Requirements for Cerebrovascular Surgery in Comprehensive Stroke Centers in South Korea

  • Objective : Cerebrovascular disease (CVD) was the third most common cause of death in South Korea in 2014. Evidence from abroad suggests that comprehensive stroke centers play an important role in improving the mortality rate of stroke. However, surgical treatment for CVD is currently slightly neglected by national policy, and there is still regional imbalance in this regard. For this reason, we conducted a survey on the necessity of, and the requirements for, establishing regional comprehensive cerebrovascular surgery centers (CCVSCs). Methods : This investigation was performed using the questionnaire survey method. The questionnaire was consisted with two sections. The first concerned the respondent's opinion regarding the current status of demand and the regional imbalance of cerebrovascular surgery in South Korea. The second section asked about the requirements for establishing regional CCVSCs. We sent the questionnaire to 100 board members of the Korean Society of Cerebrovascular Surgeons. Results : Most experts agreed that cerebrovascular surgery patients were concentrated in large hospitals in the capital area, and 83.6% of respondents agreed that it was necessary to alleviate the regional imbalance of cerebrovascular surgery. With regards to personnel, over 90% of respondents answered that at least two neuro-vascular surgeons and two neuro-interventionists are necessary to establish a CCVSC. Regarding facilities, almost all respondents stated that each CCVSC would require a neuro-intensive care unit and hybrid operating room. The survey asked the respondents about 13 specific neurovascular surgical procedures and whether they were necessary for a regional CCVSC. In the questions about the necessity of cerebrovascular surgical equipment, all seven pieces of equipment were considered essential by all respondents. A further five pieces of equipment were considered necessary on site: computed tomographic angiography, magnetic resonance angiography, conventional angiography, surgical microscope, and surgical navigation. Our results may provide a basis for future policy regarding treatment of cerebrovascular disease, including surgery. Conclusion : Raising the comprehensiveness of treatment at a regional level would lower the national disease burden. Policies should be drafted regarding comprehensive treatment including surgery for cerebrovascular disease, and related support plans should be implemented.

한국 경찰관들의 뇌혈관 질환 요인 분석 (Factor Analysis on Cerebrovascular Disease of Korean Police Officers)

  • 본 연구는 국내 경찰관들을 대상으로 요인별 뇌혈관 질환의 요인을 파악함으로써 추후 경찰관들의 체계적이고 효율적인 자원 관리를 위한 기초자료로 이용하고자 한다. 국립경찰병원을 방문한 경찰관을 대상으로 뇌혈관 질환 진단을 위한 영상의학과 검사 중 자기공명영상검사를 시행한 경찰관을 대상으로 분석을 실시하였다. 연구 결과 나이가 많은 사람이 적은 사람 보다 2.63배 뇌혈관 질환이 높게 나타났다. 고혈압 있는 사람의 경우는 고혈압이 없는 사람 보다 1.63배 뇌혈관 질환이 높게 나타났다. 심혈관 질환이 있는 사람의 경우는 없는 경우 보다 1.95배 높았다. 이외에 음주를 하는 사람이 안 하는 사람 보다 0.54배 뇌혈관 질환이 높게 나타났으며 Homocysteine 수치에서는 비정상인 사람이 정상인 보다 0.32배 뇌혈관 질환이 높게 나타났다.

뇌졸중 환자 가족과 정상인에 있어서 혈압과 Na, K 섭취경향간의 상관관계 연구 (A Study on Correlation between Blood Pressure and Dietary Na, K Intakes Pattern in the Family Members of Normal and Cerebrovascular Disease Patients)

  • Purpose of this study was to investigate correlation between blood pressure (systolic and diastolic) and dietary sodium, potassium intake pattern in the family members of normal cerebrovascular (CVA) disease, excluding patients themselves. Both mean values of systolic (125.8$\pm$23.7 vs 119.3$\pm$19.2mmHg) and diastolic(76.1$\pm$16.7 vs 71.6$\pm$12.5mmHg) bllood pressure in the family members of cerebrovascular disease patients were significantly higher than those of normal subjects. Systolic blood pressure was positively correlated with age, weibght, sodium in soybean paste, potassium in hotpepepr paste, soybean paste and meats in normal subjects group. In the family members of cerebrovascular patient, systolic blood pressure was possively correlated with age, weight, sodium in soy sauce, drinking water and potassium in soups. Interestingly, table salt intake was positively correlated with systosolic blood pressure in the family members of cerebrovascular disease patients. Diastolic bolld pressure was positively correlated with age, weight, table salt intake potassium in hotpepper paste and soybean paste in normal subjects group. Diastolic blood pressure was positively correlated with age, weight and table salt intake in the family members of cerebrovascular disease patients. Urinary potassium excretion was negatively correlated with both systolic and diastolic blood pressure in the family members of cerebrovascular disease patients.

