글루타티온 S-전환효소 다형성과 뇌혈관질환(腦血管疾患) 및 사상체질 사이의 연관성(聯關性)에 관한 연구 (Interrelationships among Glutathione S-Transferase Polymorphisms, Cerebrovascular Disease and Sasang Constitution)

  • Glutathione S-transferase polymorphisms (GST) were examined in 98 cases with cerebrovascular disease (CVD) to test the hypothesis that GST polymorphisms confer a risk to an individual to develop CVD. Tobacco smoke is a major cause of both cancer and vascular disease. We therefore were stratified the subjects with CVD for smoking status, and then examined whether polymorphisms in this detoxification enzyme gene, GST, influence risk of CVD. Neither GSTM1 nor GSTT1 genotypes in the CVD group was significantly different from the control group (n=230), even in smokers. We attempted the combined analyses for GSTM1 and GSTT1 genotypes in CVD for smoking status. No significant association observed between the combined genotypes and CVD. We also classified the subjects and control group into four types according to Sasang Constitutional Medicine, Korean Traditional Oriental Medicine, and investigated the association among GST genotypes, CVD, and Sasang constitutional classification. Our observations do not confirm the effect of the GSTM1 and GSTT1 genotypes as a risk factor for CVD, even in smokers. Furthermore, we first attempted to evaluate the efficacy of Sasang Constitutional Medicine, and to find an association with CVD.

Extracranial-Intracranial Bypass Surgery Using a Radial Artery Interposition Graft for Cerebrovascular Diseases

  • Objective : To investigate the efficacy of extracranial-intracranial (EC-IC) bypass surgery using a radial artery interposition graft (RAIG) for surgical management of cerebrovascular diseases. Methods : The study involved a retrospective analysis of 13 patients who underwent EC-IC bypass surgery using RAIG at a single neurosurgical institute between 2003 and 2009. The diseases comprised intracranial aneurysm (n=10), carotid artery occlusive disease (n=2), and delayed stenosis in the donor superficial temporal artery (STA) following previous STA-middle cerebral artery bypass surgery (n=1). Patients were followed clinically and radiographically. Results : Bypass surgery was successful in all patients. At a mean follow-up of 53.4 months, the short-term patency rate was 100%, and the long-term rate was 92.3%. Twelve patients had an excellent clinical outcome of Glasgow Outcome Scale (GOS) 5, and one case had GOS 3. Procedure-related complications were a temporary dysthesia on the graft harvest hand (n=1) and a hematoma at the graft harvest site (n=1), and these were treated successfully with no permanent sequelae. In one case, spasm occurred which was relieved with the introduction of mechanical dilators. Conclusion : EC-IC bypass using a RAIG appears to be an effective treatment for a variety of cerebrovascular diseases requiring proximal occlusion or trapping of the parent artery.

  • IT산업 발전에 따라 일상생활은 점차 편리해지고 있으며 이와 비례하여 환경오염의 확산과 각종 질병들에 대한 위험도 점차 높아지고 있다. 인간의 생명을 위협하는 위험한 질병 중에는 사전에 예방하지 않으면 돌이킬 수 없는 사태로 확산되기도 하지만 바쁜 현대인들에게는 자신의 건강상태를 사전에 파악하고 관리하기 힘든 실정이다. 이에 유비쿼터스 환경에 접어든 IT기술을 바탕으로 의료진단 및 예방 시스템을 구축하여 사용자들이 센서를 통해 컴퓨터를 직접 이용하지 않더라도 자신의 건강상태를 유지 관리할 필요가 있다. 그러므로 본 논문에서는 이와 같이 유비궈터스 환경으로 변화해가는 기술적 변화를 바탕으로 의료 진단 및 예방이 가능한 시스템 구축을 위한 모델로서 뇌혈관 질환에 대한 모델을 제시하고자 한다. 또한, 이를 통해 향후 구축하고자 하는 u-헬스 케어 의료진단 시스템 구축 모델에 활용하고자 하며, 본 모델을 통해 의료정보를 활용한 산업 발전과 인류의 편이성 증대 및 건강상태의 주기적 검사를 통해 향상된 복지문화를 유도할 수 있을 것이다.

