Basic Research on the Environment of Oral Health Promotion in the Parent Cooperating Daycare Facilities Based on the Survey for the Nursery Teacher (일부 부모협동보육시설 보육교사 대상의 시설내 구강건강증진 환경에 관한 조사)

  • The purpose of this research was to collect basic data to develop a project of oral health that fits for particularity for parent cooperating daycare facilities for infants and children. For this purpose, we gave out the self administrated surveys to the teachers at parent cooperating daycare facilities, and analyzed surveys made by teachers. The results of this research could be summarized as follows ;1.The snacks and drinks provided by parent cooperating daycare center tend to provide more non-cariogenic snacks and drinks such as fruit, vegetables and fruit juice. 2. Activities related to dental health was done in parents corporative daycare center are: Children participate in the activities and training related to food (87.9%), catering staff receive education about nutrition (78.2%), avoiding sugary food at a birthday party (74.0%), annual dental health check-up by a dentist (33.5%). 3. 88.9% of teachers agreed parents' involvement of developing policies of oral health. 4. The percentages of guiding principles based on documentation in the topic of the oral heath were: Involving parents in the formation of the pre-school group's health policy (47.8%), advising the needs of a child for dental service to parents (44.9%), coping with a situation where a child injured his or her teeth (44.9%).

A DESCRIPTIVE STUDY ON THE CONTRIBUTING FACTORS OF EARLY CHILDHOOD CARIES OF 5 YEARS CHILDREN IN KANGNUNG CITY (강릉시 5세 아동의 "조기 유아기 우식증" 관련 추정요인의 기술 역학적 연구)

  • The purpose of this study was to get descriptive statistics of the contributing factors for early childhood caries and to predict the relationship of dietary, behavior factors and health status factors of the mother and child at pregnancy and after birth. 411 first caregivers of 5-year-old children in 12 kindergartens in Kangnung city were selected by stratified random cluster sampling. They were asked to fill out questionnaires and 364 of them responded. The obtained results were as follow: 1. Over the three-Fourth of children used nursing bottle or had breast feeding habit beyond the age of 1 year. 2. 8.7% of respondents didn't recognize the necessity of the preventive measures immediate after eruption of primary tooth, and only 35.1% replied that they had begun tooth cleaning. 3. Over 90% of children brush the teeth more than once per day. But over half (614%) of them brush their teeth without parents instruction. Sixty percent of children eat between the meals as often as 1-3 time(s) a day and the remainder at any times. 4. The first time of dental visit was for most children (87%) at over 3 years, recommending the earlier dental visit. Notwithstanding the rate of routine dental visit experience was relatively high(40.2%), implicating positive parents' attitude about oral health at Kangnung area. 5. The relationships between oral health state of the parents and the variables such as the timing of the first tooth cleaning, the frequency of brushing, the time of first dental visit, and the reason of first dental visit were not statistically significant. Together, there was no statistically significant difference between rural and urban area, private and public kindergarten, and boy and girl($x^2-test$, p>.05 or Fisher's exact test, p>.05).

Identification and inhibiting effect of Lactobacillus salivarius the formation of plaque and the production of volatile sulfur compounds by anaerobic bacteria (치태형성과 혐기성 세균의 황화합물 생성에 대한 Lactobacillus salivarius의 억제효과 및 동정)

  • There are normal inhabitants doing medically useful functions in the body. There are many kinds of bacteria performing specific functions in the oral cavity. Two strains of lactic add bacteria were isolated from normal inhabitants of children's oral cavity, which inhibited the production of volatile sulfur compounds by anaerobic bacteria. The authors identified the isolates by 16S rDNA partial sequencing. 1. Two isolates were Gram-positive bacilli and produced hydrogen peroxide. 2. When Streptococcus mutans was cultured in the media, the mean weight of formed artificial plaque on the orthodontic wires was $124.4{\pm}30.4$ mg, whereas being reduced to $5.2{\pm}2.0$ mg and $10.6{\pm}6.6$ mg in the media cultured with Streptococcus mutans and each isolate, respectively(p<0.05). 3. The number of viable cells of Streptococcus mutans was $3.4{\times}10^9$ per ml in the cultured solution, whereas those of Streptococcus mutans in the combined culture with each of isolates were $4.6{\times}10^8$ and $2.4{\times}10^8$ per ml. 4. The optical density was 1.286 in the supernatant of Fusoacterium nucleatum after vortexing for 30 minutes, whereas in the supernatant of combined Fusoacterium nucleatum and each isolate, they were reduced to 0.628 and 00497, which the percentages of coaggregation between them were 2904% and 57.8%, respectively. 5. The optical density of Fusoacterium nucleatum precipitate was 1.794 in the culture media containing cysteine and $FeSO_4$ being reduced to 1.144 and 0.915 in the coaggregated precipitates of Fusoacterium nucleatum and each isolate. 6. The similarity values of 16S rDNA sequence between each of isolates and Lactobacillus salivarius subsp. salicinius were 99.60% and 99.73%, respectively, meaning that isolates were Lactobacillus salivarius subsp. salicinius. These results indicated that two strains isolated from children's saliva, which inhibited the formation of plaque and the production of volatile sulfur compounds, were identified as Lactobacillus salivarius subsp. salicinius.

