A Patient with Dyspnea, Chest Pain and Fatigue due to Angina Pectoris Treated with Saengmaeg-san-hap-Bojungikgi-tang-gami: A Case Report (협심증으로 발생한 호흡곤란, 흉통 및 피로 환자에 대한 생맥산 합 보중익기탕 가미 1례 증례보고)

  • Objectives: This case report describes good clinical progress in a patient with symptoms related to angina pectoris treated with a herbal medication. Methods: The 66-year-old male patient hospitalized in Chonnam National University Hospital (CNUH) in May 2020 for symptoms of dyspnea and chest pain was diagnosed with angina pectoris, medicated, underwent PCI in August 2020, and continued to take the medicine prescribed at CNUH during hospitalization. Korean medical treatments, including herbal medicines (Saengmaeg-san-hap-Bojungikgi-tang-gami, Yeonggyechulgam-tang), acupuncture, and cupping therapy were administered for 22 days. Symptom severity was assessed with a daily visual analog scale (VAS) for dyspnea and chest pain and standard deviation of NN (SDNN) score of heart rate variability. Results: Post-treatment, severity of dyspnea, chest pain, and fatigue reduced from VAS 7 to VAS 2, VAS 6 to VAS 1, and VAS 7 to VAS 1, respectively. The SDNN score improved from 16.474 ms to 23.270 ms. There were no side effects from Korean medicine treatment. Conclusion: Traditional Korean treatment could effectively treat symptoms related to angina pectoris.

GERD-unrelated Non-cardiac Chest Pain may be Associated with Depression and Anxiety (위식도역류질환과 관련 없는 비심인성 흉통 환자의 우울 및 불안)

  • Objectives : Non-cardiac chest pain (NCCP) can be divided into gastroesophageal reflux disease (GERD) related NCCP and non-GERD related NCCP. Our study was designed to examine the differences in clinical characteristics and psychological mood states between the two clinical syndromes. Methods : After some cardiologic evaluations such as treadmill exercise, coronary angiography, and echocardiography, 27 patients with NCCP were enrolled in this study. They were divided into patients with GERD related NCCP (12 patients) and those with non-GERD related NCCP (15 patients) using the upper gastrointestinal endoscopy and the ambulatory 24 hour esophageal pH monitoring. Clinical characteristics such as typical reflux symptoms and psychological mood states were measured. Patients who showed scores more than 10 on the Beck Depression Inventory (BDI) or Beck Anxiety Inventory (BAI) were defined as depressed or anxious group. Anxiety sensitivity Index (ASI) was also measured in all patients. All parameters were compared between patients with GERD related NCCP and those with non-GERD related NCCP. Results : The two groups showed a difference in typical reflux symptoms. Patients with non-GERD related NCCP had higher scores on the BDI, BAI and ASI than those with GERD related NCCP. Among all NCCP patients, 14 patients (51.9%) were suggested to have possible depression or anxiety disorders. Conclusion : The non-GERD related NCCP was shown to be associated with psychological mood states such as anxiety and depression. Thus, we suggest that routine measurement of psychological mood states should be necessary in the evaluation and treatment of NCCP.

Significance of Bone Scan in Chest Trauma (흉부외상에서 골스캔의 의의)

  • Clinical analysis of the relation between the clinical data of chest trauma patients and bone scan results was done to investigate the sensitivity and specificity of bone scan for the diagnosis of rib or chondral fracture. Material and Method: 445 patients treated due to chest trauma in Dongguk University Kyungju Hospital from July 1998 to December 2001 were studied. Age and sex distribution, causes of chest trauma, interval from the injury to arrival at hospital, final diagnosis, relationship between the chest pain and bone scan results, relationship between local tenderness and bone scan results, and sensitivity and specificity of bone scan were studied. Result: The male was 61.6% and the female was 38.4%. Age distribution was nearly even from the twenties to the sixties. The traffic accident was the most common cause of chest injury(60.4%). Most patients(76.6%) were arrived to the hospital within 6 hours after chest trauma. Rib fracture was the most common final diagnosis(46.5 %). The relationship between the duration of chest pain and tenderness and the bone scan results were significant, and the same in the relationship between the duration of local tenderness and the bone scan results, but the latter is more significant. The sensitivity of bone scan was 99.4% and the specificity was 90.4%. Conclusion: If the patient complains the chest pain continuously and the local tenderness around the chest is continued over 3 weeks, it is good to perform the bone scan. Further study including the MRI may be helpful for more evaluation in chest trauma patients.

