The patient and his mother showed a frameshift mutation on the AVPR2 gene on chromosome Xq28:.847_851delTGCTG (p.C283fsX90). The patient showed normal growth and development by treatment with combinations of hydrochlorothiazide ($65mg/m^2$), amiloride (0.3 mg/kg/d) and indomethacin ($100mg/m^2$), yet after five years he needed adjuvant cystostomy to relieve him from the residual symptoms of urgency with polyuria.

High Prevalence of Helicobacter pylori Resistance to Clarithromycin: a Hospital-Based Cross-Sectional Study in Nakhon Ratchasima Province, Northeast of Thailand

  • Background: Helicobacter pylori is a cause of chronic gastritis, peptic ulcer disease, and gastric malignancy, infection being a serious health problem in Thailand. Recently, clarithromycin resistant H. pylori strains represent the main cause of treatment failure. Therefore this study aimed to determine the prevalence and pattern of H. pylori resistance to clarithromycin in Suranaree University of Technology Hospital, Suranree University of Technology, Nakhon Ratchasima, Northeastern Thailand, Nakhon Ratchasima province, northeast of Thailand. Materials and Methods: This hospital-based cross-sectional study was carried out between June 2014 and February 2015 with 300 infected patients interviewed and from whom gastric mucosa specimens were collected and proven positive by histology. The gastric mucosa specimens were tested for H. pylori and clarithromycin resistance by 23S ribosomal RNA point mutations analysis using real-time polymerase chain reactions. Correlation of eradication rates with patterns of mutation were analyzed by chi-square test. Results: Of 300 infected patients, the majority were aged between 47-61 years (31.6%), female (52.3%), with monthly income between 10,000-15,000 Baht (57%), and had a history of alcohol drinking (59.3%). Patient symptoms were abdominal pain (48.6%), followed by iron deficiency anemia (35.3%). Papaya salad consumption (40.3%) was a possible risk factor for H. pylori infection. The prevalence of H. pylori strains resistant to clarithromycin was 76.2%. Among clarithromycin-resistant strains tested, all were due to the A2144G point mutation in the 23S rRNA gene. Among mutations group, wild type genotype, mutant strain mixed wild type and mutant genotype were 23.8%, 35.7% and 40.5% respectively. With the clarithromycin-based triple therapy regimen, the efficacy decreased by 70% for H. pylori eradication (P<0.01). Conclusions: Recent results indicate a high rate of H. pylori resistance to clarithromycin. Mixed of wild type and mutant genotype is the most common mutant genotype in Nakhon Ratchasima province, therefore the use of clarithromycin-based triple therapy an not advisable as an empiric first-line regimen for H. pylori eradication in northeast region of Thailand.

Evaluation of Community Health Education Program by Newsletter (건강정보소식지를 이용한 지역사회 건강증진 교육프로그램의 평가)

  • This questionnaire survey was conducted to evaluate the impact of health newsletter and related factors in the rural area. The Subjects were 354 individuals of 45 years of age and over, living in Chonnam province. The results of this study were as follows: The 225 surveyed persons (64%) recognized the health newsletter, and 196 persons (55%) read it least once and more. Newsletter-recogniton related factors were sex and age. Newsletter-reading related factors were chronic disease and sex. And Newsletter-reading pattern was determined by age and education level. 147 persons(75.8%) reported that they had got the important health information from newletter, and 76 persons(39.8%) reported the behavior change. The quiz scores of hypertension and health care in summer were higher in the persons of newsletter-recognition than in persons of non-recognition. The quiz score of hypertension was not higher in newsletter reader than non-reader, but that of health care in summer was higher in newsletter reader than in non-reader.