DISTRIBUTION AND TRANSMISSION OF STREPTOCOCCUS MUTANS AMONG CHILDREN AND THEIR MOTHERS (어린이와 어머니의 Streptococcus mutans 분포와 전이)

  • Recently, the number of multi-cultural families have been on the rise for years in Korea. Although social concerns are on the increase, there is still no basic data of their oral health state. In this study, we examined the caries prevalence of mother-child pairs from some korean families and multi-cultural families, and collected some saliva from them. We evaluated Streptococcus mutans level and investigated the serotype distribution of Streptococcus mutans of them to confirm the transmission in mother-child pairs. As a results, to the Korean families, the percentages of serotype c, e, f, k were 95%, 10%, 10%, and 10%. And to the multi-cultural families, the percentages of serotype c, e, f, k were 68.8%, 31.25%, 43,75%, 18.8%. But, there is no significant relationship between the number of serotype per person, caries prevalence, and Streptococcus mutans level(p>0.05). In conclusion, there are differences of serotype distribution of Streptococcus mutans between the two families. Nevertheless, matching rate of more than one serotype between mother and their child was high (92.86%). So, vertical transmission of Streptococcus mutans in mother-child pairs could be demonstrated.

Cariogenicity of Vitamin Supplements for Children (시판용 어린이 비타민 보충 제제의 치아 우식원성)

  • The purpose of this study is to analyze the cariogenicity of vitamin supplements for children by the Caries Potentiality Index (CPI), pH drop capacity, proliferation rate of Streptococcus mutans. Four vitamin supplements were selected - Noma (NM), Cenovis Kids (CK), Animal Parade (AP), and Character Vitamin (CV). CPI value decreased in the order of AP, CV, CK, and NM. Initial values of all experimental groups showed acidity below pH 7.0. Analysis of the colony forming units of Streptococcus mutans showed that NM and CV resulted a higher proliferation rate (p < 0.05) than CK and AP (p < 0.05). Bacterial activity of the control group was lower than other groups when observed with a confocal laser scanning microscope. Considering the bacterial activity and acidity of vitamin supplements, it is necessary to pay close attention when children taken the vitamin supplements for their oral health.

Change of Quality of Life in Children with ADHD after 12 Weeks OROS-Methylphenidate Treatment (12주 OROS-Methylphenidate 약물 치료 후 ADHD 아동의 삶의 질 변화)

  • Objectives : The aim of this study is to investigate the effectiveness of treatment with osmotic-release oral system methylphenidate (OROS-MPH) on quality of life (QOL) in children with attention-deficit hyperactivity disorder (ADHD). Another aim is to assess the relationship between change in QOL and other factors including children's symptoms and academic performance or parents' depression and parenting stress. Methods : A total of 111 medication-naive children with ADHD in a multicenter, open-label, 12-week trial of OROS-MPH completed an evaluation using diverse rating scales at two time points; at baseline and after 12 weeks of treatment. Scales for investigation of children included the Parent Report Form-Children's Health and Illness Profile-Children's Edition (PRF-CHIP-CE) on QOL, the ADHD Rating Scale-IV on symptoms, and the Academic Performance Rating Scale (APRS). The Beck Depression Inventory and Parenting Stress Index were used for assessment of their parents. Results : Total scores for mean PRF-CHIP-CE increased from $207.9{\pm}26.7$ at baseline to $226.3{\pm}25.9$ after 12 weeks of treatment (p<.001). The change of APRS showed the strongest correlation with the increment of PRF-CHIP-CE scores (Pearson coefficient=0.561, p<.001), even after controlling for other factors (partial correlation coefficient=0.420, p<.001). Conclusion : Treatment with MPH-OROS results in improvement of QOL in children with ADHD in Korea. The advance in academic performance plays a key role in this change of QOL.

