Glutathione S-transferase polymorphisms and traditional classification in Korean population with cerebrovascular disease

  • Glutathione S-transferase polymorphisms (GST) were examined in 98 cases with cerebrovascular disease (CVD) to test the hypothesis that GST polymorphisms confer a risk to an individual to develop CVD. Tobacco smoke is a major cause of both cancer and vascular disease. We therefore were stratified the subjects with CVD for smoking status, and then examined whether polymorphisms in this detoxification enzyme gene, GST, influence risk of CVD. Neither GSTM1 nor GSTT1 genotypes in the CVD group was significantly different from the control group (n=230), even in smokers. We attempted the combined analyses for GSTM1 and GSTT1 genotypes in CVD for smoking status. No significant association observed between the combined genotypes and CVD. We also classified the subjects and control group into four types according to Sasang Constitutional Medicine, Korean Traditional Oriental Medicine, and investigated the association among GST genotypes, CVD, and Sasang constitutional classification. Our observations do not confirm the effect of the GSTM1 and GSTT1 genotypes as a risk factor for CVD, even in smokers. Furthermore, we first attempted to evaluate the efficacy of Sasang Constitutional Medicine, and to find an association with CVD.

  • The purpose of this study is to refort the effect of oriental demonstration treatments for the incontinence of urine and stool by cerebrovascular disease. He was treated with herbal medication, acupunture, moxibustion and physical therapy. Through oriental demonstration treatments, Improvement in incontinence of urine and stool by cerebrovascular disease, therefore reported here. The study suggests that therapeutic value of oriental demonstration treatments for the incontinence of urine and stool by cerebrovascular disease will De higher if more clinical studies and researching more cases are accomplished.

뇌혈관질환 후 실어증의 한의학적 치료에 대한 국내 임상연구에 대한 고찰 (Review of Clinical Research about the Treatment of Aphasia after Cerebrovascular Disease)

  • Objectives: The aim of this review is to investigate clinical studies on Oriental medicine treatment for aphasia after cerebrovascular disease. Methods: Using the keywords 'Aphasia', 'Oriental medicine', 'Stroke' 'Cerebral infarction', 'Cerebral hemorrhage', and 'Clinic', we searched domestic databases, including "NDSL (National Discovery for Science Leaders)", "Korean Traditional Knowledge Portal", "OASIS (Oriental Medicine Advanced Searching Integrated System)", and "RISS (Research Information Sharing Service)". Each keyword was not searched individually, but combined in various ways. To investigate recent trends, we limited our search to papers published after 2000. Papers that did not include a specific treatment method or did not match the subject "Aphasia after stroke" were excluded. Results: Using the searching method, 13 studies were found. Of these, 12 studies were in the form of case reports, while one was in the form of a non-randomized controlled trial. These studies showed positive results for the use of Oriental medicine in terms of the Korean version of the Western Aphasia Battery (K-WAB), the evaluation form on functional performance capability and accuracy of articulatory organs developed by Lee, aphasia screening test refered in 'Assessment in Speech-Language Pathology' and adapted properly to Korean, the Communicative Ability in Daily Living Test (CADLT), the Korean Version-Boston Naming Test (K-BNT), and language assessment items included in CNS, and NIHSS. Conclusions: Of the 12 case reports, 11 studies showed positive results of the use of Oriental medicine for treatment of aphasia after cerebrovascular disease. However, more sophisticated and large-scale clinical research on aphasia should be conducted.

  • Objectives: This study investigated the effect of complex Korean medicine treatment on decreased mental status in a progressive supranuclear palsy patient with a cerebrovascular accident. Case presentation: The patient was treated with Korean medicine composed of herbal medicine (modified Guibi-tang), acupuncture, moxibustion, and cupping combined with Western medicine (a dopamine precursor and an anticholinergic) and physical therapy. The Glasgow Coma Scale, Mini-Mental State Examination, and Hoehn and Yahr Scale were used to evaluate the effects of the treatments. After treatment, Glasgow Coma Scores increased from 9-10 to 12-13, and Mini-Mental State Examination for dementia screen scores increased from 1 to 2. The Hoehn and Yahr score remained at 5. There were no side effects of the treatment. Conclusion: This study suggests that complex Korean medicine treatments may be effective for the decreased mental status of a progressive supranuclear palsy patient with a cerebrovascular accident, but further studies are needed.