Association of milk and dairy product consumption with the incidence of cardio-cerebrovascular disease incidence in middle-aged and older Korean adults: a 16-year follow-up of the Korean Genome and Epidemiology Study

  • BACKGROUND/OBJECTIVES: Unhealthy dietary behaviors constitute one of risk the factors for chronic and cardiovascular diseases, which are prevalent in middle-aged and older populations. Milk and dairy products are high-quality foods and important sources of calcium. Calcium protects against osteoporosis and cardiovascular disease. Therefore, this study investigated the association of milk and dairy product consumption with cardio-cerebrovascular disease incidence in middle-aged and older Korean adults. SUBJECTS/METHODS: Data were derived from the Ansan-Anseong cohort study, and a total of 8,009 individuals aged 40-69 years were selected and followed up biennially. Cox proportional hazard models were used to examine the association of milk and dairy product consumption with cardio-cerebrovascular disease incidence. RESULTS: During a mean follow-up period of 96.5 person-months, 552 new cases of cardio-cerebrovascular disease were documented. Milk consumers (< 1 serving/day) exhibited a 23% lower risk of cardio-cerebrovascular disease incidence than non-milk consumers (hazard ratio [HR], 0.77; 95% confidence interval [CI], 0.61-0.97; P for trend = 0.842). High yogurt consumption was associated with a 29% lower incidence risk (≥ 0.5 servings/day vs. none: HR, 0.71; 95% CI, 0.53-0.96; P for trend = 0.049), whereas high ice cream consumption was associated with a 70% higher risk of cardio-cerebrovascular disease incidence (≥ 0.5 servings/day vs. none: HR, 1.70; 95% CI, 1.01-2.88; P for trend = 0.070). CONCLUSIONS: This study indicates that less than one serving of milk and high yogurt consumption are associated with a lower cardio-cerebrovascular disease risk in the middle-aged and older populations.

일개 대학병원을 대상으로 한 뇌혈관질환의 시간적 변동양상 (Temporal Variations of Cerebrovascular Diseases in a University Hospital)

  • 연구배경: 뇌혈관 질환은 지역 및 인종에 따라 발생양상의 차이를 보이는 것으로 알려져 있다. 그러므로 뇌혈관질환의 진단, 치료, 및 예방적인 목적에 활용하기 위한 특정지역의 발생양상을 파악하는 것은 매우 중요하다. 본 논문에서는 계절별, 성별, 연령별, 출혈성 뇌혈관질환과 허혈성뇌혈관질환 발생양상을 정량적으로 분석하고자 한다. 방 법: 연구대상은 일개 대학교병원 신경외과와 신경과에 출혈성 뇌혈관질환과 허혈성뇌혈관질환으로 입원한 환자 1603명을 대상으로 하였다. 연구대상 환자에 대해서 뇌혈관질환의 유형, 발생연도, 성별, 뇌혈관질환이 발생한 계절을 조사하였다. 자료의 산술적인 분석은 마이크로소프트사의 엑셀 2003을 이용하였고, t-test는 MICROCAL사에서 제공하는 ORIGIN 6.0을 이용하였다. 결 과: 연구대상 기간인 4년 동안의 출혈성 뇌혈관질환과 허혈성뇌혈관질환의 발생비는 38.55%, 61.45%이었다. 출혈성 뇌혈관질환의 발병 연령의 평균과 표준편차 $58.20{\pm}12.60$이었고, 40대, 50대, 60대, 70대에서 각각 20% 정도로 비슷하게 발병하였다. 이에 비하여 허혈성뇌혈관질환은 발병 연령의 평균과 표준편차는 $65.01{\pm}13.59$이었고, 출혈성뇌질환에 비하여 평균 연령이 높게 나타났고, 발생 연령분포에서도 60대 15.53%, 70대 37.06%, 80대 27.72%이었다. 출혈성 뇌혈관질환이 발병이 많이 나타난 계절은 겨울, 여름, 봄, 가을 순이었고, 출혈성 뇌혈관질환이 발병이 많이 나타난 계절은 봄, 여름, 겨울, 가을이었다. 일반적으로 알려진 겨울철 출혈성 뇌혈관질환 많이 나타났다. 출혈성 뇌혈관질환과 허혈성뇌혈관질환 환자의 남녀비율은 52.09% 47.91%로 나타났다. 결 론: 연구대상을 출혈성 뇌혈관질환과 허혈성뇌혈관질환으로 일개 대학교병원에 입원한 환자만을 대상으로 하였으므로 전체 뇌혈관 질환에 대한 결과를 대변한다고 볼 수 없다. 그러나 의료종사자에게서 일반적으로 알려진 계절별, 성별, 연령별, 출혈성 뇌혈관질환과 허혈성뇌혈관질환 발생비를 통계적으로 분석하였다는데 연구논문의 의의가 있다. 본 논문에서 출혈성 뇌혈관질환과 허혈성뇌혈관질환의 발생률은 38.55%, 61.45%로 허혈성뇌혈관질환의 비중이 높아지고 있다. 적절한 뇌혈관질환의 예방 및 질환관리계획을 세우기 위해서는 장기적인 뇌혈관질환의 역학조사가 필요할 것이다.