Comparative Analysis and Evaluation of Dietary Intakes of Koreans by Age Groups: (5) Meal Patterns (서울 및 근교에 거주하는 한국인의 연령별 식생활 비교 및 평가: (5) 끼니별 섭취)

  • This study was performed to investigate the characteristics of meal patterns among Koreans of different age groups. The study subjects consisted of preschool children (PC), elementary school children (EC), high school students (HS), college students (CS), and adults over 30 years old living in Seoul and the surrounding areas. A dietary survey was conducted using 24-hour recall method from 1996-1998 and data were collected from 2392 subjects. The subjects over 30 years old were classified as 30∼49 years, 50∼69 years, and over 70 years. EC, HS, and CS groups consumed less than 20% of their total energy intake from breakfast. PC and EC consumed over 30% of their energy from snacks. In all age groups, the proportion of subjects who skipped a meal was highest for breakfast and lowest for dinner. Especially, 31.4% of the CS group skipped breakfast. Breakfast and snacks provided higher quality meals than the others when each meal was assessed by the index of nutritional quality (INQ). Especially, lunch of the HS group, each meal of the EC group and subjects over 70 years of age had a lower diet quality than the others did. The major snack items for PC, EC, HS, and CS groups were milk, ice cream, and snack cakes, but those for subjects over 30 years of age were fruits. The PC group obtained 35.5% of their energy from snacks, but the proportion of snack energy decreased with increasing age. PC and EC groups obtained a great proportion of calcium, vitamin B2, fat, and cholesterol intake from snacks. Snack foods provided 53.2%, 42.4%, 45.4%, and 24.6% of those nutrients, respectively, for the PC group and 54.2%, 43.5%, 37.4%, and 26.8%, respectively, for the EC group. These results suggested that each age group had specific meal patterns. Especially, breakfast and snack intake seemed to complement overall diet quality. However, more intervention is needed to improve overall dietary nutrition, e.g., efforts to decrease fat and cholesterol proportion and increase breakfast consumption. These dietary characteristics among different age groups present foundation data for the development of methods to resolve the nutritional problems of each age group.

Study on inhibition effects on metastasis and recurrence of Traditional Herb Medicine(THM) (한약의 전이재발억제 효과에 관한 연구)

  • Recently, the occurrence and death rates of cancer have increased rapidly. Metastasis and recurrence is the most important factor of death from cancer. So inhibition of recurrence and metastasis can increase the cure rate of cancer disease. At the basic chinese traditional medicine(TCM), there are a lot of theory related to neoplasm treatment. The metastasis and recurrence of neoplasm was the basis of yudu(餘毒) on remained neoplasm cell and stagnation of blood, thermotoxo, phlegm, asthenia of healthy enerngy and stagnation of vital energy. The principles therapy of neoplasm on metastasis and recurrence was based on knowledge of the method of support the healthy energy and strengthen the body resistance, promote blood circulation to remove blood stasis, clear away heat and toxic materials, dissipate phlegm and disperse the accumulation of evils. But the major clinical features of neoplasm was to be considered in developing a treatment plan include (1) distinguish between clinical and pathologic staging - acute and chronic, (2) classification of pathologic pattern, and (3) distingction of body situation : for examples asthenia - sthenia etc. It was most important to distinguish between supporting the healthy and eliminating the evil factors and to treat differently at the root and the branch cause of a neoplasm. This paper's results indicate that identification and effective use of THM can inhibite netastasis and recurrence and then it will help increasing survival rate. Also as BioTechnology(BT) has developed rapidly nowadays, as genes related to cancer have revealed. So it will give rise to the development of searching the mechanism of herb medicines which have inhibition effects of metastasis and recurrence.

The Effect of Soy Isoflavone Supplementation on Urinary Isoflavone Excretion in Korean Postmenopausal Women (폐경 후 여성에서 이소플라본 공급에 따른 소변 중 이소플라본 배설량 변화에 관한 연구)

  • It is well known that soy isoflavones play beneficial roles in the prevention of chronic diseases such as breast cancer, cardiovascular disease and osteoporosis. However current data are not sufficient for estimating the level of isoflavone intake. To use the urinary isoflavone excretion as a maker of isoflavone intake,26 participants consumed the isoflavone extract capsule (90 mg soy isoplavones/capsule) daily for 3 months. The study compared effects of pre- and post-isoflavone supplement in the following parameters; physical examination, dietary recalls, and urinary isoflavon excretion. The average age, height and weight were 65.7 years, 149.4 cm, and 57.3 kg. Subjects maintained regular diet pattern, and average daily nutrient intakes between pre- and post- supplementation were not significantly different except vitamin A, carotene and vitamin C. There was no significant difference between pre- and post- supplementation in terms of daily isoflavone intake. The basal urinary isoflavone excretion was 8.37 mg/day (daidzein 6.23 mu genistein 2.14 mg), and average daily excretion rate was IS.8%. Urinary isoflavone excretion was significantly increased, after isoflavone supplementation for 3 months as compared the basal level (p<0.01). Our data suggest that urinary isoflavone level, especially daidzein and genistein, may be a useful maker to estimate isoflavone intake.