Cerebrovascular Events in Pediatric Inflammatory Bowel Disease: A Review of Published Cases

  • Pediatric inflammatory bowel disease (PIBD) is a multisystem disorder characterized by intestinal and extraintestinal manifestations and complications. Cerebrovascular events (CVE) are rare extraintestinal complications in patients with PIBD. Statistics show that 3.3% patients with PIBD and 1.3-6.4% adult patients with inflammatory bowel disease (IBD) experience CVE during the course of the disease. Therefore, this study aimed to review the records of children with IBD who developed CVE during the course of the disease. We retrospectively reviewed 62 cases of PIBD complicated by CVE. The mean patient age at the time of thrombotic events was 12.48±4.13 years. The incidence of ulcerative colitis was significantly higher than that of Crohn's disease (43 [70.5%] vs. 13 [21.3%] patients). Most patients (87.93%) were in the active phase of IBD at the time of CVE. The mean time interval between the onset of IBD and CVE was 20.84 weeks. Overall, 11 (26.83%) patients showed neurological symptoms of CVE at disease onset. The most frequent symptom on admission was persistent and severe headaches (67.85%). The most common site of cerebral venous thrombosis was the transverse sinuses (n=23, 53.48%). The right middle cerebral artery (n=3, 33.34%) was the predominant site of cerebral arterial infarction. Overall, 41 (69.49%) patients who were mostly administered unfractionated heparin or low-molecular-weight heparin (56.09%) recovered completely. Patients with IBD are at a risk of thromboembolism. CVE may be the most common type of thromboembolism. Based on these findings, the most common risk factor for CVE is IBD flares. In patients with CVE, anticoagulant therapy with heparin, followed by warfarin, is necessary.

Ginsenoside Rg1 ameliorates chronic intermittent hypoxia-induced vascular endothelial dysfunction by suppressing the formation of mitochondrial reactive oxygen species through the calpain-1 pathway

  • Background: As the major pathophysiological feature of obstructive sleep apnea (OSA), chronic intermittent hypoxia (CIH) is vital for the occurrence of cardiovascular complications. The activation of calpain-1 mediates the production of endothelial reactive oxygen species (ROS) and impairs nitric oxide (NO) bioavailability, resulting in vascular endothelial dysfunction (VED). Ginsenoside Rg1 is thought to against endothelial cell dysfunction, but the potential mechanism of CIH-induced VED remains unclear. Methods: C57BL/6 mice and human coronary artery endothelial cells (HCAECs) were exposed to CIH following knockout or overexpression of calpain-1. The effect of ginsenoside Rg1 on VED, oxidative stress, mitochondrial dysfunction, and the expression levels of calpain-1, PP2A and p-eNOS were detected both in vivo and in vitro. Results: CIH promoted VED, oxidative stress and mitochondrial dysfunction accompanied by enhanced levels of calpain-1 and PP2A and reduced levels of p-eNOS in mice and cellular levels. Ginsenoside Rg1, calpain-1 knockout, OKA, NAC and TEMPOL treatment protected against CIH-induced VED, oxidative stress and mitochondrial dysfunction, which is likely concomitant with the downregulated protein expression of calpain-1 and PP2A and the upregulation of p-eNOS in mice and cellular levels. Calpain-1 overexpression increased the expression of PP2A, reduced the level of p-eNOS, and accelerated the occurrence and development of VED, oxidative stress and mitochondrial dysfunction in HCAECs exposed to CIH. Moreover, scavengers of O2·-, H2O2, complex I or mitoKATP abolished CIH-induced impairment in endothelial-dependent relaxation. Conclusion: Ginsenoside Rg1 may alleviate CIH-induced vascular endothelial dysfunction by suppressing the formation of mitochondrial reactive oxygen species through the calpain-1 pathway.