MRI와 MRA를 이용한 허혈성 뇌혈관 질환의 뇌혈관별 분포에 대한 연구 (Intracerebral Regional and Vasculature-Specific Distributions of Ischemic Cerebrovascular Diseases: Using MRI and MRA)

  • 본 연구에서는 MRI와 MRA를 이용하여 우리나라 사람의 뇌혈관 질환 중 뇌혈관의 파열에 의한 뇌혈관 질환을 제외하고 뇌혈관이 좁아지거나 막힌 허혈성 뇌혈관 질환의 분포(호발부위)를 분석하여 임상에서 활용할 수 있도록 하였다. 뇌혈관이 좁아지거나 막힌 허혈성 뇌혈관 질환만의 분포를 분석한 이유는 뇌혈관의 파열에 의한 출혈성 뇌혈관 질환과 뇌경색(brain infarction) 등 허혈성 뇌혈관 질환의 발생학적 기전(mechanism)이 다르기 때문이며 그 결과는 아래와 같다. 1. 연구대상자 총 626명 중 성별로는 남자가 55.0%로 여자 45.0%보다 높은 분포를 보였다. 2. 위치별 허혈성 뇌혈관 질환의 분포에서 우측 뇌혈관이 37.5%로 가장 많았으며, 좌측 뇌혈관 35.1%, 양쪽 뇌혈관 27.3% 순으로 나타났다. 3. 허혈성 뇌혈관 질환의 뇌혈관 별 분포는 ICA가 38.9%로 가장 많았으며, 다음으로 MCA 35.7%, PCA 13.4%, ACA 6.0%, VA 3.3% 순으로 나타났다. 4. A-com에 질환이 있는 경우는 한 명도 없었으며 P-com 에도 남자 1명만이 발생했다. 5. MCA에는 여자가 54.6%로 남자 42.2%보다 많아, 성별에 따라 통계적으로 유의미한 차이를 보였다($x^2$=9.64, p < .01). 6. ICA에서는 남자가 56.4%로 여자 46.8%보다 많아 통계적으로 유의미한 차이를 보였다($x^2$=5.71, p < .05). 7. BA에서도 남자가 2.3%로 여자 0.4%보다 많아 성별에 따라 유의미한 차이를 보였다($x^2$=4.25, p < .05). 8. 연령에 따른 혈관별 질환의 분포에 대한 차이는 없었으며 협착과 폐색의 분포에서는 폐색보다 협착이 많았다.