소아중풍(小兒中風) 환자(患者)에 대한 치험(治驗) 1례(例) (One Case of Stroke in Childhood)

  • Objectives : The purpose of this study is case report of stroke in childhood. The childhood cerebrovascular diseases are interesting than those seen in adulthood, because the etiology and prognosis of the disease in children are quite different from those of adults. Cerebrovascular diseases in children is more common than once recognized. However, relatively little attention has been paid to the epidemiology of childhood stroke. Methods : We experienced one case of ischemic stroke in childhood whose symptoms are similar to that of adults. There is no evidence of cerebrovascular malformation at MRI, MRA film. The cause of stroke is undetermined and just we can see the little infarction lesion on Lt. pons. We treated the patient with herb medicine, acupuncture-Tx, negative-Tx, moxa-Tx and physical treatment. Results : After the treatment of oriental medicine, the patient recovered from the stroke symptoms. Conclusion : we thought that the more follow-up is needed. So we repoert one case of cerebral infarction in children that recovered from stroke symptoms after the treatment of oriental medicine.

애엽(艾葉) 추출(抽出) 약침(藥鍼)이 허혈성(虛血性) 손상(損傷)에 미치는 효과(效果) (Effects of FOLIUM ARTEMISIAE ARGYI' Herbal Acupuncture on the Expression of nNOS protein Following Transient Forebrain Ischemic Injury in Rats)

  • Objectives : Acupuncture has been used to prevent and treat the cerebrovascular accident, such as a stroke, and many studies of acupuncture and moxibustion concerning to the stroke have been undertaken in the human and various animals. Also, herbal acupuncture, namely aqua acupuncture has been applied and developed to various diseases including the cerebrovascular accident. FOLIUM ARTEMISIAE ARGYI is the dry leaf of Artemisia argyi Levl. et Vant. collected in summer before the plant blooms and used to moxibustion and has been recommended for use as an analgesic and hemostatic. In this study, effects of FOLIUM ARTEMISIAE ARGYI (艾葉)' herbal acupuncture on the $LR_3$, namely Taechung on neuroprotection after the transient forebrain ischemia were investigated in Sprague-Dawely rats. Methods : Expressions of neuronal nitric oxide synthase (nNOS) protein in the hippocampus and cortex were observed at 2 hrs after transient forebrain ischemia by immunohistochemistry. Results : Expression of nNOS protein was increased in the hippocampus and cortex at 2 hrs after transient forebrain ischemia. However, pretreatment with FOLIUM ARTEMISIAE ARGYI' herbal acupuncture on $LR_3$ significantly decreased expression of nNOS protein protein compared to ischemia group. These features were observed in the motor cortex and the hippocampus. Conclusions : These results suggest that pretreatment with FOLIUM ARTEMISIAE ARGYI' herbal acupuncture on $LR_3$ inhibits the expression of nNOS protein induced by transient forebrain ischemia and may modulate excitatory toxicity of neuron related to neuronal cell death.

심뇌혈관질환 위험인자를 가진 폐경 중년여성의 질환예방 지식과 건강행위 (Knowledge on Cardio-cerebrovascular Disease and Health Behaviors among Middle-aged Postmenopausal Women at Risk)

  • Purpose: This study examined knowledge about cardio-cerebrovascular disease (CVD) and its relationship to health behaviors among middle-aged postmenopausal women with CVD risk factors. Methods: The study was a cross-sectional descriptive study. One hundred and thirty-six postmenopausal women were recruited from outpatient departments of four hospitals. The women were $60.69{\pm}6.5$ years old. Self-reported questionnaires were administered, and waist-hip ratios (WHR) were measured. Results: Among the women, 72.8% reported hypertension, 19.1% reported diabetes, 33.8% reported hypercholesterolemia, and 24.2% reported angina pectoris. Moreover, 73.9% of the women reported not knowing of CVD prevention, and only 26.1% reported exercising regularly. A majority of the women (80.9%) had a WHR > 0.85. Multiple linear regression analysis after adjusting for age and marital status indicated that the risk of myocardial infarction and stroke increased (p<.001). Waist-hip ratio${\leq}0.85$ (p=.022) and living with family members (p=.006) were significant predictors of healthier behaviors ($R^2$=0.21, p<.001). Knowledge of CVD and health behaviors were not correlated. Conclusion: Obese women and women who live alone are no more likely to practice health behaviors aimed at CVD prevention than their counterparts in the sample. Education and exercise interventions are needed, especially for obese women, to promote healthy behaviors among middle-aged postmenopausal women with CVD risk factors.