A Case Report of Non-cardiac Chest Pain in a Non-erosive Reflux Disease Patient Treated with Beewha-eum (비화음으로 호전된 비미란성 역류질환(Non-erosive Reflux Disease) 환자의 비심인성 흉통(Non-cardiac chest pain) 치험 1례)

  • Objective: The purpose of this study was to report the effectiveness of Korean medicine (Beewha-eum) on the treatment of non-cardiac chest pain (NCCP) in a patient with non-erosive reflux disease (NERD). Methods: The patient was diagnosed with a spleen-stomach weakness pattern identified by Korean medicine and was treated with herbal medicine (Beewha-eum). The severity of symptoms was assessed with a numerical rating scale (NRS) for chest pain, self-reported dyspepsia degree (%), Korean gastrointestinal symptom rating scale (KGSRS), gastrointestinal symptom score (GIS), and Functional Dyspepsia-Quality of Life(FD-QOL) score. Results: After Beewha-eum treatment, the severity of chest pain was decreased from NRS 8 to NRS 0 and the self-reported dyspepsia degree also decreased from 100% to 65%. The KGSRS score was decreased from 49 to 35, the GIS score was also decreased from 16 to 9, and the FD-QOL score was increased from 20 to 25. Conclusions: The study findings suggested that Korean medical treatment with Beewha-eum could be an effective option for treating NCCP in patients with NERD.

A Case Report of an Ischemic Heart Disease Patient with Chest Discomfort and a Headache (허혈성 심질환 환자의 두통 및 흉통 한방치험 1례)

  • Objective: This case study describes the use of traditional Korean medicine to relieve chest pain and headaches in a patient with ischemic heart disease. Methods: The patient was treated with a range of traditional Korean medicine, including acupuncture, moxibustion, and herbal medicines (Gamiondam-tang and Cheonmabanhwa-tang-gagambang). The Numerical Rating Scale (NRS) was used to measure the patient's status and improvements in the frequency of symptoms. Results: After the treatment, the NRS score for chest pain and headaches decreased from 6 to 0. In addition, chest discomfort and palpitations almost subsided. Conclusion: This clinical case study suggested that Gamiondam-tang and Cheonmabanhwa-tang-gagambang could be effective in relieving chest discomfort and headaches.

Association of Waist Circumference with Risk Factors for Coronary Artery Diseases in Women Patients with Chest Pain (흉통을 호소하는 여성 환자에서 허리둘레와 관상동맥질환 위험인자간의 관계에 관한 조사연구)

  • Purpose: The purpose of this study was to compare the associations of body mass index (BMI) and waist circumference (WC) with coronary artery diseases (CAD) in women patients with chest pain. Method: BMI, WC, and flow-mediated vasodilation (FMD) were measured for 162 women patients with chest pain whose mean age was $54.9{\pm}9.2$ years. CAD was diagnosed by coronary angiography. Results: In comparing BMI and WC, WC was found to be more strongly associated with cardiovascular risk factors. For example, correlations with the high density lipoprotein cholesterol were r=-.266, p=.001 (WC) vs. r=-.131, p=.104 (BMI), and for homocystein, r=.378, p<.001 (WC) vs. r=.150, p=.068 (BMI). Obstructive CAD develops more frequently in women patients with abdominal obesity than in patients without abdominal obesity. Conclusion: The results of the study indicate that WC is a better index of adiposity than BMI.

  • Coronary artery vasospasm results in transient, abrupt chest pain that's due to the increased vasomotor tone of the coronary artery, and this can cause myocardial ischemia. We report here or one case of the right coronary artery vasospasm after aortic valve replacement surgery, and this was due to severe aortic regurgitation.

  • Few patients with traumatic aortic laceration remain undiagnosed and survive long enough to develop a chronic aneurysm. Such aneurysms are frequently asymptomatic: alternatively, they may manifest chest pain, dysphagia, bronchial irritation, or sudden death. A case of aortobronchial fistula secondary to a chronic post-traumatic aneurysm of the aortic isthmus is presented. Hemoptysis was the main sign. The affected segment of the thoracic aorta was repaired with a Hemashield patch and a left upper lobectomy was performed.

  • Intramural dissection of the esophagus is rare esophageal disorder which has been seen predominantly in women in their seventh or eighth decade and presents as acute chest pain, accompanied by dysphagia. The etiology of this disorder remain uncertain and the diagnosis is made by esophageal endoscopy, contrast esophagography, or both. Patient with this disorder is best managed conservatively with nothing by mouth and intravenous hydration.

  • Diffuse esophageal spasm(DES) is a rare disease seen in 4% of all patients studied in an esophageal motility laboratory, and its diagnosis and surgical management is still controversial. Recently, we treated two patients by extended esophageal myotomy for diffuse esophageal spasm which was diagnosed by the clinical symptoms of patients, esophagoscopy, esophagography, and esophageal manometry. The successful result of treatments was proved with subsidence of previous clinical symptoms(dysphagia and chest pain), postoperative esophagography and esophageal manometry. We present the results together with the review of literatures.
