ChoSeokJin coach has continued the lifeline of the Korean wood furniture Jeonbuk region. It is already famous coach to award-winning Olympic coach and tournament of the world. Check his mental world and his life through the work of ChoSeokJin coach. Through a harsh process until the Republic of Korea master craftsman, it was an opportunity to reaffirm once again the importance of effort and commitment. South Korea wood chief coach (the field of furniture) growth phase of the No. 1 ChoSeokJin coach, Ripe stage and spirit such as through his neck furniture fabrication methods, he is him then and the situation until the coach and the intangible cultural heritage had asked We look at the world.   • The chest X-rays are a common way to diagnose lung cancer or pneumonia. In particular, the finding of a lung nodule is the most important problem in the early detection of lung cancer. Recently, a lot of automatic diagnosis algorithms have been studied to find the lung nodules missed by doctors. The algorithms are typically based on segmentation network like U-Net. However, the occurrence of false positives that similar to lung nodules present outside the lungs can severely degrade performance. In this study, we propose a multi-task learning method that simultaneously learns the lung region and nodule-labeled data based on the prior knowledge that lung nodules exist only in the lung. The proposed method significantly reduces false positives outside the lung and improves the recognition rate of lung nodules to 83.8 F1 score compared to 66.6 F1 score of single task learning with U-net model. The experimental results on the JSRT public dataset demonstrate the effectiveness of the proposed method compared with other baseline methods.

Bronchogenic Cyst (Two Case Report) (기관지성 낭종(2례수술보고))

  • Bronchogenic cysts, though known as a relatively common malformation of the mediastinum, were rarely discussed in Korean literatures. Since the surgical removal of asymptomatic lesions was adopted as the reasonable therapeutic principle, the incidence of bronchogenic cysts were found to be higher than previously expected. Two cases of bronchogenic cysts operated on in The Dept. of Thoracic Surgery, Korea University Woo Sok Hospital were reported and related literatures were reviewed. Case 1. 5 year old boy with chief complaints of bulging cystic mass on left supraclavicular region during straining or coughing since his age of 2 was admitted, and cystogram with Lipiodol revealed hen-egg sized mass in the left antero-superior mediastinum without any communication with bronchus or esophagus. Cyst was successfully removed under general anesthesia thru left supraclavicular incision, and pathological examination of the cyst revealed thin cystic wall lined with stratified columnar epithelium and it`s content was milky white mucoid fluid devoid of any bacterial growth. Case 2. 15 year old school girl has been noted slowly growing walnut-sized mass on anterolateral side of the neck for 4 months without any subjective symptom except cosmetic problem... Mass was aspirated to find milky white mucoid fluid in some loculation and yellowish turbid fluid in other part due to chronic infection. Cystic tumor was removed under local anesthesia, which was loculated in between the trachea and esophagus without any communication, and pathological diagnosis of the cyst was bronchogenic cyst with columnar epithelial cell lining with moderate chronic inflammatory cell infiltrations. Postoperative conditions of the two cases were all excellent with normal life.

Clinical Study of Neck Base Injury (경저부 혈관손상의 임상적 고찰)

  • Injuries to the major vessels in the thoracic inlet require early recognition and expedient operative approach. Delayed diagnosis difficulties encountered in the operative exposure of the region are the major factors limiting successful management. This report is a review of 13 patients with vascular injuries to the neck base who were managed at Busan National University Hospital from March 1975 to September 1978 about 3 years and 6 months. The important clinical problems are delineated with emphasis on the technical aspects of operative management. 1] Among 13 cases, 8 cases were male 5 cases were female. 2] Of 28 vascular injuries, subclavian axillary vascular injuries were 22 [78%]. Stab wound was the cause in 70% of these patients. 3] Without extension 7 cases[53.8%] were managed successfully with supraclavicular, and axillary incision. Posterolateral thoracotomy one of extending 4 cases, 2 cases were used right musculoskeletal flap for management of proximal part of the subclavian artery and innominate vessel, 2 cases were used left supraclavicular incision with anterolateral thoracotomy for management of left proximal subclavian artery. One Expired. 4] Repair of vascular injury was accomplished by lateral suture of debridement and end-to end anastomosis in 17[74%]. Autogenous vein was used one for interposition graft. Ligation was required 2 arterial, 6 venous injuries. Of 8 cases which were pulseless preoperatively, 5 cases were able to palpable distal pulse. 5] Post operative complications occurred 50%. Complication of vasular repair was rare. The majority was neurologic deficit (33.3%).