Pre- and Post-Angioplasty Perfusion CT with Acetazolamide Challenge in Patients with Unilateral Cerebrovascular Stenotic Disease

  • Objective : Perfusion computed tomography (PCT) has the ability to measure quantitative value and produce maps of mean transit time (MTT), cerebral blood flow (CBF), and cerebral blood volume (CBV). We assessed cerebral hemodynamics by using these parameters and acetazolamide (ACZ) challenge for pre- and post-procedural evaluation in patients with unilateral cerebrovascular stenotic disease. Methods : Thirty patients underwent pre-procedural PCT with ACZ challenge, and 24 patients (80%) was conducted follow up PCT after angioplasty with same protocol. The mean MTT, CBF, and CBV were measured and compared in both middle cerebral arterial (MCA) territories before and after ACZ challenge. Hemispheric ratio and percent change after ACZ challenge were calculated before and after angioplasty. Results : The mean stenosis rate was 76.6%. Significant increases in MTT (32.6%, p=0.000) and significant decreases in CBF (-14.2%, p=0.000) were found in stenotic side MCA territories. After ACZ challenge, there were significant changes in MTT (37.4%, p=0.000), CBF (-13.1%, p=0.000), and CBV (-10.5%, p=0.001) in pre-procedural perfusion study. However, no significant increases were found in MTT, or decreases in CBF and CBV in post-procedural study. There were no significant changes after ACZ challenge also. In addition, the degrees of these changes (before and after ACZ challenge) were highly correlated with the stenotic degrees in pre-procedural perfusion study. Conclusion : PCT with ACZ challenge appears to be a useful tool to assess the cerebral perfusion status especially in patients with unilateral symptomatic stenotic disease.

뇌혈관질환 후 발생한 욕창에 대한 치험 2례 (A Clinical Observation of two cases of Pressure Sore complicated by cerebrovascular diseases)

  • Pressure sore(decubitus ulcer) is the an ulceration and necrosis of the skin and underlying tissue usually occuring on a bony prominence of the body after prolonged or repeated pressure. This study was designed to evaluate the effects of an oriental medicine(Sipjeondaebo-tang) on pressure sores complicated by cerebrovascular diseases. Methods: As For treatment, we used Sipjeondaebo-tang. Results: Both patients were treated with Sipjeondaebo-tang. After treatment, the pressure sores of two patients were improved. Conclusions: This study suggests that Sipjeondaebo-tang is effective in treatment of pressure sores(decubitus ulcer) complicated by cerebrovascular diseases.

뇌졸중의 회복에 영향을 미치는 요인에 관한 연구 -한방병원 입원환자를 중심으로- (A Study on Factors Related to Recovery from Cerebrovascular Accidents)

  • This study was designed to investigate factors related to recovery from cerebrovascular accidents(CVA). Medical charts of 100 CVA patients(40 males and 60 females) who had been treated at Bul-Guang hospital in Teagu from June to December 1994 were reviewed to assess their recovery from CVA. Not only types of CVA, blood pressure and serum cholesterol and triglyceride were factors affecting recovery from CVA, but also smoking and drinking status and food preference seemed to be important factors. The percentages of recovered patients were higher in the order of cerebral thrombosis(83.3$\%$), subarachnoid hemorrhage(57.1$\%$), cerebral embolism(50.0$\%$), and cerebral hemorrhage(26.7$\%$). Recovery rates of patients with serum cholesterol below 200mg/dl, 200-239mg/dl, over 240mg/dl were 81.8$\%$, 66.6$\%$, and 16.6$\%$ respectively. Recovery rates of patients with serum triglyceride below 160mg/dl, 160-209mg/dl, and oover 210mg/dl were 84.6%, 72.8$\%$, and 35.7$\%$ respectively. Patients with standard weight recovered better than those with overweight or obesity. Recovery rates of underweight, standard weight, overweight and obesity patients were 73.3$\%$, 85.7$\%$, 45.8$\%$, and 31.6$\%$ respectively. Smoking and drinking seemed to be important factors which inhibited recovery from CVA. Patients preferring spiced foods were recovered better than those preferring salty or pungent foods.
