A Clinical Study of Succinylcholine-Atracurium for Cesarian Section (제왕절개환자에 있어서 Succinylcholine-Atracurium의 임상적 고찰)

  • Atracurium was used in 10 parturients undergoing elective cesarian section under general Anesthesia. An initial bolus of 0.4 mg/kg of the atracurium was injected after recovery from succinylcholine block. Complete neuromuscular block resulted in all cases. Muscular relaxation was maintained by incremental dose of 0.2mg/kg of atracurium. Administration of atracurium did not cause significant change in blood pressure, pulse rate and infants Apgar score. The residual neuromuscular block could be antagonized at the end of the procedure by mixture of 0.005mg/kg glycopyrrolate and 0.03 mg/kg of neostigmine. In all parturients, antagonism was adequate as evidenced by respiratory response and head lift test. Atracurium may be advantageous in parturients undergoing cesarian section under general anesthesia cause it maintained cardiovascular stability, is non-cumulative, is readily antagonized by neostigmine and has no deleterious effect on the newborn.

  • Maternal and fetal effects of anesthesia for 423 cesarian section, performed during the past three and quarter years period in this hospital, have been evaluated. It is emphasized that the selection of anesthetic agent and method should depend upon the physical status of the patients and the ability of both obstetrician and anesthesiologists. Author results were as follows: 1. The incidence of c-section was 13.9% of total deliveries. 2. More than about three fourths of total cases were emergency cases. 3. Almost all cases were operated under general anesthesia. 4. The good physical states of patients resulted in better Apgar score of infants. 5. The shorter the interval from induction of anesthesia to delivery, the better was the Apgar score of infants.

The Effects of Labor Support Behaviors (LSB) One-to-one Application and Partner's Delivery Participation on the Delivery Satisfaction and Delivery Results among Mothers Who Delivered Premature Birth and Low Birth Weight Infant (조산 및 저출생체중아를 분만한 산부의 Labor Support Behaviors의 일대일 적용 및 배우자의 분만참여에 따른 분만만족도와 분만결과 비교)

  • Purpose: This study was aimed to evaluate the effects of Labor Support Behaviors (LSB) one-to-one application and partner's delivery participation on the delivery satisfaction and delivery results among mothers who delivered premature birth and low birth weight infant. Methods: The data were collected from 30 mothers in the experimental group and 27 in the control group from April 23, 2009 to April 22, 2010. The collected data were analyzed using percentage, mean, standard deviation, $x^2-test$ (Fisher's exact test) and t-test with SPSS. Results: The satisfaction levels of the experimental group and the control group women were $3.73{\pm}0.43$ and $3.72{\pm}0.34$ as mean values, respectively, showing not statistically different (t=0.07, p=.945). None showed less than 7 point of Apgar score at 1 minute in the experimental group while 5 out of 27 did (18.5%) in the control group, which was statistically meaningful. Also, the emergent cesarian section cases were 3 out of 33 (9.1%) in the experimental group and 5 out of 32 (15.6%) in the control group, demonstrating lower emergency cesarian section rate in the experimental group than the control group, but showing not statistically different ($x^2=0.643$, p=.475). Conclusion: The results of this study show that LSB one-to-one application and partner's participation has affirmative effects on 1 minute Apgar scores of newborns.

The effect of Critical Pathway on the patients with Cesarean section (제왕절개술 환자에 대한 Critical Pathway 적용효과)

  • With the drastically changing healthcare market, the adoption of DRG system and the use of Critical Pathway will be increased gradually in order to care hospitalized patients qualitatively and manage cost-effectively in Korea. The purpose of this quasi-experimental research is to apply and analyze the developed critical pathway to the patients with Cesarean section. 35 eligible subjects for the control group were chosen from postpartum patients who has delivered at H-hospital in Kwang-ju, and were questioned after appling previous care plan from May to December, 1998. For the experimental group, 37 subjects were chosen and questioned after applying Critical Pathway service from January to June, 1999. The instruments are a critical pathway for the patients with Cesarean section developed by chung(1998) and a questionnaire that evaluate patients and family members' satisfaction level. Also, educational materials were used to inform them in the process of Critical Pathway application. As a result of chi-square test on general characteristics, there is significant difference between control group and experimental group only in terms of room size(p= .010). There are no significant differences in the average length of stay in the hospital between control group and experimental group(t=.078). Also, the average medical consult fee has no difference significantly between two groups(t= .105). The findings showed that the experimental group with critical pathway service scored significantly higher than control group on the satisfaction level toward care services(p=.000). The items are post-operative care, postpartum exercise technique, breast feeding and breast manage through educational materials. In conclusion, the research indicates that the application of planned Critical Pathway can have a positive impact on satisfaction level of inpatients with Cesarian section.