Isolation and identification of Lactobacillus inhibiting the production of halitosis by anaerobic bacteria (구취를 유발하는 혐기성 세균의 증식을 억제하는 유산 간균의 분리 및 동정)

The optical density was 1.286 in the supernatant of Fusobacterium nucleatum after vortexing for 30 minutes, whereas in the supernatant of combined Fusobacterium nucleatum and each isolate, they were reduced to 0.628 and 0.497, which the percentages of coaggregation between them were 29.4% and 57.8%, respectively. 3. The optical density of Fusobacterium nucleatum precipitate was 1.794 in the culture media containing cysteine and $FeSO_4$, being reduced to 1.144 and 0.915 in the coaggregated precipitates of Fusobacterium nucleatum and each isolate. 4. The optical density of Porphyromonas gingivalis precipitate was 1.932 in the culture media, being reduced to 1.170 and 1.266 in the coaggregated precipitates of Porphyromonas gingivalis and each isolate. 5. When two isolates were tested with API 50 CHL medium kit, those were identified as Lactobaciallius salivarius and Lactobacillus delbrueckii subsp. lactis.

A World Tour of the Museum of Dentistry (세계 치과 박물관 여행)

  • Although museums of dentistry and dental museums are both museums related to dentistry, they may differ significantly in the scope and the content regarding the audiences. Museums of dentistry, which display a variety of dental instruments, materials, and photographs, help you learn about the development of dentistry and primarily attract a lot of attention from those who work in dental fields. These museums, however, don't get much attention from the general public. The dental museum, which displays everything related to teeth and dental care, can attract a lot of curiosity and attention not only by those in dental fields, but also by the general public. Dental museums should purpose public health education; they should not just be a place for looking around. This is because the experience of the dental museum has the power to engage people. Therefore, if you add educational functions to the dental museum and make it a place to learn about dental history and oral health care, it can be the institution where culture and health are combined. We need to study history to better understand which improvements we have made and also to see what we need to aim for in the future. The purpose of this study is to examine the actual way each of 28 dental museums in 7 different countries is operated by describing what I saw when I visited them. In addition, the current state of museums of dentistry in Korea is examined and the future development directions is also considered.

SCISSOR BITE CORRECTION IN PATIENTS WITH WILLAMS SYNDROME: A CASE REPORT (Williams Syndrome 환자에서의 scissor bite correction: 증례보고)

  • Williams syndrome(WS) is a congenital disorder caused by a deletion of the Elastin gene and other contiguous genes at chromosome 7. Patients with WS are at a high risk of dental caries, and they also have a higher frequency of dental malocclusion compared to normal children. Malocclusion occurs in 85% of individuals with WS, which results from combined causes, such as tongue thrust, hypotonia, and connective tissue abnormality. An 11 year-old girl with WS presented scissor bite on the lower right second premolar and the first molar, and she complained of difficulty in chewing. Active lingual arch was used instead of removable appliance, considering the patient's cooperation ability. Unilateral posterior scissor bite was corrected in 7 months. Although patients with WS are sociable and friendly, dental treatment can be a fearful experience for them. Efforts to build rapport with the patients with WS resulted in improved relationship between the doctor and patient, and desired outcome of dental treatment was achieved with patient's improved cooperation.

A Survey on Procedural Sedation and Analgesia for Pediatric Facial Laceration Repair in Korea

  • Background Most children with facial lacerations require sedation for primary sutures. However, sedation guidelines for invasive treatment are lacking. This study evaluated the current status of the sedation methods used for pediatric facial laceration repair in Korea. Methods We surveyed one resident in each included plastic surgery training hospital using face-to-face interviews or e-mail correspondence. The health care center types (secondary or tertiary hospitals), sedation drug types, usage, and dosage, procedure sequence, monitoring methods, drug effects, adverse events, and operator and guardian satisfaction were investigated. Results We included 45/67 hospitals (67%) that used a single drug, ketamine in 31 hospitals and chloral hydrate in 14 hospitals. All health care center used similar sedatives. The most used drug administered was 5 mg/kg intramuscular ketamine (10 hospitals; 32%). The most common chloral hydrate administration approach was oral 50 mg/kg (seven hospitals; 50%). Twenty-two hospitals (71%) using ketamine followed this sequence: administration of sedatives, local anesthesia, primary repair, and imaging work-up. The most common sequence used for chloral hydrate (eight hospitals; 57%) was local anesthesia, administration of sedatives, imaging work-up, and primary repair. All hospitals that used ketamine and seven (50%) of those using chloral hydrate monitored oxygen saturation. Median operator satisfaction differed significantly between ketamine and chloral hydrate (4.0 [interquartile range, 4.0-4.0] vs. 3.0 [interquartile range, 3.0-4.0]; p <0.001). Conclusion The hospitals used various procedural sedation methods for children with facial lacerations. Guidelines that consider the patient's condition and drug characteristics are needed for safe and effective sedation.