Comparing Korea Occupational Safety & Health Agency and National Health Insurance Service's cardio-cerebrovascular diseases risk-assessment tools using data from one hospital's health checkups

  • Background: Cardio-cerebrovascular diseases (CVDs) are the most common cause of death worldwide. Various CVD risk assessment tools have been developed. In South Korea, the Korea Occupational Safety & Health Agency (KOSHA) and the National Health Insurance Service (NHIS) have provided CVD risk assessments with health checkups. Since 2018, the KOSHA guide has stated that NHIS CVD risk assessment tool could be used as an alternative of KOSHA assessment tool for evaluating CVD risk of workers. The objective of this study was to determine the correlation and agreement between the KOSHA and the NHIS CVD risk assessment tools. Methods: Subjects of this study were 17,485 examinees aged 20 to 64 years who had undergone medical examinations from January 2021 to December 2021 at a general hospital. We classified subjects into low-risk, moderate-risk, high-risk, and highest-risk groups according to KOSHA and NHIS's CVD risk assessment tools. We then compared them with cross-analysis, Spearman correlation analysis, and linearly weighted kappa coefficient. Results: The correlation between KOSHA and NHIS tools was statistically significant (p-value < 0.001), with a correlation coefficient of 0.403 and a kappa coefficient of 0.203. When we compared risk group distribution using KOSHA and NHIS tools, CVD risk of 6,498 (37.1%) participants showed a concordance. Compared to the NHIS tool, the KOSHA tool classified 9,908 (56.7%) participants into a lower risk category and 1,079 (6.2%) participants into a higher risk category. Conclusions: In this study, KOSHA and NHIS tools showed a moderate correlation with a fair agreement. The NHIS tool showed a tendency to classify participants to higher CVD risk group than the KOSHA tool. To prevent CVD more effectively, a higher estimation tool among verified CVD risk assessment methods should be selected and managements such as early intervention and treatment of risk factors should be performed targeting the high-risk group.

뇌혈관 질환 검사를 위한 뇌혈관 조영술(TFCA)과 뇌혈관 전산화 단층 촬영 검사(CCTA)의 입사표면선량(ESD) 및 조영제 사용량 분석 (Analysis of the Entrance Surface Dose (ESD) and Contrast Usage of Trance Femoral Cerebral Angiography (TFCA) and Cerebral Computed Tomographic Angiography (CCTA) for Cerebrovascular Disease Examining)

  • 혈관 질환을 검사하기 위한 방법으로 방사선 장비를 이용한 검사들이 주를 이루기 때문에 본 연구를 통해 뇌혈관 질환 검사에 있어 혈관 질환 검사에 사용되는 뇌혈관 조영술과 뇌혈관 전산화단층촬영검사의 입사표면선량(ESD; Entrance Surface Dose)을 비교 분석하여 뇌혈관 질환 검사 시 사용된 선량 결과에 따른 최적의 검사 방법 선택 유도 및 선량 저감화 방안에 대해 알아보고자 하였으며, 조영제 사용량을 측정 및 평가하여 조영제로 인한 부작용 발생 우려 시 권장 할 수 있는 검사선택 방법에 대해 알아보고자 연구를 진행하게 되었다. 대상으로는 2018년 6월부터 2018년 12월까지 여수지역 병원에서 뇌혈관 전산화 단층 촬영 검사를 시행한 70명 (남43, 여27)과 2018년 6월부터 2018년 11월까지 평택지역 병원에서 뇌혈관 조영술을 시행한 61 (남34, 여27)명을 대상으로 하였고, 분석 방법으로는 입사표면선량 데이터 값을 M-view와 PACS PLUS를 통해 후향적으로 획득하였으며 조영제 측정은 실제 사용된 량을 측정하는 방법으로 진행하였다. SPSS를 이용한 T-검정 분석결과 뇌혈관 조영술의 선량이 $245.74{\pm}71.91mGy$로 전산화 단층 촬영검사의 선량 $277.79{\pm}79.65mGy$보다 $32.05{\pm}7.74mGy$만큼 낮았으며 t=3.249, p=0.017로 통계적으로 유의했고(p<0.05) 조영제 총 사용량 비교 분석 결과에선 뇌혈관 조영술 시 사용된 평균 조영제 사용량이 $55.05{\pm}17.68ml$로 전산화 단층 촬영 검사에서 사용된 70 ml의 조영제 양보다 약 14.95 ml만큼 적었으며 t=-4.548, p<0.001로 통계적으로 유의했다. 결론적으로 뇌혈관 조영술의 선량이 전산화 단층 촬영검사보다 통계적으로 유의하게 낮았고, 조영제 사용량 또한 전산화 단층 촬영검사보다 유의할 만큼 적었으므로 뇌혈관 질환 검사에 있어 뇌혈관 조영술의 활용을 늘리는 방안이 피폭선량 저감화를 위한 방법임과 동시에 조영제 사용량을 감소시킬 수 있는 방안이라 생각된다.

