StrokeBase: A Database of Cerebrovascular Disease-related Candidate Genes

  • Complex diseases such as stroke and cancer have two or more genetic loci and are affected by environmental factors that contribute to the diseases. Due to the complex characteristics of these diseases, identifying candidate genes requires a system-level analysis of the following: gene ontology, pathway, and interactions. A database and user interface, termed StrokeBase, was developed; StrokeBase provides queries that search for pathways, candidate genes, candidate SNPs, and gene networks. The database was developed by using in silico data mining of HGNC, ENSEMBL, STRING, RefSeq, UCSC, GO, HPRD, KEGG, GAD, and OMIM. Forty candidate genes that are associated with cerebrovascular disease were selected by human experts and public databases. The networked cerebrovascular disease gene maps also were developed; these maps describe genegene interactions and biological pathways. We identified 1127 genes, related indirectly to cerebrovascular disease but directly to the etiology of cerebrovascular disease. We found that a protein-protein interaction (PPI) network that was associated with cerebrovascular disease follows the power-law degree distribution that is evident in other biological networks. Not only was in silico data mining utilized, but also 250K Affymetrix SNP chips were utilized in the 320 control/disease association study to generate associated markers that were pertinent to the cerebrovascular disease as a genome-wide search. The associated genes and the genes that were retrieved from the in silico data mining system were compared and analyzed. We developed a well-curated cerebrovascular disease-associated gene network and provided bioinformatic resources to cerebrovascular disease researchers. This cerebrovascular disease network can be used as a frame of systematic genomic research, applicable to other complex diseases. Therefore, the ongoing database efficiently supports medical and genetic research in order to overcome cerebrovascular disease.

소규모 사업장 근로자에 대한 뇌.심혈관질환 예방 프로그램 적용 효과 (Effects of Cerebrovascular Disease Prevention Program Designed for Workers in Small Scale Workplaces)

  • Objectives: This study was conducted in order to prepare fundamental data and assess the short-term effects of applying cerebrovascular disease prevention programs by the Ministry of Employment and Labor and the Korea Occupational Safety and Health Agency (KOSHA) in Korea. Methods: The number of study subjects was 2,676 workers (58.5%) who were able to evaluate the level of incidence risk at pre- and post-applications of the program, among the 4,576 total workers who were enrolled in the cerebrovascular disease prevention program during 2011. The guidelines for this prevention program were adopted from KOSHA GUIDE H-1-2010. To determine the program' effectiveness, the workers'risks for cerebrovascular disease were assessed pre- and post-application of the program. Results: The blood pressure level was significantly reduced by 4.09 mmHg for the mean systolic blood pressures and 5.47 mmHg for diastolic blood pressures, respectively. The mean level of total cholesterol and BMI were also reduced significantly by 2.07 g/dl and 0.1 $0.1kg/m^2$. The rate of smoking was decreased by 4.0% and the percentage of workers engaging in regular exercise was increased by 29.8%. The level of overall cerebrovascular disease risk was reduced among 1,451 (70.7%) of 2,052 workers at low risk and above. The level of cerebrovascular disease risk was lower in the improved group for health behavior than the unimproved group (odds ratio =1.7, p < 0.001). Conclusions: The author considers that the application of the cerebrovascular disease prevention program by the Korea Occupational Safety and Health Agency reduced cerebrovascular diseases risks among workers who were enrolled in the cerebrovascular disease prevention program and it must be accompanied by an improvement in health behavior for prevention of cerebrovascular disease.

뇌졸중 환자 가족의 음주, 흡연 및 가족병력에 관한 연구 (A Study on Drinking, Smoking and Family Disease Histories in the Family Members of Cerebrovascular Disease Patients)

  • 뇌졸중은 한국인에 있어서 발병율이 높은 성인병이며 또한 음주, 흡연과 관련이 있는 질병으로 알려져 있는바, 건강한 구성원만을 가진 정상인군과 뇌졸중 환자가 있는 가족의 구성원을 대상으로(환자 자신은 제외) 음주, 흡연정도, 수입과 가족병력을 조사하여 상관관계를 규명하고자 하였다. 뇌졸중 환자 가족의 평균 음주량이 정상인 가족 보다 높게 나타났는데 이는 가족 구성원 중 30대, 40대, 50대 남자의 음주량이 높은 것에 기인하는 것으로 나타났다. 뇌졸중 환자 가족의 흡연량이 정상인 보다 높았는데 이는 주로 뇌졸중 환자 가족중 40대와 50대 남자의 높은 흡연량에 기인하는 것으로 나타났다. 가구당 월 평균 수입은 정상인 세대가 뇌졸중 환자 가족에 비해 유의적으로 높았다(정상인 가족: $673,000\pm187,000원,$ 뇌졸중 환자 가족: $483,000\pm213,000원)$ 뇌졸중 환자가족에서 역시 뇌졸중 병력이 많은 것은 유전적 소인과 더불어 음주, 흡연과 상당한 관련이 있을 것으로 사료된다.