Correlation between Prognosis and Factors on Acne Patients (여드름의 치료예후에 영향을 미치는 인자들에 대한 상관성 고찰)

  • Objective : The purpose of this study was to explore the correlation between efficacy and various factors of the acne patients. Method : We reviewed 30 acne patients who visited Allergy/Derma-cosmetic Clinic at Kyung Hee Neo Medical Center from June, 2006 to December, 2007 and were treated with herbal medicine and acupuncture for 4 weeks. We investigated premenstrual aggravation of acne, facial burning sensation, constipation, Heart Rate Variability(HRV) results and regional distribution of acne. And we evaluated inflammatory acnes according to the Korean Acne Grading System(KAGS). Results & Conclusions : 1. The average age of total patients was 23.0 years old. The most frequent age group was 21-25 years old. Patients evaluated as KAGS Grade 2 were most common. 2. In total patients, inflammatory acne were reduced significantly after 4 weeks of treatment. 3. The result of analysis on premenstrual aggravation of acne, facial burning sensation and constipation, there weren't any difference of treatment results on statistical meaning. 4. The results show that there were significant correlations between the Low Frequency at HRV and the rate of acne reduction. (Pearson correlation = -0.565) 5. If the distribution of acne did not involve the T-zone of the face, chest, or back of the body, inflammatory acne were significantly decreased after 4 weeks of treatment.

Surgical Correction of Discrete Membranous Suaorti Stenosis (선천성 대동맥판막하 막상협착증 치험 2례)

  • We experienced surgical correction of 2 cases of discrete membranous subaortic stenosis. Case 1 was 19 years old male patient. His complaints were fatigue, exertional dyspnea, syncope and angina for 8 years. Ejection. systolic murmur was heard at the second right intercostal space and diastolic murmur was heard at the apex. A thrill was palpated over the second right intercostal space and area of the carotid artery. 2-D echo, cardiac cath and left ventriculogram revealed discrete membranous subaortic stenosis and VSD. Complete excision of discrete membrane without mymectomy was done. VSD was closed with dacron patch and aortic valve was replaced with St. Jude medical valve. Case 11 was 16 years old female whose complaints were exertional dyspnea and syncope. Ejection systolic murmur was heard at second right intercostal space, but diastolic murmur was not heard. A thrill was palpated over the second right intercostal space and the area of carotid artery. 2-D echo, cardiac cath and left ventriculogram revealed discrete membranous subaortic stenosis. Complete excision of fibrous tissue and myotomy were made and aortic valve was replaced with St. Jude medical valve. Operative finding was followed: both aortic valves showed deformity of leaflets. Subaortic region had a thickened central fibrous body from which the ridge protruded. Both patient`s postoperative course were uneventful and short-term follow-up results were good except soft systolic murmur at the aortic area.

Penetrating Neck Trauma by Gunshot Injury: 1 Case Report (총상에 의한 경부 관통상 -치험 1례-)

  • Penetrating neck trauma by gunshot injury involving tracheobronchial tree is rare in Korea. Extensive tissue damage by cavitation, tissue fragmentation and shock wave transmission of high-velocity projectile along with associated organ injury renders high rate of mortality and morbidity. A 28 year old man in military service with gunshot wound in left cervical area presented initial symptoms of severe dyspnea and subcutaneous emphysema. Computed tomography of chest and cervical region as well as bronchoscopic evaluation was performed to confirm highly suspected injury to cervical trachea. Surgical exposure was established through a low collar incision; the damaged segment of 3.5 cm length including 2-4th tracheal rings was resected out and end-to-end anastomosis was performed. Bleeding from lacerated anterior jugular vein was controlled by ligation of both ends and a K2 bulllet was found upon inner border of body of first rib, medial to right carotid sheath and removed out. Cervical esophagus, carotid artery, internal jugular vein and recurrent laryngeal nerve were spared. Extubation was done on the first postoperative day and postoperative course until discharge on nineth postoperative day remained uneventful.