뇌졸중 환자에 적용한 핫라인 전화상담 프로그램의 효과 (Effects of Telephone Hotline Counseling Program on Stroke Care)

  • Background: This study focuses on the establishment and operation of a stroke patient hotline program to help patients and their caregivers determine when acute neurological changes require emergency attention. Method: The stroke hotline was established at the Gyeonggi Regional Cerebrovascular Center, Seoul National University Bundang Hospital, in June 2016. Patients diagnosed with stroke during admission or in outpatient clinics were registered and provided with stroke education. Consulting nurses managed hotline calls and made decisions about outpatient schedules or emergency room referrals, consulting physicians when necessary. The study analyzed consultation records from June 2016 to December 2020, assessing consultation volumes and types. Outcomes and hotline satisfaction were also evaluated. Results: Over this period, 6,851 patients were registered, with 1,173 patients (18%) undergoing 3,356 hotline consultations. The average monthly consultation volume increased from 29.2 cases in 2016 to 92.3 cases in 2020. Common consultation types included stroke symptoms (22.3%), blood pressure/glucose inquiries (12.8%), and surgery/procedure questions (12.6%). Unexpected outpatient visits decreased from 103 cases before the hotline to 81 cases after. Among the 2,244 consultations between January 2019 and December 2020, 9.6% were recommended hospital visits, with two cases requiring intra-arterial thrombectomy. Patient satisfaction ratings of 9-10 points increased from 64% in 2019 to 69% in 2020. Conclusion: The stroke hotline program effectively reduced unexpected outpatient visits and achieved high patient satisfaction. Expanding the program could enhance the management of stroke-related neurological symptoms and minimize unnecessary healthcare resource utilization.

뇌혈관질환에서 SPECT와 PET의 임상적 응용 (Clinical Application of SPECT and PET in CerebroVascular Disease)

  • Single photon emission computed tomography(SPECT) and positron emission tomography(PET) are modern imaging techniques that allow for both qualitative and quantitative assessment of hemodynamic changes in cerebrovascular diseases. SPECT has been becoming an indispensable method to investigate regional cerebral blood flow because equipment and isotope are easily available in most general hospitals. Acetazolamide stress SPECT has also been proved to be useful to evaluate the cerebrovascular reserve of occlusive cerebrovascular diseases and to select surgical candidate. PET has gained wide spread clinical use in the evaluation of the hemodynamic and metabolic consequences of extracranial or intracranial arterial obstructive disease despite its complexity and limited availability. PET has been established as an invaluable tool in the pathophysilogy investigation of acute ischemic stroke. The potentials, limitations, and clinical applications of SPECT and PET in various cerebrovascular diseases will be discussed in this article with reviews of literatures.

쥐 전체 뇌의 단면 이미지에서 뇌혈관의 구조 재현 및 정량적 측정 기법에 관한 연구 (A Study on the Reconstruction and Quantitative Measurement Method of Cerebrovascular Structure in Cross-sectioned Images of the Whole Mouse Brain)

  • Cerebrovascular disease is a common disease in the elderly population. However, we do not have enough understanding of brain-related diseases. Recent advances in microscopy technology have resulted in the acquisition of vast amounts of image data sets for small organs, and it has become possible to handle vast amounts of image data sets due to improved computer performance and software technology. In this paper, the author proposes introduce a method for classifying and analysing only cerebrovascular information in the mouse brain image, as well as a quantitative measure of the portion of the cerebrovascular in the mouse brain. The study of the cerebrovascular structure is significant, and it can be helpful to improve the understanding of cerebrovasculature. As a result, the author expects that this study will be useful for neuroscientists conducting clinical research.

중풍 후 운동장애에 대한 지황음자(地黃飮子) 투여 2례 (Two Cases of Motor Impairment after Cerebrovascular Disease by Jihwangeumja)

  • Cerebrovascular disease causes various symptoms like hemiparalysis, facial palsy, dysphagia, dysphasia etc. Although patients take lots of time to treatment, there are many cases symptoms could not be cured completely and patients could get sequelae. Jihwangeumja(dihuangyinzi) is used the cerebrovascular disease with dysphasia and motor impairment of the lower extremities due to weakness of kidney. The purpose of this study was to investigate the effect of Jihwangeumja on lower extremity weakness caught by cerebrovascular disease. In this study, we administered Jihwangeumja to two stroke patients with lower extremity weakness and analyzed the clinical progresses. As a result of the treatment, patients showed improved symptoms.