뇌혈관 질환자의 기능 회복과 체성감각 유발전위의 변화 (Changes of Somatosensory Evoked Potential and Functional Recovery in Patients of Cerebrovascular disease)

  • This study was designed to test the effects of comprehensive rehabilitation management on functional recovery after attack of cerebrovascular disease. 16 cerebrovascular disease patients applied comprehensive rehabilitation management of physical therapy at department of physical Therapy, C medical center in Gwang-ju. The collection of the data had been executed for 4months(April 15, 2007${\sim}$July 15, 2007). For evaluating, Functional Independence measure(FIM) and Somatosensory Evoked Potential(SSEP) were used to assess functional recovery. The results of this study were as follows: 1. In the comparison of latency of median nerve SSEP before and after treatment, the lat. N20 and P25 increased, the ampl. P25/N20 was decreased. In the comparison difference data of median nerve SSEP, however there was no significant difference in the group(p>0.05). 2. In the comparison of latency of Post Tibial nerve SSEP before and after treatment, the lat. P40, P50, P60 increased, the ampl. P40 was decreased. In the comparison difference data of post tibial nerve SSEP, however there was no significant difference in the group(p>0.05). 3. In the comparison of FIM scores of Self-care, Sphincter Control, Mobility: Transfer, Locomotion before and after treatment, the scores of FIM was significantly increased. In the comparison of difference of the motor part of FIM, however there was significant difference(p<0.05). 4. In the comparison of FIM scores of Communication, Social Cognition before and after treatment, the scores of FIM was significantly increased. In the comparison of difference of the motor part of FIM, however there was no significant difference in the group(p>0.05). Based on these results, it is concluded that the comprehensive rehabilitation management for cerebrovascular disease case was not significant difference in the SSEP, was significant difference in the motor part of FIM. Further study should be done to analyze the effect of intervention duration of treatment, optimal time to apply the treatment in more long period.

뇌혈관질환과 관상동맥성 심질환의 위험요인에 관한 환자-대조군 연구 (The Case-Control Study on the Risk Factors of Cerebrovascular Diseases and Coronary Heart Diseases)

  • Cerebrovascular disease and coronary heart disease are the first and the fourth common causes of death among adults in Korea. Reported risk factors of these diseases are mostly alike. But some risk factors of one of these diseases may prevent other diseases. Therefore, we tried to compare and discriminate the risk factors of these diseases. We recruited four case groups and four control groups among the inpatients who were admitted to Wonju Christian Hospital from March, 1994 to November, 1995. Four control groups were matched with each of four case groups by age and sex. The number of patients in each of four case and control groups were 106 and 168 for acute myocardial infarction(AMI), 84 and 133 for subarachnoid hemorrhage(SAH), 102 and 148 for intracerebral hemorrhage(ICH), and 91 and 182 for ischemic stroke(IS) respectively. Factors whose levels were significantly higher in AMI and IS than in responding control group (RCG) were education, economic status, and triglyceride. Factors whose levels were significantly lower in hemorrhagic stroke than in RCG were age of monarch, and prothrombin time. The factor whose level was higher in AMI than ill RCG was uric acid. The factor whose level was higher in AMI, ICH, and SAM than in RCG was blood sugar. Factors whose levels were significantly higher in all the case groups than in RCG were earlobe crease, Quetelet index, white blood cell count, hemoglobin, hematocrit, and total cholesterol. The list of risk factors were somewhat different among the four diseases, though none of the risk factors to the one disease except prothrombin time acted as a preventive factor to the other diseases. The percent of grouped cases correctly classified was higher in the discrimination of ischemic diseases(AMI and IS) from hemorrhagic diseases(SAM and ICH) than in the discrimination of cerebrovascular disease from AMI. The factors concerned in the discrimination of ischemic diseases from hemorrhagic diseases were prothrombin time, earlobe crease, gender, age, uric acid, education, albumin, hemoglobin, the history of taking steroid, total cholesterol, and hematocrit according to the selection order through forward selection.