The Yields of Volume, Thiamin and Riboflavin from the Milk of Cesarian-Section Women, and Their Ingestion in Infants (제왕절개 분만 수유부의 모유, 티아민, 리보플라빈의 분비량과 영아의 섭취량)

  • The present study was conducted to investigate the yields of volume, thiamin and riboflavin from the milk of twenty-five Cesarean-section women, and their ingestion in infants. We measured the consumed volume of human milk of infants by the test-weighing method, and thiamin riboflavin contents were determined by fluorescence spectroscopy during the first 7 days postpartum. Thiamin contents of the milk averaged 24.3, 24.0, 27.4 and 30.2ug/100g. Thiamin and riboflavin contents of the milk both increased during the lactation ps mod(p<0.05). The first secretion of breast milk occurred from the 4th to 5th day postpartum in Cesarean-section women. The consumed volume of human milk was 63, 81, 104 and 133g/day at 4, 5, 6 and 7 days postpartum and the consumed volume of formula milk was 58, 176, 292, 323, 308, 321 and 318g/day at 1, 2, 3, 4, 5, 6 and 7 days postpartum and the consumed volume of formula from the human and formula milk were 26.6, 91.0, 151.0, 172.5, 169.2, 175.9 and 172.9ug/day. Total daily riboflavin ingestion from the human and formula milk were 26.6, 91.0, 151.0, 172.5, 169.2, 175.9 and 172.9ug/day. Total daily riboflavin ingestion from the human and formula milk was 48.7, 166.8, 277.6, 318.2, 312.0, 327.0 and 335.7ug/day at 1, 2, 3, 4, 5, 6 and 7 postpartum. These results suggest that the milk secretion of Cesarean-section women is delayed a few days and the yield volume of milk is less than that of normal delivery women, but thiamin and riboflavin contents of the milk are not different from those of milk of normal-delivery women.

Effects of Cesarean Section in Dogs on Blood Coagulation Profiles (개 제왕절개 수술이 혈액응고계에 미치는 영향)

  • Patients often present with spontaneous bleeding, or a bleeding disorder may be discovered when an otherwise healthy dog develops marked bleeding during or after surgery. In this study, we were aimed to elucidate whether the cesarean section in dogs has influence on the coagulation profiles. And we gained the normal data on a panel of screening laboratory tests which allow accurate characterization of a hemostatic defects in dogs. Of the 20 healthy adult dogs, buccal mucosa bleeding time (BMBT) was $83.0{\pm}10.5$ seconds, platelet count was $24.0{\pm}3.5{\times}10^4/{\mu}l$, activated partial thromboplastin time (APTT) was $8.8{\pm}2.0$ seconds, the concentration of fibrinogen was $288.5{\pm}77.9mg/dl$, and the concentration of fibrin degradation products (D-dimer) was <250.0 ng/ml. Coagulation profiles before and after cesarean section of 13 cesarean sectioned dogs were in the normal range and there were no statistical differences in coagulation profiles between normal dogs and cesarean sectioned dogs (p>0.05). The results suggested that labor and cesarean section in healthy dogs did not alter coagulation profiles.

  • A 28 years old pregnant woman(Gestational age 35 weeks) had been operated emergency Cesarian section for delivery and emergency graft replacement of ascending aorta and total arch for acute type A aortic dissection. 1 year and 6 months later, she underwent aortic graft replacement from descending thoracic aorta to both common iliac arteries because of further progression of aortic dissection. So far she has a complete artificial graft aorta.

  • The concept of PCA(Patient Controlled Analgesia) was first described in 1968, by Sechzer. The earliest descriptions of actual self-administered PCA machines were by Forrest et al. In gastric bypass surgery, cesarian section, orthopedic surgery etc, PCA is widely used in the control of postoperative pain. Previous Studies have shown that PCA provides effective pain-control for the postoperative patient. The postoperative pain-control is a problem that should be solved in surgery. Especially in orthognathic surgery, it is not same as in the case of maxillofacial trauma surgery or of tumor surgery: most orthognathic surgery patients are under operation not accustomed to pains, and difficulties in pain-complaint due to IMF(Intermaxillary fixation), postoperative nausea, and vomiting are additional problem. In this study, we have compared PCA and IM analgesics with respect not only to time request but also to the quality of postoperative pain control.

  • Objectives: This study is to report the effect of oriental medicine on a cervical incompetence patient. Methods: The effectiveness of surgical cerclage in the cervical incompetence have had prolonged controversy. Herbal medicine was given to a cervical incompetence patient whom was suggested surgical cerclage. The patient was a 40-year-old female and treated with Dojeukgangki-tang gagambang. Results: After oriental herbal treatment, the clinical symptom of a cervical incompetence was improved and maintained, the patient was delivered in 38 weeks through cesarian section. Conclusions: Oriental herbal medicine and treatment have possibility to be a useful alternative to the surgical cerclage on cervical incompetence on the basis of more persistent study.

  • Purpose: Many infertile women are receiving in vitro fertilization-embryo transfer (IVF-ET). But side effects occur after IVF-ET and pregnancy rate is still low. So this study is to report the effect of herbal medicine on a woman receiving IVF-ET and herbal medicine's low risk on a pregnant woman. Methods: The patient was underwent artificial insemination five times and IVF-ET three times. In this study, the patient steadily took a herbal medicine before IVF-ET to birth. Results: The side effects of IVF-ET and the miscarriage symptoms ware decreased. And by taking a herbal medicine during IVF-ET, the patient was pregnant and gave birth when she was 33 weeks pregnant by cesarian section. Conclusions: This case study shows that herbal medicine is effective for reducing side effects of IVF-ET and increasing pregnancy rate. And in this case the patient gave birth to healthy babies, althogh steadily taking herbal medicine during pregnancy.