뇌혈관질환에 대한 다빈도 처방 한약의 임상적 유효성 : 후향적 연구 (The Clinical Effectiveness of Frequently Used Herbal Medicines for Cerebrovascular Disease: A Retrospective Study)

  • Objectives: With the aging of the population, the incidence of a variety of diseases is increasing. Cerebrovascular disease is a major cause of mortality worldwide and therefore requires timely treatment and management. We aimed to evaluate the clinical effectiveness of herbal medicines in treating cerebrovascular disease by screening the most frequently prescribed herbal medicines and analyzing the clinical data of patients with cerebrovascular disease taking these medicines. Methods: In this retrospective study, we analyzed the electronic medical records of patients with cerebrovascular disease who received herbal medication at the Pusan National University Korean Medical Hospital from 10 March 2010 to 31 March 2017. The inclusion criteria were as follows: diagnosis of cerebrovascular disease, use of frequently prescribed herbal medicines, available records of laboratory tests and functional evaluation that was conducted within two weeks before and after admission and discharge, and herbal medicine usage exceeding half the period of hospitalization. Results: Of the total patients with cerebrovascular disease, the number of patients analyzed in this study was 49. There were some changes in the various variables examined after frequently used herbal medicines had been taken, such as complete blood count, liver function test, lipid test, and functional evaluation. No serious adverse events related to herbal medicine were found in this study. Conclusions: We concluded that herbal medicines frequently used for cerebrovascular disease may have some clinical effectiveness. However, because of the retrospective nature of this study and only one hospital involved, the generalizability of our findings is limited.

중년여성의 심뇌혈관질환 관련요인 및 취약군 규명 (Factors related to the Identification of Middle-Aged Women Who are Disadvantaged by Cardio-cerebrovascular Disease)

  • Purpose: To investigate factors related to cardio-cerebrovascular disease and groups disadvantaged by it in Korean middle-aged women, using the Korea National Health and Nutrition Examination Survey (KNHANES). Methods: The present study was conducted with 1,627 middle-aged women, aged 40 to 64 years, who participated in the 7th (2016) Korea National Health and Nutrition Examination Survey conducted by the Korea Centers for Disease Control and Prevention. Results: Cardio-cerebrovascular disease among middle-aged women was associated with quality of life, menopause, diabetes mellitus, body mass index, and family history of hypertension. The incidence of cardio-cerebrovascular disease in middle-aged women was found to be the most prevalent in women who have entered menopause, have a family history of hypertension, and have a body mass index greater than $30.0kg/m^2$. Conclusion: This study classifies the subjects according to the risk level of each disadvantaged group for cardio-cerebrovascular disease prevention and management in middle-aged women. The results provide evidence to support a tailored cardio-cerebrovascular disease prevention and management program based on the related factors of disadvantaged groups and to establish strategies in educational and practical aspects.

뇌혈관질환의 예측인자로서의 악력 (Grip Strength as a Predictor of Cerebrovascular Disease)

  • Background: Cerebrovascular disease is included in four major diseases and is a disease that has high rates of prevalence and mortality around the world. Moreover, it is a disease that requires a high cost for long-term hospitalization and treatment. This study aims to figure out the correlation between grip strength, which was presented as a simple, cost-effective, and relevant predictor of cerebrovascular disease, and cerebrovascular disease based on the results of a prior study. And furthermore, our study compared model suitability of the model to measuring grip strength and relative grip strength as a predictor of cerebrovascular disease to improve the quality of cerebrovascular disease's predictor. Methods: This study conducted an analysis based on the generalized linear mixed model using the data from the Korea Longitudinal Study of Ageing from 2006 to 2016. The research subjects consisted of 9,132 middle old age people aged 45 years or older at baseline with no missing information of education level, gender, marital status, residential region, type of national health insurance, self-related health, smoking status, alcohol use, and economic activity. The grip strength was calculated the average which measured 4 times (both hands twice), and the relative grip force was divided by the body mass index as a variable considering the anthropometric figure that affects the cerebrovascular disease and the grip strength. Cerebrovascular diseases, a dependent variable, were investigated based on experiences diagnosed by doctors. Results: An analysis of the association between grip strength and found that about 0.972 (odds ratio [OR], 0.972; 95% confidence interval [CI], 0.963-0.981) was the incidence of cerebral vascular disease as grip strength increased by one unit increase and the association between relative grip strength and cerebrovascular disease found that about 0.418 (OR, 0.418; 95% CI, 0.342-0.511) was the incidence of cerebral vascular disease as relative grip strength increased by unit. In addition, the model suitability of the model for each grip strength and relative grip strength was 11,193 and 11,156, which means relative grip strength is the better application to the predictor of cerebrovascular diseases, irrespective of other variables. Conclusion: The results of this study need to be carefully examined and validated in applying relative grip strength to improve the quality of predictors of cerebrovascular diseases affecting high mortality and prevalence.