Perspective on Population Characteristics and Health Problems of Koreans in 21st Century (21세기 초 한국의 인구구조 및 질병양상)

  • In order to anticipate disease pattern and health problems of Koreans in the 1st part of 21st century (by the year 2020), transition of population characteristics, mortality and morbidity data during the last 30 years Koreans have experienced were reviewed. On the actual basis of epidemiologic transition process that has undergone during last 30 years since 1960 along with socioeconomic development and successful implementation of selective national health policies (family planning, medical insurance and etc.), following changes can be expected in the 21st century in Korea, under the assumption that the current rate of progress is maintained. The population of South Korea alone will be doubled the population of 1960 by the year 2013 : aged Population older than 65 years will be increased from 3.3% in 1960 to 11.4% in 2020 with increased average age of the population from 23.6 year in 1970 to 39.2 year in 2020; urban population from 28% in 1960 to 83% in 2005. GNP/capita has increased tremendously from U.S. $120 in 1970 to $6,749 in 1992, and the government estimated it would be 519,350 in 2010 and $29,460 in 2020. Growth and developmental indices of children, educational achievement and social status of women also showed a remarkable improvement and anticipated to make futher progress. Leading causes of mortality and morbidity have shown a striking change during the last 30 years, from infectious diseases to chronic degenerative diseases and man-made injuries. Occurrence of communicable diseases may become minimal although viral hepatitis, venereal diseases Including AIDS, and well adapted herpes virus infections will maintain their endemic level. Newly evolving infectious agents, however, should be carefully monitored because of rapidly changing environments and human behaviours. Tuberculosis may increase up to the epidemic level when AIDS prevails. Ischemic heart diseases may increase steadily with increasing occurrence of hypertension and diabetes mellitus whereas cerebrovascular diseases may be decreased slowly. Musculaskeletal diseases which contribute a lot to the disability of aged people may be a major health problems due to increased aged population. Mental diseases, particularly that caused by alcohol and drug abuse, and senile dementia may become a prominent health problem. On the other hand injuries caused by traffic and industrial accidents that have shown most striking increase till now may be decreased considerably by intensive intervention. The health policies in the 21st century will be oriented to the health promotion for good quality life rather than life-savings.

Design of Dynamic Buffer Assignment and Message model for Large-scale Process Monitoring of Personalized Health Data (개인화된 건강 데이터의 대량 처리 모니터링을 위한 메시지 모델 및 동적 버퍼 할당 설계)

  • The ICT healing platform sets a couple of goals including preventing chronic diseases and sending out early disease warnings based on personal information such as bio-signals and life habits. The 2-step open system(TOS) had a relay designed between the healing platform and the storage of personal health data. It also took into account a publish/subscribe(pub/sub) service based on large-scale connections to transmit(monitor) the data processing process in real time. In the early design of TOS pub/sub, however, the same buffers were allocated regardless of connection idling and type of message in order to encode connection messages into a deflate algorithm. Proposed in this study, the dynamic buffer allocation was performed as follows: the message transmission type of each connection was first put to queuing; each queue was extracted for its feature, computed, and converted into vector through tf-idf, then being entered into a k-means cluster and forming a cluster; connections categorized under a certain cluster would re-allocate the resources according to the resource table of the cluster; the centroid of each cluster would select a queuing pattern to represent the cluster in advance and present it as a resource reference table(encoding efficiency by the buffer sizes); and the proposed design would perform trade-off between the calculation resources and the network bandwidth for cluster and feature calculations to efficiently allocate the encoding buffer resources of TOS to the network connections, thus contributing to the increased tps(number of real-time data processing and monitoring connections per unit hour) of TOS.