A Survey on actual state or patients visited one emergency medical center (일개 응급센타에 내원한 일부 내원자의 실태조사)

  • We have reviewed 2.876 patients who visited the emergency room from May, 1996 to september, 1996 in order to look for a more appropriate management of the emergency medical system. The result were summarized as followings; 1. Considering sexual distribution, male patients were 59.6% more than female patients 40.4% and according to age categories, first decade of life was most frequent. 2. Distribution according to ca use is most common for the disease 79.8%, and next injury and traffic accident is 11.6%, 6.8%, respectively. 3. According to the department distribution, pediatrics accounted for 37.4%, internal medicine 21.1%, and emergency medicine 10.0%, and these three departments was 66.3% of total number of patients. 4. 67.0% of patients visited emergency room were discharged. Arrived time in emergency room, patients of 41.9% was visited from 17 to 1 hours. 5. Among 196 patients of traffic accident, 160 were male(81.5%) and 36 were female (18.5%). Distribution of the age is most common in 3rd and 4th decade for 63.2%, and region of injury were face 33.3%, head 22.2%, and chest 22.2% and follow-up measures after visiting emergency room is most common transfer hospital 40.8%.

Analysis of Sleep Breathing Type According to Breathing Strength (호흡 강도에 따른 수면 호흡 유형 분석)

  • Sleep apnea refers to a condition in which a person does not breathe during sleep, and is a dangerous symptom that blocks oxygen supply in the body, causing various complications, and the elderly and infants can die if severe. In this paper, we present an algorithm that classifies sleep breathing by analyzing the intensity of breathing with images alone in preparation for the risk of sleep apnea. Only the chest of the person being measured is set to the Region of Interest (ROI) to determine the breathing strength by the differential image within the corresponding ROI area. The adult was selected as the target of the measurement and the breathing strength was measured accurately, and the difference in breathing intensity was also distinguished using depth information. Two videos of sleeping babies also show that even microscopic breathing motions smaller than adults can be detected, which is also expected to help prevent infant death syndrome (SIDS).

Blunt Cardiac Injuries That Require Operative Management: A Single-Center 7-Year Experience

  • Purpose: Blunt cardiac injuries (BCI) have a wide clinical spectrum, ranging from asymptomatic myocardial contusion to cardiac rupture and death. BCIs rarely require surgical intervention, but can be rapidly fatal, requiring prompt evaluation and surgical treatment in some cases. The aim of this study was to identify potential factors associated with in-hospital mortality after surgery in patients with BCI. Methods: The medical records of 15 patients who had undergone emergency cardiac surgery for BCI between January 2014 and August 2020 were retrospectively reviewed. We included trauma patients older than 18 years admitted to Regional Trauma Center, Gachon University Gil Medical Center during the study period. Clinical and laboratory variables were compared between survivors and non-survivors. Results: Non-survivors showed a significantly higher Injury Severity Score (p=0.001) and Abbreviated Injury Scale in the chest region (p=0.001) than survivors. American Association for the Surgery of Trauma-Organ Injury Scale Grade V injuries were significantly more common in non-survivors than in survivors (p=0.031). Non-survivors had significantly more preoperative packed red blood cell (PRBC) transfusions (p=0.019) and were significantly more likely to experience preoperative cardiac arrest (p=0.001) than survivors. Initial pH (p=0.010), lactate (p=0.026), and base excess (BE; p=0.026) levels showed significant differences between the two groups. Conclusions: Initial pH, lactate, BE, ventricular injury, the amount of preoperative PRBC transfusions, and preoperative cardiac arrest were potential predictors of in-hospital mortality.