한국 경찰관들의 뇌혈관 질환 요인 분석 (Factor Analysis on Cerebrovascular Disease of Korean Police Officers)

  • 본 연구는 국내 경찰관들을 대상으로 요인별 뇌혈관 질환의 요인을 파악함으로써 추후 경찰관들의 체계적이고 효율적인 자원 관리를 위한 기초자료로 이용하고자 한다. 국립경찰병원을 방문한 경찰관을 대상으로 뇌혈관 질환 진단을 위한 영상의학과 검사 중 자기공명영상검사를 시행한 경찰관을 대상으로 분석을 실시하였다. 연구 결과 나이가 많은 사람이 적은 사람 보다 2.63배 뇌혈관 질환이 높게 나타났다. 고혈압 있는 사람의 경우는 고혈압이 없는 사람 보다 1.63배 뇌혈관 질환이 높게 나타났다. 심혈관 질환이 있는 사람의 경우는 없는 경우 보다 1.95배 높았다. 이외에 음주를 하는 사람이 안 하는 사람 보다 0.54배 뇌혈관 질환이 높게 나타났으며 Homocysteine 수치에서는 비정상인 사람이 정상인 보다 0.32배 뇌혈관 질환이 높게 나타났다.

Association of milk and dairy product consumption with the incidence of cardio-cerebrovascular disease incidence in middle-aged and older Korean adults: a 16-year follow-up of the Korean Genome and Epidemiology Study

  • BACKGROUND/OBJECTIVES: Unhealthy dietary behaviors constitute one of risk the factors for chronic and cardiovascular diseases, which are prevalent in middle-aged and older populations. Milk and dairy products are high-quality foods and important sources of calcium. Calcium protects against osteoporosis and cardiovascular disease. Therefore, this study investigated the association of milk and dairy product consumption with cardio-cerebrovascular disease incidence in middle-aged and older Korean adults. SUBJECTS/METHODS: Data were derived from the Ansan-Anseong cohort study, and a total of 8,009 individuals aged 40-69 years were selected and followed up biennially. Cox proportional hazard models were used to examine the association of milk and dairy product consumption with cardio-cerebrovascular disease incidence. RESULTS: During a mean follow-up period of 96.5 person-months, 552 new cases of cardio-cerebrovascular disease were documented. Milk consumers (< 1 serving/day) exhibited a 23% lower risk of cardio-cerebrovascular disease incidence than non-milk consumers (hazard ratio [HR], 0.77; 95% confidence interval [CI], 0.61-0.97; P for trend = 0.842). High yogurt consumption was associated with a 29% lower incidence risk (≥ 0.5 servings/day vs. none: HR, 0.71; 95% CI, 0.53-0.96; P for trend = 0.049), whereas high ice cream consumption was associated with a 70% higher risk of cardio-cerebrovascular disease incidence (≥ 0.5 servings/day vs. none: HR, 1.70; 95% CI, 1.01-2.88; P for trend = 0.070). CONCLUSIONS: This study indicates that less than one serving of milk and high yogurt consumption are associated with a lower cardio-cerebrovascular disease risk in the middle-aged and older populations.

중풍 후 운동장애에 대한 지황음자(地黃飮子) 투여 2례 (Two Cases of Motor Impairment after Cerebrovascular Disease by Jihwangeumja)

  • Cerebrovascular disease causes various symptoms like hemiparalysis, facial palsy, dysphagia, dysphasia etc. Although patients take lots of time to treatment, there are many cases symptoms could not be cured completely and patients could get sequelae. Jihwangeumja(dihuangyinzi) is used the cerebrovascular disease with dysphasia and motor impairment of the lower extremities due to weakness of kidney. The purpose of this study was to investigate the effect of Jihwangeumja on lower extremity weakness caught by cerebrovascular disease. In this study, we administered Jihwangeumja to two stroke patients with lower extremity weakness and analyzed the clinical progresses. As a result of the treatment, patients showed improved symptoms.

뇌졸중 환자 가족과 정상인에 있어서 혈압과 Na, K 섭취경향간의 상관관계 연구 (A Study on Correlation between Blood Pressure and Dietary Na, K Intakes Pattern in the Family Members of Normal and Cerebrovascular Disease Patients)

  • Purpose of this study was to investigate correlation between blood pressure (systolic and diastolic) and dietary sodium, potassium intake pattern in the family members of normal cerebrovascular (CVA) disease, excluding patients themselves. Both mean values of systolic (125.8$\pm$23.7 vs 119.3$\pm$19.2mmHg) and diastolic(76.1$\pm$16.7 vs 71.6$\pm$12.5mmHg) bllood pressure in the family members of cerebrovascular disease patients were significantly higher than those of normal subjects. Systolic blood pressure was positively correlated with age, weibght, sodium in soybean paste, potassium in hotpepepr paste, soybean paste and meats in normal subjects group. In the family members of cerebrovascular patient, systolic blood pressure was possively correlated with age, weight, sodium in soy sauce, drinking water and potassium in soups. Interestingly, table salt intake was positively correlated with systosolic blood pressure in the family members of cerebrovascular disease patients. Diastolic bolld pressure was positively correlated with age, weight, table salt intake potassium in hotpepper paste and soybean paste in normal subjects group. Diastolic blood pressure was positively correlated with age, weight and table salt intake in the family members of cerebrovascular disease patients. Urinary potassium excretion was negatively correlated with both systolic and diastolic blood pressure in the family members of cerebrovascular disease patients.
